7 저널리뷰 Journal Review Treatment of Endometriosis-Associated Pain with Elagolix, an Oral GnRH Antagonist N Engl J Med 2017;377:28-40.DOI: /NEJMoa Intervention Model: Parallel Assignment Study Design Masking: Quadruple (Participant, Care Provider, Investigator, Outcomes Assessor) Primary Purpose: Treatment Official Title Brief Summary A Randomized, Double-Blind, Placebo-Controlled Study to Evaluate the Safety and Efficacy of Elagolix in Subjects With Moderate to Severe Endometriosis-Associated Pain A randomized study evaluating the safety and efficacy of elagolix in the management of moderate to severe endometriosis-associated pain in adult premenopausal female subjects. Background 자궁내막증은월경통및골반통증을유발하는에스트로겐의존성만성질환이다. 경구용비펩타이드 Gonad otropin-releasing hormone(gnrh) 길항제인 Elagolix는이전연구들에서에스트로겐을부분적또는거의완전히억제하였다. Methods Elagolix의위약대비두용량의효과를평가하기위하여유사한두개의무작위, 이중맹검, 3상임상 (Elaris Endometriosis I and II [EM-I and EM-II]) 을 6개월간진행하였다. 외과적으로자궁내막증진단을받고이와관련된중등증 ~ 중증의통증이있는환자를대상으로각각 Elagolix 1일 1회 150mg( 저용량군 ) 또는 1일 2회 200mg( 고용량군 ), 위약 ( 위약군 ) 을투여하였다. 일차종결점은 3개월후월경통, 비월경성골반통에대한임상반응을보인비율이었다. 이종결점들은일일전자의무기록을바탕으로임상적으로유의한통증점수감소와구제진통제의사용횟수감소또는안정적유지등을종합하여측정하였다. Results Elaris EM-I은 872명, EM-II는 817명의여성을무작위배정하였고이중각각 653명 (74.9%), 632명 (77.4%) 이 intervention을완료하였다. 3개월후, 위약을복용한군보다각각의 elagolix 용량을복용한여성에서더많은비율로임상적반응의일차적종결점기준을충족하였다. Elaris EM-I에서월경통에대한임상반응이있는여성비율은위약군 19.6% 에비해저용량군 46.4%, 고용량군 75.8% 로나타났다. Elaris EM-II에서는위약군 22.7% 에비해저용량군 43.4%, 고용량군 72.4% 이었다 (P<0.001 for all comparisons). Elaris EM-I에서비월경성골반통의반응을보인여성의비율은저용량군 50.4%, 고용량군에 54.5% 이었으며위약군 36.5% 이었다 (P<0.001 for all comparisons). Elaris EM-II에서는관련비율이저용량군 49.8%, 고용량군 57.8%, 위약군 36.5% 이었다 (P=0.003 and P<0.001, respectively). 월경통과비월경성골반통의반응은 6개월간평가되었다. Elagolix를복용하는여성에서위약대비일과성열감 ( 대부분경증또는중등증 ) 이더많은비율로나타났으며, 혈중지질농도가높고 baseline 대비골밀도가감소하였다. 자궁내막의다른이상소견은발견되지않았다. 7
