1 고위험군유두상갑상선암환자에대한방사성옥소최대허용선량측정법및치료법의검증 Verification of Measurement Methods and Therapeutic Efficacy of Maximum Permissible Dose of I-131 in High Risk Patients with Differentiated Papillary Thyroid Cancer Jung Chul Kim, M.D., Jung Han Yoon, M.D., Young Jong Jaegal, M.D. and Hee Seung Bum, M.D. 1 Purpose: Radioiodine (I-131) therapy is an effective modality to reduce both recurrence and mortality rates in differentiated thyroid cancer. Whether higher doses shows higher therapeutic responses was still debatable. The purpose of this study was to validate curve-fitting (CF) method measuring maximum permissible dose (MPD) by a biological dosimetry using metaphase analysis of peripheral blood lymphocytes. Methods: Therapeutic effects of MPD was also evaluated in 58 patients (49 females and 9 males, mean age 50±11 years) of papillary thyroid cancer. Among them 43 patients were treated with 7.4 GBq, while 15 patients with 9.25 GBq. The former was defined as low-dose group, and the latter high-dose group. Therapeutic response was defined as complete response when complete disappearance of lesions on follow-up I-131 scan and undetectable serum thyroglobulin levels were found. Statistical comparison between groups were done using chi-square test. P value less than 0.05 was regarded as statistically significant. Results: MPD measured by CF method using tracer and therapeutic doses were 13.3±1.9 and 13.8±2.1 GBq, respectively (P=0.20). They showed a significant correlation (r=0.8, P ). Exposed doses to blood measured by CF and biological methods were 1.54±0.03 and 1.78±0.03 Gy (P=0.01). They also showed a significant correlation (r=0.86, P=0.01). High-dose group showed a significantly higher rate of complete response (12/15, 80%) as compared to the low-dose group (22/43, 51.2%) (P=0.05). While occurrence of side effects was not different between two groups (40% vs. 30.2%, P=0.46). Conclusions: measurement of MPD using CF method is reliable, and the high-dose I-131 therapy using MPD gains significantly higher therapeutic effects as compared with low-dose therapy. (B) A 44-year-old female patient with stage 3 papillary thyroid cancer shows partial response to 5.55 GBq of I-131. 였고 21명(48.8%)에서는 부분치유를 보인 반면, 고용량 치 료군 15명 중 12명(80%)에서 완전치유를 보였고 3명(20%) 에서만 부분치유를 보여 고용량 치료군에서 유의하게 높 은 완전치유를 얻을 수 있었다(P=0.05). 한편 부작용 발생 빈도는 저용량 치료군 43명 중 13명(30.2%), 고용량 치료 군 15명 중 6명(40%)로 양 군 간에 유의한 차이가 없었다 (P=0.46). 임상적인 병기, 연령 및 성별에 따라서는 치유의 차이가 없었다(모두 P 0.05). 완전치유와 부분치유를 보 인 환자 증례를 Fig. 4에 보였다. 고 찰 본 논문에서 저자는 산술적으로 측정한 MPD가 실제로 환자의 말초혈액림프구에 대한 선량을 잘 반영하고 있다 는 사실을 밝힘으로써 현재 치료하고 있는 방법의 적절성 을 검증하였으며, 치료용량의 방사성옥소를 투여한 후에 산술적으로 MPD를 측정하는 방법도 가능하다는 사실을 밝혔다. 이처럼 측정한 MPD를 이용하여 고용량 방사성옥 소 치료를 시행한 환자의 치료결과는 고식적인 저선량치 료법에 비해 유의하게 높은 치료율을 보였다. 분화갑상선암은 일반적으로 예후가 좋다고 알려져 있지 만, 재발하는 경우는 암의 분화도가 낮아지기 때문에 예 후가 급격히 나빠진다. Samaan등(1)과 Mazzaferri등(2)은 크기가 1.5 cm 이상인 분화갑상선암 환자를 대상으로 갑 상선암 절제술 후 방사성옥소를 이용하여 내부방사선치 료를 함으로써 재발률과 사망률을 유의하게 낮출 수 있다 는 사실이 분명하게 보고하였다.(1,2) 방사성옥소 치료법의 문제는 부작용이다. 치료 직후에 생길 수 있는 부작용으로는 위염, 타액선염, 혈구수 감소 등을 들 수 있으며, 만성적인 부작용으로 타액선염, 혈구 수 감소, 불임, 그리고 방사선발암을 생각할 수 있다.(13) 방사성옥소 치료 후 가임여성의 약 30%에서 첫 1년째 난 소기능부전을 경험한다고 알려져 있다.(14) 누적치료용량 이 14.8 GBq 이하에서는 발암의 위험이 증가하지 않으나, 누적치료용량이 37 GBq 이상일 때는 백혈병 발생률이 증 가한다고 알려져 있다.(15-17) 방사성옥소 치료를 반복적으로 시행하면 방사선에 민감 한 암세포는 사멸하므로, 방사선에 저항성을 가지고 있는 세포가 살아남아 재발하는 경우는 방사성옥소 치료효과가 차츰 감소하게 된다. 이를 극복하기 위해 레티노이드를 이 용한 재분화치료를 시도하기도 하지만 그 효과는 아직 높지 않다.(18,19) 따라서, 첫 번째 방사선치료를 시행할 때 최대
