A wide excision of the causative tumor is considered as the treatment of choice, following which, a dramatic recovery is expected. Authors report a case in which the symptoms and bone mineral density were dramatically recovered after an excision of the causative tumor around the tibialis posterior muscle in oncogenic osteomalacia. Conflict of interests: None. 종양성골연화증은종양에서분비하는 fibroblast growth factor (FGF)23 으로인하여발생되는저인산혈증, 신장인산염소모 (renal phosphate wasting), 골연화증및다발성부전골절을특징으로하는드문부종양성증후군이다 년 McCance 1) 에의해처음기술되었으며, 이후전세계적으로약 350예이상이보고되었다. 2) 환자는골통, 근력약화, 근무력감, 골절등비특이적인임상증상을호소하기때문에진단및종양의존재를확인하는데상당한시간이소요되는사례 가많다. 3) 궁극적인치료로는원인이되는종양을찾아내어완전절제하는것이며, 이로써수술후극적인회복을기대할수있다. 저자들은종양성골연화증으로진단된 31세남자환자에서종양제거만으로도다발성부전골절유합과증상및골밀도가극적으로회복된사례를보고하면서빠른진단과종양의탐색및제거가이병의치료에얼마나중요한가를강조하기위하여증례보고하고자한다. 해효소(alkaline phosphatase, ALP)의 증가, 부갑상선 호르몬 특별한 과거력이나 가족력은 없었으며, 전신 골 스캔상 늑 골, 골반골, 근위 대퇴골 등에서 다발성 열소가 관찰되었다 A B Fig. 1.

뇨 검사상 과인산뇨증은 아니지만 신장에서 원 후에 퇴원, 외래 추시하였다.

27 mg/dl, 정상치: 대퇴골 경부 내측 기저부에 부전골절 소견이 관찰되어 본과 2.34.3 mg/dl).4) 지속적인 저인산혈증은 1alpha hydroxylase를 자극하여 1,25(OH)2 vitamin D의 농도를 증가시켜서 장내 인의 흡수를 촉진시킴으로써 저인산혈증을 교정한다.

$하퇴부에 자기공명영상(magnetic resonance imaging, MRI) 검사를 시행하였으며 후 경골근내 에 종양을 확인하였다(Fig. 4). 종양은 피막에 쌓여 주위와 잘 분리되어 있었으며, 수술적으로 완전히 절제하였다(Fig. 5). Fig. 3. Insufficiency fracture of the femoral neck.$

3 Journal of the Korean Fracture Society Vol. 30, No. 3, July 2017 (parathyroid hormone, PTH)의 증가, 25(OH) vitamin D는 로 입원하였다(Fig. 3). 환자의 통증 조절과 조기 보행을 위하 감소되었다(Table 1). 신장 및 갑상선 기능 검사는 정상이었 여 예방적 금속정 삽입술을 시행하였다. 혈액검사상 지속적 으며, 부갑상선 스캔 상에서 특별한 병변은 관찰되지 않았다. 인 저인산혈증과 1,25(OH)2 vitamin D의 감소, ALP의 증가 PTH 피하 주사 투여 및 칼슘, 비타민 D를 복용하며 1주일 입 소견을 보였다. 뇨 검사상 과인산뇨증은 아니지만 신장에서 원 후에 퇴원, 외래 추시하였다. 인산염이 부적절하게 소모되는 소견을 보였다(24 hr urine P: 퇴원 후 4개월 뒤, 보행 중 좌측 고관절 통증이 갑자기 악 782 mg, 정상치: 4001,300 mg/24 hr; tubular maximum 화되어 응급실로 내원하였으며, 고관절 방사선 사진상 좌측 of phosphate/glomerular filtration rate: 1.27 mg/dl, 정상치: 대퇴골 경부 내측 기저부에 부전골절 소견이 관찰되어 본과 mg/dl).4) 지속적인 저인산혈증은 1alpha hydroxylase를 자극하여 1,25(OH)2 vitamin D의 농도를 증가시켜서 장내 인의 흡수를 촉진시킴으로써 저인산혈증을 교정한다. 본 증례와 같이 저인산혈증임에도 불구하고 1,25(OH)2 vitamin D의 농도가 낮은 것은 1alpha hydroxylase를 억제하는 물질이 혈중에 있는 것을 의미하며, 이런 물질 중의 하나인 FGF23을 측정한 결과 608 RU/ml (정상치: <180 RU/ml)로 크게 증가된 것이 확인되었다(Table 1). FGF23은 골연화증 을 일으키는 종양에서 자주 분비하기 때문에 종양의 위치 추 적을 진행하였다. 원인이 되는 종양의 위치를 확인하기 위해 111 Indiumoctreoscan을 시행하였으며, 좌측 하지의 경골 주 위에서 열소가 확인되었다. 하퇴부에 자기공명영상(magnetic resonance imaging, MRI) 검사를 시행하였으며 후 경골근내 에 종양을 확인하였다(Fig. 4). 종양은 피막에 쌓여 주위와 잘 분리되어 있었으며, 수술적으로 완전히 절제하였다(Fig. 5). BMC: bone mineral content. 혈중 인 수치는 수술 후 1일째 정상이 되었다. 수술 후 7일 고 찰 째 시행한 혈액검사상 1,25(OH)2D는 증가하였으며, FGF23 (72 RU/ml)은 정상으로 회복되었다(Table 1). 이후 다른 Jiang 등5)이 1987년부터 2011년까지 총 308명의 종양성 치료 없이 경과 관찰하였으며 수술 후 1년 6개월째 시행한 골 골연화증 환자를 분석하여 발표한 논문에 따르면, 평균 연령 밀도상에서 제3 요추의 Z값은 3.2에서 0.6으로, 제4 요추 은 45±16세(286세)였으며, 여성 47%, 남성 53%의 비율이 의 Z값은 3.6에서 1.1로 회복되었다(Fig. 6). 좌측 전자부의 었다. 골 조직 기원이 40%, 연부조직 기원이 55%였으며, 약 부전골절도 치유되어 금속정을 제거하였다. 3%의 환자군에서는 종양 위치를 찾지 못하였고, 2%의 환자 군에서는 다발성 종양이 발견되었었다. 149

5 Journal of the Korean Fracture Society Vol. 30, No. 3, July 2017 종양에서분비되는 FGF23은근위세뇨관에서 type II sodiumphosphate cotransporters (NaPiIIa and NaPi IIc) 의발현을감소시켜며, 인의재흡수를막아서신장에서의인배출을증가시켜서저인산혈증을일으킨다. 또한 1α hydroxylase 를억제하여 25(OH) vitamin D가 1,25(OH) 2 vitamin D로활성화되는것을억제한다. 종양성골연화증의임상증상은비특이적이고모호하여진단이종종늦어지며, 증상발현후진단되는데까지 10여년이상이걸리기도한다. 3) 본증례에서도비특이적증상을호소하였으며진단까지 2년 4개월이걸렸다. 따라서전형적인생화학적인패턴인저인산혈증, 신장인산염소모및골연화증소견을보일시에는종양성골연화증을반드시의심해야한다. 원인이되는종양은보통크기가작고전신의어디에도발생할수있어종양탐색시많은어려움이따른다. 일반적으로전신에대한 fluorodeoxyglucosepositron emission tomography/computed tomography (PET/CT), octreotide scintigraphy 등의기능적이미지검사 (functional imaging) 를시행해야하며, 의심되는병변이확인되면 MRI 같은해부학적이미지검사 (anatomical imaging) 로위치를확인하여야한다. 6) 그러나종양의위치가확인되지않을시에는낮은인과 1,25(OH) 2 vitamin D로인한이차성부갑상선항진증을예방하기위해인과 1,25(OH) 2 vitamin D를보충하면서 1년마다주기적으로종양을찾기위한검사를시행하여야한다. 최근에는 68 Ga DOTATATE PET/CT 가정확하다는발표들이있었으며, 정확성은 97.7%, 민감도 100%, 특이도 90.9% 로보고되었다. 7) 또한다발성병변이발견되거나수술전정확한종양의위치확인이필요할때는 selective venous catheterization 을시행하는것이도움이되는것으로보고되었다. 8) 치료의원칙은종양의완전절제이며완전절제시에는극적인치료효과가있다. 절제후특별한치료없이임상증상의호전및생화학적검사소견의정상화가수일내빠르게진행되며, 6) 감소된골밀도는수술후약 1년째에정상에가깝게회복되는것으로몇몇보고들에서보고되었다. 9) 본증례에서도수술후특별한치료없이임상증상및생화학적비정상소견이빠르게정상화되었으며, 골밀도또한정상으로회복되었음을확인하였다. 결론적으로종양성골연화증치료에서빠른진단과종양의탐색및제거가무엇보다중요하며, 저자들이경험한증례를통하여다시한번강조하기위하여보고한다. 요약 종양성골연화증은종양에서분비하는 FGF23 으로인하여발생되는저인산혈증, 신장인산염소모, 골연화증및다발성부전골절을특징으로하는드문부종양성증후군이다. 치료는원인이되는종양을찾아내어완전절제하는것이원칙이며, 수술후극적인회복을기대할수있다. 저자들은후경골근주위에발생한종양성골연화증의원인이되는종양을제거하여증상및골밀도가극적으로회복된사례를보고하는바이다. 색인단어 : 종양성골연화증, 부전골절, 저인산혈증 ORCID 박영창, 서준오, 양규현, References 1.
