1 대한소화관운동학회지 2005;11:89-96 종설 염증과기능성위장관질환 건양대학교의과대학내과학교실 허규찬 Inflammation in Functional Gastrointestinal Disorders Kyu Chan Huh, M.D. Department of Internal Medicine, Konyang University College of Medicine, Daejeon, Korea 서 론 기 전 기능성위장관질환은분명한기질적이상이없이다양 한임상증상을기초로진단하는것으로그기전은잘알려 져있지않다. 과거에는기능성위장관질환은정신적인 원인에서기인한다고생각했지만최근에는급성감염이사 라진후에도계속남아있는염증반응으로인한장관기능 의변화가중요한원인중의하나로알려져있다. 즉비록 현재의진단방법으로는쉽게찾지못할점막이나장관신 경계의변화가계속존재하여장기능의변화를초래한다 는것이알려졌다. 박테리아, 바이러스등의감염이나독소그리고면역체 계이상등으로인해위장관은염증상태에놓이게된다. 이때염증을일으키는여러가지세포에서분비되는물질 들이위장관신경의과민성을일으킬뿐아니라, 점막의 분비, 흡수및장관운동의부조화를일으켜증상을일으 키기도한다. 본저자는위장관에서염증을가장잘일으키는감염 후에나타나는여러가지기능성장질환에대해문헌고찰 과함께기술하고자한다. 감염후발생한기능성소화불 량증, 위마비그리고가장잘알려진감염후과민성장 증후군을중심으로해서간략히서술하고자한다. 접수: 2005년 12월 7일책임저자 : 허규찬, 대전시서구가수원동 685 ( ) 건양대학교병원소화기내과학교실 Tel: (042) , Fax: (042) kchuh2020@hanmail.net 위장관은수많은신경들이서로관장하는기관으로 " 제3 의뇌" 라고도불린다. 기능성위장관질환은이러한구심 성혹은원심성신경의이상과점막기능의이상으로발생 한다. 감염후에생기는기능성위장관질환은감염에의 해생긴염증반응이신경을포함한조직을손상시키고 치유과정을통해서신경이재생되는데이때재생된신경이 자극에대해과민성을나타내어여러가지기능성위장관 질환을일으키는중요한원인이된다. 기전을크게염증의시간경과에따른관점과각각의 기전에따라나눌수가있다 1. 시간에따른변화 급성기에는감염성염증이생기면세균에서여러가지 염증성사이토카인을분비하고이뒤를이어백혈구, 림프 구들이모이고여기에서염증의매개체인 ATP, H +, 프로스 타글란딘등이분비되어조직을파괴한다. 이과정뒤를 이어염증을감소시키고조직을치유하는재생과정이일어 나는데이때신경의과민성이나타난다. 즉기존의통각을 느끼는침해수용체(nociceptor) 의활성이증가되고기존의 무반응침해수용체(silent nociceptor) 가염증부분에모이 게되어과민성이나타난다. 2 위장관의점막에염증이 생기면화학적매개체인브라디키닌(bradykinin), 히스타민 (histamine), 세로토닌(serotonin), 카테콜라민 (catecholamine), 프 로스타노이드 (prostanoids), 아데노신(adenosine), 서브스탄스 P (substance P), 사이토카인 (cytokine), 신경펩타이드 (neuropeptide)

Disorders of Gastrointestinal Motor and Sensory Function After Known and Unknown Inflammatory Insults 1 Disorders of function Esophageal dysmotility Delayed gastric emptying Functional dyspepsia IBS Guillain-Barré syndrome complicated by ileus pseudo-obstruction Neuroimmune appendicitis Symptomatic diverticular disease Inflammatory insult Infection by T. cruzi Rotavirus infection, Lyme disease, Post vaccination, Cytomegalovirus, herpes zoster, Epstein-Barr virus, Human immunodeficiency virus Presumed infective Postinfectious IBS IBS in quiescent IBD Myenteric ganglionitis Postinfectious polyneuropathy, Herpes zoster Resolved acute appendicitis Acute diverticulitis 등이분비되어염증조직에통각과민을유발하게되며, 3-6 통증을유발하는자극은말초신경의세포간질(matrix) 을변 화시킴으로써감각역치를간접적으로낮출수있다. 7,8 또 염증은척수신경의전달을증강시키는 vanilloid 수용체 (VR1) 9 와 nerve growth factor 수용체 10 의기능을향상시킨다. 염증반응의후기단계에는대식세포와림프구가침윤 하는데, 이것은궁극적으로신경에변화를초래한다. 즉부 교감신경의장관근육의활성도를증가시키고교감신경인 노에피네프린의분비를감소시킨다 과정에따른기전 감염후기능성장증후군을일으키는기전을보면첫째, 염증회복의불완전성이다. 급성염증에서회복되는단계 에들어가면염증반응을감소시키는 IL-10을생성하는 조절세포(T-reg) 와면역반응을억제하는 TGF- β (tumor growth factor- β ) 를분비하는 Th 3 의역할이항염증성과정에있어서 중요하다. 감염후과민성장증후군에서는이것이억제되어 감염이사라진후에도염증이오랫동안존재한다. 14 둘째, 점막기능의변화이다. 염증은점막의상피세포를파괴하여 상피세포의보호기능인장벽기능에도영향을줄뿐아니 라, 점막의기능에도영향을준다. 초기에는장투과성이 증가하고흡수가감소하며재생과정에서는림프구와연관 된사이토카인에의해서생성된 paneth 세포와장내분비세 포(enterochromaffin cell) 의과증식을일으키고 T 15,16 담즙산과 염분(sodium) 의흡수를방해하여설사와같은증상을일으 킨다. 셋째, 장신경계의변화이다. 염증의치유도중에는 염증에의해손상된혈관이나신경들이재생되는데이때 신경들은평소보다과하게증식이되고수개월이지나야 정상으로돌아오게된다. 17 과하게증식된신경들은어떤 자극에대해과민성을나타나는데, 대표적인경우가치아 농양(dental abscess), 호흡기감염, 비뇨생식계감염등이 다 대식세포에서유래된염증성사이토카인인 IL-1 β 는 근육층신경총(myenteric plexus) 에서신경신경전달물질의 유리를억제시켜미주신경과뇌간을통해서장관에대한 중추신경계의조절능력을변화시킨다. 21 넷째, 내장감각능 의변화이다. 조직이손상되면신경을감작시키는 ATP, 프 롬빈(thrombin), 프로스타글란딘, 세로토닌등이분비된다. 22 염증에의해활성화된 nerve growth factor는즉각적으로나 지속적으로신경을감작시키고수용체의발현을변화시킨 다. 수용체의상향변화는특히침해수용체에서잘나타 나는데이러한변화는손상된조직에활동을억제시켜치유 를돕는다. 9,23,24 치유에도움이되는운동성의저하는환자에 서는오심과구토와같은증상으로나타나고, 또장관의과민 성으로인해분비를촉진시키고밀어내는힘을증가시켜병 원균을배출시키는설사와같은증상이나타나기도한다. 감염후기능성소화불량증 기능성소화불량증의기전은크게위배출시간의지연, 위적응력감소, 그리고위장관과민성으로나눌수있다. 특발성기능성소화불량증과감염후에나타나는기능성소 화불량증을비교해보았을때위배출시간이나위의과민 성등에는차이가없었으나위의적응력이현저히떨어짐 을관찰하였다 (67 vs 30%). 25 급성염증이선택적으로억제 성 nitrergic 신경에장애를주어위적응력의감소를초래 한다고알려져있다. 26 급성 Helicobacter pylori 감염에의한 위염에서나타나는증상인오심, 복통구토등의증상이 위적응력의감소로나타난다는보고들이있다. 27

Mechanisms and Mediators of Altered Gastrointestinal Function After Inflammation1 Neuronal loss Mechanisms Mediators Clinical examples Altered nerve phenotype (increased substance P, VR1 expression, galanin) Increased gut permeability Altered muscle function (hypertrophy/responsiveness) GDNGF, glial-derived nerve growth factor; VR1, vanilloid receptor Acute inflammatory products Neurotrophic virus Autoimmune Nerve growth factors (GDNGF) Cytokines T lymphocytes, increased macrophages, increased cyclooxygenase-2 Acute diverticulitis, Acute appendicitis Herpes zoster, Epstein-Barr, Human immunodeficiency virus Guillain-Barré syndrome Crohn's disease/ulcerative colitis, neuroimmune appendicitis Diverticular disease Postinfectious IBS 대부분의기능성소화불량증은인지하지못하거나특 별한원인없이나타나는데전세계의기능성소화불량 증환자의유병율은 약 25% 내외로알려져있고, 28 그중의 1/4 은과거에급성감염의과거력을가지고있다. 25 기능성소화불량증의위험인자로는남자와흡연자로 29 알려져있지만기존의소화불량증환자와감염후소화불 량증의비교시감염후에잘발생하는특별한위험인자 는없으나감염후증상이발생하는경우 연령이었다. 25 4살정도어린 감염후소화불량증은열, 오심, 구토, 설사등의증상이 나감염균을동정하는등분명한감염의증거가있어야 하고내시경등검사등을시행하여기질적질환이배제가 된후기능성소화불량증의진단기준 30 에부합될경우진단 이가능하다. 기능성소화불량증의증상은상복부불쾌감, 오심및구 토, 식후포만감, 조기팽만감등이고이러한증상으로인 해음식섭취의감소로체중감소가나타난다. 31 특발성에 비해감염후생하는소화불량증의특징은오심및구토가 좀더심하고 5% 이상의체중감소가특징이다. 25 치료는안정시키고정신적격려가제일중요하다식이 방법은소량자주먹는것이중요하다. 약제로는양성자 펌프억제제가실험적치료로약 40% 에서호전을보이나 위약에비해큰차이가없다. 32 운동촉진제도증상호전의 효과가없다. 단지위의적응력을높이는 sumatriptan은효 과가있는것으로알려져있지만좀더많은연구결과를 기대해보아야한다. 33 예후에대한전향적연구는별로없 으나 6년간추적관찰한결과특발성보다예후가좋은것 으로알려져있다. 34 만성적 Helicobacter pylori 감염과기능성소화불량증 위장관에세균감염에의해염증이생기면여러가지 면역과정을거쳐고유의기능에도영향을미친다는것은 잘알려진사실이다. 하지만기능성소화불량증발생과만 성적 Helicobacter pylori 감염과의연관성에대해서는아직 까지논란이많다. 먼저역학적인연구에서소화불량증환 자에서 Helicobacter pylori 감염률이높은것으로보고되었 으나이에상반된보고도많다. 35,36 기능성소화불량증의 증상발현의주요기전의하나로생각되는위배출기능의 저하에대한연구에서 Helicobacter pylori 감염유무에따른 위배출시간의차이는없다고보고하였다. 37 감염의결과 로염증이만성화가되면감각신경에영향을미쳐자극에 대해과민성으로변화하는데많은 Helicobacter pylori 감염 이기능성소화불량증에서위감각의예민도를증가시키 고, Helicobacter pylori 균이많이감염된경우적은수에의 한감염보다장관의팽창에대한감각역치가낮추어위배 출능의이상을초래한다는보고는있었다. 38,39 하지만다른 보고에서는 Helicobacter pylori 감염이기능성소화불량증을 일으키는데관여하는감각신경에영향을미치지않으며 팽창에대한위의과민반응과연관이없다고보고하여 40 Helicobacter pylori 감염이감각기능을변화시킨다는분명 한증거는없다. 만약 Helicobacter pylori 감염이만성기능성소화불량증 의원인이라면제균하면소화불량증의증상이사라지는것 이당연하다. 하지만제균후증상소실에대해서도서로 상반된보고들이많아아직까지논란이있다. 여러연구에 서제균치료군과대조군사이에증상소실율에차이가 없음을보고하였을뿐만아니라동시에조직학적소견의

4 92 Kor J Neurogastroenterol Motil: Vol. 11, No. 2, 2005 호전과증상소실은상관관계가없음을보고하였다. 41,42 결론적으로역학적, 병태생리적및임상적연구에서 Helicobacter pylori 감염과기능성소화불량증과의상호연 관관계에대한분명한증거는없다. 기능성소화불량증은 여러가지요소가함유된증상복합체이므로 Helicobacter pylori 감염하나로만설명할수는없기때문에이와같은 결과가나온것으로생각된다. 하지만 Helicobacter pylori 감염은소화불량증을초래할수있는여러가지원인중 하나일가능성은충분히있다. 감염후위마비 동물실험모델에서로타바이러스와 Norwalk 바이러스 감염후생기는급성바이러스위장관감염시오심, 구토 등이생기는데그원인은위배출이지연되어나타나는것 으로알려져있다. 43 또 1990년대 Mayo clinic 보고에의하면 바이러스감염후오심, 구토, 심와부불편감을호소하는 위마비환자들을조사해보니위배출의지연이분명히관 찰되고전정부의저운동성과연관되어있었다. 44 바이러스 성위장관염은흔하나위마비의합병증은극히드물다. 이 때생기는위마비는주로전정부의저운동성과연관되어 있고식이에대한전정부반응의손상은자율신경계의이 상에의해초래된다. 44 감염후위마비를일으키는바이러 스로는거대세포바이러스 (Cytomegalo virus), 45 Herpes zoster 바이러스, 46 Epstein-Barr 바이러스 47 등이있고그리고예방 접종 48 후에도위마비의발생이가능하다. 거대세포바이 러스와 Herpes zoster 바이러스는신경을직접손상을일으 키고, 9 Guilian-Barre 증후군을발생시키는 Epstein-Barr 바이 러스와예방접종후에는자가면역질환에의해자율신경 이손상되어위마비를일으킨다. 46,47 유병율을포함한역학조사는전형적연구는없으나한 연구에서 143명의환자중 52명은특발성이고 12명은바이 러스성감염과연관된것으로보고하였다. 47 진단은정형화 된기준은없으나분명한감염의증거와함께음식물의위 배출지연이확인되면가능하다. 임상적특징은급성으로발생하고초기에는오심, 구토, 체중감소등의증상이매우심하나시간이갈수록점차증 상은소실이된다. 잘일으키는위험인자는없으며특발성보 다는예후가좋고입원하는빈도나삶의질도훨씬좋다. 47 치료는특별한것이없으며예후를잘설명해주고안 정시켜주는것과영양공급이중요하다. 50 소화관운동촉 진제의효과는아직불분명하다. 51 식이는소량자주먹는 것이영양부족을해소하는좋은방법이다. 는 예후는특발성위마비보다는훨씬좋으며 100% 회복되는것으로알려져있다. 47 감염후과민성장증후군 32개월후에 과민성장증후군은아직까지정확한병태생리가알려 지지않는질환으로다른기질적원인이없는상태에서복 부불쾌감이나복통이배변습관의변화와함께관찰되고, 배변장애의양상이나타나는기능성질환의한군이다. 소화기전문의가가장흔하게접하는질환으로외래환자의 50% 까지보고된다. 52,53 감염후과민성장증후군은전체과 민성장증후군환자의 최초문헌보고는 6~17% 정도로알려져있다. 54 2차대전중아프리카에주둔했던영 국군인의일부가아메바성이질을앓은후오랜시간이 지난후에도설사와복통을호소하는것을정리발표한 것이다. 55 그후 Chaudhary 등 56 이세균성또는아메바성 이질후과민성대장증상과유사한증상을발생한경우 를따로구분하여이질후과민성대장증후군으로이름 지었다. 감염후과민성장증후군의발생빈도는 6~30% 까지다양 하게알려져있고 57 위험인자로는설사의기간, 58 감염된균 의독성 59 및기존의심리적인요인 60 등이라고알려져있 다. 이중에서유병기간이제일중요한데기간이길면길 수록감염후과민성장증후군이생길확률이높다. 58 여성 이남성에비해높은빈도로발생한다고하였으나, 58 이것 은남녀간의면역체계의차이라기보다는여성들이건강염 려증, 신체화증상등과같은정신과적인요인이복합되어 더욱잘나타나기때문이다. 61 감염후과민성대장증후군 의아형을보면 70% 는설사우세형, 26% 는교대형나머지 8% 가변비우세형이다. 62 급성위장염을앓은 3개월후에거의대부분의환자에 서대장통과시간과직장확장에대한불쾌감의역치가감 소된다는보고가있었다. 이러한변화는과민성장증후군 을앓는환자에서더욱많이나타난다. 60 원인은염증에대 한이상반응으로나타나며가장심한곳이원위부소장과 근위부대장이다. 감염이없어지고오랜시간후에도염증 이남아있는이유는전술한것처럼면역억제물질인 IL-10과 TGF- β 의생성이감소된때문이다. 감염후과민성대장증후군을일으키는기전을생각해

Inflammation in Functional Gastrointestinal Disorders 93 보면첫째직장을포함한장조직소견의변화, 둘째세로 토닌의역할, 셋째소장기능의변화, 넷째정신심리적인 요인등으로나누어볼수가있다. 감염후과민성장증후군환자의전층조직검사시, 특 히심한감염후발생한과민성장증후군환자에서의조 직검사에서림프구와같은만성염증세포가근육층신경총 (myenteric plexus) 에침윤및신경의변형이함께관찰되고 63,64 IL-1 β 의 mrna 의증가도관찰된다. 이러한현상의결 과로장의과민성이나타난다. 65 조직소견에서의또다른 특징은세로토닌을함유하는 enterochromaffin cell이많이 관찰되는데, 증가된 enterochromaffin cell이많은양의세로 토닌분비한다. 66 enterochromaffin cell의증가는과증식에 의해발생하고데주로염증이심할수록과증식이더욱잘 생긴다. 일어난다. 세로토닌의분비가과도하게일어나면 위장관에기계적인운동과통증의경로도촉진시킨다. TNBS (Trinitrobenzene sulphonic acid) 로유도된대장염을가진쥐 의모델에서대표적으로관찰된다. 67 감염 또다른중요한병태생리는소장기능의변화이다. Campylobacter 3개월후모든환자에서장투과성이증가됨이관찰 되었는데이러한현상은감염후과민성장증후군에서도 유사하게관찰되어염증에의한장투과성의증가가병태 생리의한부분이라고생각할수있다. 68 저등급염증이 있는경우라도지속적인기능장애가생길수있는데, 특히 담즙산의흡수기능이감소된다. 흡수되지않은답즙산이 장관을자극하여장운동을증가시키고분비를촉진하여 설사와같은증상을일으킨다. 69 답즙산흡수가감소된이 유는담즙산을이동시킬수있는운반체(transporter) 가염증 에의해억제되기때문이다. 70 정신심리적인요인인불안, 우울증, 건강염려증등은 대장운동기능을변화시킬뿐만아니라장관의자극에대한 중추신경계의진행과정을변화시켜증상을일으킨다. 60,61 진단은 Rome II 기준에의한과민성장증후군의진단 기준에합당하고분명한급성위장관염의증거가있을때 가능하다. 진단시가장중요한것은다른질환을의심할만 한체중감소, 항생제사용여부, 혈변, 대장암의가족력등 경고징후(alarm sign) 가있는지를병력청취로잘찾을수 있어야한다. 또감별진단으로만성염증성장질환인크 론병이나현미경적장염 (Microscopic colitis), 유당불내성 (lactose intolerance), 기생충감염등을잘감별진단해야한다. 71 치료는최근환자에게발생된여러가지증상들이감염 으로생긴염증에대한정상적인방어기전이라고설명하고 안심시키는것이중요하며, 72 흡수가잘되지않는과일이 나대변양을증가시키는채소의섭취를피한다. 73 약제로서 는 opiate제제인 loperamide 가효과가있다. loperamide는물 의흡수를촉진시키고대장과소장의통과시간을지연시 킬뿐아니라, 항문괄약근의긴장도를증가시켜설사우세 형과민성장증후군에효과가있는것으로알려져있 다. 74,75 항경련제는가장많이처방되는약이나여전히효 과에대해서는분명한증거가없다. 76 세로토닌의증가가 증상발현의중요한기전으로알려져있는데세로토닌수용 체길항제인 alosetron 도설사형과민성장증후군에효과가있다. 77 또답즙산의흡수장애로인한설사시에는 cholestyramine이 도움을줄수있다. 78 과민성장증후군에서는정신심리적 인치료약제로 amitryptyline을가장많이처방하지만감염 후과민성장증후군에대한치료효과는확립되지않고있다. 예후는다른원인의과민성장증후군과유사하며 추적검사결과 6년 43% 에서회복되었다는보고가있다. 79 예후 의가장중요한요소로는과도한건강염려증과같은정신 심리적요인이다. 기타염증과연관된기능성위장관질환 1. 염증성장질환과연관된과민성장증후군 염증성장질환을가진환자에서관해기간중에종종 설사나복통과같은과민성장증후군과유사한증상이나 타나서염증성장질환의활동도가증가한것인지아니면 과민성장증후군이발생한것인지환자를치료하는이들 을고민에빠지게하는경우가있다. 염증성장질환에서재발의증거없이과민성장증후군 과유사한증상을나타내는경우가궤양성대장염에서는 33%, 크론씨병에서는 57% 로나타난다. 80 이러한증상은병 의범위보다는환자의심리적인상태나병의유병기간에 따라달라지는데, 불안등과같은정신적인증상이심할수 록, 유병기간이길수록잘나타난다. 1 발생하는기전은궤양성대장염의경우급성활동기에 는염증에의해신경이손상되고만성기에는염증이심할 수록 substance P 가함유된점막신경이증가한다. 81 크론씨 병에서는염증이심한부분은신경분포가변형이생기고 심지어염증이없는부분에서도신경화학적반응에변화 가일어난다. 이러한변화는신경수용체의말단에변화를 주어장관의감각능을증가시켜과민성장증후군과같은 증상을일으킨다. 82

6 94 대한소화관운동학회지 : 제11권 2 호, Chagas 씨병 감염과관련되어가장대표적인식도운동질환은 Chagas 씨병이다. Chagas씨병은 Trypanosoma cruzi의감염으로생 기는전신질환인데식도에서는연하곤란이제일흔한증상 이다. 무편모충체(amastigote) 가심장이나골근육계와위장 관의근육에침범하는질환이다. T 림프구가위장관신경, 특히대장과식도의신경에침윤되어신경손상을일으켜 장관의운동성을떨어뜨리고장관을확장시킨다. 83 식도운동이상이발생하는기전은 Chagas씨병에의해 생긴 Ig G가 M2 아세틸콜린수용체에결합하여식도근 육의수축을조장하거나, 84 ganglionic acetylcholine 수용체의 자가항체에의해자가면역성신경염을일으켜발생시킨 다. 식도내압검사소견은하부식도괄약근이완부전, 무연 동운동, 다중정점수축등이나타난다. 85 요 최근수년간기능성위장관질환의대한병태생리에대 한연구에많은발전이있었다. 그중제일주목받는것이 염증과관련된장관기능의변화에대한것이다. 염증은급 성감염이사라진뒤에도장기간장관점막에존재해서소 화관기능을변화시킨다. 장관신경의손상, 통각신경경로 의활성화, 그리고점막의분비나흡수이상등이대표적 인기전이다. 과거에비해병태생리가많이알려져있다고 는하나확실하지않으며치료의표준지침이없어실제 치료시의사를곤혹스럽게만드는것이사실이다. 향후병 태생리에대한전향적대단위연구와함께, 항염증제등의 약 치료약제및치료지침의개발이필요하다. 참고문헌 1.
