Wash slides in distilled water for 10min.. Place slides in 0.0ml of pre-warmed 0% silver nitrate solution for 1min in a 7 oven.. Pour silver nitrate solution into clean container and save.. A), 6개월이상 경과 Strich는 심한 두부외상후 초래된 치매환자들에서 대뇌 후 사망했던 례에서는 축삭종창을 관찰하기 어려웠다. 이 백질의 미만성 변성을 처음으로 기술하였고19), 그 후 전단 상 관찰된 축삭종창은 손상의 호발부위인 대뇌의 백질부, 뇌 손상16), 급성충격에 의한 미만성 손상6), 미만성 백질 전단 량 및 뇌간부 등에서 특히 잘 관찰되었으며, 그외에 해마, 소 손상0)이라는 용어가 사용되기도 하였다. 이러한 병변은 외 뇌, 내섬유막, 뇌실주위의 백질부에서도 관찰되었다. 력에 의한 두부 충격시에 신경섬유가 비틀림을 받아 축삭손 한편 이번 증례의 상당수에서는 미만성 축삭손상 이외에도 상이 발생하기 때문에 초래된다고 하였으며19), 어떤 학자들 대뇌 부종, 뇌좌상, 지주막하 또는 뇌실질내 출혈, 소뇌 천막 은 이 병변이 저산소성 손상이나 외상성부종 또는 소뇌천막 을 통한 구상돌기의 탈출, 뇌간부 출혈, 저산소성 허혈성 손 을 통한 탈출현상으로 인해 뇌간부가 압박을 받음에 의해 상, 운동신경로의 변성, 척수 손상 등이 관찰되었다. 대뇌부 이차적으로 나타난다고 하였다1). Gennarelli등10)11)은 뇌압 종은 LBPAS 염색상 쉽게 관찰할 수 있었으며, 또한 사고 상승이나 저산소증을 유발시키지 않은 상태에서 두부에 가 후 6개월 이상 생존하였던 례에서는 뇌량, 뇌간부 등의 신 속력을 가했을 때 일차적으로 미만성 축삭손상이 초래됨을 경로에서 다수의 유전분체(amyloid body)가 관찰됨으로써 실험적으로 증명하였다. 이번 연구에서 손상 후 1시간에 (ig. B) 이 부위에서 퇴행성 변화가 진행되었음을 알 수 사망한 증례에서 신경세사단백질에 대한 면역조직화학적 염 색상 염주목걸이 처럼 종창된 소견은 두부충격시 축삭돌기 가 여러부위에서 비틀려져 축삭손상이 초래됨을 뒷받침하는 소견이라고 생각된다. Adams 등7)은 신경병리학적 검색 결과 미만성 축삭손상 의 특징적인 병변을 뇌량과 문측 뇌간부의 후측방에 국소적 손상과 미만성 축삭손상의 두가지로 요약하였다. 그 후 보 다 많은 증례들에 대한 임상적 및 신경병리학적 연구를 통 하여 미만성 축삭손상에 대한 등급분류를 시도하였으며, 그 결과 1등급은 대뇌백질, 뇌량, 뇌간부, 드물게는 소뇌의 축 삭손상에 대한 병리조직학적 소견이 관찰되는 경우, 등급 은 뇌량에 국소성 병변이 존재하는 경우, 등급은 뇌량과 문측 뇌간부의 국소성 병변이 존재하는 경우라고 하였다9). 이상과 같은 연구보고들은 폐쇄성 두부손상에 의한 미만성 축삭손상을 진단하는데 있어서 임상적 소견과 더불어 병리 조직학적 검색이 필수적임을 시사하고 있다. 이번 연구는 두부외상에 의한 미만성 축삭손상을 분석하 기 위하여 부검 후 각종 신경병리학적 검색을 시행하여 진 단이 확정된 17예를 대상으로 하여 후향적으로 임상적 소 견과 비교하였다. 모든 증례에서 신경병리학적 검색을 통하 여 미만성 축삭손상을 관찰할 수 있었는데, 이것은 두부외 상과 관련되어 부검이 시행되었던 예의 8.7%에 해당하였다. ig..
