Healthrelated quality of life (HRQoL) has been identified as a factor in various medical outcomes including death, and has become an important consideration in health care. We aimed to analyze the association between sarcopenia and HRQoL in Korean adults older than 19 years. Methods: We included 2,300 men and 2,849 women who participated in the Korea National Health and Nutrition Examination Survey from 2010 to Sarcopenia was defined as appendicular skeletal muscle mass divided by weight (%) below 2 standard deviations of the sex-specific mean for young adults. The cutoff point for sarcopenia was 29.1% in men and 23.0% in women. HRQoL was measured with the Euro quality of life-5 dimension (EQ-5D) instrument. Logistic regression analysis was performed to evaluate the relationship between sarcopenia and HRQoL. Results: The prevalence of sarcopenia was 14.5% in men and 19.7% in women. The EQ-5D index score was significantly lower and the rate of difficulty with individual components of HRQoL was higher in the sarcopenic group. After adjustment for age, body mass index, lumbar spine BMD, chronic disease, smoking, alcohol, regular exercise, and education, the odds ratios for problems with mobility were 1.62 in men and 1.31 in women; however, the difference was not statistically significant for women. Conclusion: Sarcopenia was associated with poor quality of life, especially with regard to mobility and depression/anxiety. Greater consideration of sarcopenia is needed at a younger age to prevent and manage poor quality of life at an older age. Keywords: Sarcopenia; Euro Quality of Life-5 Dimension; Health-Related Quality of Life 서론 경우로 정의하였고, 이차성 근감소증은 노화 이외의 다른 원인(좌식 생활에 의한 신체활동의 감소, 영양 불균형, 암, 심뇌혈관질환 등의 근감소증(sarcopenia)은 신체 위약의 대표적인 특징으로서 근육 1) 중증 질환)에 의해 발생한 경우로 정의하였다. 이러한 근력의 저하는 의 양과 근력의 감소로 정의 되는 개념으로 다양한 원인에 의해 발 당뇨, 심혈관질환 등 만성 질환자에서 더 크게 발생3,4)하고, 근감소증 생한다. 2 오정민외. 한국성인의근감소증과건강관련삶의질과의연관성 을중시하는방향으로나아가고있는데, 건강관련삶의질 (health related quality of life, HRQoL) 이사망이나재입원같은다양한의료결과에영향을미치는독립요인으로알려지면서 6,7) 보건의료중재의중요한고려사항이되고있다. 근감소증은보통노년층에서논의되고있는문제이지만, sedentary lifestyle에익숙한청장년층에서도이차성으로발생할수있는건강문제로, 본연구에서는우리나라만19세이상의전체성인인구를대상으로하여남성과여성을나누어각각에서근감소증과건강관련삶의질의관계를알아보고, 각연령대별로근감소증과건강관련삶의질과의연관성에어떤차이가있는지를파악하고자한다. 방법 scale (EQ-VAS), 그리고우리나라평가기준을적용하여가중치를계산한 EQ-5D index 11,12) 로평가하였다. EQ-5D는 운동능력, 자가관리, 일상활동, 통증 / 불편, 불안 / 우울 로각 5개차원에대하여 문제없음, 다소문제있음, 심각한문제있음 의 3가지수준으로평가 13) 하고, EQ-VAS는오늘자신의전반적인건강수준을최저건강상태인 0점부터최고의건강상태인 100점까지로 20 cm 수직자막대에표시하여나타냈다. EQ-5D index는완전한건강상태 1부터가장낮은점수 0.171까지의가중지표값으로계산한다. 본연구에서는, EQ-5D 5개차원의 3가지응답을다시 2가지로분류 ( 문제없음, 문제있음 ) 하여문제를호소한분율과 EQ-VAS, EQ-5D index의평균과표준편차를제시하였다. 1. 연구대상본연구는제5기 1, 2차년도국민건강영양조사 ( 년 ) 에참여한만19세이상의대한민국성인남녀를대상으로하였다. 전체 17,476 명중신체계측및 dual energy X-ray absorptiometry (DEXA) 미시행자, EuroQoL 5 dimension (EQ-5D) 설문항목무응답자, 삶의질에영향을줄수있는중증질환 ( 악성종양및신부전, 뇌혈관질환, 심장혈관질환, 간경변증 ) 을가진경우를제외하고, 근감소증에영향을줄수있는 8) 비만도, 흡연여부, 교육수준, 규칙적운동에대한정보를얻을수있는남성 2,300명, 여성 2,849명, 총 5,149명을분석대상으로하였다. 2. 근감소증의정의현재근감소증을정의하는여러기준이제시되고있지만, 표준화된진단기준은없는실정으로, 본연구에서는이중에너지방사선흡수계측기 (DEXA) 로양측상하지근육량 (appendicular skeletal muecle) 을측정하였으며, 제4기국민건강영양조사자료를이용한 Kim 등 9) 의근감소증진단법에따라양측상하지근육량을체중으로나눈후 100을곱한값이젊은성별기준집단평균값의 2 표준편차미만인값을근감소증으로정의하였고, 절단값 (cutoff value) 은남성 29.1%, 여성 23.0% 를적용하였다. 3. 건강관련삶의질측정건강관련삶의질 (HRQoL) 을측정하는도구로 EQ-5D 10) 를사용하였다. EQ-5D는 EuroQol 그룹에서개발한선호도기반 HRQoL 측정도구로써현재의건강상태를묻는 5개의문항으로구성된 EQ-5D와온도계형식의시각아날로그척도인 Euro quality of life-visual analogue 4. 국민건강영양조사자료활용방법제5기 1, 2차년도국민건강영양조사 ( 년 ) 원시자료중건강면접조사와보건의식행태조사자료에포함된나이, 비만도, 운동, 흡연, 음주, 소득수준을이용하였고, 골밀도체지방검사를이용하였다. 음주는한달에 1회이상술을마시는경우로, 비음주는한달에 1 회미만으로마시는경우로구분하였으며, 1989년세계보건기구 (World Health Organization) 가개발한 alcohol use disorder identification test (AUDIT) 13) 10문항자가보고용설문을시행해 10개항목점수총합이 12점이상을문제음주군, 12점미만을정상음주군으로분류하였다. 흡연력은설문응답에따라과거흡연, 현재흡연, 비흡연자로분류하였으며, 운동행태는 international physical activity questionnaire 를참고하여, 격렬한신체활동을 1회 20분이상, 주 3회이상실천하거나중등도신체활동을 1회 30분이상, 주 5회이상실천한경우에규칙적운동을한것으로정의하였다. 교육수준은교육을받은기간에따라 9년이하, 10 15년, 16년이상의그룹으로분류하였다. 신체계측은이동검진차량에서직접계측을하였으며, 신장과체중은각각 0.1 cm과 0.1 kg까지측정하였고, 체질량지수는 ( 체중 [kg]/ 신장의제곱 [m 2 ]) 식을이용하여계산하였다. 5. 통계분석통계분석은질병관리본부의국민건강영양조사원시자료이용지침에따라가중치를사용하여 survey 분석을시행하였다. 근육량은성별에따라차이가있어남녀간절대적비교가어려워본연구는근감소증과건강관련삶의질의관계를성별을나누어각각분석하였다. 각성별에따른연구대상자들의일반적특성을연속변수는독립표본 t-검정을이용하여평균 ± 표준오차로제시하였고, 범주변 871

Association between Sarcopenia and HRQoL in Korean Adults 수는카이제곱검정을이용하여빈도 ( 백분율 ) 로제시하였다. 각각남 성과여성의연령그룹별근감소증과 HRQoL 과의연관성을파악하 기위해독립표본 t- 검정과일원분산분석을이용해평균 ± 표준오차 로제시하였으며, 근감소증과 EQ-5D 의연관성을확인하기위해로 지스틱회귀분석을이용하여분석하였고, 연령, 체질량지수, 요추골 밀도를보정한모형 (model 1), model 1 에만성질환현재유병유무 ( 고 혈압, 이상지질혈증, 관절염, 갑상선질환, 천식, 폐결핵, 아토피피부염 ) 을보정한모형 (model 2), model 2 에흡연력, 음주력, 규칙적운동여 부, 교육수준을보정한모형 (model 3) 을이용하여유의성을확인하 였다. 분석결과에는교차비와 95% 신뢰구간을제시하였다. 통계분석은 Table 1. IBM SPSS statistics ver 22.0 (IBM Co., Armonk, NY, USA) 을이용하여 시행하였고, 통계적유의수준은 P-value<0.05 로하였다. 결과 1. 연구대상자의일반적인특성 Table 1 에서연구대상자들의일반적특성을성별로나누어표기 하였다. 남성 2,300 명, 여성 2,849 명으로, 남성은평균연령은 49.02± 세, 사지근육량 22.06±3.365 kg, 근감소증유병률은 14.5% 였으며, EQ-5D 에서통증 / 불편문제호소비율이 17.4% 로가장높았 고, 그다음으로운동능력의문제호소비율이 9.8% 로나타났다. 여성 의평균연령은 45.49± 세, 사지근육량 14.36±2.144 kg, 근감소증 유병률은각각 19.7%, EQ-5D 중통증 / 불편문제호소비율이 24.7% 로 가장높고, 그다음으로운동능력의문제가 12.8% 로나타났다. 4 오정민외. 한국성인의근감소증과건강관련삶의질과의연관성 2. 연령증가에따른평균 SMI와근감소증유병률 Table 2는남, 여각각에서연령그룹별 skeletal muscle index (SMI) 평균값과표준편차와근감소증유병률을나타낸결과로, 남, 여모두에서연령이증가함에따라 SMI 평균값은유의하게감소하였고, 근감소증유병률은유의하게증가하였다 (P<0.05). 에서도연령이증가함에따라 EQ-5D index와 EQ-VAS가감소하였고, 통계적으로도유의하였다. 결론적으로남성, 여성모두에서근감소증그룹에서정상그룹에비해건강관련삶의질지표가낮았고, 연령이증가함에따라건강관련삶의질만족도가낮게나타났다 (P<0.05). 3. 연령증가에따른근감소증과건강관련삶의질지표와의연관성 Table 3은남, 여각각의정상그룹과근감소증그룹에서의연령별 EQ-5D index와 EQ-VAS의 t-test 결과로평균과표준편차를나타냈다. 남성의경우정상그룹에서는연령이높아질수록 EQ-5D index와 EQ-VAS 값이감소하였고, 통계적으로도유의하였다. 근감소증그룹에서는연령이증가함에따라 EQ-5D index가유의하게감소하였으나, EQ-VAS는통계적유의성이없었다. 여성의경우, 정상인그룹에서는연령이높아질수록 EQ-5D index 와 EQ-VAS가감소하였고통계적으로유의하였으며, 근감소증그룹 4. 근감소증과 EQ-5D와의관계 Table 4는성별에따른근감소증과 EQ-5D의연관성에관한로지스틱회귀분석결과이다. 남자의경우근감소증이있는사람이정상인과비교했을때운동능력의문제발생위험이 1.62배증가하였다 (odds ratio [OR], 1.62; 95% confidence interval [CI], ). 여성에서는 model 1에서만근감소증그룹에서정상그룹에비해운동능력문제발생위험이 1.50배증가하였고 (OR, 1.50; 95% CI, ), model 1, 2에서근감소증그룹에서불안 / 우울의문제발생위험이각각 1.40배 (OR, 1.40; 95% CI, ), 1.37배 (OR, 1.37; 95% CI, ) 증가하였다. Association between Sarcopenia and HRQoL in Korean Adults 5. 연령그룹별근감소증과 EQ-5D와의연관성 Table 5는연령그룹별근감소증에따른 EQ-5D 발생과의연관성을보기위한로지스틱회귀분석결과이다. 남성에서는 20 30대그룹에서근감소증일때 운동능력, 통증 / 불편, 불안 / 우울 의문제발생위험도가각각 OR, 7.49 (95% CI, ), 2.26 (95% CI, ), 3.63 (95% CI, ) 로유의하게높았으며, 40 50대그룹에서는근감소증일때 운동능력 문제발생위험도가 OR, 2.62 (95% CI, ) 로유의하게높았고, 60대이상에서는근감소증과 EQ-5D의통계적으로유의한연관성은없었다 (P<0.05). 여성에서는 20-30대그룹에서근감소증일때 운동능력 의문제발생 OR, 3.68 (95% CI, ), 40 50대그룹에서근감소증일때 불안 / 우울 의문제발생 OR, 1.68 (95% CI, ) 로유의하게높았으며, 60대이상에서는근감소증과 EQ-5D는통계적으로유의한연관성은없었다 (P<0.05). 고찰근감소증은 1988년에 Rosenberg 14) 에의해처음도입된개념으로, 최초 Baumgartner 등 15) 이신장으로보정한근감소증진단기준을제시한이후다양한연구가이루어지고있는데현재근감소증유병률은진단기준과성별에따라매우다른결과를나타낸다. 가장많이사용되는 Baumgartner 등 15) 이제시한진단기준은절대적인근육량을비교한것으로, 체지방이많은사람이근육량도많다는것을반영하지못해체지방량이많을수록근감소증으로분류될가능성이낮아진다는단점이있다. 15,16) 따라서남성보다상대적으로체지방량이더많은여성에게적용하기에제한이있어이런단점을극복한 Janssen 17) 의진단법을이용한 Kim 등 9) 에의해제시된체중으로보정한진단기준을사용하였다. 최근에는근감소증의진단기준으로신장과체중두가지모두를반영하는 body mass index로보정하는진단기준 18) 에대한연구가이루어지고있으며, 이진단기준에따른근감소증은어느한가지로보정했을때보다신체기능의감소와더큰상관관계를보였다는연구결과가있었다. 한국인을대상으로한연구 18,19) 도있으나남성, 여성모두를포함한연구와대표성을띤절단점을제시한연구는없어앞으로도지속적인연구가필요할것으로보인다. 이전 Go 등 20) 의신장으로보정한진단기준을적용한한국남성에서골밀도와근감소증과건강관련삶의질의연관성을본연구에서는근감소증이동반될수록 운동능력, 자가관리, 일상활동, 불안 / 우울 항목에서문제가발생할위험도가더높다는결과를밝혔는 데, 체중보정진단기준을사용한본연구에서는남자에서근감소증이동반될수록 운동능력 문제발생위험도만높게나타났다. 여러진단기준을비교한 Kim 등 19) 의연구에따르면신장보정진단기준은신체적장애혹은신체적허약과연관성이크며, 15,21) 체중보정진단기준은기능적손상과장애와더큰연관성 9,17,22) 이있다고밝히고있어이러한연구결과의차이는적용한진단기준의차이에서발생한것으로판단된다. 본연구에서는남성과여성에서근감소증이건강관련삶의질에미치는영향에차이가있었는데, 남성의경우신체적기능측면 ( 운동능력 ) 에영향을미쳤지만, 여성의경우신체적기능 ( 운동능력 ) 뿐아니라정신건강 ( 불안 / 우울 ) 측면에도영향을미쳤다. 특히, 불안 / 우울 영역은 model 3에서유의하지않게나온것으로미루어여성에서의불안 / 우울감은근감소증보다다른생활요인들 ( 흡연, 음주, 규칙적운동, 교육수준 ) 이더큰영향을미치는것으로판단된다 (P<0.05). 또한, 연령대별로근감소증과 EQ-5D의관계가다르게나타났는데, 20 30대남성에서근감소증이동반되어있을때정상인에비해 운동능력, 통증 / 불편, 불안 / 우울 문제발생위험도가높았고, 여성에서는 운동능력 문제발생위험도가높았는데, 가장활발하게사회생활을해야하는청장년층의근감소증은신체적영역뿐아니라정신건강영역까지영향을미치는것으로나타났다. 국민건강영양조사자료의한계로인해 20 30대의근감소증이어떠한원인에의해발생했는지를파악할수는없었으나, 근감소증은노년층뿐아니라젊은성인에게도건강관련삶의질에영향을미칠수있다는점을고려하여청장년시기부터장기적인의료적개입이필요할지에대해서는더연구가필요할것으로생각된다. 또한, 고령일수록근감소증과 HRQoL의관계가밀접할것이라고예상했던것과는달리남녀모두 60세이상군에서근감소증과 HRQoL 사이에통계적유의성이없었다. 그원인으로는근감소증단일요인보다는노화에따라동반되는만성질환들 (model 2-chronic disease) 과개개인의통증관리능력 (model 3-lifestyle 및 socioeconomic factor) 이 HRQoL에더큰영향을미치기때문으로판단된다. 이번연구에는몇가지제한점을가지고있다. 첫째, 이번연구는단면연구이기때문에근감소증과건강관련삶의질의시간적선후관계를알수없어인과관계를밝히기어려우며, 청장년층의근감소증이노년기에영향을미치는지에대한선후관계를알기어렵다. 둘째, 근감소증을진단하는기준이표준화되어있지않아각진단방법에따른연구의결과를직접적으로비교하기엔한계가있었으며, 연구에적용하는진단기준에따라결과가달라질수있다는한계가있다

6 오정민외. 한국성인의근감소증과건강관련삶의질과의연관성 셋째, 국민건강영양조사자료의부족으로이차성근감소증의원인과신체적기능에영향을줄수있는유년기와학동기시절의건강상태와신체활동, 골절사고등에대한정보를확인할수없었다. 그럼에도불구하고본연구는다음과같은장점이있다. 첫째, 우리나라인구전체의대표성을가진국민건강영양조사자료를이용하여혼란변수를보정한후근감소증과건강관련삶의질의연관성을분석하였다는점이다. 둘째, 근감소증과관련된위험인자의자료를모아 model 1에서 model 3까지보정을하여분석을하였고, 나아가연령집단별로근감소증이건강관련삶의질에미치는영향까지분석했다는점이다. 결론적으로, 이연구는대한민국만19세이상의성인에서근감소증은건강관련삶의질에영향을미치며, 남성의경우신체기능장애를유발하고, 여성의경우신체기능적측면뿐아니라정신건강측면까지도영향을미친다는것을확인하였다. 이번연구결과를토대로근감소증과건강관련삶의질의인과관계를밝히는것뿐아니라더나아가청장년층의이차성근감소증이노년기근감소증과건강관련삶의질에미치는영향에대한전향적연구가필요할것으로판단된다. 요약연구배경 : 본연구는우리나라성인인구를대상으로근감소증과건강관련삶의질의관계에대한연구를시행하였다. 방법 : 제5기국민건강영양조사에참여한만19세이상성인남녀를대상으로 DEXA로측정한양사지근육량의합을체중으로보정한값으로근감소증을정의하였고, 절단점은남 29.1%, 여 23.0% 로근감소증을진단하였다. 건강관련삶의질평가는 EQ-5D를사용했으며, 현재의건강상태를묻는 EQ-5D, EQ-VAS, 그리고우리나라평가기준을적용한 EQ-5D index를평가하였다. 근감소증에따른건강관련삶의질의연관성을파악하기위해 chi-square, analysis of variance, t-test, logistic regression analysis 를이용하였다. 결과 : 남성과여성각각근감소증유병률은 14.5%, 19.7% 였으며, 남녀모두에서연령이증가함에따라근감소증유병률은유의하게증가하였고, EQ-5D index는감소하였다. 근감소증과 EQ-5D의로지스틱회귀분석에서남성은근감소증그룹에서 운동능력 OR, 1.62 (95% CI, ) 였고, 여성에서는 운동능력 OR, 1.31 (95% CI, ), 불안 / 우울 OR, 1.33 (95% CI, ) 였으나, 통계적유의성은없었다. 남성의경우, 20-30대그룹에서근감소증일때 운동능력, 통증 / 불편, 불안 / 우울 OR가각각 7.49, 2.26, 3.63였고, 40 50대그룹에서는근감소증일때 운동능력 OR는 2.62였다. 여성의경우 대그룹에서근감소증일때 운동능력 OR 는 3.68, 대그룹에서 는 불안 / 우울 OR 는 1.68 였다. 결론 : 본연구를통해우리나라만 19 세이상의성인남녀에서근감 소증은건강관련삶의질과관계있으며, 남성에서는신체적기능장 애, 여성에서는신체적기능장애와정신적측면에영향을미친다는 것을확인하였다. 중심단어 : 근감소증 ; EuroQoL-5 Dimension; 건강관련삶의질 REFERENCES 1.
