Serum GGT levels are associated with oxidative stress, and oxidative stress is associated with physical activity intensity and duration. We studied the relationship between exercise and GGT levels in Korean adults. Methods: Data from the fifth Korean National Health and Nutrition Examination Survey ( ) were used to analyze 10,159 women and men. Subjects were divided into four groups based on the quartile of GGT. Multiple logistic regression analysis was performed after adjusting for age, sex, body mass index, alcohol drinking, income, education, occupation, living alone and presence of diabetes mellitus, hypertension, or dyslipidemia in order to determine the association between physical activity and serum GGT levels. Results: Walking was significantly associated with serum GGT levels in men. We found a significant association between walking time per week and low GGT levels in men (P for trend ). However, there was no statistically significant relationship between other exercise types and GGT levels, nor was there a statistically significant relationship between exercise type and GGT levels in women. After adjusting for confounders, the odds ratio (95% confidence interval) for serum GGT levels compared with the non-walking group in men was ( ). Conclusion: Walking is significantly associated with serum GGT levels in Korean men. 2 손효재외. 운동과혈중감마지티피와의관계 승과연관되는것이알려져있다. 이에본연구는운동을할수록산소성손상지표인 GGT가상승하고, 운동을하지않을수록 GGT가감소할것이라는가설하에진행하고자한다. 국내운동과혈중 GGT의관계를직접적으로본연구는없으며 2차지표로분석한연구가존재한다. 그러나운동횟수및시간만고려하고강도를고려하지않은연구, 18) 특정지역만분석한경우 19) 로한계점이존재하여본연구에서는운동의강도와질을고려하여운동과혈중 GGT의관련성에대해분석하고자한다. 방법 1. 연구대상및자료수집본연구는국민건강영양조사의제5기 (1차 2010년, 2차 2011년 ) 의응답자를대상으로분석하였다. 건강설문조사의교육및의료이용, 경제활동, 질병이환항목은면접을통하여조사되었고, 건강행태영역은자기기입식으로실시되었으며, 검진조사는신체조사자의직접계측으로이루어졌다. 제5기 1차및 2차의총응답자는총 17,476 명으로, 20세미만 4,170 명을제외한 19세이상의성인 13,306명을연구대상자로삼았다. 그중간질환설문중 B형간염, C형간염, 간경변증설문중 (1) 지금까지앓은적있음, (2) 의사에게진단을받았음, (3) 현재앓고있음, (4) 현재치료여부설문중어느것이라도 Y에체크한자를 GGT에영향을주는간질환자로정의하여이상 234명을제외하였고, 음주설문중 1회평균음주량설문에서 1회평균음주량이남 7잔, 여 5잔이상인동시에 (and) 음주빈도설문주 2회이상인경우 1,241명을고위험음주자로간주하여제외하였다. 다음으로독립변수 ( 운동관련설문 ) 에대한응답에결측인사람 1,066명및종속변수 ( 혈중 GGT) 가결측인경우 606명을제외하여최종적으로성인남녀 10,159 명을분석하였다. 2. 연구방법및내용 1) 설문조사 (1) 운동력대상자들은국민건강영양조사설문지에따라자기기입식으로경도, 중등도, 격렬한운동상태에대한설문지를작성하였는데, 격렬한운동은 최근 1주일간평소보다몸이매우힘들거나숨이많이가쁜격렬한신체활동을 1회 20분이상, 주 3일이상실천한분율, 중등도운동은 최근 1주일간평소보다몸이조금힘들거나숨이약간가쁜중등도신체활동을 1회 30분이상, 주 3일이상실천한분율, 걷기운동은 최근 1주일간걷기를 1회 30분이상, 주 5일이상실천한분율 로나타내었다. (2) 사회경제적요인및생활습관사회경제적요인으로는나이, 교육기간 (6년이하, 7 9년, 10 12년, 13년이상 ), 근로여부, 결혼상태 ( 결혼, 별거 / 사별 / 이혼, 독신 ), 보험상태 ( 의료보험, 의료보호 ), 월수입 (100 만원미만, 100만원이상 300만원미만, 300만원이상 ) 을고려하였다. 생활습관으로는음주, 흡연, 하루총섭취열량을고려하였다. 음주의경우하루섭취하는알코올의평균량을기준으로하여상중하 3그룹으로나누었다 ( 하루섭취하는알코올의평균량 = 한번에음주하는잔수 주당횟수 /7). 음주관련설문의 1회평균음주량및음주빈도에대한응답자료를이용하였고, 음주설문중 1회평균음주량설문에서 1회평균음주량이남 7잔, 여 5잔이상인동시에 (and) 음주빈도설문주 2회이상인경우 1,241명을고위험음주자로간주하여제외하였다. 하루섭취열량은 24시간회상으로실시한설문의응답을기준으로상중하 3그룹으로나누어계산하였다. (3) 동반질환이환력비만의경우체질량지수 (body mass index, BMI) 25 kg/m 2 이상 (World Health Organization에서제시한아시안 -태평양비만기준 [BMI 25.0 kg/m 2 ]) 이거나복부비만 ( 남자 90 cm, 여자 85 cm), 당뇨의경우현재약을복용하고있거나인슐린주사요법을시행중이라고답한사람들및 fasting blood sugar 126 mg/dl 이상또는당화혈색소 6.5% 이상으로분류하였고, 고혈압, 고지혈증약을복용하고있다고답한사람들또는 systolic blood pressure 140 mm Hg 이상 or diastolic blood pressure 90 mm Hg 이상, low density lipoprotein cholesterol 130 mg/dl 이상, total cholesterol 150 mg/dl 이상인군을질환자로분류하였다. 2) GGT 측정국민건강영양조사의임상검사분석방법은 GGT를 Hitachi 7600 (Hitachi, Tokyo, Japan) 장비를사용하여 Pureauto S GGT (Sekisui, Tokyo, Japan) 시약을사용하여효소법 (G5CMP) 으로분석하였다. 단위는 IU/L로표기하였다. 3) 통계분석수집된자료의통계적분석은 SAS ver. 9.3 (SAS Institute Inc., Cary, NC, USA) 을이용하였으며, 성별에따라 GGT 4분위를나눠서하위 1 분위와나머지를비교하였다. 연속형변수에대해서는평균 ± 표준오차, 범주형변수에대해서는 % ( 표준오차 ) 로제시하였으며, 각각 t-test 및카이제곱검정을하였다. 정규분포를따르지않은변수에대해서는로그변환을실시하고기하평균 (95% 신뢰구간 ) 으로제시하였다. GGT와운동의연관성을다중로지스틱회귀분석을이용하여 odds ratio (95% 신뢰구간 ) 으로제시하였으며, 복합표본설계를고려하여 SAS Survey Procedure (SAS Institute Inc.) 를이용하였다. 별도의가중치분석을하지않았으며, 통계적유의수준은 P값이 0.05 미만일때유의한것으로하였고모든통계적유의수준은 0.05로정의하였다. 결과연구대상자들은 2010년과 2011년각 8,958명, 8,518명으로합계 17,476 명이었으며, 19세미만 4,170 명, 운동설문결측 1,066명및간질환자 234명, 고위험음주자 1,241명및 GGT 결측 606명을제외한총 10,159 명을분석하였다. 연구대상자들의 GGT 평균은기하평균으로남평균 30.6 ( ), 여평균 17.3 ( ) 이었다. GGT가높은순서대로사분위로 Q1, 2, 3, 4 네개의군으로나누었는데, 각군의기준은남자에서하위군부터 20, 28, 44이고, 여자에서는하위군부터 13, 16, 22였다. Q1군의숫자는남녀각 964, 1,549명, Q2군의숫자는남녀각 987, 1,396명, Q3군의숫자는남녀각 975, 1,800명, Q4군의숫자는남녀각 970, 1,518 명이었다. 연구대상자들을 GGT에따라사분위 4그룹으로나누고, GGT가제일낮은 Q4군과다른 Q1, 2, 3군의기본특성을비교하였다. 두군의현재흡연분율, 고위험음주분율, 고졸이하분율, 동거분율, 직업이있는사람의분율, 가구소득하위 25% 인사람의분율, 고혈압유병자의분율, 고지혈증유병자의분율, 당뇨유병자의분율, 나이, BMI, 허리둘레, alanine aminotransferase 및 aspartate aminotransferase Table 2. Model 1에서는 age, 성별, BMI, 음주수준을보정하였고, model 2에서는 age, 성별, BMI, 음주수준에더하여소득정도, 교육정도, 직업유무및동거여부를보정하였고, model 3에서는 age, 성별, BMI, 음주수준, 소득정도, 교육정도, 직업유무, 동거여부에더하여당뇨, 고혈압, 고지혈증의만성질환을보정하였다. 다중로지스틱회귀분석을통해 95% 신뢰구간에서 OR을구하였을때남자에서걷기운동을하면 GGT 하위군으로변화할 OR이 model 1에서 1.341, model 2에서 1.327, model 3에서 로확인되었다 (Table 4). 고찰혈청 GGT는간세포손상에대한생체지표로널리이용되어왔으며, 최근에는산소성손상을나타내는생체지표로서의미가있는것으로알려졌다. 산소성손상은유리산소기 (free radical) 의발생을증가시켜이를억제하는 glutathione의고갈을일으키고 glutathione의평형유지에관여하는 GGT의상승을유발한다고알려지고있다. 즉산소성손상에의해 GGT의상승이일어나며, GGT는산소성손상을나타내는하나의생체지표 (biologic marker) 로볼수있는것이다. 산소성손상을유발하는인자들은운동부족, 비만, 흡연등이있다. 이와같이운동부족, 비만, 흡연등과산소성손상이관련이있어산소성손상지표인 GGT와각종심혈관계위험요인과의관련성이밝혀지고있다. 본연구에서는심혈관계위험요인인 GGT와운동과의관련성을분석하고자한국성인남녀의 GGT와운동수준을비교하였다. 그결과남자에서걷기운동그룹에서운동정도가많을수록 GGT가유의하게감소하는것을확인할수있었다 ( 중등도운동그룹, P=0.036; 걷기운동그룹 P<0.001). 본연구결과기존에알려진음주량의지표, 담도폐쇄의증거, BMI 의증가지표로서의 GGT의효용성이외에산소성손상을반영하는생체지표로서이용될수있음을알수있다. 음주, 흡연, 운동부족, BMI 등은산소성손상을증가시키는요인으로알려져있으며, 이러한요인들과 GGT와의연관성에대해서는과거의연구들에서잘나타난다. 즉 GGT는여러요인들에의해서발생되는산소성손상의지표로사용할수있는것이다. GGT는서론에서언급하였듯이고혈압, 당뇨, 비만등각종성인병과연관이있고, 관상동맥질환 20-22) 등의질병과도산소성손상과관련하여연관성을보이기때문에이들질병을예견할수있는요소가될수있다. 그러므로운동부족은뇌경색, 관상동맥질환, 고혈압, 당뇨등의질병과도장기적으로는연관이있을것으로볼수있다. 남자에서의걷기이외에격렬한운동및중등도운동과는유의한관계가없는이유로생각해볼수있는점은걷기운동유무와의관련성을본단면연구이기때문에걷기운동외에다른운동을동시에했을경우를고려하지않은점이다. 또한같은걷기운동을하였더라도여자에서는유의한관련성이없는이유로도이와같은점을고려해볼수있겠다. 일반적으로남성의신체활동및운동량이많고, 걷기외에다른운동을할가능성이더높을수있다는점을생각해볼수있겠다. 본연구의제한점으로는첫째, 제5차국민건강영양조사 ( ) 자료만을기본으로운동부족에따른산소성손상표지자로 Korean J Fam Pract. 2016;6(1):

Relationship between Exercise and Gamma-Glutamyltransferase 혈청 GGT를선정했다는점, 둘째, 최근일주일간의운동의양과질의설문에기초한 GGT를분석한단면연구인점, 셋째, 다양한산소성손상에영향을주는요인이있지만, 만성질환등을보정한바있지만운동부족에의한 GGT에초점을맞추어진행했다는점이다. 본연구의가설에서산소성손상은운동량부족에기인하는것으로가정하였으므로정상범위라도 GGT가높은성인남성의경우걷기운동을권장하여이들질병의예방에도움이될것으로생각된다. 그러나이연구자체만으로는앞의여러제한점이있기때문에운동과 GGT가명확한관련성이있다고단정짓기는어렵다. 향후연구대상자에대한지속적추적, 운동량에대한정확한조사및흡연등생활습관에대한조사를보완하여본연구에서나타난남성에서걷기운동의증가에따른 GGT의감소에대한연구를전향적으로수행해나가는것이필요할것이다. 요약연구배경 : 최근연구에서운동부족과혈청감마지티피 (gamma-glutamyltransferase, GGT) 와의관련성이밝혀지고있다. 또한 GGT는심혈관계질환에영향을주는인자로서연구되고있다. 이는산소성손상의기전에의하며, 운동과산소성손상지표인 GGT의관계를알아보고자한다. 방법 : 제5차국민건강영양조사 ( ) 자료를통해 10,159명의건강한성인남녀를분석하였다. 연구대상자는 GGT 사분위에따라 4그룹으로나누었고, 로지스틱회귀분석을통해일반사항및만성질환을보정하여격렬한, 중등도, 걷기운동그룹의분율에따른 GGT 의관계를분석하였다. 결과 : 남자에서혈청 GGT가낮은그룹과걷기운동과의연관성이높음을확인하였다. 이는일반특성, 만성질환, 직업요소등을보정하여도역시남자에서걷기운동을많이할수록혈청 GGT가낮아지는음의상관관계를확인하였다 (P for trend ; odds ratio [OR], 1.334; 95% confidence interval [CI], ), 혼란변수를보정한 mode l에서도남자에서걷기운동유무에따른 OR이 (95% CI, ) (model 3) 로유의한결과를보였다. 결론 : 한국성인남자에서걷기운동을많이할수록유의하게혈청 GGT가감소하는경향을보인다. 중심단어 : 운동 ; gamma-glutamyltransferase; 심혈관계질환 REFERENCES 1.
