1 담석증과 허혈성 뇌졸중의 연관성 중앙보훈병원 가정의학교실 길은선, 김현주, 박영윤, 방인호, 정영진 연구배경: 뇌졸중은 주요 사망 원인의 하나로 전체 사망원인의 3위를 차지하며 향후 인구노령화로 급격한 뇌졸중의 증가가 예상된다. 또한 생활습관 및 식단의 서구화로 담석증의 발생이 증가하여 담석증으로 병원을 찾는 환자가 지속적으로 연평균 6~7% 정도씩 증가하고 있다. 그 연관성에 관하여 아직 논란의 여지가 있어 뇌졸중과 담석증과의 연관성을 알아보고자 이 연구를 시행하게 되었다. 연구방법: 연구대상은 2008년 1월부터 2010년 12월까지 중앙보훈병원을 방문하여 복부초음파 또는 복부전산화단층촬영를 시행한 환자604명의 환자를 대상으로 하였다. 본 연구의 의무기록 검토는 연구자 1인이 시행하였고, 고혈압, 당뇨병, 고지혈증, 비만 (BMI 25),심방세동, 관상동맥질환, 음주력, 흡연력을 의무기록과 혈액검사, 심전도, 심혈관조영술 결과를 조사하였다. 연구결과: 허혈성 뇌졸중의 위험인자 중 연령, 성별, 고혈압, 관상동맥질환이 관련이 있었지만 비만, 당뇨, 고지혈증, 심방세동, 음주력, 흡연력과는 관계가 없었다. (P value<0.05) 단변량 분석에서 담석증과 허혈성 뇌졸중은 Odds ratio 2.120(95% 신뢰구간 )로 연관성이 있었으며 나이, 성별, 흡연력, 고혈압, 당뇨, 심방세동, 관상동맥질환, 고지혈증으로 보정하여 시행한 로지스틱 회귀분석에서도 허혈성 뇌졸중에 대한 담석증의 odds ratio는 (95% 신뢰구간 )로 연관성이 있었다. 결론: 담석증과 허혈성 뇌졸중의 여러가지 공통된 위험인자를 보정하여도 둘 사이에 연관성이 증명된 것으로 보아 담석증과 허혈성 뇌졸중은 유의한 연관성이 있다. keyword : 담석증, 허혈성 뇌졸중

2 The association of vitamin D deficiency and thyroid dysfunction in the Korean population: Korean National Health and Nutrition Examination Survey 인하대학교 의과대학 가정의학교실 김춘영, 이연지, 최지호, 이수연, 이혜영, 정다혜, 최연준 Background Vitamin D deficiency has been associated with several autoimmune diseases, but its association with autoimmune thyroid disease is still unclear. This study aimed to analyze the association between the vitamin D deficiency and the thyroid dysfunction in the Korean population. Method This study was based on the data acquired in the Korean National Health and Nutrition Examination Survey (KNHANES) VI (2013~2014). We excluded subjects with no vitamin D or thyroid function test data, those with pregnancy, those with a history of thyroid disease, thyroid cancer, or chronic renal failure, and those who had thyroid dysfunction but negative anti-thyroid peroxidase antibody (TPOAb). We classified the subjects into TPOAb(+) or TPOAb(-) group, and the TPOAb(+) group was further classified to euthyroid or thyroid dysfunction (subclinical or overt hypothyroid) group. Result

3 A total of 4,265 subjects were enrolled in this study. Vitamin D deficiency (<10ng/mL) was found in 37.6% of males and 62.4% of females. TSH (3.35mIU/L vs. 2.61mIU/L, p=0.005) and the prevalence of TPOAb positivity (8.6% vs. 5.3%, p=0.003) were higher in the vitamin D deficiency group. The results of the logistic regression analysis showed that vitamin D deficiency was significantly associated with an increased risk of thyroid dysfunction. In the TPOAb(+) group, the adjusted odds ratios (OR) for vitamin D deficiency was higher in the thyroid dysfunction group (OR=2.26; 95% CI= ; p=0.037) than in the euthyroid group (OR=1.50; 95% CI= ; p=0.035) after adjustment for sex and age. Conclusion Vitamin D deficiency was significantly associated with the TPOAb positivity and the thyroid dysfunction in Korean population. keyword : vitamin D deficiency, thyroid dysfunction

4 Relationship between obesity and serum 25hydroxyvitamin D (25(OH)D) concentrations in adult male and female of a certain areas. 울산대학교 의과대학 가정의학교실 김선정, 김문찬, 김대영, 김정호, 황혜아 Back ground Vitamin D is known to play an important role not only in calcium homeostasis and bone metabolism but also in diseases other than musculoskeletal disorders such as diabetes mellitus, cardiovascular disease, and immune diseases. In relation to serum vitamin D (25 (OH) D), several cross-sectional studies from abroad have found that vitamin D levels are negatively correlated with body mass index, body fat percentage, and visceral fat. In Korea, however, the results of these studies were inconsistent. Methods The study subjects were composed of 585 adults (380 male and 205 female) over 18 years of age, who visited a health promotion center at a university hospital and checked serum vitamin D levels from March, 2015 to February, They were divided into two groups according to sex, and t-test and chi-square test were used to evaluate baseline characteristics. Correlation coefficients were calculated using Pearson correlation analysis between body mass index, waist circumference, body fat percentage and serum vitamin D level. Finally subjects were classi fied as deficient (<20 ng/ml) based on serum vitamin D level. Bivariate logistic regression analysis was used to determine the odds ratio of serum vita min D defeciency. 5 There was no correlation between obesity and serum vitamin D level in male. In female, there was a significant negative correlation between obesity and serum vitamin D level (p <0.05) but no correlation was observed between waist circumference and serum vitamin D. Significantly obesity was associated with vitamin D deficiency in female but not in male. The details of findings were as below; 25 in BMI(adjusted OR = 4.24; 95% CI = ), 85cm in waist circumference(adjusted OR = 2.21; 95% CI = ), 30% in body fat(adjusted OR = 2.09; 95% CI = ), resptectively. 6 간접흡연의 지식의 영향을 미치는 요인과 간접흡연 지식 정도에 따른 금연효과 양산부산대학교 의과대학 가정의학교실 이상엽, 이새롬 서론 : 간접흡연의 위해성이 많은 논문을 통해 밝혀지면서 금연 구역 확대와 금연 교육이 활발하게 일어나고 있다. 간접흡연에 대한 지식에 영향을 미치는 요인과 간접흡연에 대한 지식과 실제 금연 행동을 알아보기 위한 연구를 시행하였다. 대상 및 방법 : 본 연구는 양산의 한 대학 병원에서 근무하고 있는 직원 총 2166명 대상으로 자가기입 설문지로 회수된 328개의 설문지로 자료 분석을 시행하였다. 설문 내용 총 40문항으로 인구학적 특성, 흡연력과 주된 흡연 장소, 금연 시도 경험과 계획 및 그 이유, 간접 흡연에 대한 내용으로 이루어져 있다. 간접 흡연의 실제 지식에 대한 검사 항목이 8개가 있었으며, 간접흡연에 노출되었을 때 대처 행동 및 공공장소에서의 금연구역 설정하여 담배를 못 피우는 것에 대해 어떻게 생각하는지 물었다. 간접흡연 지식에 대해 영향을 미치는 요인을 구하기 위해서 간접흡연 지식을 묻는 8개 항목의 총 점수와 다른 변수에 대하여 다중회귀분석을 시행하였다. 연구결과 : 전체를 대상으로 하였을 경우에 간접흡연에 노출되었을 때 아무런 증상을 느끼지 못하는 경우(beta=1.287, p-value<0.001)가 간접흡연 지식에 영향을 미치는 것으로 나타났으면 다른 변수들은 영향을 미치지 않았다. 남성을 대상으로 하였을 경우 금연 교육을 통해 간접흡연을 알게 된 경우(beta= 1.227, p-value 0.050), 간접흡연에 노출되었을 때 아무 증상을 느끼지 못하는 경우(beta= , p-value= 0.050)에 간접흡연의 지식에 영향을 미치는 것으로 나타났다. 비흡연자를 대상으로 하였을 경우에는 간접흡연에 노출되었을 때 아무런 증상을 느끼지 못한 경우(beta=-1.353, p-value= 0.004)가 간접흡연의 지식에 영향을 미치고 다른 변수는 영향을 미치지 않았다. 결론 : 이 연구를 통해 간접흡연에 대한 지식을 높이기 위해 금연 교육이 중요함을 알 수 있다.

8 Mental health of South Korean Adolescence Depending on Smartphone Overuse 성균관대학교 의과대학 삼성서울병원 가정의학교실 Hyunjeong Kim, Yun-mi Song, Hyeonyoung Ko, Minkyoung Cho Background: Smartphones are becoming increasingly indispensable in everyday life for most adolescent in South Korea. But, It is reported that excessive smartphone use is associated with physical health symptoms, as well as mental health problems including depression. This study aimed to evaluate the relationship between Smartphone use, depressive symptoms, and suicide attempts among Korean adolescents. Methods: This cross-sectional study included a total of Korean adolescents (26930 male and female) using smartphone who participated in the Korea Youth Risk Behavior Survey in Multiple logistic regression analysis was performed to evaluate the association between smartphone use and depressive symptoms and suicide attempts after adjusting for covariates. Results: A total of 13,850 (25.4%) had depressive symptoms for more than 2 weeks and 1,387 (2.5%) had attempted suicide within the last year. As smartphone usage is increased, high tendencies of depressive symptom and suicide attempt are seen. Compared to participants who are smartphone overuse group(high tertile), lowuse group(low tertile) had greater odds of depressive symptoms (odds ratio [OR], 1.56; 95% confidence interval [CI], ) and suicide attempts (OR, 2.03; 95% CI, )

9 Conclusion: Smartphone overuse is associated with worse mental health problem including depressive symptom, suicide attempt among Korean adolescents. 10 Correlation between Cortisol, DHEA-S, and Testosterone 아주대학교 의과대학 가정의학교실 정상훈, 김광민 Backgrounds: Cortisol, Dehydroepiandosterone sulfate (DHEA-S), and Testosterone are used to predict fatigue, andropause, psychophysiological conditions such as each person ᤀ猀 stress condition. We aimed to determine the association of Testosterone, Dehydroepiandrosterone sulfate (DHEA-S), and Cortisol in Korean men. Methods: We studied data from general medical exam at Ajou hospital. In this study, one thousand and a hundred one men from aged 33 to 88 years old participated. Plasma Total testosterone, DHEA-S and Cortisol were determined to collect at 8:00 AM after overnight fasting. Results: Cortisol and DHEA-S were correlated ( r = 0.130, p 0.001) after adjustmenting for age, BMI, SBP, DBP, FBS, LDL and total cholesterol. However, association between Total testosterone and Cortisol was not significant. In addition, correlation in Total testosterone and DEHA-S was not significatnt, too. Conclusion: These result suggested that only Cortisol and DHEA-S shows association among three hormones. keyword : cortisol, DHEA-S, testosterone, correlation

11 부산과 경남 5개 대학을 대상으로 연구한 대학생의 과체중 및 비만과 가족기능 간의 연관성 동아대학교 의과대학 가정의학교실 문성현, 박영진, 신보경 연구배경 : 비만과 가족기능 간의 연관성에 관한 연구는 많이 진행되지 않았으며, 대부분 소아, 청소년을 대상으로 연구가 이루어졌다. 또한 이러한 연구에서 유 의미한 결론을 도출해내지 못하였고, 일관성이 결여되어 있었다. 이에, 본 연구에서는 대학생을 대상으로 과제중 및 비만과 가족기능 간의 연관성을 평가하였다. 방법 : 2017년 8월과 9월, 2개월 동안 부산과 경남 (진주) 지역 5개 대학을 대상으로 175 명의 대학생에게 olson 의 가족 적응력과 가족 결속력 평가척도 한국어판, 국문으로 번역된 가족기능지수(Family APGAR), 사회경제적 가정 환경, 생활 습관을 묻는 자기기입식 설문조사를 하여, 성실히 응답하였고, 비만과 가족기능에 영향을 미칠 병력이 없는 163 명을 최종 연구대상으로 하였다. 결과 : 연구에 참여한 총 163명의 대상자 중 과체중 및 비만에 해당하는 사람은 54명이었다 과체중 및 비만군과 정상체중군 간의 비교에서 식사량과 흡연여부에서 유 의미한 차이를 보였다. 하지만 가족 적응력과 결속력, 가족기능지수(Family APGAR), KOBES(신체존중감척도) 에서는 두 그룹 간의 통계적으로 유 의미한 차이가 없었다. 결론 : 연구 결과, 가족기능 (가족 적응력과 결속력, 가족기능지수) 과 과체중 및 비만 사이에 통계적으로 유의미한 차이가 없었다. 과체중 및 비만과 가족기능 간의 연관성을 정확하게 알아보기 위해서 더 많은 데이터를 통한 연구가 필요할 것으로 생각된다. 또한 가족기능을 평가하기 위한 도구의 대표성과 정확성에 대해서도 추가적인 연구가 필요할 것으로 생각된다.
