1 간세포암종조기진단의종양표지자 Biomarkers in screening and surveillance of HCC 충북대학교의과대학내과학교실 채희복 Abstract In Korea with a high incidence of hepatocellular carcinoma, surveillance tests with abdominal ultrasonography and alpha-fetoprotein (AFP) are recommended for high risk groups of patients. The optimal interval of surveillance may be 6 months. AFP was discovered earlier than ultrasonography, but its role of hepatoma surveillance is still controversial. Though AFP is useful to monitor therapeutic response and detect recurrence, its role is limited in screening. The diagnostic values of AFP-L3 and PIVKA-II are not superior to that of AFP. Glycoprotein-3 and squamous cell carcinoma antigen are novel and promising tumor markers, but the efficacy of them are not yet proven. Some investigators tried to combine each tests to improve the diagnostic accuracy. Because this trial is accompany by the cost rise, it cannot be adopted for surveillance test. We have not to forget the low sensitivity and positive predictive value of tumor marker when interpreting its meaning in clinical practice. At this time, tumor marker is more useful to confirm the diagnosis and detect recurrence than to screen the patients with cancer risks. Key words: Biomarker; Alpha-fetoprotein; PIVKA-II; Glycoprotein-3; Squamous cell carcinoma antigen 서 론 간세포암종의조기진단에사용되고있는종양표지자 (biomarker, tumor marker) 들로는 α-feto protein(afp), prothrombin in vitamin K absence PIVKA-II, glypican-3, osteopontin 등이나와있으며, 임상에서는 AFP와 PIVKA II가주로이용되고있다. 본고에서는먼저간암에있어서종양표지자의유용성과한계점을개괄하여본뒤, 이어서개별표지자들의특성에관하여자세히알아보고자한다. 종양표지자는혈중 car

2 2010 년대한간학회 Single Topic Symposium cinoembryonic antigen(cea) 치가대장암환자에서상승되어있음이보고된이래로많은종양표지자들이해당종양의치료반응과예후평가에사용되어오고있다. 종양표지자는또한치료전질병의진행상태를알아보고치료후재발위험을예측하고감시하는데에도사용되고있다. 그러나, 종양표지자의암조기검진기능은종양에따라서편차가매우크고, 종양표지자검사건수가증가함에따라실제로환자의생명을연장시키거나생활의질을향상시킨다는증거는아직없다. 따라서, 임상의사는진단과치료에있어서종양표지자가가지는의미를제대로이해할필요가있다. 암의조기발견즉, screening에서그효과를인정받은종양표지자는없다. 단지 AFP만이간염, 간경변을가진아시아환자를대상으로, 그리고초음파검사와병행하였을때에만그효능을인정받고있는실정이다. 간세포암종의조기검진에서 AFP는더이상의미가없으며, 다만영상학적검사에서결절이보일경우간세포암종진단의보조적인의미를가진다고주장하는학자들이있는것을보면큰흐름은조기검진에서초음파의중요성이더욱강조되고있으며, 지금까지이용되고있는것들의한계를뛰어넘는새로운종양표지자를찾기위해모색중인것으로보인다. 본 론 1. AFP 1956년 Bergstrand와 Czar가처음발견하였으며, 년 Tatarinov와 1968년 Abelev 등이 2 이를임상진단에처음으로도입하였다. 당시는초음파와같은영상기술이없었던때이므로진단에있어서매우유용하였다. AFP는분자량 70kDa의 glycoprotein이다. 성인에서 AFP가상승하는경우는간세포가재생되거나간암이발병한경우로나눌수있다. 질병별로는바이러스간염, 간경변, 간세포암종, 간모세포종 (hepatoblastoma), 전이된간암등이있다. 그외소화기계종양중췌장암의 24%, 위암의 15%, 대장암의 3%, 담낭암등에서도 AFP가상승한다. 간세포암종의조기검진프로그램에는간초음파, AFP 두가지가있으며, AFP는검사실에서쉽게시행될수있는간단한검사이기때문에조기검진항목으로널리이용되고있다. 그럼에도불구하고 AFP 는임상에이용할때몇가지사항을고려해야한다. 첫번째, AFP를간암진단에이용하기위해서는 cut-off 치를결정하여야한다. 예를들어, 500 ng/ml이상으로높게잡을경우에는, 소간암을가진환자들은 500 ng/ml 이하의낮은수치를갖게되어진단에서누락된다. 3,4 또한 AFP는만성 B, C 간염환자의재활동기 (reactivation period) 에도 500 ng/ml 이상의높은수치로나올수있다. Cut-off value에따른민감도, 특이도의변화를표 1에나타내었다 5,6. 두번째, AFP의양성예측도가간세포암종의원인에따라서즉, 바이러스성과비바이러스성간에 (70% vs 94%) 도편차가있다. 세번째, AFP은간세포암종의크기에따라서그수치가증가한다. 네번째, 간세포암종검진목적으로 AFP를사용할때그대상환자집단을잘선정하여야하는데, 그이유는민감도와양성예측도가낮은편이기때문이다. 특이도는 cut-off 치가 - 6 -

Diagnostic value of AFP as HCC biomarker 5,6 Cut-off value (μg/l) Sensitivity (%) Specificity (%) 20 55~60 88~ AFP: α-fetoprotein 올라가면 76~96% 로높아지지만그반면민감도의경우는 3cm 이하소간암의경우 25%, 3cm 이상의경우 50% 로낮다. 즉, cut-off 치를 100 ng/ml로정할경우민감도는 20~30% 이며, 이것은간세포암종환자의 70~80% 에서는위음성이나온다는의미이다. 즉, 치료가능한 70~80% 의환자가 AFP만으로는진단이불가능하다는이야기이다. 5명중오직한명의환자만이 AFP 검사로간세포암종을진단받을수있으며, 나머지환자들은진단이늦어진다. AFP 검사가간암조기진단에유용한가? 서론에서잠깐언급을하였던부분이다. 질문에대한답부터이야기하자면, 그렇지않다 가정답이다. 고위험군에서초음파검사가 AFP보다훨씬민감도높은검사였다. Bolondi 등에따르면, 7 간경변환자 313명을약 3년간추적조사한결과, 연간간암발생률은 3% 이었다. 조기검진프로그램은 6개월마다실시하는초음파, AFP 검사로구성되었으며, 연구결과에따르면치료가능한종양을조기검출하여환자의생존을연장시키는이득은없었다. 한편, 초음파와 AFP 검사를조합하는프로그램을반대하는사람들은다음의이유를든다. 첫번째, 초음파의해상도가진보하여 3cm 이하의소간암을거의잡아낼수있다. 두번째, 초음파에의해새롭게진단되는결절의 80% 이상이악성이다. 세번째, 소간암에대한 AFP의민감도와양성예측도가매우낮다. 네번째, AFP검사를조기검진프로그램에추가함으로써직접적인비용상승뿐아니라, 불필요하게값비싼추가진단검사를유발하여간접비용의상승을일으킨다. AFP를검진프로그램에추가할것인가, 아니면단독으로초음파검사만시행할것인가하는문제는전향적인연구를통해서만증명할수있을것이다. 현재까지조기검진프로그램으로인한효과는간암환자의 30~60% 에서간세포암종을조기에진단할수있고, 2cm 이하의소간암상태에서의진단은전체간세포암종진단의 30% 에이른다. 일본의경우이러한조기검진프로그램과근치절제술등의효과적인치료술이상호연동되어생존률의향상을가져오고있다 AFP-L3 AFP는당연쇄구조 (sugar moiety) 에따라서아형 (subtype) 별로렉틴-반응성 (Lectin-reactive fraction) 이다른것이밝혀졌으며, 이 L1, L2, L3 중 AFP-L3 분획이간세포암종에보다특이적인아형임이알려지게되었고, 이아형을따로구분하여검사함으로써진단률의상승이기대되었다. 4 2010 년대한간학회 Single Topic Symposium 관련이있고, L2는임신, 난황낭종양 (yolk sac tumor) 과관련이있다. 3cm 이하의소간암의경우 L3 분획이전체의 35% 이상을차지하게된다 선행연구에 12,13 의하면, AFP-L3 검사법으로간세포암종의영상진단 9~12개월전에미리간세포암종을진단할수있다. 2cm 미만의간세포암종은 45%, 5cm 이상은 90% 이상에서진단할수있다. AFP-L3/AFP 비율이 10% 이상이면악화된간기능, 종양의조직학적분화도가나쁨, 크기가큰종양, 보다침습적이고, 조기전이가잘됨을의미한다. 그러므로, L-3분획은종양의진단과더불어예후를예측하는데에도쓰이고있다. 국내에서이루어진 AFP-L3 연구들을살펴보면, 이등은 14 AFP-L3 15% 이상을 cut-off치로정하였을경우, 환자등록이후 6개월이내의간암병발혹은재발에대한예민도는 63.6% 이었으며, 특이도는 87.0% 로보고하고있다. 변등은 15 간암환자 69명을대상으로한 AFP 검사의민감도는 59.4%, 특이도는 41.0% 이라고보고하였다. AFP-L3의경우민감도는 32.5%, 특이도는 85.7% 로간암진단의민감도측면에서, AFP와비교하여열등한결과를나타내었다. 3. Prothrombin in vitamin K absence(pivka)-ii, Des-gamma-carboxy prothrombin(dcp) PIVKA-II는간세포암종세포에서프로트롬빈전구물질이 carboxylation되는과정의결함으로초래되는단백인데, N-terminus의 10개 glutamic acid-residue의 carboxylation이안되어비정상적인 prothrombin 이만들어지므로 des-gamma-carboxy prothrombin(dcp) 이라는다른이름을갖고있기도하다. 이러한결함은간세포암종세포에서만들어진 prothrombin 단백전구체의유전암호해독후결함 (post-translational defect) 에기인한것이다. PIVKA-II는프로트롬빈과같은혈액응고활성이없다. 1984년 Liebman 등은 16 간세포암종진단혹은간세포암종으로수술받은후재발환자에서 PIVKA-II가높음을보고하였다. 간세포암종환자의 50~60% 에서 DCP가높게검출되며, 조기간암의경우 15~30% 에서높았다. 17 Nakagawa 등은 18 이검사법의민감도 48~62%, 특이도 81~98% 로보고하였으며, 종양표지자로서의의의는 AFP 와 필적할만하다고하였다. Grazi 등은 19 위두검사간의상관성이없어서이를조합할경우진단률이민감도 74.2%, 특이도 87.2% 로향상됨을보고하였다. Carr 등은 년전향적인연구를통하여수술하 Table 2. 5 채희복 : 간세포암종조기진단의종양표지자 지않은환자들에서간조직생검을통하여진단검사들의각조합에따른민감도특이도를보고하였다. 표 2에따르면, 가장성적이좋은것은 AFP-L3와 DCP 검사두가지의조합이다. 우리나라에서시행된유사한연구들을살펴보면, 최등은 23 간암진단의민감도및특이도측면에서 PIVKA-II가 AFP에비하여못미쳤으며, 이두검사를조합할경우 AFP 단독보다더낫다고하였다. 윤등은 명의간세포암종환자들과 274명의만성간질환환자들을대상으로한횡단면연구에서간암환자군의 40% 에서 AFP가양성 (>20 ng/ml) 이었고, 67% 에서 PIVKA-II가양성이었다 (>40 mau/ml). AFP 치가 20 ng/ml 이하로음성인환자들중에서 52% 가 PIVKA-II 양성반응을보였다. 전체간암환자군 129 명을대상으로한 PIVKA-II의진단민감도는 67%, 특이도는 93% 로보고하였다. 김등의 25 연구는간세포암종환자 42명과만성간질환환자 185명에서혈중 PIVKA-II를측정하여간세포암종진단에유용한 PIVKA-II 치의양성기준점을찾고자하였다. 또한김등은 AFP 치의간세포암종진단민감도는 61.9%, 특이도는 85.9% 였고, PIVKA-II는 (40 mau/ml) 의민감도는 66.7%, 특이도는 74.1% 로우리나라간세포암종환자에서 PIVKA-II의진단적유용성은 AFP 치에미치지못함을보여주었다. AFP와 PIVKA-II 치를결합하였을경우 85.7% 로증가하나특이도는 64.3% 로감소하였다. 김등은 ROC curve에서 PIVKA-II 의간세포암종진단의최적기준치가 250 mau/ml이라고제시하였다. 4. γ-glutamyl transpeptidase(ggt) GGT는담세관에 (bile canaliculus) 연하는간세포막과간쿠퍼세포에서 (Kupffer s cell) 분비하는효소이다. 본효소의활성은여러생리적, 병리적조건에따라서달라진다. 본효소는담즙정체와관련되며, 간암에특이적인효소는아니므로 AFP 검사를도와서간세포암종진단의민감도를높일수있다. 26 Xu 등은 27 본효소의분획을 9개에서 11개까지로구분할수있었는데, 그중두번째분획인 GGT II가대부분의간세포암종환자혈청에서발견되었다. GGT II 효소의양성률은 90% 이었으며, AFP, DCP와서로관련이없었다. 간세포암종환자를대상으로한후속연구에서 GGT 양성률은 90% 이었으며, 급성, 만성간염, 임산부, 간이외의암종에서는검출되지않았다. 5. Glypican-3(GPC-3) GPC-3는 oncofetal 단백으로세포막에부착된 heparan sulfate proteoglycans의일종이다. 28 GPC-3는정상적으로태아발생기에세포증식과생존의조절에관여하며, 종양억제인자로기능한다. 유방암, 난소암, 폐선암에서저발현 (downreulated) 되어있으나, 29 간세포암종에서는과발현되어있다. 30 GPC-3는정상인, 만성간질환환자의간세포에는존재하지않으며, 간세포암종의 50% 에서검출되고 AFP, DCP 음성간암의 33% 에서검출된다. 본검사의특이도는 100% 에달한다. 31 그래서 AFP와이검사를조합하였을경우간세포암종진단의민감도를향상시킬수있다

6 2010 년대한간학회 Single Topic Symposium 6. Squamous cell carcinoma antigen(scca) SCCA는 serine protease 가계를대표하며, serpins이라고도불린다. 두개의 SCCA 1과 SCCA2 상동유전자는각각다른두개의 SCCA 동종단백을만들며, 이는대부분의정상편평상피세포에서발견된다. Pontisso 등은 33 간세포암종조직에서 SCCA 과발현을관찰하였으며, 편평상피기원이아닌간세포암종에서 SCCA가과발현되는현상을관찰하였다. 간세포는편평상피와공통되는태생기원즉 carcionembryonic origin을가지고있기때문이라는가설을세우고간세포암종환자에서 SCCA의민감도, 특이도는각각 84, 46% 로조사하였다. SCCA의높은민감도, 낮은특이도와 AFP의낮은민감도, 높은특이도를조합할경우간세포암종진단의정확도를 90% 까지향상시킬수있다고보고하였다 AFP, SCCA, DCP의면역복합체이미알려져있는항원들과체내에서형성된면역글로불린인 IgM의면역복합체를 (immuno-complexes) 측정하여간세포암종의진단률을향상시키려는노력을하고있다. SCCA-IgM IC, AFP-IgM IC, DCP-IgM IC를측정하면작은크기의간세포암종혹은간세포암종의작은변화도검출할수있다는것이다 Giannelli 등은 35 AFP IgM IC, SCCA, SCCA IgM IC 세가지표지자를이용하여 AFP 혈중치가 20 ng/ml 이하인간암환자들을대상으로 25.6% 에서간세포암종을진단하였다. SCCA IgM IC 검사는간세포암종진단시민감도를 100%, 특이도 70%, 양성예측률 100%, 음성예측률 83% 로보고하였다. 36 AFP-IgM IC는 AFP 검사를보완하고, 위두검사를조합할경우진단의정확도를향상시킬수있다. 37 DCP-IgM IC의경우 DCP, AFP보다간세포암종환자에서더흔하게검출되며간세포암종진단에유용하다. 앞으로간암표지자분야의새로운물질의발견과아울러비침습적인진단법이간암조기검진에서주도적인역할을할것이기대된다. 38 결 론 우리나라는간세포암종의유병률이높은지역이고, 고위험군을대상으로 6개월마다 AFP, 초음파검사가권고되고있다. 종양표지자는영상검사보다더역사가오래되었음에도불구하고간세포암종조기검진프로그램에있어서그효과에대하여논란이많은실정이다. AFP는치료반응과재발감시에서그역할은인정받고있지만, 스크리닝측면에서는그진단적가치가제한적이다. AFP-L3, DCP의진단적가치는 AFP에도못미치고있으며, GP-3, SCCA 등은전망이있어보이지만아직상용화단계에이르지못하고있다. 39 위방법들을서로조합하여진단적정확도를높이려는시도가있으나그만큼비용이상승하기때문에조기검진의애초목적과는거리가멀어지는문제가있다. 조기검진측면에서종양표지자는분명히한계점이있다. 종양표지자가낮은수치일지라도간세포암종일수있으며, 높게나오더라
