To integrate the conventional CRP test detecting acute phase response and the hs-crp assay with a lower detection limit, we evaluated the performance characteristics of hs-crp assay methods with a wide range of concentrations. Methods : Immunonephelometric assay (BNII) and two turbidoimmunometric assays (TIA), the Hitachi 76 with Daiichi reagent (Daiichi-Hitachi) and the Roche Cobas Integra 7 (Integra), were evaluated for the precision with 8 levels of pooled sera ranging from.3 to 12 mg/l and the agreement of TIA with the BNII assay using regression analysis and Bland-Altman analysis with 89 patient samples. Results : The within-day coefficients of variation (CVs) of BNII were less than 1% in all levels of pooled sera. The CVs of Daiichi-Hitachi and Integra exceeded 1% in pooled sera below 1. mg/l and.5 mg/l, respectively. Both Daiichi-Hitachi and Integra appeared to be linear over the entire range of CRP concentrations used and were comparable with the results of BNII (Daiichi-Hitachi: y=.98x+.13, r=.97, Integra; y=1.2x+.22, r=.99). However, at the concentrations over 1 mg/l TIA and BNII showed discrepant results. Conclusions : Both Daiichi-Hitachi and Integra showed a good precision over a wide range of CRP. However, the discrepant results found at very high concentrations require standardization among different assay methods or instruments. 2 228 강소영 서진태 김정훈외 2 인 환에대한가장강력한예측인자로알려졌다 [5, 6, 8, 9]. 이에미국심장협회 (American Heart Association, AHA) 와질병통제예방센터 (Center for Disease Control and Prevention, CDC) 는건강인에서심혈관질환의예방목적으로혹은심혈관질환을가진환자에서임상적으로 CRP를이용하기위한지침을공표하였으며이지침에의하면 CRP농도 <1 mg/l, 1-3 mg/l, >3 mg/l를각각저위험, 평균위험, 고위험으로정의하고있다 [4]. 일반적으로경미한염증이나바이러스감염시 1-5 mg/l, 활동성염증이나세균감염시 5-2 mg/l, 중증의감염증이나상해의경우 2 mg/l 이상의 CRP농도를보이며 [2], 이러한급성기반응을찾아내는것을목적으로하는통상적인 CRP 검사들은 3-8 mg/l의측정한계치를갖는다. 그러나건강인에서 CRP를측정하여만성염증여부를판별함으로써심혈관질환의위험도를평가하기위해서는통상적인 CRP 검사의측정한계치보다훨씬낮은범위인.2 mg/l 미만의측정한계치가요구되며, 심혈관질환예측을위한임상적 cut-off치인 1 mg/l와 3 mg/l를포함하는 -5 mg/l 사이에서정확하고정밀한측정이보장되는고도로예민한 high sensitivity CRP (hs-crp) 검사의필요성이대두되었다 [1, 1-12]. 본원의경우임상적효용에따라기존의 CRP와 hs-crp측정이이원화되어있고검사방법도 CRP의경우혼탁면역법 (turbidoimmunometry, TIA) 을이용하며검사결과단위는 mg/dl로보고하고있으며, hs-crp의경우면역비탁법 (immunonephelometry) 을이용하며검사결과단위는 mg/l로보고하는방법을취하고있다. 따라서저자들은급성기반응을측정하는기존의 CRP 검사와 hs-crp에서요구되는저농도의측정범위를동시에아우르는넓은측정범위를갖는 CRP 검사방법으로통합하여경제성및검사실의효율성을높이고자 Daiichi사와 Roche사의시약을사용하여각각 Hitachi 76 (Hitachi High Technologies Co., Tokyo, Japan) 과 Cobas Integra 7 (Roche Diagnostics, Basel, Switzerland) 에서측정하여 hs-crp검사에서요구되는저농도범위와일반 CRP검사에서요구되는고농도범위에서의정밀도, 기존기기로부터얻은결과와의일치성을보고자하였다. (Hitachi High Technologies Co.) 으로검사를시행하였고, 반응성이다른두단클론성항체를크기가다른두종류의라텍스입자에각각코팅시켜 hs-crp검사에서요구되는측정한계치로부터기존의 CRP검사에필적할만한고농도범위까지측정범위를제공하는 dual radius enhanced latex법의 Integra CRPLX 3 (Roche Diagnostics) 를사용하여 Cobas Integra 7 (Roche Diagnostics) 으로측정하였다 [13]. 2. 검사일내정밀도평가각검사방법의검사일내정밀도검사는저농도에서고농도까지 ( mg/l) 다양한농도로준비된 8단계의 pooled serum (Pool No 1-8) 을이용하여각농도별로 1회씩측정한후 3분이상의간격을두고다시각농도별로 1회씩반복검사를시행하여얻은결과로평균, 표준편차및변이계수를구하는방법으로시행하였다. 3. 기존검사방법과의비교기존검사방법과의비교를위해 CRP 및 hs-crp 검사가의뢰된환자들의혈청검체를사용하여기존의 hs-crp 검사장비인 BNII로측정하여저농도인.16 mg/l부터고농도인 253 mg/l 에걸쳐다양한농도를보이는 89검체를선택하여사용하였다. 이로부터얻은결과들을이용하여검사방법간일치정도를보기위해 Passing-Bablok 회귀분석과 Bland-Altman 분석을시행하였다 [14, 15]. 특히 AHA/CDC에서제안한심혈관질환예측을위한임상적 cut-off치를포함하는 -5 mg/l의저농도구간을분리하여 BNII와의일치정도를보았으며이들의통계분석은 MedCalc version 7.5 (MedCalc software, Mariakerke, Belgium) 를이용하였다. 또한이들측정값을적용하여 AHA/CDC에서제안한심혈관질환예측을위한임상적 cut-off치를적용하여환자들을세개의위험군으로분류할경우검사방법간분류상의차이를조사하였다. 대상및방법 결 과 1. CRP 측정장비및시약 1. 검사일내정밀도평가 기존에 hs-crp 측정에사용중인면역비탁장비인 BNII (Dade Behring, Marburg, Germany) 로 N High Sensitivity CRP assay (Dade Behring) 시약을사용하여제조사의지시대로 hs-crp검사를시행하여기준결과치로삼았다. 비교대상검사로는통상적인 CRP검사및 hs-crp검사에서요구되는측정범위를동시에만족시키는 latex particle enhanced TIA 시약들을사용했는데 PureAuto S CRP (Daiichi, Tokyo, Japan) 는 Hitachi 76 농도별로시행한 hs-crp의검사일내정밀도평가에서 BNII 의변이계수는저농도에서고농도에걸친모든농도에서 1% 미만이었으며, Daiichi 시약으로 Hitachi 76에서시행한 hs-crp검사 ( 이하 Daiichi-Hitachi로표시 ) 에서 1. mg/l 이하의저농도값을갖는 Pool No 1, 2 및 3에서변이계수가 1% 를초과하였으나고농도로갈수록 1% 미만의낮은변이계수를보였고, Roche 사의 Cobas Integra 7으로시행한 hs-crp ( 이하 Integra로표

BNII (x) 와 Daiichi-Hitachi (y) 에대한 Passing-Bablok 회귀분석결과기울기.98 (95% 신뢰구간 : ), y절편.13 (95% 신뢰구간.8-.19) 이고 Spearman 상관계수값은.97로나타났으며, BNII (x) 와 Integra (y) 는기울기 1.2 (95% 신뢰구간 : ), y절편.22 (95% 신뢰구간 : ) 이 고상관계수값은.99로나타났다 (Fig. 1). 한편 -5 mg/l의저농도범위를갖는예들을따로분리하여시행한 Passing-Bablok 회귀검정에서 BNII (x) 와 Daiichi-Hitachi (y) 는기울기 1.1 (95% 신뢰구간 : ), y절편.11 (95% 신뢰구간 :.8-.15) 이고상관계수값은 r=.91로나타났고, BNII (x) 와 Integra (y) 는기울기 1.5 (95% 신뢰구간 : ), y절편.19 (95% 신뢰구간 : ) 이고상관계수값은.94로나타났으며 (Fig. 2) 5 mg/l 이상의농도를갖는예들에서는 BNII (x) 와 Daiichi- Hitachi (y) 는기울기.88 (95% 신뢰구간 : ), y절편.63 (95% 신뢰구간 ) 이고상관계수값은.96이었으며, BNII (x) 와 Integra (y) 는기울기 1.4 (95% 신뢰구간 ), y절편 -.9 (95% 신뢰구간 : ) 이고상관계수값은.99로나타났다. Bland-Altman 그래프에서 BNII 결과와 Daiichi-Hitachi 및 Integra 결과와의차이값의평균이각각 1.4 mg/l (95% CI, ), -.2 mg/l (95% CI, ) 로나타났으며,

5 측정범위가넓은 C-Reactive Protein 검사법비교 mg/l까지는 BNII와양호한일치를보이다가이를넘어가면서증가되는산포양상을보였다. Daiichi-Hitachi의경우농도가증가할수록 BNII와의차이가양의방향으로커지는양상을보이는반면 Integra의경우농도가증가할수록 BNII와의차이가음의방향으로증가하는양상을보이며 (Fig. 1), 1 mg/l 이상에서는 2SD 이상의검사방법간불일치가관찰되었다. 5 mg/l 미만의저농도에서는 Daiichi-Hitachi와 Integra 두가지비교방법모두 BNII와의차이값들이비교적고른분포를보였다 (Fig 2). 3. AHA/CDC 기준분류상일치도 Table 2는각각의검사방법으로측정된 hs-crp 결과들을이용하여심혈관질환예측을위한임상적 cut-off인 1 mg/l와 3 mg/l를기준으로세개의상대적위험군으로분류한결과이다. Daiichi-Hitachi와 BNII 간에는 89명중 78명 (87.6%) 에서, Integra와 BNII 간에는 89명중 8명 (89.9%) 의분류일치율을보여위험군분류에있어서불일치를보이는경우는 Daiichi-Hitachi 12.4% 와 Integra 1.1% 였다. 또한이때관찰된불일치는 Daiichi- Hitachi와 Integra 모두한 class의차이를나타냈다. 고찰 hs-crp의측정이죽상경화증의발생과진행에기여하는것으로알려진만성염증상태를찾아내는표지자로이용되고있으나, 아직까지 CRP의공인된참고방법 (reference method) 이없는상태에서여러 hs-crp검사법간의가변성을무시하고서로다른검사방법으로측정된값을일률적으로 AHA/CDC에서공표한상대적위험군분류표에적용하는것은환자의분류오류를초래한다 [1, 1]. 따라서 hs-crp 검사방법에상관없이일률적으로임상결정치 (medical decision level) 를적용하여상대적위험군분류에사용하려면 hs-crp검사의정밀도와정확성이보장되어야하는데이를위해선 hs-crp검사의표준화가필요하며표준화의기본조건으로국제적으로공인된 calibrator 를사용해야한다. 급성기반응을보기위한통상적인 CRP 측정을위한공인된표준물질로 WHO 1st International Reference Preparation for CRP Immunoassay 85/56과 Certified Reference Material 47 (CRM47) 이사용되고있었으나 [2, 1, 12], 심혈관질환위험도를평가하기위한저농도의 hs-crp 측정을위한공인된표준물질은없었다. 그러나최근 CDC에서 hs-crp검사의표준화작업의일환으로시행한연구에서 CRM47을 hs-crp검사의표준물질로공인하였고 [1], 본연구에서비교한 Dade Behring, Daiichi 및 Roche사모두 CRM47으로부터각제조사의 calibrator를표준화하여사용하고있었다. 심혈관, 뇌혈관및말초혈관질환의위험군분류를위한 hs-crp 검사의정밀도는변이계수 1% 미만으로권장되는데 [2, 16], 기 존의보고에의하면 BNII나 Daiichi-Hitachi는 -2 mg/l의농도범위에서 total imprecision은변이계수 1% 미만이며두방법모두.3-1 mg/l에서직선성을보였으나 1 mg/l 이상의농도범위에서는정밀도평가나직선성평가가이루어지지는않았다 [12]. Integra를평가한국내보고에의하면비록 1 mg/l 미만의저농도에서의정밀도는평가하지않았지만 15 및 3 mg/l 농도수준에서변이계수가 5% 미만이며 -2 mg/l에서 %relative nonlinearity가.78% 로직선성판정기준인 2.5% 미만을만족한다고보고하였다 [17]. 본연구에서는비록검사일간정밀도를평가하지못했다는한계점을가지나 1 mg/l 미만의저농도에서 1 mg/l 이상의고농도까지광범위한범위에서검사일내정밀도를평가하였으며이로부터면역혼탁법은면역비탁법인 BNII에비해 1 mg/l 미만의저농도에서변이계수가 1% 를넘는것으로나타났으며, 고농도가될수록면역혼탁법이면역비탁법보다변이계수가낮게나오는경향이있음을알수있었다. 또한, 면역비탁법과면역혼탁법간일치도를평가하기위해잠정적으로면역비탁장비인 BNII를참고방법으로간주하고면역혼탁법에의해 hs-crp를측정하는 Daiichi-Hitachi 및 Integra에대해 Passing-Bablok 회귀분석과 Bland-Altman 분석을시행한결과 Daiichi 시약을사용한 Hitachi와 Integra는 BNII와의일치도가양호한것으로나타났다. 그러나 Daiichi-Hitachi 및 Integra 모두 1. mg/l 미만의농도에서 1% 가넘는변이계수를보이고, 농도가증가할수록 Daiichi-Hitachi는 BNII보다낮게측정되고 Integra는 BNII보다높게측정되는경향을보여 1 mg/l 이상에서는 Bland-Altman 분석상 2SD 이상의불일치를보였다. 표준물질 CRM47으로표준화된 calibrator를제조사별로공히사용함에도불구하고검사방법및검사장비에따라검사결과의불일치가관찰되며, 이는저농도때보다고농도에서큰폭의불일치를보인다는여러보고들이있고 [12, 16, 18, 19], 본연구에서도마찬가지로고농도에서 2SD 이상의불일치는검사방법혹은검사장비의차이와관련된문제로생각되며 [19], 그가능성으로검체희석, 사용된희석액의 matrix 효과, 검사법간채택한표준곡선적합알고리즘의차이등을제시하고있어 [18] 다양한검사방법간의일치를위한표준화작업이앞으로도더연구되어야할것으로생각된다. 이러한검사방법간불일치에대해대부분의연구들이면역비탁법을참고방법으로간주하고면역혼탁법을평가하지만 CRP에대한공인된참고방법이없는상태에서실제로면역비탁법과면역혼탁법중어느것이더정확한지는판단하기어려운문제이며, 저농도및고농도에서의불일치가있더라도 BNII를기준으로상대적위험군을분류한결과와 Daiichi-Hitachi 및 Integra에서얻은값으로분류한결과를비교했을때면역혼탁법과면역비탁법검사방법간불일치로인해분류오류가일어난경우는 1% 내외이며이러한불일치정도는대부분에서한 class의차이만을보이고있으므로각검사실의실정에따라어떤방법을선택하든임상적으로큰문제를초래할것으로생각되지않는다. 따라서검사실의효율

6 232 강소영 서진태 김정훈외 2 인 성이나결과보고의일관성을유지하기위해기존의염증반응을보기위한 CRP검사와심혈관질환의위험도예측을위한 hs-crp검사를아우르는 Daiichi-Hitachi 및 Integra 두방법모두넓은범위의 CRP를측정하는방법으로대체할수있을것으로생각된다. 요약배경 : C-reactive protein (CRP) 측정은임상적효용에따라급성기반응을측정하는통상적인 CRP검사와심혈관질환의위험도를예측하기위해요구되는 high sensitivity-crp (hs-crp) 로나뉘어서이루어진다. 본연구에서저자들은아주낮은농도의측정한계치를갖는 hs-crp검사와염증반응에서나타나는통상적인 CRP 검사를통합하기위하여넓은측정범위를갖는 CRP 검사법들을평가하고자한다. 방법 : 면역비탁장비인 BNII와두개의면역혼탁장비인 Hitachi 76 (Daiichi-Hitachi) 과 Roche Cobas Integra 7 (Integra) 의정밀도와검사방법간일치도를분석하였다. 정밀도는.3-12 mg/l사이의 8단계의 pooled serum을이용하였고, 검사방법간일치도평가를위해 89개의검체를사용하여 BNII 결과와두가지면역혼탁법결과를비교하여 Passing-Bablok 회귀분석과 Bland- Altman 분석을시행하였다. 결과 : 검사일내정밀도는 BNII의경우모든단계의 pooled serum에서변이계수가 1% 미만이었고, Daiichi-Hitachi는 1. mg/l 이하에서, Integra는.5 mg/l 이하에서변이계수가 1% 를넘었다. Daiichi-Hitachi 및 Integra 모두평가가이루어진전 CRP 범위에서 BNII와의우수한일치성 (Daiichi-Hitachi: y=.98x+.13, r=.97, Integra; y=1.2x+.22, r=.99) 을보였으나 1 mg/l 이상에서면역혼탁법과면역비탁법간에 2SD 이상의불일치가관찰되었다. 결론 : Daiichi-Hitachi 및 Integra 모두 CRP 측정법은저농도에서고농도까지정밀도및 BNII와의일치성에있어서우수하였다. 그러나검사방법및검사장비의차이에따라나타나는고농도에서의불일치현상은계속적인표준화과정으로개선되어야할것으로생각된다. 참고문헌 1.
