4 신경간내주사 반드시국소마취제나스테로이드주사약을함께청구해야인정. 이때사용하는국소마취제등은예외코드55( 치료나검사에사용되는주사제 ) 로따로등록 물리치료와병행시둥중하나는 100/100청구가능. 스테로이드는한달에 2회만인정하므로국소마취제로청구하는것이좋음.

5 신경차단술 통증을유발하는신경에주사를통해직접약을주입하는방법으로, 통증완화랑신경염증을치료하는방법 말초신경차단은, 주로신경및신경주위나그것을함유한조직에국소마취약을주입하여, 그전도를화학적으로차단할수있는것을이용해서무통을얻는것을말한다.

8 산정기준 ( 고시제 호 ( 행위 )) 통증완화또는치료목적으로실시하는신경차단술은상병명, 환자의상태및신경차단술에대한환자의반응에따라그종류와실시간격및횟수등이달라질수있으나적정치료기간등을감안하여동시술에대한산정기준은다음과같이함. 가. 신경차단술은상병에따라주 2-3 회시행함을원칙으로하 되, 최초시술부터 15 회까지는소정금액의 100% 를, 15 회를초 과시는 50% 를산정함.

9 신경차단술산정기준 나. 신경차단술을장기간연속적으로실시하는것은바람직하지아니하므로일정기간신경차단술후제통이되지않는경우에는치료의방향등을고려하여야하는점등을감안하여실시기간은치료기간당최대 2개월까지인정함. 다만, 대상포진후신경통, 척추수술실패후통증, 신경병증성통증 (neuropathic pain), 척추손상후통증, 말기암성통증인경우에는예외로적용함.

10 신경차단술산정기준 다. 동일병소에날짜를달리하여서로다른신경차단술을실시하는경우에는실시의종류를불문하고실시횟수를합산함. 라. 동일병소에동시에서로다른 2가지이상의신경차단술을실시하는경우에는 2가지의신경차단술만산정하되, 주된신경차단술은해당소정금액의 100% 를산정하고, 제2의신경차단술은해당소정금액의 50% 를산정하며, 횟수는 1회로산정함. 다만, 주신경계에서세분한분지신경차단을주신경차단과동시에실시한경우에는주신경차단에따른효과를고려하여주신경차단의소정금액만인정함.( 예 : Sapheneous N/B과동시에 articular Br. Block of saphenous nerve)( 시행 )

54 외측대퇴피신경차단술 청구코드 LA 요부질환에따른대퇴부, 대퇴후외측통 병명은 M169+G571 또는 M5449, M5439 등. 대퇴부와대퇴후외측부위에따라찌름점이다르다.

55 외측대퇴피신경차단술 시술 -> 환자를바로누운자세로눕히고, ASIS를표시한후아래로 2cm, medial로 2cm 위치한곳을바늘로뚫고, 대퇴근막을통과하는느낌이올때까지밀어넣은다음국소마취제 5ml를대퇴근막위아래로넓게주사한다. 복와위에서시행하는경우에는, 압통부에서대퇴골로바늘을 직접닿게하여, 1% 메피바카인 ( 리도카인 ) 10ml 을주입하 여광범위하게침윤시킨다.

58 경막외신경차단술 청구코드 LA 좌골신경통, 요통에효과좋으나 risk 도크다. 반드시충분한해부학지식을갖추고시행. 되도록이면수액을달고, aseptic 하게시행하며, 시술중혈액 등출혈소견이보이면바로멈추고지혈을시행한다.

60 경막외신경차단술 Prone position에서, 하복부에베개를넣어천골부를높게한다. 발을안쪽으로모은다. 천골열공 (hiatus) 를찾아손톱등으로눌러표시후포타딘으로소독하고멸균장갑을착용하여, 약물을준비하여찌른뒤, 흡인테스트하여혈액역류여부를확인하고, 마취제를조금만주사하여요천부하지의중압감을확인한뒤나머지마취제를서서히주입한다. 이때, 왼손바닥을천부에대어약물주입시팽윤을느끼게된다면, 피하조직에들어간것이므로다시찌름점을찾아재시도한다.

61 경막외신경차단술

62 경막외신경차단술 천골열공촉지는많은경험이필요한기술이다. 제통효과를얻으며, 하지근력저하가없으려면 1% 메피바카인 5ml(1% 리도카인 5~10ml) 정도가적량. 스테로이드는덱사메타손을쓰며, 이때는반드시15일이상간격을두고재시술한다. 시술전배뇨를시행하지않으면요실금이일어날수있다.( 특히, 여성 ) 반드시시술후 20~30분침상안정. 혈관투여나과량투여시경련등이올수있다.

64 요추후지신경차단술 적응증 : 추간관절증을일으키는모든질환 급성염좌, HNP, 압박골절, 대상포진후신경통, 척추분리증, 전방전위증, 수술실패후요통, 척추관협착증,,,, 등등 방법 : 요추 - 극돌기외측 4cm 수직 or 약간두측약 4-5cm

65 요추후지신경차단술 PDLNB(posterior division lumbar nerve block) 소독후번요추극돌기의상연에서외측 0.5cm 두측, 외측 4cm의선에교점마킹후 23G 6cm주사바늘을피부에수직으로찔러주사바늘의끝이횡돌기에닿은것을확인한후바늘끝을미세조정하여약간내측으로향하게한후리도카인을주사하여척수신경후지차단
