We sought to identify the different clinical manifestations between patients treated with and without oseltamivir. Methods: We retrospectively studied the medical records of 72 inpatients or outpatients from two medical centers diagnosed with influenza B infections by either a rapid antigen test or multiplex reverse transcriptase PCR between January 2012 and July We compared gender, age, past medical history, admission period, total fever duration, fever duration after hospitalization, post-oseltamivir medication peak temperature, laboratory test, chest X-ray, antibiotic medication, and the presence of concomitant viral or bacterial infections. Results: The number of subjects in our study was 72 who were diagnosed with influenza B pneumonia, acute bronchitis, acute bronchiolitis, croup, and mean age was 3.6±2.8 year old. The demographic characteristics and clinical manifestations of oseltamivir and the non-oseltamivir groups, including hospitalization period (4.18±2.10 vs 4.79±1.49 days, P =.17) and total fever duration (5.32±2.07 vs 6.41±3.25 days, P =.09), demonstrated no significant differences. Notably, the oseltamivir group did have significantly reduced usage of antibiotic treatment than the non-oseltamivir group (P =.04). When we limited our patient group to patients under the age of three, similar results were seen. The group prescribed oseltamivir within 48 hours of fever onset had less antibiotic usage, in addition to a shorter fever duration. Conclusion: Oseltamivir appeared to have no benefit in improving the clinical course. However, if it is prescribed within the first 48 hours of symptoms, it may be more effective. 2 Korean J Pediatr Infect Dis Vol.21, No.3, 2014 변화시켜교대로또는같은시기에유행한다 1-3). 바다표 범과인간외에야생보유숙주가없어더낮은빈도로감 염되고 2) 상대적으로좁은지역에서유행하는특징이있 다 1). 국내연구는주로봄에유행하는경향을보이며 3, 4), 상대적으로발열기간이길고 5-7) 폐렴발생률과입원률이 높으며, 특히소아에서사망률이높다고보고하고있다 2). 국외보고에서는인플루엔자관련소아사망률의 52.3% 가인플루엔자 B 바이러스가원인이었다 3, 10). 인플루엔자 B 바이러스치료제인 neuraminidase 억제 제 (NAIs, oseltamivir, zanamivir) 중에서 oseltamivir 는 경구약제로영유아에서도복용이허가되어영유아에서 2, 8, 도복용허가되어소아에서주로사용되는약제이다 9). Oseltamivir 는입원기간과호흡기합병증의발생을 2, 8, 10- 감소시키지만소아에서효과가낮다고보고되었다 12). 인플루엔자 A 바이러스에비하여상대적으로낮은유 행률을 2) 보이는인플루엔자 B 바이러스에이들의효용성 에대한연구가부족하고특히국내인플루엔자 B 바이러 스감염에대한 oseltamivir 의효용성에대한연구는거 의없다 10). 이에저자들은인플루엔자 B 바이러스감염소아환자 를대상으로 oseltamivir 치료군과비치료군간의비교를 통하여 oseltamivir 의임상적효용성과이에관련된인자 에대하여알아보고자연구를시행하였다. 1. 연구 대상 및 방법 대상 및 방법 본연구는 2012 년 1 월부터 2012 년 7 월까지경희대학 교병원과강동경희대학교병원소아청소년과에입원하거 나외래진료실을방문한소아환자를대상으로하였다. 이들중 38 도이상의발열과기침, 콧물, 인후통과같은 호흡기증상, 구토, 설사와같은위장관증상이동반되거 나항생제치료에도발열이지속된환자에대하여인플루 엔자신속항원검사 (rapid antigen test) 또는 multiplex reverse transcriptase-polymerase chain reaction (RT-PCR) 을시행하여인플루엔자 B 바이러스에양성 을나타낸환자 72명을대상으로후향적으로의무기록을분석하였다. 인플루엔자 B 바이러스감염에대한진단시기 ( 월 ), 성별, 연령, 보육시설또는학교등교유무, 인플루엔자예방접종, 과거력과기침, 인후통, 비염, 위장관증상, oseltamivir 복용시점, 입원유무와입원기간, 발열시작과진단일까지의기간, 최고발열온도, 발열후최고온도발생시까지의기간, 입원후발열기간, oseltamivir 치료후최고온도발생시기, 치료후발열기간, 총발열기간, 입원당시시행한일반혈액검사, CRP 등의검사소견, 단순흉부방사선사진 (chest X-ray, CXR) 을분석하였다. 약제의투여는고열이지속되는경우, 고열과함께 CXR 에서심한폐렴소견이없고검사실검사결과에서세균감염보다는바이러스감염이의심되었을때, 신속항원검사에서확진된경우 oseltamivir 투약에대하여하루 2 회, 5일간권장용량을복용하였다 13). 대상환자들을 oseltamivir 복용유무에따라치료군과비치료군으로나누었고 3세이하의환자들을추가분류하여 oseltamivir 치료의유용성을분석하였다. 그리고 oseltamivir 치료시점을기준으로비치료군, 발열 48시간이내치료군, 발열 48시간을초과하여치료한군으로나누어임상양상을비교분석하였다. 2. 인플루엔자 바이러스 검사신속항원검사법과 RT-PCR 은면봉으로비인후검체를채취하여멸균튜브에넣어검사실로이동하였으며경희대학교병원에서는 Alere BinaxNow Influenza A & B card (Alere Scarborough, USA), 강동경희대학교병원에서는 SD Bioline Influenza Ag A/B/A (H1N1) Pandemic (Standard Diagnostics, Inc., Yongin, South Korea) 를사용했다. RT-PCR 은두병원모두에서 Gene AMP PCR System 9700 (Applied Biosystems, USA) 를이용하여인플루엔자 A와 B 바이러스를분류할수있었고그외에아데노바이러스, 호흡기세포융합바이러스 A와 B, 파라인플루엔자바이러스 1-3, 라이노바이러스, 코로나바이러스 229E/NL63, 코로나바이러스 OC43/ 200

3 윤송이외 4 인 : 소아에서 B 형인플루엔자바이러스의감염에서 oseltamivir 치료의유용성 HKU1 등의바이러스들도검사하였다. 3. 통계 분석 자료의분석은 SPSS 21 (IBM, New York, USA) 을 이용하여명목변수에대하여 Fisher, Pearson chi- square 검정을하였고연속변수에대하여 student t 검 정, Mann-Whitney 검정을하였다. 투약시기에따른세 군의비교에서는일원배치분산분석 (oneway ANOVA), 사후검정을시행하였다. P 값이 0.05 미만인경우를유 의하다고판정하였다. 통계적 power 는증상의유지기간 을대표하는발열기간에대해서 G-power (G* Power, HEINRICH HEINE, Dusseldorf, Germany) 을 이용하여분석하였다. 1. 월별 분포 결 과 인플루엔자 B 바이러스는 72 명의소아환자에서 1 월 에 2 명, 2 월에 11 명이발생하였고 3 월에 41 명으로검출 율이가장높았으며 4 월부터감소를보여 4 월 13 명, 6, 7 월에는각각 1 명만이진단되었다 (Fig. 1). 2. 환자들의 임상적 특징 연구대상환자 72 명에서 51 명이신속항원검사로 21 Fig. 1. Oseltamivir 치료군은 38명 (53 %), 비치료군은 34명 (47%) 이었고비치료군에서는 PCR 로진단된환자가 44% 였다. 인플루엔자예방접종은군간차이가없었다. 남녀각각 37명과 35명이었으며평균연령은 3.6세이었다. 보육시설에다니는경우가 45명이었고천식, 알레르기비염, 아토피피부염의알레르기질환 6 명, 크룹, 편도선염등의상부호흡기질환과거력 4명, 세기관지염과폐렴등하부호흡기질환력 4명, 심장질환을포함한기타과거력이있는환자는 2명으로총 16명이었다. 염색체이상이나청색증을동반한심장질환의과거력을가진환자는없었다 (Table 1). 전체환자들중에서 3세이하는 44명이며이들중치료군은 20명 (45 %), 비치료군은 24명 (55%) 이었다. 1) Oseltamivir 치료군과 비치료군 간의 비교 Oseltamivir 치료군, 비치료군간에항생제투약이비치료군에서유의하게높았다 (P=.04). 그이외에성별, 나이, 보육시설유무, 알레르기, 호흡기질환과거력, 치료전기침, 인후통, 비염, 위장관증상등에대한유의한차이는없었다 (Table 1). 인플루엔자 B 바이러스감염에의한입원은전체환자 72명중 67명이었고각군의환자에대한입원율은통계적으로는유의하지않으나비치료군이높은경향을나타냈다 (P=.06). 총발열기간도통계적으로유의하지는않았으나전체환자에서는 5.9일, 비치료군은 6.4일, 치료군은 5.3일로비치료군이연장되는경향을보였다 (P=.09, power 0.36). 검사실소견중 C-반응성단백이비치료군에서유의하게높았고 [0.46 mg/dl ( ) vs 0.85 mg/dl ( ), P=.04], 그이외에는일반혈액검사를포함한검사실소견에서유의한차이가없었다. 2) 투약 시기에 따른 효과항바이러스제투약시기는발열 48시간, 72시간후가가장많았으나발열 3-8일후에투약한비율이 72.2% 로발열발생 48시간내투약비율 (27.8%) 보다높았다. 비치료군, 48시간이내치료군, 48시간이후치료군, 이들세군간의비교에서입원후발열기간과 (2.81± 201

4 Korean J Pediatr Infect Dis Vol.21, No.3, 일 vs 2.14±1.95 일 vs 2.44±1.54 일, P=.51) 총발열기간에서 (6.52±3.44 일, 3.88±1.81 일, 5.68±1.77 일, P=.06, power 0.92) 48시간이내치료군이가장짧았으나통계적으로유의하지는않았다. 항생제투약은비치료군이가장많고 48시간이후치료군, 48시간이내치료군순으로나타났다 (P=.001, Table 2). 3) 연령군에 따른 약제의 효과 3세이하의환자군을치료군과비치료군으로나누어비교했을때두군간의성별, 연령, 보육시설통학유무, 알레르기, 호흡기또는심장질환과거력등에대하여유의한차이가없었다. 각군에서의항생제복용율은비치료군이높은경향을보였지만 (P=.06) 이외에는총발열기간 (P=.82, power 0.19) 을포함한임상양상에서차이를보이지않았다 (Table 3). C-반응성단백은치료군과비치료군에서각각 0.35 mg/dl ( ), 0.69 mg/dl ( ) 으로비치료군에서더높고 (P=.01) 혈청알부민은각각 4.42± 0.19 g/dl, 4.14±0.71 g/dl로비치료군에서더낮았다 (P=.02). 고 찰저자들의연구는인플루엔자 B 바이러스감염소아만을대상으로 oseltamivir 의효용성을연구한국내최초연구이다. 인플루엔자 B 바이러스감염소아환자에서 oseltamivir 투여는약제투여군에서신속항원검사율이높았고임상경과에통계적으로유의성은없었으나발열 48시간이내치료군에서전체발열기간이짧아지는경 Table 1. 6 Korean J Pediatr Infect Dis Vol.21, No.3, 2014 향을보여주었다. 인플루엔자 B 바이러스의감염률은기후, 지역과유행 정도에따라차이가있다 4, 12) 년 1 월부터 7 월에는 예년과비교하여소아환자의감염률이증가하여이기간 을연구의대상시기로하였다. 검출률은 3 월에가장높 았고이후감소하여 6, 7 월에는각각 1 명만이진단되었 다. 국내외연구결과들처럼봄철에유행하고, 국내에서는 3 월에 3, 4, 14, 15) 호발한다는보고와같은결과를보여주고 있다. 전체환자들중 3 세이하환자만을분류하여분석하였 다. 호흡기바이러스검출률이생후 24 개월이상에서는 떨어지고 4) 2 세이하환자들은인플루엔자감염에의한 합병증발생가능성이높은고위험군이지만 8) 어린소아 2, 10, 에서 oseltamivir 효과가저하된다는보고들이있다 16). 항바이러스제투약률은전체환자에서는 53% 였으나 3 세이하에서는치료군의비율이 45% 로낮았다. 이는 oseltamivir 가 1 세이상에서승인되었고비치료균에서 더높은 C- 반응성단백과항생제처방률을고려할때영 유아에서세균감염을먼저의심했던것과관련이있을 것으로생각된다. 전체환자의비치료군에서항생제투약비율과 C- 반 응성단백이 3 세이하군분석처럼유의하게높았다. 또한 전체환자군에서항생제투여가 71% 로 oseltamivir 복 용율보다높았다. 혈액검사에서 C- 반응성단백수치의 상승이소아, 특히영유아들에게항생제처방율을증가시 켰을것으로추측된다. 발열후투약시기를고려하지않 았을때통계적으로유의하지는않았지만 oseltamivir 치 료군에서총발열기간이감소하는경향 (P=.09) 을보여 주었다. Oseltamivir 투약시점에따라비치료군, 발열 48 시간 이내치료군, 발열 48 시간이후치료군으로구분하여비 교분석했을때에는통계적으로유의성은없으나발열 48 시간이내치료군이발열기간이제일짧고비치료군이 가장지속되는경향을보였고 (P=.06, Table 2) 비치료 군과 48 시간이내치료군만을비교했을때발열기간이 유의하게짧게나타났다 (P=.04). 그러나비치료군과 48 시간이후치료군과는총발열기간에유의한차이가없었다. 인플루엔자 A, B 바이러스모두를대상으로 oseltamivir 복용시에발열기간의호전을보여준연구들이있었다. Nicholson 등의연구에서는발열발생 24시간내에투약한환자들은위약투여환자군보다 43시간 (75 mg) 혹은 47시간 (150 mg) 먼저발열이호전되었다 11). 발열 48시간내에투여했을때 44% 가 1일만에해열 (< 37.5 ) 되고환자의 86% 가 48시간이내에발열이호전되어투약후평균발열기간이 1.7일이었다 17). 인플루엔자 B 바이러스만을대상으로한무작위대조연구는감염된 1-12세소아들에서질병발생 48시간이내에 oseltamivir 투약시발열기간의유의한감소를보여주었다 18). 하지만핀란드에서 년기간동안인플루엔자 B 바이러스감염 1-3세소아를대상으로한무작위대조군연구는 24시간내에투약도뚜렷한효과를보여주지못하여위의결과들과상반된결과를보여주었다 19). 연구결과들의차이는대상자수, 연령, 인종등연구설계과정에서의차이와계절별, 유행지역별 NAIs에감수성이다른인플루엔자종이원인일수있다. 하지만공통적으로인플루엔자 B바이러스감염유행시기에 oseltamivir를시도하고자할때는증상발생조기에투여하고 48시간의경과관찰이중요할것이다. Oseltamivir 의인플루엔자 B 바이러스에대한효과는연령, 약제내성과관련이있다. Sugaya 등은 oseltamivir의인플루엔자 B 바이러스에대한 50% 억제농도 (IC 50, half maximal inhibitory concentration) 가인플루엔자 A (H3N2) 보다높아서인플루엔자 B 바이러스에대한항바이러스제의효과가인플루엔자 A 보다낮아, 특히 5세미만에서현저하고, 세에서는차이가없어져연령과면역상태에따라약에대한감수성이다르다고보고하였다 10). NAIs 에대한인플루엔자 B 바이러스의내성정도는낮으나다른종류의저항성분자적표지자수는인플루엔자 B 바이러스가 A바이러스보다높다 2). Fujisaki 등은항바이러스제를복용한적이없는환자를대상으로인플루엔자 B 바이러스에서단일 E105K 변형으로 oseltamivir 에 204

7 윤송이외 4 인 : 소아에서 B 형인플루엔자바이러스의감염에서 oseltamivir 치료의유용성 대한감수성의감소를보여주었다 20). 아시아태평양지역 의 2011년동안 oseltamivir 의내성연구는인플루엔자 A (H1N1) 보다는인플루엔자 B 바이러스에서훨씬낮은내성을보고하고있으나 IC 50 이인플루엔자 A보다 30 배더높기때문에 NA 돌연변이에대한지속적감시의필요를강조하였다 9). 신속항원검사는 의 H1N1 대유행시기에민감도와특이도에대한많은연구가이루어졌다 21). 저자들의결과에서약제투여군에서신속항원검사의빈도가높았다. 신속한결과가항바이러스제투여에영향을주었을것이다. 하지만투여군과비투여군간에진단까지의시간에차이가없고, 투여군에서도 PCR 검사시에 oseltamivir의투여가동시에이루어져 oseltamivir 의효과에검사방법의영향은낮을것이다. 본연구는후향적연구이며증상발생 48시간이내치료군의환자수 (10명) 가적었고외래진료환자들에대해서는 oseltamivir 복용후재내원이적어 oseltamivir 의효용성을정확히판단할수없다. 그러나 년국립보건원보고 22) 에서유행한인플루엔자 B 바이러스의예방접종약제와의일치율도높았고, 약제내성도발견되지않았다. 이러한시기의일정한연령대의인플루엔자 B 바이러스감염소아를대상으로시행된저자들의연구는대부분입원후에 oseltamivir 투약을했기때문에약제에순응도가확인되고대상환자들이면역이나호흡기등의기저질환이없는환자들에서이루어졌다. 이는약제반응에대한다른변수의차이가적어 oseltamivir 의투여효과와관련된인자에대한기초정보를제공할것이다. 저자들의연구는전체환자들에서 oseltamivir 복용전치료군과비치료군간의임상적특성에서유의한차이가없었고 oseltamivir 군에서임상경과의유의한차이가없었다. 그러나투약시점에따른비교에서는발열시작 48 시간이내 osetamivir 의투여는발열기간에대하여 1.7 일감소를보여주었다. 향후, 이러한결과의확인과임상적용을위해서는감염여부에대한신속한검사를통한전향적연구를통해투약시기, 연령과다양한변수를포함하는연구가필요하다. 요 약 목적 : 본연구는인플루엔자 B 바이러스에대한 oseltamivir 의효능을알아보고자치료군과비치료군간의임 상적특징의차이를비교하였다. 방법 : 2012 년 1 월부터 2012 년 7 월까지입원하거나 외래진료실을방문하여인플루엔자 B 바이러스에감염된 소아환자를대상으로후향적으로의무기록을분석하였다. 결과 : 인플루엔자 B 바이러스에의한폐렴, 급성기관 지염, 급성세기관지염, 크룹으로진단된 72 명의평균나 이는 3.6±2.8 세였다. oseltamivir 치료군은 38 명 (53 %), 비치료군은 34 명 (47%) 이었고치료시기를고려하 지않았을때두군의입원기간 (4.18±2.10 일 vs 4.79± 1.49 일, P=.17), 총발열기간 (5.32±2.07 일 vs 6.41± 3.25 일, P=.09) 을포함한임상적양상에유의한차이가 없었다. 비치료군, 48 시간이내치료군, 48 시간이후치 료군간의비교에서임상적양상에유의한차이가없었 다. 3 세이하의환자들을대상으로두군간의비교에서 도유의한차이가없었다. 결론 : Oseltamivir 복용전치료군과비치료군간의임 상적특성에서유의한차이를나타내는항목이없어 oseltamivir 의적응증을도출하기어려웠으며치료후임상 경과의유의한차이도없었다. 그러나투약시점에따른 비교에서는발열시작 48 시간이내에 osetamivir 를투여 한군에서발열기간이감소하는경향을보여주었으므로 향후투약시기를고려하여연령별분석, 인플루엔자 B 바이러스종에대한분석을포함하는대단위연구가필 요하다.
