A-76-year old female patient who had pain on both hands due to osteoarthritis was treated with miniscalpel-acupuncture for 7 weeks. The main outcome were the Numeral Rating Scale (NRS) and the grip strength by dynamometer. After miniscalpel-acupuncture treatment, NRS was decreased and the strength of grasp was increased. The result of this study suggests that the treatment of miniscalpel-acupuncture may be effectively used to decrease pain and increase grip strength of patients who suffer from hand osteoarthritis. Further research is necessary by the reason of the limited sample size of this study. (J Korean Med Rehabil 2018;28(4): ) Key words Case report, Miniscalpel-acupuncture, Hand, Osteoarthritis, Acupotomy Copyright 2018 The Society of Korean Medicine Rehabilitation 서론»»» 손가락관절염 (hand osteoarthritis, Hand OA) 은손가락의관절에만성적인문제를일으키는질환으로 1), American College of Rheumatology (ACR) 의기준에따르면손가락관절의통증, 강직과함께지절관절 (interphalangeal joint, IP joint) 골조직의비후 (hard tissue enlargement) 와변형이나타나는경우 Hand OA로진단한다 2). 이학적진단이외에손가락관절염은 plain radiography 영상을통해함께진단하며, Kellgren과 Lawrence (K-L) 가제시한기준에따라네가지 grade로나누어진다 3). 2005년발표된연구에의하면네덜란드의 55세이상인구중여성의 67%, 남성의 55% 가영상의학적으로 Hand OA 소견을보였다 4). 이러한 Hand OA는통증뿐만아니라손의기능을감소시켜 일상생활을어렵게하고삶의질을저하시키기때문에치료가필요하다 5). 2018년발표된 European League Against Rheumatism (EULAR) 의가이드라인에의하면 6), Hand OA의치료로는운동요법, 보조기착용, 비스테로이드성항염증제 (non-steroidal anti-inflammatory drugs, NSAIDs) 도포, NSAIDs 복용, chondroitin sulfate 복용, 관절강내스테로이드주사, 수술이있었다. 가이드라인에따르면 NSAIDs 장기적인복용은소화기계, 심혈관계또는신장의문제를일으킬수있기때문에단기간복용만추천되었으며, 관절강내스테로이드주사요법과수술은특별한경우를제외하고는추천되지않았다. 한의학적치료에관한연구는침치료 7) 와화침치료 8) 의효과에대해서진행되었고, 그외의치료법에대한연구는보고되지않았다

2 정지철 윤상훈 김신애 도침은침끝에작은칼날이결합된침의일종으로, 연조직의유착 (adhesions) 과긴장 (contracture) 을완화시켜무릎관절염에효과가있다는연구가보고되었다 9,10). 저자들은약물치료를비롯한보존적치료에반응하지않던 K-L grade 4의 Hand OA 환자가도침치료후호전되어이를보고하고자한다. 증례»»» 1. 대상환자및현병력대상환자는시골에서농사를짓는 76세여성이다. 본증례는치료시행시연구출판및개인정보보호에관해고지하였고, 환자의동의를받아진행되었다. 우측엄지손가락의 IP joint, 우측두번째손가락의원위지절관절 (right 2nd distal interphalangeal joint, Rt. 2nd DIP joint) 과좌측두번째손가락원위지절관절 (left 2nd distal interphalangeal joint, Lt. 2nd DIP joint) 의수년전부터반복되는통증을주소로 2018년 1월 25일에본원에내원하였다. 과거력이나가족력에서특이점은없으며수십년동안농사일을반복하였다. 내원당시손가락관절의통증으로인해복용중인약물은없었다. 시진상환자양손의 1st IP joint, 2nd DIP joint, 3rd DIP joint와좌측의 4th DIP joint, 5th DIP joint에서골조직비대가나타났으며, 그중양손의 2nd DIP joint와좌측손의 5th DIP joint에 서는골관절의변형이나타났다 (Fig. 1). Plain radiography 상우측의 2nd DIP joint, 좌측의 5th DIP joint는관절간격이거의소실되고골극과함께분명한연골하골경화, 관절변형이관찰되어 K-L grade 4로진단되었으며, 좌측 1st IP joint와 2nd DIP joint는관절간격감소와연골하골경화, 골극이관찰되어 K-L grade 3로진단하였다 (Fig. 2). 그중통증을호소한관절은우측의 1st IP joint와 2nd DIP joint, 좌측의 2nd DIP joint였다. 세관절모두에서부종과압통이나타났으며굴국이제한되고근력이떨어졌다. 손가락의굴곡제한으로손을완전히쥐지못했으며, 근력이떨어져서농사일을하거나일상생활에서불편함을느낀다고호소하였다. ACR의 Hand OA 진단기준 2) 은손가락관절의통증과경직감과함께 1) 양쪽 2nd, 3rd IP joint와 1st metacarpophalangeal (MCP) joint 중 2개이상에서골조직의비후, 2) 2개이상의 DIP joint 에서골조직의비후, 3) 3개이하의 MCP 관절에서의부종, 4) 양쪽 2nd, 3rd IP joint와 1st MCP joint 중적어도 1개이상의관절변형 (deformity) 이나타나야한다. 네가지진단기준중세가지이상만족하면 Hand OA로진단되며, 본환자의경우이학적검사와 plain radiography모두에서위의조건네가지를모두만족하므로 Hand OA로최종적으로진단되었다. 또한혈액검사상 RA factor는음성으로나타나 rheumatoid arthritis는배제하였다. 시술 방법은 먼저 통증을 호소하는 손가락 관절의 배측 정중앙을 중심으로 그 주변을 압진하여 가장 압통을 심하 게 느끼는 지점에 아시혈을 찾아 외과수술용 펜으로 표시 Fig. 3. Miniscalpel-acupuncture treatment. 했다. 환자는 DIP joint의 신전 건 부착부에 가장 큰 압통 을 호소하였다.

R: Right. 2. 치료 치료 도구는 칼날 너비 0.5 mm, 길이 50 mm 규격의 1회 용 도침(Dongbang Dochim, Dongbang Medical, Boryeong, Korea)을 사용하였다. 시술은 총 7주간 주 1회의 빈도로 7회 실시하였으며, 1회 시술에 세 관절 모두 도침 치료를 시행하였다. 자침 부위는 환자가 통증을 가장 많이 호소 하는 DIP joint의 신전 건 부착부에 위치한 아시혈에 시 행되었다(Fig. 3). 7주간 도침 치료 이외에 침 치료나 약 물 치료, 물리 치료 등 다른 치료는 시행되지 않았다. 시술 방법은 먼저 통증을 호소하는 손가락 관절의 배측 정중앙을 중심으로 그 주변을 압진하여 가장 압통을 심하 게 느끼는 지점에 아시혈을 찾아 외과수술용 펜으로 표시 Fig. 3. Miniscalpel-acupuncture treatment. 했다. 환자는 DIP joint의 신전 건 부착부에 가장 큰 압통 을 호소하였다. 시술 부위에 수술포를 덮고 그 다음 요오 압박지혈과 함께 지혈밴드를 붙여주며, 환부가 오염되지 드스틱으로 시술 부위를 소독하고 시술하였다. 시술자는 않도록 교육하였다. 본 침 치료법 서술은 Standards for 1회용 마스크와 수술용 라텍스 장갑을 착용하였다. Changes in Numeral Rating Scale and Grip Strength Before and After Treatment Baseline 1 week 2 weeks 3 weeks 4 weeks 5 weeks 6 weeks 7 weeks NRS Rt. 1st IP Rt. 2nd DIP Lt. 2nd DIP Grip strength (kg) Rt. hand Lt. hand NRS: numeral rating scale, Rt.: right, IP: interphalangeal joint, DIP: distal interphalangeal joint, Lt.: left. 해악력을측정하였다. 악력은 Jamar 유압식악력계 (Jamar dynamometer, Lafayette Instrument Co., Lafayette, IN, USA) 를사용하여측정하였고, 정확성을높이기위해 3번측정하여측정값의평균값을기록하였다. 7주간의치료후세관절의 NRS는각각 6~8점에서모두 2점으로감소하였고, 악력의경우우측손은 15.3 kg에서 22.3 kg, 좌측손은 20 kg에서 24 kg으로각각증가하였다 (Table Ⅰ). 7주간치료후각관절의굴곡제한이호전되어손을꽉쥘수있었다. 부작용으로는도침치료후 1-2일간시술부위의통증을호소하는경우가있었으며, 통증은별도의처치없이자연호전되었다. 병력청취상도침시술후농사일을하였거나감기에걸렸을때통증이발생하였으므로, 도침시술로인한통증이손의과사용이나환자의몸상태에따라회복이지연되어통증이발생된것으로사료되었다. 그외에특별한부작용은관찰되지않았다. 고찰»»» 상기증례를통해변형을동반한 Hand OA에도침치료가통증완화와기능회복에도움이될수있다는것을확인하였다. 단일치료만시행하여도침치료만의효과를평가하였으며, 기타치료에반응하지않던환자의증상이호전되었다는점에의의가있다. Hand OA는노인인구에서흔한질환으로여성에서많고, 단일관절보다는다관절을동시에침범한다 4). 특히 Hand OA는최근 MRI 연구를통해병리기전이연구되고있는데, 측부인대의부종이연골하골의부종과연골손상 을촉발하며, 신근건의퇴행성변화는건부착부에영향을줘골극형성에기여하는것으로알려졌다 11,12). 이러한연조직의변화는관절에비가역적인변화를야기하며통증과기능저하를고착시킨다. 무엇보다손가락관절의통증이 2차적으로기능저하와악력의약화를일으키기때문에 13), 통증에대한치료가중요하다. 2018년에개정된 Hand OA의 EULAR recommendations 에따르면 Hand OA의치료는크게약물적치료와비약물적치료로나뉘고, 비약물적치료로는보조기착용, 운동등이있으며약물적치료로는 topical NSAIDs, oral analgesics, chondroitin sulfate, intra-articular injections, 수술요법이있었다 6). Topical NSAIDs 치료는통증완화와기능개선의효능이있으며, 위험대비효과가가장뛰어나추천되었다 14). 반면 oral analgesics 와 intra-articular injections는부작용이있고 placebo에비해효과가뛰어나지못해제한된경우에만허용되었다 15,16). 상술한보존적치료에도호전되지않으며관절의변형이관찰된경우에한해수술요법이추천되었지만, 통증과불안전성, 신경손상, 감염등의부작용이보고되었다 17). 상기환자는진통제및물리치료에도호전되지않았으며, 관절의변형이있었으므로수술적응증환자로볼수있다. 하지만수술에비해비교적위험이적은도침치료를통해호전반응을이끌어냈다는것이본연구의강점이라고할수있다. Hand OA에대한도침치료의효과가보고된바는없었으며, 무릎관절을대상으로몇몇연구가보고되었다. 2014년 Lin 등 18) 의연구에서, 무릎관절염의도침치료, 침치료군두군에대해치료전후 the synovial fluid의 interleukin 1 beta (IL-1β), interleukin 6 (IL-6), tumor necrosis factor-alpha (TNF α) 수치의증감 128 J Korean Med Rehabil 2018;28(4):

5 도침치료를통한손가락관절염치료 : 증례보고 과통증을비교하였고도침치료후 IL-1β, IL-6, TNF α 수치가감소되며침에비해증상완화효과가뛰어남을보고하였다. 2017년 Ding 등 9) 은동물실험을통해도침치료후손상된관절연골이회복되는것을관찰하였다. 2017년 MA 등 10) 은도침치료후관절연골이회복되는기전이 FAK-PI3K Signaling Pathways와연관이있을수있다는동물실험을보고하였다. 상술한세연구에서도침치료는경직된관절주변의연조직을이완시켜연골, 인대, 관절낭등관절주변조직의회복을촉진하는것으로고찰하였다 9,10,18). 본증례의치료점인 DIP joint의신전건부착부의연조직변화는 Heberden s nodes 와골극형성에관여하여손가락관절염의진행에서중요한역할을하고있다 11). 본증례에서도침치료를통해신전건부착부를자극해재생을유도하고, 자락을통해염증물질을배출시켜통증이완화된것으로추정된다. 그러나기존생리적연구는무릎관절염에국한된연구로, Hand OA에도침치료가미치는영향에대해서는독립적인연구가필요할것으로보인다. 본연구의가장큰한계는단일증례보고라는것이다. 이를통해도침치료가 Hand OA 개선에도움이될수있다는가설은제시했지만, 증례가부족하고대조군이없으므로치료에충분한근거가되지못한다. 또한치료종결후에지속적인경과관찰을하지못하여장기적인치료효과를살피지못하였다. 마지막으로치료는도침치료만을시행하여교란인자를최소화하였지만, 기타일상생활이나노동등통증에영향을미칠수있는인자들을제대로통제하거나, 노동의양등을객관적으로평가하고수치화하지못하였다. 본증례에서치료후증상이호전되었다가오히려통증과악력이악화되는경우가있었는데, 이러한반응은치료후환자가손가락관절을많이사용한경우나타났다. Hand OA는반복적으로손을과사용하는경우증상이악화되는것이기존에보고되었으므로 19), 추후연구에서는결과에영향을줄수있는노동과같은인자는체계적인평가와기록이필요할것으로사료된다. 결론»»» 단일증례보고라는한계점과손의반복적인사용과같이결과에영향을미치는교란인자를차단하지못했다는 한계점이있지만, 본증례는수술적응증에해당하는 Hand OA에대해도침의단일중재로증상과악력을호전시켰다는점에서의미가있다. 또한 Hand OA는높은유병률에비해한의학연구가부족한질환으로추후지속적임상연구가필요할것으로사료된다.
