The purpose of this study was to compare the levels of serum IL-31 and 25 (OH) D between the allergic rhinitis (AR), nonallergic rhinitis (NAR), and control groups and to investigate the relationship between IL-31 and 25 (OH) D. Methods: We recruited 59 children with only rhinitis and 33 controls without any allergic diseases. Serum IL-31 and 25(OH) D levels were assayed using an enzyme-linked immunosorbent assay and high-performance liquid chromatography, respectively. The patients were considered to have atopic sensitization if the levels of serum specific IgE to inhalant allergens as assessed using immunocap were 0.35 IU/mL or if they tested positive for one or more allergens by the skin prick test. Results: Of children with rhinitis, 25 had nonatopy (NAR), and 34 children had atopy (AR). Serum 25(OH) D levels were significantly lower in the rhinitis group than in the control group, while there was no significant difference serum 25(OH) D levels between the AR and NAR groups. Children with rhinitis demonstrated higher serum IL-31 levels than controls; however, there was no difference in serum IL-31 levels between the AR and NAR groups. Serum 25(OH) D levels were inversely correlated with serum IL-31 levels and blood eosinophil counts. On the other hand, serum 25(OH) D levels were not correlated with total serum IgE levels. Conclusion: Serum 25(OH) D and IL-31 may play a role in the pathogenesis of rhinitis via mechanisms other than IgE-related pathway. Relationship of serum vitamin D and IL-31 to rhinitis in children 알레르기질환과의연관성에관한수많은연구들이진행되고있다. 5 특히알레르기질환중아토피피부염, 알레르기천식과비타민 D와의관련성에관한많은연구들이있는데일부상충되는결과들을보고하고있는연구들도있지만, 대부분의연구에서비타민 D 결핍이아토피피부염, 알레르기천식과관련성이있는것으로보고하였다. 6,7 알레르기비염과의관련성에관한연구들은아토피피부염및알레르기천식과의관련성에대한연구에비해많이시행되지않았지만몇몇관련성이없다는연구보고뿐만아니라 8,9 관련성있다는연구보고들도있어서 아직은더많은연구가필요할것으로생각한다. Interleukin-31 (IL-31) 은네개의나선다발구조로된사이토카인으로활성화된 CD4+ T 세포, 비만세포, 단핵구, 대식세포, 수지상세포들과같은여러종류의세포에서분비된다. 13 IL-31 은다형핵세포, 단핵구와 T 세포등을끌어들이는염증촉진사이토카인중하나로 T-helper (Th) 2 반응의중요한조절자역할을하는것으로알려져있다. 14 알레르기질환과 IL-31의관련성은아토피피부염에서주로연구가되었다. IL-31 을과발현시킨형질전환쥐에서쥐의긁는행동이증가하고, 심한피부염이발생하는것을보고한연구가있었고, 15 아토피피부염환자의피부병변에서 IL-31 발현이증가되고 staphylococcus 초항원에의해 IL-31 발현이급속히증가하며, 16 아토피피부염의중증도가증가할수록혈중 IL-31 의농도도유의미하게높게나타난다는연구결과가있었다. 17 이에반해 IL-31 과비염과의상관성에관한연구는드문편인데한알레르기비염환자를대상으로하는연구에서정상대조군과비교하여 IL-31 의농도의유의미한차이가없다는결론을도출해냈으나 14 알레르기비염환자를대상으로하는몇몇다른연구에서는 IL-31 의농도가알레르기비염환자에서정상대조군에비해유의미하게높게나타나는상반되는결과를보여 13,18,19 알레르기비염에서의 IL-31 의역할에대한더많은연구가필요할것으로보인다. 지금까지알레르기질환에서 IL-31 과비타민 D는각각따로연구되었고 IL-31 과비타민 D를같이연구한경우는거의없었다. 정상대조군에서는 IL-31 과비타민 D가양의상관관계를보이지만알레르기질환에서는상관관계가없었다는연구결과가있었고, 18,20 반면저자들의이전연구에서는아토피피부염환자에서음의상관관계를보였다. 17 또한지금까지아토피피부염및알레르기천식환자들에서비타민 D와 IL-31 에대한연구들이많았지만알레르기비염과의관계에대한연구결과는부족한실정이며, 특히알레르기비염뿐만아니라비알레르기비염과의관계를알아보는연구는거의없었다. 저자들은혈청비타민 D와혈청 IL-31 의농도를비염과정상군간에, 또한알레르기비염과비알레르기비염사이에차이가있는지비교해보고비타민 D와 IL-31 간의상관관계를분석하고자하였다. 대상및방법 1. 대상 2014년 6월부터 2015년 8월까지강북삼성병원소아청소년과알레르기클리닉을방문한비염의증상과징후가있는환아 59명과같은기간내원한알레르기질환의증상, 징후및과거력이없고비염환아와나이와성별이맞는 33명의대조군을대상으로전향적으로시행되었다. 비염군은각각알레르기비염과비알레르기비염군으로분류하였고, 알레르기비염의진단은맑은수양성콧물, 후비루, 재채기, 코막힘, 코가려움증등의비염의특징적인증상을보이면서, 얼굴에특징적인징후인알레르기샤이너 (allergic shiner), 수평코주름 (horizontal nasal crease), 알레르기경례 (allergic salute), 결막염등을보이거나비점막이창백하고부어있는소견을보이고, 혈청특이 IgE 항체검사나피부반응검사 (skin prick test) 에서하나이상의흡입항원에양성인경우로정의하였다. 2 비알레르기비염은비염의특징적증상이있으면서혈청특이 IgE 항체검사나피부반응검사에서모두음성으로나온경우로정의하였으며, 감염, 약물, 호르몬, 식품등의원인이있는경우는대상에서제외하였다. 2 이렇게분류된알레르기비염군과비알레르기비염군, 정상대조군사이의 IL-31, 25(OH) D, 혈청총 IgE 농도와총호산구수 (total eosinophil count, TEC) 를비교하였다. 이연구는부모혹인보호자의동의를얻어시행되었고, 강북삼성병원연구윤리위원회의심사 ( 승인번호 : ) 를받았다. 2. 혈청 IL-31, 비타민 D, 혈청총 IgE 농도, 항원-특이 IgE 농도및총호산구수측정비염군과정상대조군에서정맥혈을채취하여원심분리후 (3,000 rpm, 15분 ) 혈청을영하 20 C에서보관하였다가일시에 IL-31과비타민 D를측정하였다. 비타민 D는 25(OH) D 농도를 high-performance liquid chromatography (Neodin, Seoul, Korea) 로측정하였고, IL-31 농도는 enzyme-linked immunosorbent assay kit (R&D systems, Minneapolis, MN, USA) 를이용하여측정하였다. 혈청총 IgE와항원-특이 IgE 농도는 ImmunoCAP (Pharmacia Diagnostics, Uppsala, Sweden) 을이용하여측정하였다. 흡입항원은 Dermatophagoides farinae, cat epithelia, dog epithelia, Alternaria, tree mix, weed mix를이용하였고 0.35 IU/mL일때양성이라고판정하였다. 피부반응검사는 16가지흡입항원 (Dermatophagoides farinae, Dermatophagoides pteronyssinus, cat epithelia, dog epithelia, Alternaria, Aspergillus, birch, alder, hazel, pine, rye grass, Japanese hop, timothy, orchard, ragweed, and mugwort) 에대해시행하였고, 생리식염수와히스타민을각각음성대조와양성대조로사용하였다. 판독은 15분후경결의크기가 3 mm 이상인경우양성으로판정하였다. 총호산구수는자동화 42

3 박성준외 혈청비타민 D 및 IL-31 수치와비염과의관계 된분석기기를통해측정하였다. 3. 통계분석통계분석은 STATA ver (StataCorp LLC., College Station, TX, USA) 를이용해분석이이루어졌으며두군간의차이는 t-test 혹은 chi-square test를이용하였고, 세군간의평균값비교는 oneway analysis of variance 를이용하였다. 연속변수들은평균 ± 표준편차로표현하거나, 정규분포를따르지않을때는로그치환하여분석하였다. 상관관계의분석은 Spearman correlation test를이용하였으며, P-value 가 0.05 미만일때통계적으로의미있다고해석하였다. 결과 1. 대상자특성연구에참여한총대상자 92명 ( 남자 45명, 48.9%) 중알레르기비염군은 34명 ( 남자 16명, 47.1%), 비알레르기비염군은 25명 ( 남자 12 명, 46.2%), 정상대조군은 33명 ( 남자 17명, 51.5%) 이었다. 알레르기비염군, 비알레르기비염군과정상대조군의평균나이는각각 6.0 ±1.3세, 5.5±2.1세, 5.3±2.5세였으며세군간의성별, 평균나이에통계적으로유의한차이는없었다. 알레르기비염의가족력은알레르기비염군에서 62.5%, 비알레르기비염군에서 60.9% 로정상대조군 12.9% 보다높은빈도를보였고, 알레르기비염군과비알레르기비염군사이에는차이가없었다 (Table 1). 2. 비염군과정상대조군의 logige 값과 logtec 값의비교비염군과정상대조군의 logige 값은각각 4.9±1.4 IU/mL, 3.8 ±1.4 IU/mL로비염군에서높았으나통계적으로유의한차이는없었고 (P = 0.06, Table 2), logtec 값은비염군에서 5.9±0.8/μL, 정상대조군에서 4.8±1.1/μL 로정상대조군과비교하여비염군에서통계적으로유의하게높았다 (P = 0.00, Table 2). 3. 알레르기비염군과비알레르기비염군의 logige 값과 logtec 값의비교알레르기비염군과비알레르기비염군의 logige 값은각각 5.8± 1.2 IU/mL, 3.7±0.7 IU/mL로알레르기비염군에서유의하게높았다 (P= 0.00, Table 3). logtec 값은알레르기비염군에서 6.3±0.5/μL, 비알레르기비염군에서 5.4±0.8/μL로알레르기비염군에서유의하게높았다 (P = 0.00, Table 3). 4. 비염군과정상대조군의혈청비타민 D와 IL 31 농도의비교비염군과정상대조군에서의 logvitd 값은각각 2.7±1.3 ng/ml 과 3.4±0.9 ng/ml로비염군에서정상대조군에비해통계적으로 Table 1. Characteristics of study subjects Characteristic 유의하게낮았다 (P = 0.03, Table 2). logil-31 값은비염군에서 6.9 ±1.2 pg/ml, 정상대조군에서는 5.0±2.1 pg/ml 로비염군에서 통계적으로유의하게높았다 (P = 0.04, Table 2). 5. 알레르기비염군과비알레르기비염군의혈청비타민 D 와 IL 31 농도의비교 Allergic rhinitis (n= 34) Nonallergic rhinitis (n= 25) 알레르기비염군의 logvitd 값은 2.6±1.3 ng/ml 이었고, 비알레 르기비염군은 2.7±1.3 ng/ml 으로두군간의통계적으로유의한 차이는없었다 (P = 0.89, Table 3). 또한알레르기비염군의 logil-31 값은 6.3±1.1 pg/ml, 비알레르기비염군의 6.4±1.5 pg/ml 와비교 하여유의한차이는없었다 (P = 0.80, Table 3). There were significant negative correlations between logvitd and logil-31 levels and between logvitd and logtec levels. Levels of logvitd showed no correlation to logige levels. LogTEC, logarithmic transformation of blood total eosinophil count; LogIgE, logarithmic transformation of serum total IgE; LogVitD, logarithmic transformation of serum vitamin D; LogIL-31, logarithmic transformation of serum IL 혈청비타민 D 농도와혈청총 IgE 농도, 총호산구수, 혈청 IL 31 농도와의상관관계혈청 logvitd 농도와 logil-31 농도는음의상관관계를나타냈고 (r = 0.59, P = 0.00), 혈청 logvitd 농도와 logtec 간에도음의상관관계를보였다 (r = 0.27, P = 0.01) (Fig. 1A, B). 한편, 혈청 logvitd 농도와혈청 logige 값은상관관계가없었다 (r = 0.10, P = 0.40) (Fig. 1C). 고찰이연구에서는소아비염환자에서비타민 D 수치가정상대조군에비해유의하게낮았고, IL-31 수치는유의하게높음을알수있었는데, 이는비슷한결과를보고한국내외다른연구들을통해서도확인할수있다. 캐나다의소아들을대상으로한연구에서는알레르기비염환자들이건강한아이들과비교하여비타민 D 결핍이더많다고보고하였고, 10 이란소아들을대상으로한연구에서비슷한결과를보고하였다. 11 국내소아알레르기비염환자에서비타민 D 수치가건강한아이들에비하여유의하게낮았다고보고한연구가있었다. 12 비타민 D는이미언급한대로많은연구들을통해알레르기질환과의관련성을인정받고있는데비타민 D가알레르기비염의발병에관여하는기전은아직불명확하다. 한연구에서는선천면역계에서단핵구의 Toll-like receptors (TLRs) 와같은, 선천면역반응을활성화시키는수용체를억제하여 TLR 매개염증반응을감소시키고, 피부나폐의상피세포에존재하는내인성항균성펩타이드를생성시켜환경적인항원에대한장벽을강화시키며, 후천면역계에서 T 세포의분화와염증을촉진하는사이토카인의분비를억제하고, IL-10 의생성촉진및 IL-2의생성을억제함으로써조절 T 세포의면역관용상태를유도하는것으로비타민 D의역할을설명하였다. 21 하지만이는알레르기비염에국한되는가설로비알레르기비 염과비타민 D의관련성유무및역할에대한연구는거의없었다. 이연구에서는알레르기비염뿐만아니라비알레르기비염군도비타민 D와관련이있다는사실을확인하였다. 이를통해비타민 D 가알레르기비염및비알레르기비염에관여하는기전이 IgE 매개반응이아닌다른기전에의할가능성이있음을알수있었다. 비타민 D와총 IgE 농도의상관관계를분석하였을때도상관관계가없다는결과가도출되어저자들의추정을뒷받침하였다. 이와같은결과는터키의소아들을대상으로하는연구에서도확인할수있는데, 이연구에서는알레르기비염군에속한아이들에서건강한아이들에비해비타민 D가유의하게낮았다고보고하였을뿐만아니라, 비알레르기비염군에속한아이들에서도건강한아이들에비해비타민 D가유의하게낮았으며, 알레르기비염군과비알레르기비염군간의비타민 D 수치의유의한차이는없다고보고하여이연구와비슷한결과를도출해냈다. 1 하지만아직비알레르기비염과비타민 D의관계에관한연구들이거의없는실정으로앞으로비타민 D의알레르기비염뿐아니라비알레르기비염에작용하는병태생리학적기전을밝히기위해이에대한추가적인연구가더필요할것으로생각한다. 한편 IL-31과알레르기질환과의관련성에대한연구들도많이있었는데, 한알레르기비염환자를대상으로하는연구에서정상대조군과비교하여 IL-31 의유의미한차이가없다고보고하기도하였지만 14 이탈리아의한연구에서는건강한아이들과비교하여알레르기비염과알레르기천식을가진환자들에서혈청 IL-31 이더높게측정된사실을보고하였고, 18 중국의한연구에서는알레르기비염환자들의혈청및코세척액에서측정한 IL-31 의농도가중국의건강한아이들에비해높게측정된사실을보고하였다. 13 마찬가지로독일의한연구에서는 11명의알레르기비염환자를환자를대상으로알레르기항원을코에주입한이후시간에따른콧물에서의 IL-31 농도의측정및증상발현정도와 IL-31 농도와의관련성을조사하였고, 결과적으로콧물에서의 IL-31 농도의증가및 IL-31 농 44

5 박성준외 혈청비타민 D 및 IL-31 수치와비염과의관계 도와증상의정도가양의상관관계를갖는다고보고함으로써 22 알 레르기비염에서 IL-31 이어떤역할을담당할것이라는가설에무게 감을실었다. 이러한연구들과이연구를종합하여볼때 IL-31 이비 염과관련성이있는사이토카인일가능성이높음을확인할수있 었고, 비타민 D 와마찬가지로 IL-31 농도역시알레르기비염군과 비알레르기비염군간의유의한차이를보이지않아 IL-31 도비타민 D 처럼 IgE 매개반응을제외한다른기전으로비염에영향을미칠 것으로추정하였다. 마지막으로이연구의저자들은비타민 D 와 IL-31 사이의관계 를알아보기위해상관관계분석을시행하였는데, 그결과둘은음 의상관관계를나타내는것으로나타났다. 이는비타민 D 와 IL-31 사이의관계를분석하여비타민 D 가 IL-31 과무관하게알레르기비 염및아토피피부염에영향을미친다는결론을도출한몇몇연구 들과는상반되는결과로 18,23 비타민 D 와 IL-31 의알레르기질환에 서의상관관계를더명확히밝히기위해서추후더많은연구가필 요할것으로보인다. 이번연구저자들은국내소아청소년에서혈청비타민 D 수치및 IL-31 수치를비염환자와정상소아를대상으로비교해보았다. 이 번연구는지금까지시행된연구와는다르게대상을알레르기비염 환자뿐만아니라비알레르기비염환자까지확대하였고, 비타민 D, IL-31, IgE 농도등다양한지표를동시에조사하여각변수간의상 관관계를분석하는등여러가지의미있는결과를도출한점이강 점이라생각된다. 이연구의제한점으로는우선대학병원을방문한 환자들을대상으로하여국내의전체소아비염환자군을대표하 기는어렵다고생각되는점, 둘째, 대상자수가적어더많은대상자 에서연구가필요할점, 셋째, 비타민 D 농도에영향을주는각개인 의신체활동, 음식물섭취등에대한조사가없었다는점이라생각 한다. 결론으로저자들은소아비염환자들에게서정상대조군에비 해혈청비타민 D 수치가낮고, IL-31 수치가높음을확인할수있 었고, 알레르기비염과비알레르기비염두군간의비교에서는비타 민 D 및 IL-31 의유의미한차이는없다는것을알수있었다. 이러한 결과를통하여비타민 D 부족이비염환자들에게영향을미칠수 있다는것을추정할수있었고, 비타민 D 및 IL-31 의비염에대한보 다정확한병태생리를알아내기위해알레르기비염뿐만아니라비 알레르기비염환자들대상으로하는연구및비타민 D 와 IL-31 사 이의관계를알아보기위한추가적인연구들이필요할것으로생 각한다.
