1 대한의진균학회지제 8 권제 4 호 2003 초청원고 가천의대길병원감염내과 조용균 =Abstract= Systemic New Antifungal Agents Yong Kyun Cho Division of Infectious Diseases, Gil Medical Center, Gachon Medical School, Incheon, Korea Voriconazole is a second-generation triazole that has an enhanced antifungal spectrum, compared with older triazoles. It will likely become the drug of choice for treatment of invasive aspergillosis and many Scedosporium/Pseudallescheria and Fusarium infections. Voriconazole should not replace fluconazole or other antifungal agents for treatment of most Candida infections. The drug has more side effects and drug interactions than fluconazole. The oral formulation, with its excellent bioavailability, is available it is especially beneficial in patients with renal failure, who should not be exposed to the cyclodextrin vehicle used for the intravenous formulation. Caspofungin, the first inhibitor of fungal β -1,3 glucan synthesis, is effective for the treatment of mucosal and invasive candidiasis and invasive aspergillosis. It is also active in vitro and in animal models against a number of other filamentous and dimorphic endemic fungi and in animal models of Pneumocystis carinii infection. Caspofungin has an excellent safety profile. Caspofungin may prove to be useful in empirical therapy for suspected invasive fungal infections. Additional clinical trial data that expand our knowledge of the usefulness of caspofungin for these and other mycoses is anticipated. The broad spectrum antifungal itraconazole is an effective and well tolerated agent for the prophylaxis and treatment of systemic fungal infections. The recent development of an itraconazole oral solution and an intravenous itraconazole solution has increased the options for the use of this drug. Intravenous itraconazole solution is at least as effective as intravenous amphotericin B in the empirical treatment of neutropenic patients with systemic fungal infections, and drug-related adverse events are more frequent in patients treated with amphotericin B. A large proportion of patients with confirmed aspergillosis also respond to the treatment with intravenous itraconazole followed by oral itraconazole. Liposomal nystatin is another promising antifungal agent that might be effective for treatment of invasive candidiasis and invasive aspergillosis. More clinical data, however, is needed for clinical application. 2 대한의진균학회지제 8 권제 4 호 2003 최근항암요법, 골수이식과각종장기이식의증가와면역억제제사용의증가로인해진균감염의기회가증가하고있다 1. 대부분의진균감염은 Candida나 Aspergillus spp. 에의해발생하고 2. 사망률은높은경우에침습적 candidiasis 이 50%, 침습적 aspergillosis 가 90% 에이른다. 침습적진균감염은높은사망률과이환률을가진질환이지만최근까지폴리엔화합물인 amphotericin B가유일한치료제였다. Amphotericin B는효과적인광범위항진균제이지만신독성을포함한전신부작용이심해안정성에문제가있다. 1990년대에 fluconazole 과 amphotericin B의지질복합체가개발되었으나 fluconazole 은침습적 aspergillosis 나많은 non-albicans Candida 감염에효과가없고 3 amphotericin B의지질복합체도부작용을줄이기는했으나치료율을크게향상시키지는못했다 4. 따라서높은효과와낮은부작용을가진새로운항진균제에대한필요성이상존하고원인미생물의진단과그특성에대한지식이축적되면서항진균제는매우빠른속도로개발되고있다. 본강의에서는최근 FDA의인가를받았거나받을예정인새로운항진균제를중심으로검토할것이다. 1. Voriconazole 2세대 azole로진균의세포막성분중 major sterol component 인 ergosterol 의합성을억제한다. 1세 Table 1. 3 조용균 : 새로운항진균제 대 azole 약물인 fluconazole 로부터 triazole ring 중의하나를 fluorinated pyrimidine 으로대체하고 α- methyl group을추가하여합성하였다. 1) 작용기전진균의 cytochrome P450 (CYP 450) 의존성 14α -lanosterol demethylation 을방해하여세포막내의 ergosterol 의합성을억제한다. Yeast 에대해정균성 (fungistatic) 을보이고일부 mold에대해살균성 (fungicidal) 이다 5. 이런살균력은 voriconazole 이 yeast보다 mold의 14α-lanosterol demethylase 에대해강한결합력을가지고있고따라서 ergosterol 합성을더욱완벽하게억제할수있기때문이다. 2) 항진균범위 Yeast 와 mold에광범위한항진균력을보인다. fluconazole 내성인 C. krusei 와 C. glabrata를포함한모든 Candida spp. 에효과가있다 ( 표 1). Fluconazole 감수성 C. albicans보다일부 fluconazole 내성 C. albicans의 voriconazole MIC가높고다른종에비해 C. krusei와 C. glabrata의 MIC가높지만모두감수성범위에포함된다. 그외에 Cryptococcus neoformans나 Trichosporon beigelii, Saccharomyces cerevisiae 와같은 yeast 도감수성을보인다 6,7. Voriconazole 은 amphotericin B에내성을보이는 A. terreus를포함해대부분의 Aspergillus spp. 에대해효과적이다 ( 표 2). Aspergillus spp. 의시간사멸곡선 (time-kill curve) 을보면 voriconazole 은 ampho- Table 2. 4 대한의진균학회지제 8 권제 4 호 2003 tericin B에비해서는낮으나 itraconazole 에비해서는우월한용량의존성살균력을보인다 5. 그외 Blastomyces dermatitidis 와 Histoplama capsulatum에도효과적이며 amphotericin B에내성인 Scedosporium apiospermum, Fusarium spp., Paecilomyces spp., Bipolaris spp., Alternaria spp. 도감수성을보인다. 반면에 zygomycetes 에는효과가없다. 3) 약동력학경구용과정맥주사용모두가능하며경구투여로도높은흡수율을보인다. 정맥주사용은 sulfobutyl ether β-cyclodextrin sodium(sbecd) 에용해시킨것으로 1~2 시간에걸쳐주입한다. 3~6 mg/kg 하루에두번주사하면 3~6 μg/ml의항정상태 (steadystate) 혈중농도를유지할수있다. 12시간마다 6 mg/kg로 2번투여후유지용량으로 12시간마다 4 mg/kg로투여하는것을권장한다. 경구용은식전또는식후 1시간에투여하면 90% 이상의생체흡수율을보이며지방식은흡수율을 80% 이하로감소시킨다. 200mg 을하루에두번복용하면 2~3μg/mL 의항정상태 (steady-state) 혈중농도를유지할수있다. 체중이 40kg 이상이면 12시간마다 200mg, 40kg 이하인환자는 12시간마다 100mg 복용하고, 두배의용량을첫날복용하면항정혈중농도에 1일만에도달한다. 혈중에서 58% 의 voriconazole 이단백결합을하고용적분포 (volume of distribution) 가크다. CSF 농도는혈중농도의약 50% 에달하며뇌조직의농도는 CSF보다높다. 5% 미만의약물이대사되지않고소변으로배출된다. 주로간에서 CYP 450 효소군에의해대사되며대사산물은항진균효과를상실한다. Voriconazole 의주요대사과정인 CYP2C19 pathway 의활성도는유전적특성에좌우되므로, 인구의약 20% 가이효소의낮은활성도를가진동양인 (poor metabolizer) 의혈중농도는높은활성도를가진사람에비해 4 배이상높다. 이경우에는간독성부작용이발생할가능성이높으므로간기능검사와주의관찰이필요하다. 간질환이있는환자는용량을줄여야하며부하용량은유지하고유지용량을반으로줄인다. 중증간질환환자에서의안정성은아직연구가없다. 신부전환자에서경구용 voriconazole 의용량은줄일필요가없으나정맥주사용 voriconazole 은부형제인 SBECD 가축적되므로크레아티닌청소율이 50 ml/min 인경우에는정맥주사제를피한다 8. 4) 약물상호작용간에서 CYP450 효소군에의해대사되므로약물상호작용의가능성이높다 9. CYP450 유발인자인 rifampin, long-acting barbiturates, carbamazepine 은 voriconazole 의혈중농도를낮춘다. 같은 CYP450 유발인자인 phenytoin 은 voriconazole 의혈중농도를낮추므로함께투여할때에는 voriconazole 의용량을 2배로증량해야한다. 하지만 phenytoin 이대사되는 CYP2C9 효소에 voriconazole 이경쟁적으로결합하여 phenytoin 의혈중농도를높이므로두가지약물을동시투여할때는 phenytoin 의혈중농도를모니터해야한다. Voriconazole 은 ergot alkaloids 나 terfenadine, astemizole, quinidine, cisapride 와같은약물의대사효소인 CYP3A4 와 CYP2C9 를억제하여이약물의혈중농도를독성을유발할수있으므로동시투여를피한다. 마찬가지로 tacrolimus, cyclosporine, warfarin 의용량을줄이는것이필요하다. Statins 이나 benzodiazepines, calcium channel blockers, sulfonylureas, proton pump inhibitors, vinca alkaloids 등도조심해서투여한다. 그러나cimetidine, ranitidine, digoxin, indinavir, macrolides, prednisolone 은용량조절이필요없다. 5) 부작용가장흔한부작용은일시적인시각장애 ( 광시증, photopsia) 로높게는 30% 의환자에서나타나지만대부분이것때문에약을중단하는경우는없다 8,10. 색분별의장애, 흐려보임 (blurred vision), 빛나는얼룩이나움직이는선모양의출현, 눈부심 (photophobia) 등이주된증상이다. 주로투여 1주일내에발생하고계속투여해도대부분증상이호전된다. 증상이심한경우에는약을중단하면정상화되고망막의영구적손상은없다. 피부발진이두번째로흔한부작용이다. 대부분은경미한부작용이지만드물게 Stevens-Johnson syndrome 이나 toxic epidermal necrolysis 와같은중증부작용도보고되었다. 광과민반응 (photosensiti

5 조용균 : 새로운항진균제 vity reaction) 이생길수있으므로직접적인햇빛노출은피해야한다. 이런부작용은투여를중지하면사라진다. 용량에비례해서간기능이상이발생하지만대부분무증상이고드물게치명적인간손상이올수있다. Voriconazole 투여환자는 2-4주간격으로간기능검사를시행한다. 드물게두통이나구역질, 구토, 설사, 복통, 환시등이올수있다. 6) 임상적이용 (1) Aspergillosis Voriconazole 은 amphotericin B와의대규모무작위대조군임상시험연구에기반하여침습적 aspergillosis 에대한치료제로인정되었다 11,12. 초기비대조군연구에서는 Voriconazole 을초치료제로사용한경우완전관해또는부분관해를보인환자는 59%, 다른항진균제의치료실패나부작용으로인한구제치료제 (salvage therapy) 로사용한경우는 38% 의반응률을보였다. 이는과거대조군 (historical control) 에비해동등하거나우수한반응률이다. Voriconazole 과 amphotericin B와의비교연구에서는반응률이각각 53% 와 32%, 생존률이각각 71% 와 58% 로 voriconazole 이더우수한치료효과를나타냈다. (2) Pseudallescheria/Scedosporium 감염증우리나라에서는드물지만스페인이나호주에서호발하고골수이식후발생하는 Pseudallescheria 감염증과 Scedosporium 감염증에효과가입증되었다. 이들감염증은 amphotericin B에내성을보이고다른항진균제도효과가낮은것으로알려져있다. 36예의 Scedosporium 감염증환자에대한연구에따르면 S. apiospermum감염증은 63%, S. prolificans 감염증에대해서는 29% 의반응률을보였다 13. (3) Fusarium 감염증다른항진균제에효과가없거나부작용이심한 Fusarium 감염증의치료제로 voriconazole 이허가되었다. 다른 mold 진균보다는높지만 itraconazole 에대한 MIC에비해 voriconazole 의 MIC가매우낮고 21명의 fusariosis 환자중에 9명 (43%) 의치료율 ( 완전관해와부분관해포함 ) 을보였다. (4) Candida 감염증 AIDS 환자의 esophageal candidiasis 에서 fluconazole에 비해치료효과의차이가없었다 14. Fluconazole에내성인 12명의 AIDS환자 esophageal candidiasis 를치료한결과 2명을제외하고완치되거나호전되었다는보고가있다. 다른 Candida 감염증에대한연구는현재진행중이다. (5) Febrile neutropenic patient 의경험적치료 Febrile neutropenic patient 의경험적치료제로 voriconazole 과 liposomal amphotericin B를비교한연구 15 에서 voriconazole 그룹이 liposomal amphotericin B 그룹에비해전체적인반응률과생존률은낮았지만통계적인차이를보이지는않았다. 단치료중진균감염 (breakthough fungal infection) 의발생률은 voriconazole 그룹에서유의하게낮았다. 그러나연구설계상의문제등으로 voriconazole 은 febrile neutropenic patient 의경험적치료제로 FDA의허가를받지못하였다. (6) Cryptococcosis Voriconazole 은 C. neoformans 에대한시험관내효과가뛰어나고뇌척수액농도가높아 cryptococcal meningitis 의치료제로적합하리라생각되지만충분한임상연구결과가없어아직까지는확립된적응증이아니다. 진행성 AIDS 환자에서 fluconazole 내성인재발성 cryptococcal meningitis 유지요법으로 voriconazole 을투여하여실패한증례보고가있었다 Caspofungin Echinocandin lipopeptide 계항진균제로 1,3 β -D-glucan synthase 를비경쟁적으로억제함으로써세포벽합성을저해한다. 1,3 β-d-glucan synthase 억제제는 acidic terpenoids 와 papulocandins, echinocandin 등이있으며그중에 caspofungin 이최초로개발된약제이다. Glarea lozoyensis 의발효산물인 pneumocandin B0로부터반합성된유도체로수용성 amphipathic lipopeptide 이다. 1) 작용기전 Glucan 은진균세포벽의주요구성성분으로 UDP-glucose 로부터 β-1,3 glucan이합성될때필요한효소가 glucan synthase(udp-glucosyl-transferase) 이다. Caspofungin 은 glucan synthase 을억제해서진균의세포벽합성을저해하고삼투압을유지못

6 대한의진균학회지제 8 권제 4 호 2003 하는진균은용해된다. 사람의세포막성분에도작용하는amphotericin B와같은 polyene 계열의약물과달리 glucan synthase 는포유동물의세포막에는극소수만이존재하므로 caspofungin 은사람의세포에는부작용이매우작고진균에만효과를가진다. 2) 항진균범위대부분의 Candida spp. 에대해살균력을가진다 ( 표 3). 대부분의 Candida spp. 는 azole내성여부나내성기전에관계없이시험관내감수성이있으며 amphotericin B에내성인 C. lusitaniae 에대해서도효과적이지만 C. guilliermondii 에는살균력이떨어진다. Biofilm 에서자라는 C. albicans나 C. parapsilosis에대해서도효과가있다 17,18. C. albicans, C. glabrata, C. tropicalis, C. krusei와 C. neoformans 에대해 fluconazole 과병합했을때길항혹은상승작용이없고 amphotericin B와병합했을때 C. albicans 에대한길항작용도없다. C. parapsilosis 에대해서는 fluconazole 이나 voriconazole 과병합하면상승작용이있으나 amphotericin과병합하면상승작용이없다. 반면에 fluconazole은 biofilm 에서자라는 C. albicans에대한 caspofungin 의효과를저해한다. 동물실험에서 caspofungin 은 disseminated cryptococcosis 에대해효과가없었다. 그러나실험실적연구에서 caspofungin 과 amphotericin B를병합하면 C. neoformans 에대한상승작용을보였다. 또한 caspofungin 과 fluconazole 을병합하면진균의종류에따라상승혹은부가작용을가진다. 실험실적연구에서 caspofungin 은모든 Aspergillus 종에대해감수성을가지며 Itraconazole 내성 strain에대해서도효과적이다 19. 동물실험에서도 disseminated aspergillosis 와 pulmonary aspergillosis 를가진쥐의생존률을증가시켰다. Caspofungin 과 amphotericin B의시험관내병합시에 Aspergillus spp. 에대해상승혹은부가작용이있다. Voriconazole 이나 itraconazole 과병합해도대부분상승효과가있다. 동물실험에서다른 filamentous and dimorphic endemic fungi와 Pneumocystis carinii 감염에도효과적인것으로확인되었다. 3) 약동력학경구투여로흡수가거의안되며정맥주사용으로만사용가능하다. 70mg의부하용량과 50mg의유지용량으로 Candida와 Aspergillus 감염에효과적인 1.0μg/mL 이상의 trough level을유지한다. 혈중에서는약 97% 가단백결합을하고 41% 의약물이소변으로배설되고나머지는간을통해대사되거나담도를통해대변으로배설된다. 4) 약물상호작용 Efavirenz, nevirapine, rifampin, dexamethasone, phenytoin, carbamazepine 과같은약물은 caspofu- Table 3. 7 조용균 : 새로운항진균제 ngin AUC를감소시키므로이약물들과함께투여할때는유지용량을 70mg으로올린다. Cyclosporine은 caspofungin AUC를약 35% 증가시키고 tacrolimus 는 caspofungin 농도에는영향이없으나 tacrolimus 의혈중농도는약 20% 감소한다. Amphotericin B와 itraconazole 과는약물상호작용이없다. 5) 부작용지금까지의연구에서약 12% 의부작용발생률을보이며중등증부작용비율은약 1% 이다 20. 3% 의환자가약물연관성부작용으로투여를중단했다. 가장흔한부작용은발열, 홍조, 오심, 두통, 구토, 정맥염등이며각각 3% 미만의빈도로발생한다. 히스타민분비에의한부작용이약 2% 의환자에서나타나고알러지반응은드물며아나필락시스는매우드물다. 약 3% 의환자에서호산구증가증이발생하고 14% 에서정상치의 5배이하인간기능이상이나타난다. 신독성은없다. 동물실험에서태아독성이확인되어임신부에사용되는경우 category C로신중하게투여한다. 6) 임상적이용 Esophageal candidiasis 환자에서 amphotericin B 와 caspofungin 을비교한연구에서치료성공율은각각 67~90%, 61~63% 으로비슷하였지만부작용발생은 caspofungin 군에서현저하게낮았다 21. 침습성 candidiasis 에대한치료효과는 caspofungin 군이 73%, amphotericin B군이 62% 이고약물부작용에의한투약중단이각각 3%, 23% 로 caspofungin 의부작용발생률이낮았다 20. Fluconazole 과비교한 esophageal candidiasis 치료성적은 fluconazole 군이 85%, caspofungin 군이 81%, 부작용발생률은각각 32%, 41% 였으며임상적으로 fluconazole 내성인환자대부분이 caspofungin 으로성공적으로치료되었다 22. 대조군없는임상연구에서적절한항진균제로 1 주이상투약했지만치료실패한침습성 aspergillosis 환자에서 45% 의치료율을보였고 FDA로부터침습성 aspergillosis 의 salvage therapy로허가받았다. 기존의 amphotericin B와같은항진균제에반응하지않는침습성 aspergillosis 환자에서 amphotericin B나 voriconazole 과 caspofungin 을병합투여하면개선된치료율을보이고병합투여에따른부작용발생도거의없는것으로확인되었다. 3. Itraconazole 주사제 itraconazole 은캡슐, 용액, 정맥주사용의세가지제제로개발되었으며캡슐제제는투여후흡수율의차이가많고특히항암치료후호중구감소성발열이나조혈모세포이식환자에서예방적으로투여할때문제가된다. 최근주사제와경구용액제가개발되어이런단점이보안되었다. 지용성물질인 itraconazole의용해도를높이기위해 hydroxyl- propyl-β-cyclodextrin(cyclodextrin) 이사용된다. Cyclodextrin 은외부가친수성이고내부는소수성인원통형의구조를가지고있어소수성의내부공간이지용성인 itraconazole 에결합하여용해도를높인다. 1) 작용기전진균의 cytochrome P450 (CYP 450) 의존성 14α -lanosterol demethylation 을방해하여세포막내의 ergosterol 의합성을억제한다. 결국 lanosterol 과 14 α-methyl sterol이세포내에축적되고 ergosterol 의미형성으로진균세포막의투과성이상이발생한다. 2) 항진균범위항진균범위는 fluconazole 보다넓으며 Candida spp. 와 Cryptococcus neoformans 와같은 yeast 뿐만아니라 Aspergillus spp. 와 Sporothrix schenckii와같은 mold에도효과적이다. Fluconazole 내성인 Candida에도대부분 ( 약 70%) 감수성이다. 3) 약동력학지용성이고경구흡수율이 fluconazole 보다낮으며위산도가낮거나음식과섭취하면흡수율을높일수있다. 용액제제는캡슐에비해경구흡수율이높고공복시흡수가잘된다. 흡수된약물의 99.8% 가혈액내세포와혈장단백에결합한다. 용적분포 (volume of distribution) 가크고혈청농도보다조직내농도가중요하다 23. 피부, 손톱, 발톱, 폐와여성생식기관과같이진균감염의호발부위에조직농도가높고신장, 간, 뼈, 위, 비장과근육등

8 대한의진균학회지제 8 권제 4 호 2003 에축적된다. 주로간에서대사되며주요대사산물인 hydroxyl-itraconazole 은시험관내항진균효과가 itraconazole 과유사하다. 신부전환자에서경구용 itraconazole 의용량은줄일필요가없으나정맥주사용 itraconazole 은 cyclodextrin 이축적되므로가능하면정맥주사제를피한다. 200 mg를하루에두번이틀간주사하면 250~ 500 μg/l의항정상태 (steady-state) 혈중농도를유지할수있다. 12시간마다 200 mg 로 4번투여후유지용량으로 24시간마다 200 mg 로투여하는것을권장한다. 4) 약물상호작용진균 CYP3A 효소군의억제제로서사람에서도강도는약하지만비슷한작용을가진다. 같이투여하는경우에 cyclosporin 의혈중농도를증가시키므로주의하여야하며 protease inhibitor와병용투여할때에는두약물모두를감량하여야한다. Terfenadine 과 cisapride 와같이투여하면심실성부정맥을유발할수있으므로주의가필요하다. 5) 부작용정맥주사투여시약 5% 의환자에서오심, 구토, 두통, 피부발진, 간기능이상등이발생할수있고드물게족부부종, 저칼륨혈증, 고혈압이나타난다 24. 6) 임상적이용 Febrile neutropenic patients의경험적항진균효과를보면amphotericin B에비해우수한치료효과 (48% 와 38%) 와낮은부작용발생율을나타냈으며 liposomal amphotericin B와는비교해서보다경제적이다 25. 그러나앞으로 liposomal amphotericin B 와직접비교한연구가필요하다. 골수이식환자의예방적투여연구에서 fluconazole 주사와경구투여에비해 itraconazole 주사와경구투여가침습성진균감염의발생빈도가낮았으나 ( 각각 25%, 9%) 양군간의전체사망률은유사했으며소화기계부작용은 itraconazole 군에서높았다 ( 각각 9%, 24%) 26. HIV 감염환자에서 oropharyngeal candidiasis 에 fluconazole 과유사한치료효과와부작용빈도를보이나주로경구투여제의경우이고주사용제제 에대한연구는없다. 4. Liposomal nystatin Polyene 계항진균제중가장먼저개발되었지만전신적사용은하지못했다. 1980년대말 nystatin 을 liposome 에 encapsulation 시켜독성을줄이고생체내항진균효과를개선시켰다. 최근연구에서면역능이정상인환자와호중구감소증환자에서파종성칸디다증과침습성 A. fumigatus 감염에효과적이었다. 동물실험에서전신칸디다증과파종성아스퍼질루스증에 free nystatin 은효과가없었지만 liposomal nystatin은독성이없고생존율이개선되었다. 혈액종양및불응성호중구감소성발열환자를대상으로한 1상실험에서신독성을보이지않았으며 37% 가치료에반응을보였다. 현재임상시험이진행중이다. 참고문헌 1.
