In this study, we evaluated two spectrophotometric broth microdilution methods (SBM) to determine amphotericin B and itraconazole MICs for Aspergillus species isolated from clinical specimens. Methods : A total of 80 clinical isolates (20 A. fumigatus, 20 A. flavus, 18 A. niger, 20 A. terreus, and 2 A. nidulans) were tested for amphotericin B and itraconazole susceptibility by the broth microdilution method. The MIC endpoint was calculated by the spectrophotometer with microplate reader (SBM-Spec method) or colorimetric XTT (tetrazolium dye) method (SBM-XTT method). The results of the SBM method were compared with those of NCCLS M38-A broth microdilution method. Results : The MICs of amphotericin B by the NCCLS M38-A method ranged from to 8 g/ ml, and those of itraconazole ranged from 0.25 to 2 g/ml. The agreement of SBM-Spec and SBM- XTT methods within one dilution of the NCCLS M38 reference were 98.8% and 96.3% for the amphotericin B, and 98.8% and 100% for itraconazole, respectively. The agreements between SBM-Spec and SBM-XTT methods were 97.5% for amphotericin B and 98.8% for itraconazole. Conclusions : In antifungal susceptibility testing of Aspergillus species, the SBM method including SBM-Spec and SBM-XTT methods showed high levels of agreements with the NCCLS M38-A method. The SBM methods can be useful in the clinical laboratory. 2 302 박미라 신종희 송정원외 6 인 현성이낮은점이가장큰문제였는데, National Committee for Clinical Laboratory Standards (NCCLS) 에서는 1998년사상형진균감수성검사인 M38-P법을발표하였고, 2002년에는이를개선한 M38-A법을발표하였다 [7, 9, 10]. 이들 NCCLS법은 macrodilution법보다경제적이고시간소모가적고재현성이좋은 broth microdilution법을기준으로하고있는데 minimum inhibitory concentration (MIC) 을판정하는데있어진균의탁도를단계적으로평가하는육안관찰법을선택하고있다. 최근항진균제감수성검사에있어 MIC 판정을보다객관적이고정확하게시행하기위해 NCCLS법을변형하여일반검사실에서쉽게이용할수있는다양한방법들이시도되고있다 [3, 8]. 이러한방법에는 broth microdilution법을시행한후분광광도계 (spectrophotometer) 로흡광도 (OD) 를측정하여 MIC를판정하는방법 [11] 과마이크로플레이트의판독을쉽게할수있도록산화-환원지시약을넣어색변화에따라판독하는비색분석 (colorimetric) 법 [12, 13] 이있다. 비색분석법은선택적으로지시물질을사용하여, 살아있는균의대사활성에따른색의변화로 MIC를판정한다 [14]. 이중 XTT (2,3-Bis {2-methoxy-4-nitro-5[(sulfenylamino) carbonyl]-2htetrazolium-hydroxide}) 법은미토콘드리아의탈수소효소에의해주황색 formazan으로변환되는황색의 tetrazolium salt dye를이용한것으로, 색의변화는육안으로도판정가능하다 [3]. 본연구에서는임상검체에서분리된 Aspergillus 80주를대상으로 amphotericin B와 itraconazole에대한항진균제감수성검사를분광광도계방법 (spectrophotometric broth microdilution, 이하 SBM법 ) 으로시행하여보았다. SBM법은 M38-A broth microdilution법을시행한후그대로 (SBM-Spec법) 혹은 XTT 를첨가한후 (SBM-XTT법) 분광광도계를이용하여 MIC를판정하는 2가지법으로실시하였고, 이를육안으로 MIC를판정하는 NCCLS M38-A법성적과비교하여보았다. 대상및방법 1. 대상균주대상균주는 1996년 1월부터 2003년 12월사이에전남대학교병원환자의임상검체로부터분리된 80주의 Aspergillus species 로하였다. 즉, A. fumigatus 20주, A. niger 18주, A. flavus 20 주, A. terreus 20주및 A. nidulans 2주를대상으로하였다. Aspergillus species의분리는각검체를 Sabouroud dextrose agar (SDA) 평판배지와 SDA 병배지에접종한후각각을 30 와실온에 4주간배양하여실시하였다. 배지에자란 Aspergillus species의집락에대해서는동정을위하여스카치테이프법, tease mount법혹은슬라이드배양법등을시행하였다. Aspergillus species의동정은배지의육안소견과더불어균을 lactophenol cotton blue로염색하여광학현미경으로관찰함으로써주로형태 Table 1. Aspergillus species tested in this study Species Site of isolation Respiratory Pus Others Total A. fumigatus A. flavus A. niger A. terreus A. nidulans Total 학적방법으로시행하였다 [15]. 대상균주는호흡기검체에서분리된균주가 63주 (79%) 로가장많았고, 농에서분리된균주 9주 (11%) 및기타 8주였다 (Table 1). 정도관리균주로는 NCCLS 에서제시한 A. flavus ATCC 와 A. fumigatus ATCC 를사용하였다. 2. 항진균제검사방법 1) 항진균제마이크로플레이트준비항진균제감수성검사는 broth microdilution법인 NCCLS M38- A법에따라 RPMI 배지를이용하여시행하였다 [9]. RPMI 1640 배지의제조는 L-glutamine이들어있는 RPMI 1640 (Gibco, Invitrogen Corporation, USA) 분말 16.2 g을 3-N-morpholinopropanesulfonic acid (MOPS) g이들어있는완충액 900 ml에녹인다음, 10 M NaOH로최종 ph를 7.0으로맞추어 1 L 의 RPMI 1640 배지를만든후, 사용전까지 4 에보관하였다. 항진균제는두가지를사용하였는데 amphotericin B (Fungizone, Gibco BRL, USA) 와 itraconazole (Janssen Pharmaceutica, Beerse, Belgium) 은 dimethyl sulfoxide (DMSO) 를이용하여 1,600 g/ml의용액으로만들었다. 원액은 RPMI 배지를이용하여 g/ml의농도가되도록배수희석하여 10개의농도로만들었다. 배수희석한각각의항진균제용액을 96 microwell plate (Nunc, Nalge Nunc International, Denmark) 에 1번에서 10번 well까지각 well 당 100 L씩분주하였다. 11번과 12번 well에는항진균제가들어있지않는 RPMI 배지에 2% 의 DMSO를첨가하여 100 L씩분주하였다. 항진균제가분주된 plate는 -70 에보관하여검사당일실온에서녹여서사용하였다. 2) Broth microdilution법검사에사용할 Aspergillus 균액의준비는다음과같이하였다. 1주일간 35 에서배양한균이자란 potato dextrose agar (PDA) 사면배지에 1 ml의멸균증류수를붓고멸균나무막대로사면을적당히긁었다. 혼합물에 Tween 20을 0.01 ml 첨가한후, 무거운파편이가라앉도록 3-5분간두었다. 혼합물의상층액을새로운멸균된 tube로옮겨섞은후분광광도계를이용하여 OD 가 (80-82% transmittance) 이되도록하였다. 농도를맞춘균액을다시 RPMI 배지를이용하여 1:50으로희

3 Aspergillus 의항진균제감수성검사 303 석하였고, 항진균제가분주된 plate에 1번에서 10번 well까지각각 100 L ( 최종균농도, CFU/mL) 씩을분주하였다. 11번 well 은성장대조 well 로서균액 100 L를, 12번 well은배지의대조 well로서 RPMI 배지 100 L만을분주하였다. 균접종이끝난마이크로플레이트는 35 에서 48시간동안배양하였다. 3) MIC 판정 MIC 판정은다음의세가지방법으로하였다. 먼저, NCCLS M38-A법 ( 육안판정법 ) 은 NCCLS 지침 [9] 에따라균성장의정도를육안으로판독하였는데, amphotericin B와 itraconazole 모두항진균제가들어가지않은양성대조와비교해서성장이전혀없는 score 0인농도를최소억제농도로하였다. SBM-Spec법 ( 분광광도계를이용한균탁도측정법 ) 은각 well의 OD를 405 nm 에서분광광도계 (VERSAmax Tunable Microplate Reader, Molecular Devices Corp., Sunnyvale, Calif, USA) 로측정하였고, 배지의대조 well의 OD와비교하여성장이없는최소농도를 MIC로판정하였다 [3]. SBM-XTT법 (XTT를이용한측정법 ) 은 XTT (Sigma, St. Louis, USA) 의최종농도가 200 g/ml, menadione (Sigma) 의최종농도가 25 M이되도록 XTT (1 mg/ml) 50 L와 menadione (1 mm) 을각 well에첨가하여시행하였다 [11, 16]. Menadione은 1,722 mg/ml (10 mm) 의농도로아세톤에녹인다음, 멸균식염수를이용하여 1:10으로희석하여사용하였다. XTT를분주한 plate는 35 에서 2시간추가배양한후 490 nm에서분광광도계 (VERSAmax Tunable Microplate Reader) 로 OD를측정하여대조군과비교하여발색이없는최소농도를 MIC로판정하였다. 정도관리범위가알려진 2 종의표준균주를대상으로 amphotericin B와 itraconazole에대하여 NCCLS M38-A법으로감수성검사를시행하여 48시간배양후 MIC를판정하였다. 그결과본연구에서이용한 3가지 MIC 판정법모두에서 A. flavus ATCC 의경우 amphotericin B 1 g/ml, itraconazole 0.25 g/ ml이었고, A. fumigatus ATCC 의경우 amphotericin B 0.5 g/ml, itraconazole 0.25 g/ml로서, 모두정도관리허용범위에속하였다 (Table 2). 2. NCCLS M38-A 에의한검사결과 임상검체에서분리된 Aspergillus species 80주를 M38-A broth microdilution법으로검사하여 MIC를판정한결과 amphotericin B에대한 MIC 성적은 Table 3과같았다. NCCLS M38-A법에의한 amphotericin B MIC 범위는 g/ml이었고, 균종별 MIC는 A. fumigatus와 A. niger는 g/ml, A. nidulans는 g/ml로낮은데비해, A. flavus는 g/ml, A. terreus는 2-8 g/ml의범위로매우높았다. Aspergillus species 80주의 MIC 50 과MIC 90 은전체적으로 1 g/ml와 4 g/ ml이었다. 균종별 MIC 50 과 MIC 90 은 A. niger가가장낮아mic 50 과 MIC 90 모두 0.5 g/ml이었다. A. fumigatus의 MIC 50 과 MIC 90 은 0.5 및 1 g/ml, A. flavus는모두 2 g/ml이었고, A. terreus는 2 및 4 g/ml로서높았다. MIC가 2 g/ml 이상인균주는 80주중 34주였는데, A. fumigatus 및 A. niger 중에는한주도없는반면, A. flavus에서는 85% (14/20) 이었고, A. terreus에서는 100% (20/20) 이었다. Table 3. Itraconazole MICs for clinical isolates of Aspergillus species by three methods Species/Method Table 4는 itraconazole에대한 MIC 성적으로균종간에큰차이없이 g/ml의분포를보였다. 전체적인 MIC 50 과 MIC 90 은 A. fumigatus를제외하고 0.5 g/ml와 1 g/ml이었다. A. fumigatus의 MIC 50 과MIC 90 은 1 g/ml이었다. Amphotericin B와 itraconazole에대한감수성검사를두가지 SBM법으로실시한후 NCCLS M38-A법과일치도를비교한결과 ±1 희석범위내에서모두높은일치율을보였다 (Fig. 1, 2). SBM법과 NCCLS M38-A법과의일치율은 amphotericin B의경우 SBM-Spec법은 98.8% (79/80) 로 A. flavus 1주만일치하지않았고, SBM-XTT법은 96.3% (77/80) 로 A. fumigatus 2 주및A. flavus 1주만이일치하지않았다. Itraconazole의경우에는 SBM-Spec법은 98.8% (79/80) 로 A. niger 1주가일치하지않았고, SBM-XTT법은 100% (80/80) 이었다. 또 SBM- Spec법과 SBM-XTT법의 MIC 일치율은 amphotericin B의경우 97.5% (78/80) 로 A. fumigatus와 A. niger 각 1주가일치하지않았고, itraconazole의경우 98.8% (79/80) 로 A. niger 1 주만일치하지않았다. 또한 SBM-Spec법및 SBM-XTT법의 ±2 희석범위내에서 NCCLS M38-A법과의일치율은 itraconazole의경우각각 98.8% (79/80) 및 100% (80/80) 이었으며, amphotericin B의경우두방법모두 100% 이었다. No. of isolates > >2 고 NCCLS에서는최근사상형진균에대한항진균제감수성검사방법으로 M38-P법에이어 NCCLS M38-A법 (2002년) 을문서화하였다. NCCLS M38-P법에서는 azole계항균제의 MIC 결정점을성장대조군과비교하여 50% 성장억제를보이는최소약제농도로하였는데, M38-A법에서는더정확하고신뢰성이있도록 MIC 판정기준을 100% 성장억제되는최소농도로하였다 [10, 17]. NCCLS M38-A법은비교적최근에발표되었으므로이러한기준에의한진균의감수성성적은아직많이보고되어있지않으며, 다른방법간의일치율에관한연구도거의없다. 본연구에서는임상검체에서분리된 Aspergillus species에대하여 NCCLS M38-A법을적용하여 amphotericin B와 itraconazole에대한감수성검사를실시하였고, 이를분광광도계를이용하여 MIC를판정하는두가지방법과비교하여보았다. 본연구결과, 국내임상검체에서분리된 Aspergillus species 찰 Amphotericin B Itraconaozle Fig. 1. 5 Aspergillus 의항진균제감수성검사 305 의 MIC 분포는 amphotericin B의경우 g/ml이었고, itraconazole의경우 g/ml의범위로 itraconazole의 MIC 범위가더좁았는데, Meletiadis 등 [13] 은 amphotericin B의 MIC 범위는 g/ml, itraconazole의경우 g/ml의범위를보고하여본연구와비슷한소견을보였다. 또한 Odds 등 [18] 의연구에서 amphotericin B의 MIC는모두 2 g/ml 이하이고, itraconazole의 MIC는모두 1 g/ml 이하였는데, 본연구에서도 amphotericin B에감수성이적다고알려진 A. flavus 와 A. terreus를제외한 A. fumigatus와 A. niger의 MIC는모두 2 g/ml 이하였고, itraconazole의 MIC는모두 1 g/ml 이하를보였다. Aspergillus species에대한 amphotericin B의균종별 MIC 분포는 A. fumigatus와 A. niger는 g/ml, A. nidulans는 g/ml, A. flavus는 g/ml, A. terreus는 2-8 g/ml의범위를보였는데, 신등 [19] 의보고와 Arikan 등 [20] 의보고에서도본성적과유사하거나조금낮았으며균종별차이서열은일치하였다. Itraconazole에대한균종별 MIC 분포는 NCCLS M38-P법을시행한 Pfaller 등 [21] 의성적과달리균종별차이가없었다. 항진균제감수성검사의 MIC 판정에서의문제점은균성장의부분적억제에의한 trailing 현상과육안관찰의객관성결여인데 [14, 22, 23], 분광광도계를이용하여 MIC를판정할경우육안관찰보다객관적이고빠르며 [13, 23] 균사성장을더정확하게정량할수있다 [3]. 또한산화- 환원지시약을첨가하여대사활성을측정하는비색분석법도 MIC 판정에유용하다 [10, 12]. 현재산화-환원지시약으로는 3-(4,5-dimethyl-2-thiazyl)-2,5-diphenyl- 2H-tetrazolium bromide (MTT) 와 XTT가주로사용되고있다. MTT는다양한효모와사상형진균의항생제감수성검사에사용되어왔고, NCCLS에의해제시된표준화된방법과비교하여좋은일치율을보였다 [3, 14, 16]. 그러나분광광도계로 MIC 측정시 formazan crystal의형성으로수용화과정이포함되어야하므로 [3, 12, 14] 대안적으로 XTT가제시되었는데 [12, 14], XTT 역시다양한효모의항생제감수성검사에사용되어정확한 MIC를보였다 [3, 24]. 또한 XTT는수용성 formazan으로변환되기때문에추가적인수용화과정이필요하지않아 MTT보다사용이간편하다 [12]. 본연구에서는 M38-A법에의한 broth microdilution법을시행한후 OD를측정하여 MIC를판정하는방법 (SBM-Spec) 과마이크로플레이트의판독을쉽게할수있도록 XTT를넣어색변화에따라 MIC를판독하는 Spec-XTT법을동시에사용하였다. NCCLS M38-P법과 SBM-Spec법과의일치율을 Meletiadis 등 [3] 은 amphotericin B 98.7% 및 itraconazole 81.5% 로보고하였고, Dannaoui 등 [25] 은 amphotericin B 99% 및 itraconazole 95-98% 로각각보고하였다. 그러나 NCCLS M38-A법과 SBM- Spec법을비교한연구는아직없다. 본연구에서는 NCCLS M38- A법과 SBM의두방법을최초로비교하여보았는데, NCCLS M38-A법과 SBM-Spec법과의일치율은 amphotericin B 및 itraconazole의 경우 98.8% 및 98.8% 로서매우높았다. Meletiadis 등 [3] 은 NCCLS M38-P법과육안관찰한 XTT법의일치율을 amphotericin B의경우 97%, itraconazole의경우 94% 로보고하였고, SBM-Spec법과 SBM-XTT법의일치율은 amphotericin B의경우 98.7%, itraconazole의경우 84% 로보고하였다. 이와비교하여본연구에서의 NCCLS M38-A법과 SBM-XTT법일치율은 amphotericin B의경우 96.3%, itraconazole의경우 100% 이었고, SBM-Spec법과 SBM-XTT법의일치율은 amphotericin B의경우 97.5%, itraconazole의경우 98.8% 로그들의성적과유사하거나높았다. 이는 NCCLS M38-P법에비해 M38-A법의경우 azole계 MIC 판정기준을 50% 대신 100% 성장억제되는최소농도로하였기때문으로특히 itraconazole MIC 판정에있어재현성이더높아지고용이해져서일치율이더높아진것으로판단된다. 본연구결과 SBM법의두방법은둘다 NCCLS M38-A법과높은일치율을보였고또두방법간에도 97% 이상의높은일치율을보였다. 이는실제검사실에서 SBM법을사용하여 MIC 판정이가능함을보여주었고, 이러한방법은육안으로 MIC를판정하는번거로움과오류를줄일수있으며더객관적이고정확한결과를제공하리라생각된다. 그러나실제사용에있어 SBM- XTT법의경우비용이많이들고시간소모가많기때문에, 단순히마이크로플레이트 reader로균탁도를판독하는 SBM-Spec 법이더간단하여효율적이라생각되었다. 결론적으로 Aspergillus species의감수성검사에있어분광광도계와 XTT 를이용한 broth microdilution법은둘다 M38법과일치율이높아검사실에서의이용이가능하리라생각된다. 요약배경 : 항진균제감수성검사에있어분광광도계를이용하여 minimum inhibitory concentration (MIC) 을판정할경우육안관찰보다객관적이고빠르며, 균사의성장을더정확하게정량할수있다고알려져있다. 본연구는임상검체에서분리된 Aspergillus species에대한항진균제감수성검사를분광광도계를이용하여 MIC를판정하는 spectrophotometric broth microdilution (SBM) 법으로시행하고, 이를 National Committee for Clinical Laboratory Standards (NCCLS) M38-A법과비교하였다. 방법 : 임상검체로부터분리된 80주의 Aspergillus species (A. fumigatus 20주, A. flavus 20주, A. niger 18주, A. terreus 20 주및A. nidulans 2주 ) 를대상으로 amphotericin B와 itraconazole에대한감수성검사를 broth microdilution법으로실시하였다. 6 306 박미라 신종희 송정원외 6 인 교하였다. 결과 : NCCLS M38-A법에의한 amphotericin B에대한 MIC 는 g/ml이었고, itraconazole에대한 MIC는 g/ml 사이의범위였다. SBM법과 NCCLS M38-A법과의일치율 (1배희석배수내 ) 은 amphotericin B의경우 SBM-Spec법은 98.8%, SBM-XTT법은 96.3% 이었고, itraconazole의경우 SBM-Spec법은 98.8%, SBM-XTT법은 100% 이었다. SBM- Spec법과 SBM-XTT법의두방법간의일치율은 amphotericin B 97.5% 및 itraconazole 98.8% 이었다. 결론 : Aspergillus species의감수성검사에있어분광광도계를이용한 SBM법은 SBM-Spec법및 SBM-XTT법둘다 NCCLS M38-A법과일치율이높아검사실에서이용이가능하리라생각된다. 참고문헌 1.
