Tuberculosis is an infectious disease caused by Mycobacterium tuberculosis and is characterized by granuloma with caseous necrosis. The clinical and histological similarity between sarcoidosis and tuberculosis has stimulated research searching for an association between mycobacterium and sarcoidosis. We report a case of a 38-year-old male with sarcoidosis that developed soon after treatment of tuberculous lymphadenitis. He was diagnosed as tuberculous lymphadenitis by microbiological confirmation. He showed clinical improvement after treatment for tuberculosis. One year later, his chest radiography showed bilateral hilar enlargement with diffuse bilateral nodules. A noncaseating granuloma was confirmed by endobronchial ultrasound guided transbronchial needle aspiration and he was diagnosed with sarcoidosis. To our knowledge, this is the first report describing sarcoidosis after treatment of tuberculosis in South Korea. 조직검사상괴사를동반한육아종 (Figure 2) 과항산균이관찰되었고, 림프절흡인액에서시행한결핵균 PCR (polymerase chain reaction) 에서양성 소견을보여, 결핵성림프절염으로진단하고,

외래치료중림프절흡인액에서결핵균배양이확인되었고, 항결핵제감수성검사상모든항결핵제에감수성을보였다.

Histological findings of subcarinal lymph node shows chronic granulomatous inflammation and necrotic debri (arrows) (H&E stain, 200). sound guided transbronchial needle aspiration, EBUS- TBNA) 을시행하였다. 조직검사상괴사를동반한육아종 (Figure 2) 과항산균이관찰되었고, 림프절흡인액에서시행한결핵균 PCR (polymerase chain reaction) 에서양성 소견을보여, 결핵성림프절염으로진단하고, 4제표준요법 (Isoniazid, Rifampin, Ethambutol, Pyrazinamide) 을시작하였다. 외래치료중림프절흡인액에서결핵균배양이확인되었고, 항결핵제감수성검사상모든항결핵제에감수성을보였다. 총 6개월간치료후흉부 CT를시행하였고, 흉부및종격동림프절의크기가감소되었음을확인하고치료를중단하였다 (Figure 1B). 특이증상없이지내던중내원 4개월전미열, 오한이있었고, 내원 1개월전외래방문시시행한흉부단순촬영상, 양측폐문부비대및미만성폐결절소견으로결핵재발등의원인질환감별을위해입원하였다. 내원당시에특이증상호소는없었다. 과거력 : 특이사항없음. 가족력 : 특이사항없음. 사회력 : 7갑년의흡연력이있으며 10년전흡연을중단하였고, 음주력은없었다. 베릴륨및다른금속, 살충제, 진균노출력은없었다. 이학적소견 : 내원당시혈압 104/63 mm Hg, 맥박수 79회 / 분, 호흡수 20회 / 분, 체온 36.5 o C로, 신체검사상전신상태는양호하였고, 흉부청진에서심음이나폐음은정 434

2 mg/dl, 혈액요소질소 13 mg/dl, 크레아 티닌 0.9 mg/dl, 혈청 전해질검사상 나트륨 141 mmol/l, 칼륨 3.8 mmol/l, 염소 104 mmol/l, 칼슘 8.5 mg/dl이었 다.

폐기능 검사: 노력성 폐활량(forced vital capacity, FVC) 은 정상예측치의 97%, 1초간 노력성호기량(forced expiratory volume in 1 second, FEV1)은 79%, FEV1/FVC은 68, 폐확산능(diffusing capacity for carbon

CT: computed tomography. 상이었으며, 다른 신체의 이상은 관찰되지 않았다. 3 검사실 소견: 일반혈액 검사상 백혈구 10,000/mm, 혈색소 14.4 g/dl, 혈소판 192,000/mm 3이었다. 혈청 생화학 검사상 총 단백질 7.2 g/dl, 알부민 4.4 g/dl, AST 20 IU/L, ALT 33 IU/L, 알칼리포스파타제 83 IU/L, 총빌리루빈 1.2 mg/dl, 혈액요소질소 13 mg/dl, 크레아 티닌 0.9 mg/dl, 혈청 전해질검사상 나트륨 141 mmol/l, 칼륨 3.8 mmol/l, 염소 104 mmol/l, 칼슘 8.5 mg/dl이었 다. 객담에서 시행한 항산성 염색 및 결핵균 PCR은 음성 이었고, 배양 검사에서 세균 및 진균은 배양되지 않았다. 투베르쿨린 검사 결과는 음성이었다. 폐기능 검사: 노력성 폐활량(forced vital capacity, FVC) 은 정상예측치의 97%, 1초간 노력성호기량(forced expiratory volume in 1 second, FEV1)은 79%, FEV1/FVC은 68, 폐확산능(diffusing capacity for carbon monoxide)은 66% 로 경증 폐쇄성 환기장애 및 폐확산능의 감소를 보였다. 방사선학적 소견: 흉부 단순촬영상 양측 폐문부 비대 및 양 폐야의 미세결절이 관찰되었다(Figure 3A). 흉부 CT상 양측 종격동 및 폐문부 림프절의 크기가 증가되어 있었으나 괴사를 시사하는 저밀도 음영은 없었고, 양폐에 중심 소엽성 결절(centrilobular nodule)이 미만성으로 산 재되어 있었다(Figure 4). 내시경 소견: 기관지 내 병변은 관찰되지 않았다. 기관 지 초음파 내시경 소견상, 불균질한 에코발생을 보이는 둥근 모양의 림프절이 관찰되어 TBNA를 시행하였다. 림 프절 흡인액 및 기관지 세척액에서 시행한 항산성 염색 및 결핵균 PCR은 음성이었다. 병리 소견: 조직 검사상 다수의 비건락성 육아종이 관 435

Histological findings of mediastinal lymph node shows non-caseating granuloma consistent with sarcoidosis (H&E stain, 200). 찰되었고 (Figure 5), 항산성염색및결핵균 PCR, 진균염색은음성이었다. 치료및경과 : 사르코이드증으로진단하였고, 타장기침범여부확인을위해추가검사를시행하였다. 혈청안지오텐신전환효소 (angiotensin converting enzyme) 62.1 U/L ( 정상치, U/L), 24시간소변칼슘 mg/day ( 정상치, mg/day) 이었다. 심전도검사및심초음파검사, holter monitoring, 안과검진및피부과검진상특이소견은관찰되지않았다. 림프절흡인액및기관지세척액에서시행한항산균배양검사는 8주후음성으로확인되었다. 환자는종격동림프절비대및폐실질침윤이있으면서폐기능의저하가동반되어스테로이드치료를시작하기로하였다. 스테로이드치료 ( 경구프레드니솔론 ) 2개월경과후시행한흉부단순촬영상양측종격동림프절크기는감소하였고 (Figure 3B), 현재특이증상호소없이외래경과관찰중이다. 고 사르코이드증은면역매개육아종성질환이다 1,2,7. 발병기전은항원제시세포 (antigen presenting cells) 에의해제시된특정항원에의해 CD4+ T-세포가활성화되고, 활성화된 CD4+ T-세포가제1형조력 T-세포 (type 1 T helper cell) 로분화하여인터루킨-2(interleukin-2) 와인터페론-감마 (interferon-γ) 를주로분비하고, 대식세포종양괴사인자-α (tumor necrosis factor-α) 생산을촉진하여육아종을형성및유지시키는것으로알려져있다 2,7. 이러 찰 한항원제시및처리 (processing), 시토카인 (cytokine) 분비는유전적인인자가관여하는것으로생각되며 2, 감염성인자, 유기질입자, 무기질입자등이가능한항원으로보고되고있다 2-4. 이중결핵은사르코이드증과임상적, 방사선학적, 조직학적소견이유사하고 4-6, 결핵발병후사르코이드증을진단받거나 8, 혹은드물지만결핵과사르코이드증이동시에발생하는경우가보고되어 9, 두질병사이에연관성이있을것으로추측되어왔으며, 이를규명하기위해여러연구가시행되어왔다 3,4,7, 최근에 Gupta 등 12 이사르코이드증환자및대조군 ( 폐암, 기관지확장증, 간질성폐질환등결핵, 사르코이드증이아닌다른폐질환 ) 에서 mycobacteria 에대한 PCR을시행한 31개연구에대해메타분석 (metaanalysis) 을시행한결과, 사르코이드증환자 874명의검체중 231개검체 (26.4%) 에서 mycobacteria 양성이었고, 대조군에서는 600명의검체중 20개검체 (0.3%) 에서양성소견을보였으며, 대조군과비교하였을때사르코이드증환자에서 mycobacteria PCR양성이확인된대응비는 9.67임을보고하였다. Song 등 7 은 Mycobacterium tuberculosis catalase-peroxidase (mkatg) 항체를이용한실험에서사르코이드증조직의 55% 에서는 mkatg 가존재하는반면, 대조군 ( 암, 감염, 육아종의증거가없는폐조직, 림프절, 비장조직 ) 에서는 mkatg가검출되지않았음을보고하여, mycobacterium 에서유리된항원이사르코이드증의유발원인일가능성을제시하였다. 이후에 Chen 등 11 이사르코이드증환자 150명, 대조군 ( 건강한성인 ) 53명을대상으로, IFN-γ-enzyme-linked immunospot assay (IFN-γ- ELISPOT assay) 및흐름세포측정법 (flow cytometry) 을이용하여재조합 mkatg 에대한 T-세포활성도를측정한결과, 투베르쿨린검사음성인대조군에비해사르코이드증환자에서 mkatg- 반응 T-세포의비가현저하게높게나타남을보고하여, 사르코이드증환자에서 mkatg 에대한 T- 세포반응이존재함을증명하였다. 또한 Carlisle 등 10 과 Hajizadeh 등 13 은사르코이드증환자에서투베르쿨린검사음성인대조군에비해 mycobacterial superoxide dismutase A, early secreted antigenic target protein 6, mycolyl transferase Antigen 85A에대한반응 T-세포의비가높게나타남을보고하여, 사르코이드증환자에서 mkatg 외에도다양한 Mycobacterium 항원에 T-세포반응을보임을증명하였다. 이와같은연구들은사르코이드증과결핵간에연관성이있다는점을시사하며, 향후결핵과관련한사르코이드증의발병기전에대해더많은연구가필요함을 436

No. 5, May 2011 제시하고있다. 결핵과사르코이드증의또다른쟁점은두질환의감별진단이다. 두질환은임상적, 방사선학적, 조직학적으로유사하여구분하기가쉽지않다 4-6. 사르코이드증은적절한임상증상과방사선학적소견이동반된환자에서조직검사상비건락성육아종이증명되며, 육아종을형성하는다른질환들이배제되었을때진단할수있다 1,2. 결핵과의감별점은조직염색및배양검사상항산균이확인되지않고, 눈, 피부등다른장기침범을흔히동반하며, 피부무반응 (skin anergy) 으로투베르쿨린검사상음성을나타낸다는점, 기관지폐포세척 (bronchoalveolar lavage) 세포검사상, 림프구백분율이높은것은동일하지만결핵에서는 normal CD4/CD8 ratio를보이는반면, 사르코이드증에서는 high CD4/CD8 ratio 를보인다는점등이있다 14. 또한병리소견상사르코이드육아종의경우다른질환보다단핵포식세포 ( 대식세포, 상피모양세포 ) 가치밀하게집적되어있고 15, 주변조직에호중구나호산구가관찰되면사르코이드증보다는진균이나결핵감염과같은감염성육아종을더의심할수있다 5. 본증례는배양검사에서결핵균이확인된결핵림프절염환자에서항결핵제치료후양측폐문부림프절종대가발생한경우로, 결핵재발과의감별이필요했다. 임상증상및방사선학적소견으로는감별진단이어려웠으나, 조직검사상비건락성육아종이관찰되었고, 항산성염색및결핵균 PCR, 배양검사및투베르쿨린검사에서음성소견을보이며, 모든항결핵제에감수성을보인균주에대해적절한항결핵제가 6개월간투약되었었다는점을근거로결핵재발을배제하였다. 그외진균감염, 악성종양등육아종을형성하는다른질환의증거가없고, 방사선학적소견과조직검사소견이사르코이드증에합당하다고판단하여사르코이드증을진단하였다. 본증례는국내에서최초로보고되는결핵치료후발생한사르코이드증증례로사르코이드증의발생과결핵이관련있음을시사해주는한예라고생각된다. 두질환이임상적, 방사선학적, 조직학적으로유사하나진단에따라치료및예후가다르므로감별진단이중요하며, 향후사르코이드증발생과결핵의관련성에대해지속적인연구가필요할것으로생각한다. 참고문헌 1.
