It is often diagnosed through characterized radiologic finding in abdominal examinations including computed tomography scan. However, it is important to rule out other conditions involving mesentery when diagnosing sclerosing mesenteritis. In the case of malignancy, the method of treatment and prognosis can be completely different therefore thorough examinations are essential. We herein report a 75-year-old male who suffered from frequent diarrhea and weight loss. Initially, he was diagnosed with sclerosing mesenteritis through abdominal computed tomography scan showing "misty" soft-tissue attenuation around the mesenteric vessel. However, follow up positron emission tomography scan and biopsy finding confirmed the common bile duct cancer with lymph node metastasis. 복강동맥 및 상 mg/dl), IgM 56 mg/dl (50-320 mg/dl), IgG4 14.96 mg/dl 장간막동맥 주위 연무양 감쇄상은 경화성 장간막염으로 사료 (2.4-121 mg/dl), carcino-embryonic antigen 3.75 ng/ml (0-5 되었다(Fig. 1).

이후 외래에서 장간막염의 장부에 미란을 동반한 다발성의 점상발적들이 보여, 조직검 진행 정도를 추적관찰하여 스테로이드 및 호르몬 치료를 계 사를 시행하였으며, 병리 결과에서 만성 회장염으로 판독되 획하였다. 추적관찰 목적으로 한달 후 복부 컴퓨터단층촬영 었다.

(, ) n FDG-avid lymph node around the pre-aortic and celiac trunk (arrow) that was suspected malignancy. 을 촬영하였고, 이전 복부 컴퓨터단층촬영상에 비하여 장간 고 찰 막의 침윤은 큰 변화를 보이지 않았으나 총담관의 협착은 경 미하게 진행한 양상이었다(Fig. 2). 담관의 협착에 대하여 보 경화성 장간막염은 장간막을 침범하는 특발성 염증 및 섬 다 정밀한 검사가 필요하다고 판단하였고, 이에 내시경초음 유화 과정 중에 있는 질환으로, 병태 생리학적으로 이들 과 파검사를 시행하였으며, 종괴나 담석의 증거는 관찰되지 않 정은 위장관의 관강이나 주변 장간막 혈관을 침범할 수 있 았고, 경도의 협착 및 담관벽의 비후가 의심되었다(Fig. 3). 한 다. 경화성 장간막염에 대해서 현재까지 밝혀진 명확한 병태 편, 내시경적 역행성 담관조영술에서 팽대부로 유도철선을 기전은 없으며, 지방세포의 괴사(mesenteric lipodystrophy) 삽관하였으나, 총담관 중간의 협착부위에 굴곡이 심하여, 협 에서 만성 염증상태(mesenteric panniculitis)로 진행된 뒤 마 착부 상방으로 유도철선의 진행에는 실패하였다(Fig. 4). 총 지막으로 섬유화 과정(retractile mesenteritis)으로 진행하는 담관 중간의 협착부에서 조직검사를 시행하였고, 병리 결과 일련의 과정 중 일부분으로 추정된다.5 원인 역시 명확히 밝 에서 분화도가 양호한 선암종으로 확인되었다(Fig. 5). 이후 혀지지는 않았으나 관련한 임상질환들로 복부수술 및 외상 경과관찰 과정에서 환자의 총 빌리루빈이 6.12 mg/dl로 증가 의 과거력, 자가면역질환, 부종양증후군(paraneoplastic syn- 되었고, 우상복부의 불편감을 호소하여, 경피경간담즙배액술 drome), 허혈성 손상, 감염성 질환 등이 보고되었다.1,5 또한 (percutaneous transhepatic biliary drainage)을 시행하였다. 60세 이상에서 호발하며, 남녀의 차이는 보고마다 다르나, 추가로 시행한 양성자방출 컴퓨터단층촬영술(positron emis- 대체적으로 남자에서 더 흔한 것으로 나타났다. 2 노령에서 sion tomography computed tomography)에서 복강동맥 및 호발하는 이유는 어린 연령에서는 복강내 지방이 상대적으 상장간막동맥 주변으로 고대사성 연부 조직 음영(hypermet- 로 적기 때문으로 추정된다. 경화성 장간막염의 증상으로 복 abolic soft tissue density, SUVmax 4.5) 소견이 보였으며, 경 통, 구역, 구토, 설사, 체중 감소, 발열 등이 보고되었다.1,2 경 화성 장간막염보다는 암종의 전파(cancerous encasement)에 화성 장간막염의 진단은 이에 부합하는 영상학적 이미지와 합당한 소견이었다(Fig. 6). 이후 환자에게 고식적 항암화학 더불어 조직학적 결과를 토대로 결론내려질 수 있다. 경화성 요법, 항암방사선 치료 등을 권유하였으나, 적극적인 치료는 장간막염의 컴퓨터단층촬영에서의 특징은 장간막의 연부조 원치 않아서 대증적 치료를 하면서 외래 경과관찰 중이다. 직 종괴로 가장 흔히 나타나며, 다소 특징적 소견으로 장간 06 증례 indd 오후 3:26:18

김범수 Analysis of Outcomes after Resection of Sarcomatous Hepatocellular Carcinoma Purpose: Sarcomatous hepatocellular carcinoma (HCC) is rare. Therefore, the clinicopathologic characteristics and prognosis
