This study was designed to evaluate the effects and safety of Agio 18 g per day in patients with chronic constipation. Methods: We enrolled 24 patients with constipation-dominant irritable bowel syndrome and chronic constipation diagnosed by Rome Ⅱ criteria. Agio 18 g per day was administered to these patients for 21 days. We compared the symptoms related to constipation before and after Agio administration. We also compared colon transit time before and after Agio administration. Adverse events related to Agio administration were investigated. Results: Symptom changes before and after Agio administration were as follows: 1) frequency of bowel movement (2.3 vs. 1.7, p<0.05), 2) straining during defecation (6.9 vs. 4.6, p<0.001), 3) stool forms and consistency by Bristol stool scale (2.1 vs. 3.0, p<0.05), 4) sensation of incomplete evacuation (5.9 vs. 4.3, p<0.001), 5) sensation of anorectal obstruction or blockage (7.1 vs. 4.7, p<0.05), 6) abdominal discomfort (6.5 vs. 4.8, p<0.001) and 7) satisfaction of defecation (2.3 vs. 4.1, p<0.05). Total colon transit time was 46.4±18.3 hours before Agio administration while 37.9±16.7 hours after Agio administration (p=0.020). No significant adverse events related to Agio administration occurred. Conclusions: Agio 18 g per day for 21 days seems to be effective and safe in patients with constipation-predominant irritable bowel syndrome and chronic constipation. (Kor J Neurogastroenterol Motil 2008;14: ) Key words: Agio, Constipation, Colon transit time 서 만성기능성변비에대한치료로완하제와장운동항진제등이흔히사용되고있다. 완하제는부피형성하제 (bulk forming laxative), 삼투성하제 (osmotic laxative), 자극성하제 (stimulant laxative) 등이있으며, 이중부피형성하제는다당류나섬유소의유도체로수분을흡수하고배변량을증가시켜효과를나타내며만성기능성변비환자에게서배변횟수를증가시키고배변양상을부드럽게하며장기사용 론 에따른부작용이적어가장널리사용되는기본적인완하제이다. 아기오 ( 부광약품 ) 은 1포 (6 g) 당차전차 (Plantago seed) 3.9 g과차전차피 (Ispaghula) 0.13 g로구성된변비치료제이다. 부피형성하제에속하는아기오 는변비우세형과민성장증후군및만성변비의치료에있어그효과와안정성이이미여러임상연구를통하여밝혀진바있다. 1,2 이들연구에서아기오 는대체로 1일 10~30 g을경구복용하는용량이사용되었다. 이러한아기오 는현재우리나라에서도변비우세형과민성장증후군및만성변비환자에게흔히처방되고있는약제로실제그안정성이매우높아일반의약품으로분류되어있고성인및 12세이상의소아인경우 1일 6~18 g 경구투여용량이허가되어사용되고있다. 본연구에서는그안정성이잘알려져있는아기오

2 116 대한소화관운동학회지 : 제 14 권 2 호, 2008 를 1일 18 g 경구투여했을때변비우세형과민성장증후군및만성변비로진단된국내환자들에서효과적이며안전한지를살펴보고자하였다. 대상및방법 1. 대상연구대상자는만 18세부터만 70세이하의만성기능성변비또는변비우세형과민성장증후군의로마진단기준 Ⅱ (Table 1) 3 을만족하는환자로 Table 2와같은기준의환자는연구대상에서제외되었다. 2006년 1월부터 2006년 9월까지서울아산병원과삼성서울병원소화기내과에내원한환자중에서연구에동의한 24명 ( 각기관별 14명, 10명 ) 을대상으로파일럿연구를시행하였으며, 각기관의임상연구윤리위원회승인후연구를수행하였다. 2. 방법 1) 아기오 투여환자들은아기오 투여 10일전부터배변에영향을주는약제사용을금지하였다. 10일동안의준비기간후아기오 를아침 1포 (6 g), 저녁에자기전 2포 (12 g) 씩총 21일간경구투여하였다. 실제복용여부는외래추적진료시빈포를확인함으로써판단하였고, 80% 이상복용하였을 때약물을충분히복용한것으로간주하였다. 4 2) 아기오 의효과 (1) 임상증상비교환자들의기저임상상을평가하기위해아기오 투여시작 3일전배변관련증상설문지 (Table 3) 를작성하였다. 설문은배변관련증상평가를위하여 7가지항목에대하여시행하였으며, 설문지는과거여러연구들 5,6 에서사용되었던것과동일한것으로하였다. 각각의항목은 1주일간의총배변회수 (> 주 3회 =1점, 1~3회 =2점, <1회 =3점 ), 배변시힘주기 (0: 매우경미하다 ~10점 : 매우심하다 ), 대변형태 (Bristol scale 7 : 1~7), 배변후잔변감 (0: 매우경미하다 ~10: 매우심하다 ), 배변시항문폐색감 (0: 매우경미하다 ~10: 매우심하다 ), 복부불편감및복통 (0: 매우드물고경미하다 ~10: 매우잦고심하다 ), 전체적인배변만족도 (0: 매우불만 ~10: 매우만족 ) 로구성되었고비교를위하여점수화하였다. 임상증상호전에대한아기오 의효과를확인하기위해아기오 투여 21일째배변관련증상설문지 (Table 3) 를재작성하였고, 아기오 투여전결과와비교하였다. 모든증상설문지의작성은두명의임상전문간호사에의해직접면담후이루어졌으며, 동일한환자의설문작성은동일한임상전문간호사에의해이루어졌다. Methods. 여 21일째에복부단순방사선촬영을시행하고, 이후 3일동안매일 Kolomark TM 1캡슐씩경구복용한후 24일째에복부단순방사선촬영을재시행하여아기오 투여후대장통과시간을측정하였다. 3) 아기오 의안전성이상의연구방법에대하여약제투여를위한준비기간, 약제투여기간, 약제투여후기간으로나누어도식화하여 Fig. 1에나타내었다.

Mean Colonic Transit Time (hours) Before treatment After treatment p-value Total CTT* 46.4± ± Right side CTT* 14.9± ± Left side CTT* 22.4± ± Rectosigmoid CTT* 9.5± ± * CTT: colonic transit time. 4) 통계분석통계적인분석은통계처리프로그램인 SPSS 14.0K for Windows를이용하였다. 비모수검정방법인 Wilcoxon signed rank test를이용하여약제투여전후의지표들을비교하였고, p-value<0.05인경우를유의한것으로판정하였다. 결과 1. 연구대상자의특성연구에는총 24명의환자가참여하였는데, 남성 7명, 여성 17명으로여성이 70.8% 를차지하였다. 연령분포는 24 세 ~72세로연령의중앙값은 54.5세이었다. 모든환자들은연구약물인아기오 1일 18 g을연구기간동안충분히복용하여적절한약물순응도를보였으며, 결과적으로 24 명모두약물효과및안전성분석에포함되었다. 2. 아기오 의효과 1) 아기오 투여전후배변관련증상변화 (Fig. 2) 배변관련증상은아기오 투여후 7가지항목모두에서호전을보였다. 아기오 투여후배변회수는치료전평균 2.3점에서치료후 1.7점으로호전되었으며 (p<0.05), 전체적인배변에대한만족도도아기오 투여전평균 2.3 점에서투여후 4.1점으로호전되었다 (p<0.05). 이외증상인배변시힘주기의정도, 대변형태, 배변후잔변감정도, 배변시항문폐색감정도, 그리고복부불편감및복통정도도모두전반적으로호전되는경향을보였다 (p<0.05). 2) 아기오 투여전후대장통과시간변화 (Table 4) 전체대장통과시간은아기오 투여전 46.7±18.3시간에서투여후 18.3±16.7시간으로의미있는호전을보였다 (p=0.020). 대장통과시간을구역별로분석해보았을때우측대장통과시간은 14.9±12.5시간에서 11.3±9.9시간으로감소하는경향을보였고 (p=0.056), 좌측대장통과시간은 22.4±10.2시간에서 16.0±9.8시간으로유의하게감소되었다 (p=0.033). 직장-구 불결장통과시간은아기오 투여전 9.5±9.6시간에서투여후 11.0±8.6시간으로차이가없었다 (p=0.244). 3. 아기오 의안전성아기오 18 g씩매일복용후복부팽만감으로인해복부불편감및복통의악화를호소한 2명 (Fig. 2(6)) 을제외하면임상적으로의미있는이상반응은없었다. 고찰 Psyllium을주성분으로한아기오 18 g을매일 3주동안경구투여한본연구에서아기오 는만성기능성변비또는변비우세형과민성장증후군환자의주관적인배변관련증상을호전시켰을뿐아니라객관적지표인방사선표지자를이용한대장통과시간검사결과에서도유의한호전을보여주었다. 부피형성배변완하제는경도의기능성변비에일차적으로사용되고있는약제로서 Psyllium seed, methylcellulose, calcium polycarbophil 등이포함된다. 9 이중 Psyllium은질경이과에속하는 Plantago psyllium과 Plantago indica L. 의씨앗으로서이들의외피를가공하여점액을생산할수있는친수성의섬유성물질을얻게된다. 이점액질은장관내에서물을흡수하여배변량을증가시킴으로써장관통과시간을감소시키고배변회수를증가시키며대변을부드럽게 하여만성변비에효과를보이는것으로알려져있다. 10 실제로 Ashraf 등 11 은 22명의특발성변비환자를대상으로매일 10 g Psyllium을 4주간경구투여하였을때배변회수, 배변량, 대변의경도, 배변시통증등의주관적증상이호전됨을보고한바있다. 본연구에서도 12.1 g의 Psyllium 이포함된아기오 18 g을매일 3주동안경우투여한후배변관련증상이모두호전되는결과를확인할수있었다. 한편, 아기오 에포함된 Ispaghula husk는 Plantago ovata 의껍질에서추출된성분이포함되는데장관내에서발효되는다른섬유소와는달리발효되지않아완전히분해되

Diagram of Symptoms Change Before and After Agio Administration. 지않고, 이로인하여대변에수분의함유량을높여배변량증가에더기여할수있다. 12 이와같이수용성섬유소와불용성섬유소를균형적으로함유한특성으로인해아기오 는만성변비환자의치료에효과적으로사용될수있다. Psyllium 제재의장관운동에대한효과는상반되는결과들이보고되고있다. Cheskin 등 13 은 1일 24 g의 Psyllium 를복용한군에서대장통과시간이감소함을보고하였으나, Ashraf 등 11 은 1일 10 g Psyllium을 4주간투여하였을때주관적증상은호전되나, 대장통과시간의의미있는변화는보이지않는다고보고하였다. 본연구에서는 Psyllium 12.1 g이포함된아기오 18 g을매일경구투여한결과, 치료전후전체대장통과시간이유의하게감소되었다 (46.7±18.3 분 vs. 37.9±16.7분, p=0.020). 이와같이상반된결과들이나타나는이유는명확하지않으나각연구들에사용된 Psyllium의투여량이서로다르다는점이하나의원인이될수있을것으로생각된다. 성등 14 은식이섬유섭취량과장관통과시간이유의한상관관계를보이는데, 식이섬유섭취량이많을수록대장통과시간이빨라짐을보고한바있다. 따라서섬유소로이루어진부피형성완하제인 Psyllium 또한투여량이많을수록대장통과시간을단축시키는효과가커질것을기대할수있으며, 이러한점이본연구를포함한기존연구들에서의상반된대장통과시간변화결과들을일부설명해줄수있을것이다. 대장통과시간의증가로인한서행성변비가만성기능성변비의중요한원인중하 나임을고려하면, 안정성이확보된다는전제하에저용량 Psyllium 제재에호전되지않는환자들에대해서는고용량의 Psyllium 제재를투여해볼수있을것으로생각한다. 본연구에서아기오 1일 18 g 경구투여는임상적으로의미있는부작용을보이지않아안전하였다. 따라서국내환자들에대해서도필요시아기오 1일 18 g 경구투여는안전하게고려될수있을것으로보인다. 향후, 보다다양한중증도를보이는만성변비환자들을대상으로보다대규모의추가연구를통해 Psyllium 제재의적정용량에대한확인이필요할것이다. 한편, 본연구에서아기오 투여후전체대장통과시간은감소하였으나부위에따른대장통과시간은일관된결과를보이지않았는데, 특히직장-구불결장통과시간은통계적으로유의하지는않았으나오히려다소증가하는듯한양상을보였다. 대상환자수가많지않았다는점및만성변비의중요한원인중하나인골반저부이상 (pelvic floor dyssynergia) 을가진환자가포함되었을가능성이이러한결과에기여했을개연성이있다. 본연구는전향적으로진행되었으며, 아기오 투여전후주관적증상변화뿐아니라대장통과시간의변화까지객관적으로측정하였으므로아기오 1일 18 g 경구투여의효과및안전성에대해객관적으로증명할수있었다고생각된다. 하지만, 비교적적은수의연구대상자가포함되었다는제한점이있으며, 대조군이설정되지않은연구였으므로 1일 6~12 g 투여에비해우수한지에대해서는확인할수없었다. 또한연구대상자들의최근식이섬유섭취량이확인되지않아섬유소제제인아기오의치료반응을예측, 평가하는데참고하지못했고, 약제투여기간중각환자들에게전향적으로식이일기를작성하게하여섬유소섭취에대한정량적분석을하였었다면아기오의효과에대해서보다객관적으로평가할수있었겠으나이러한과정도이루어지지못했다. 향후식이섬유섭취량을확인한대규모환자를대상으로, 식이일기를작성하면서, 아기오의다양한용량에대한비교연구를통해만성기능성변비에서아기오 의가장효과적이고안전한용량에대한탐색이필요할것으로생각한다. 요약목적 : 변비우세형과민성장증후군또는만성기능성 변비환자에서부피형성완하제인아기오 1일 18 g 경구
