Unconjugated bilirubin. 1 Kodak Ektachem Analyzer 의 Dry-slide Technology 1. 건식슬라이드 (dry-slide) 방법 건식슬라이드화학방식은시약에대신하는슬 라이드를사용하여정량분석을하는것이며슬라 이드는우표만한크기이지만습식화학에서수행하 는모든복잡한기능을한다. 건식슬라이드를두가지부분으로나누어설명 할수있는데한분야는광학분야이고하나는임 상화학분야로나눌수있다. 건식슬라이드기법은습식화학방법을그대로 옮겨놓은독특한방법으로서, 습식화학 (wet chemistry) 에서시약을회석하거나혼합하는불편을멀 어주고반응용기가필요없고특히 carryover 가 없다. 시약슬라이드는투명한포리에스터필름위에 여러층으로반응에필요한모든시약이도포, 건 조되어있어각층에각각의특별한화학적, 물리 적기능을갖고분석을행하는독특한방법이다. 슬라이드의종류는 colorimetric 과 potentiometric 두가지로구별되며, colorimetric slide 로는빛의반 사로농도와활성도를측정하는데, 그예로는 glucose, uric acid, albumin, calcium 동을측정하고 potentiometric slide 로는 electolyte 농도를측정한 다. 즉 Na, K, CI, CO 2 이다. 2. Slide 구조와기능 1) Colorimetric slide Colorimetric slide 는맨위층에서부터 spreading layer, reagent layer, indicater layer, support layer 로 이루어져있다 ( 그림 1 ). A cross-sectional view of the 확산충 (spreading layer) 확산층은슬라이드의맨위층에자리하고있으 며스폰지와같은성질을나타내는데그구조가 모세관망상으로되어있어처음검체가슬라이드 위에고루잘퍼지게하여오차를없애게한다. 또 한지질, 단백, 헤모글로빈같은고분자물질이내 려가지않게막아주는역할을하여간섭물질을제 거하고선택적인홉수기능을한다. 또한반사효과 의배경역할을하는데그작용을하는것은 BaS04 또는 Ti0 2 시약충 (reagent layer) 이층은확산충아래에있으며생화학반응이 일어나는데필요한시약이도포되어있다. 이들시 약은효소, 완충제, 촉매제동을포함한다. 때로는몇몇반응이일어나야할때하나씩또 는차례로일어나도록필요에따라만들어져있으 며이것은여러개의시약충으로이루어져있다. 예 : Glucose 검사용슬라이드의시약층 ( 그림 2) - 42-

Ektachem clinical chemistry silde(glu) 또한여기에는경우에따라 scavenger layer 라고 하는특수층이있어서생화학반응에서의방해물 질을제거한다. 즉종래의 uric acid 검사에서방해 가되어오던 ascorbic acid 를제거하여검사상의오 차를없애준다. 즉, scavenger layer 에서검체에있 는 ascorbic acid 와결합히 - 여 ascorbateo-xidase 로 되어시약층에서 ascorbic acid 를제거히 - 발색충 (indica tor lay er) 여기에는발색만응을일으키는발색제나색소 Reflectence spectrometry ( 그림 3) 색의농도를측정하는데있어서습식생화학적 검사법에서는시약과반응이일어나는것을 cuvette 을투사한빛이통과한것을읽지만건식생 화학적검사법에서는빛을투사하면확산층 (spreading layer) 에서반사한것을 photodetector 에서 읽는다. 그러므로헤모글로빈이나벌리루빈같은방 해물질이습식생화학적검사법에서는혼합된상 태로윈어지지만건식생화학적검사법에서는확 산층 (spreading layer) 에서걸러진상태에서원게 되므로그들로인한오차가없다 ( 그림 4). 가들어있는데이런발색제는분석시약농도에 /17/ Light Path 비레해있으며이층에서시약층과색소가결합하 Support layer Fig. 3. Principle of the PM slide technology 3. 장비의구조및원리 Main unit, control unit, printer 의 3 부분으로되어 았다. Main unit 는컴퓨터시스템과슬라이드를저 장, 공급하는모률로되어있다. Control unit 는키 보드와 display unit 로되어있으며, 오퍼레이터와 장비의교신역할을한다. 프린터는 2 대를연결시 킬수있어서검사실과그외다른곳에도설치하 여동시에프린트시킬수있다 ( 그림 6). 1) Modules 총 3,000 개의슬라이드를저장할수있으며, 슬 라이드는 supply 1, supply n 로두곳에각각넣 게되어있고, 각각 30 개의캐드리지를넣게되어 있다. Supply 1 : 습도가 25-45% 로유지되도록되어 있으며, TP. URIC. ALB.TRIG. CHOL. AMYL. Cl. K. Na. TBIL. LIPA. Mg. 의슬라이드를저장한다. Supply n : 습도가 20% 로유지되도록되어있 다. GLU. NH3. CREA. AST. ALKP. ALT. LDH. CK. GGT. THEO. CKMB. 의슬라이드를저장한다. 2) Rate incubator 24 개의슬라이드를 37 0 C 에서 5 분 30 초동안 Incubation 한다. Supply n 로부터슬라이드가오면 sample metering station 으로부터검체를받고 incubaton 한다. Incubation 이끝날때까지 54 회읽 는다. 54 회읽는것은효소의활성도를측정하는 것이며컴퓨터는효소의활성도를측정하여 kinetic curve 의 slope 를만들고계산치가프린터로간 다. 즉한슬라이드에서발색변화속도에따른반 사도를측정하여효소활성도를측정하는것이다

Unconjugated 로분석되는간접빌리루 빈은일차적으로알부민과결합되어있고디 - 만 methanol 과같은 mordant 아래서 diazotized sulfanilic acid 와반응한다. 이런원리는 Malloy-Evelyn method 에서또는 caffeine 과 sodium benzoate 를이용하는 Jendrassik -Groff( 이상 J.G 로표기 ) 원리에서이용되었다. 간접 직접빌리루빈이란용어는검사실이나임 상의학에서오랫동안통용되어왔 며 hyperbiliru - binemia 진단에흔히이용되어왔다. 그러나이제는직접빌리루빈이란용어가잘못 정의된것이라는게알려졌다. 더구나소변에서빌 리루빈이안나오거나적게검출되면서혈청에 conjugate bilirubin(bc) 이증가되는것이특정인케이 스에서는항상 Bc 가출현하리라는일반적인경향 과는일치되지않은것이다. 그러므로 direct bilirubin 은 mono 와 diglucuronidated form 외에다른종류가있기때문에그개 념은바뀌어져야한다 i1i rubin 의분류 Kuenzle 과 Lauff 는 LC (liquid choromatography) 를이용하여네종류의 bilirubin(bil) 분류를병적 혈청에서논증하였다. 그하나는 a-form 으로간접또는 unconjugated bilirubin(bu) 이라흔히불려지는것으로서이는 비수용성이며알부민과결합되어 lipophilic form 을 구성하며신장에서여과되지않는다. 두번째종류는 β 또는 monoconjugated bililubin glucuronide 이고셰번째는 y 또는 diglucuronide 이 며나중두형태는수용성 로서직접반응을일 으키며혈청에서이들직접빌리루빈이증가될때, 소변에서도증가된다. 마지막한종류는델타빌리루빈 (δ BIL) 으로서 가장최근에알려진것으로서알부민분획에붙어 있는빌리루빈으로구성된다. 텔타빌리루빈은전 반적으로직접반응을일으키나알부민과배위결 합 (covalently bound) 되어서 glomeruli 에의하여여 과되지않는다. 또한, 이는알부민과빌리루빈이강하게결합되었다고 biliprotein 이라고도한다. 몇몇빌리루빈측정용액체시약에는 indirect bilirubin 은 a-bil 과거의같고 β, ô, y 형태의빌리루 빈의합이직접빌리루빈 (DBIL) 과거의같다. 그 러나, 대부분의액체시약방법에서는 DBIL 이 y, ß 그리고 δ BIL 보다낮다. 3. 델타빌리루빈 (ô 81 니 델타빌리루빈은빌리루빈과알부민의복합체이 다. 이것은정상인의혈청에서는검출되지않으나 Bc 가증가되면어떤경우에도발견된다. Mc Donagh 는동물실험에서 bile-duct ligation 이 Bu, Bc, 델타빌리루빈의빠른축적을이룬다는것 을증명하였다. 실제로댈타빌리루빈은사람혈청 이나 Sprague-Dawley 쥐의혈청과함께 bilirubin diglucr onide 를 37 0 C 에서 incubate 하므로써형성될 수있다. Mc Donagh 팀은 biliprotein 의형성이알부민괴 Bc 의 nonenzymatic 반응을포함한다고결론지었 다. Gautan 동도 3TC, ph 7.4 에서쥐의혈청에서 델타빌리루빈을형성할수있었다. 길버트증후군을가진사람에대한그들의연구 는 Bc 가델타빌리루빈의 precursor 이고 Bu 는아 니라는것이다. 즉 Bu 는델타빌리루빈으로바뀌기전에 glucuronidated 되어야한다는것을확인했다. 텔타빌리루빈의반감기는 일또는알부 민과같은것으로알려져있다. 텔타빌리루빈은 glucuronidated bilirubin 보다쥐에서 serum halflife 가더길다. Mc Donagh 연구에서쥐에서의담관폐색은신 체밖으로 fistula 를통한담즙배설을수반하며담 관폐색을제거한후 2 시간안에 glucuronide bilirubin 의빠른감소를보였으며 7 시간안에완전히 소실되었다. 담즙배설후 7-12 시간후에혈청의 LC 분석 에서 9 마리쥐중에서 6 마리에서 predominent bilirubin fraction 은델타빌리루빈이었다. 텔타빌리루빈의반감기는 Weiss 둥에의해서 일또는알부민과같다고추정되었다. 분명히텔타빌리루빈은알부민으로부터분리되 지않고또한알부민과독립적으로소실되지않는다. 4. 표준화의경향 최근거의모든임상검사실에서실시하고있는 임의의수치는표준액, 시약의촉매또는다른인 자에달려있다. 다음그림은 John A. Lott 팀이신선한환자혈청 으로 Bc 를 aca 와 Astra 방법으로측정하여비교한 것이다 ( 그림 7)

Dye 는 nm 에서 plateau 를이룬다. 적당한 computer algorithms 을 이용해서 nm 에서반사를측정하여빌리 루빈의양을알아낸다. 이방법은적어도 210 mg/ dl 까지는근사치를측정할수있다. 혈청을슬라이 드에직접떨구어서각층으로이루어진시약과 슬라이드에서반응을하게한다 ( 그립 8). Sample Regresion plot 는그림 7 에서와같이두방법에 서우수한관계를보여주는데 aca 와 Astra 의값 의차이는 Astra 의결과에대히 - 여 aca 의결과가 지속적으로평균 0.7 mg/dl 가높게니 - 왔다. 이와같이 Bc 의표준액이없을때는그검사결과 를정확하디 - 고말할수없다. Bc 의결과가정확하지못하면총빌리루빈의결 과가믿을만해도간접빌리루빈은부정확할것이 다. 그이유는간접빌리루빈은총빌리루빈의차 로계산하기때문이다. Ou 동은 DBIL 이어떤방 법에서는임상적으로중요할수있는간접빌리루 빈분류를과대평가한나머지특히 neonates 에서 너무낮게나온다는것을증명하였다. 간접빌리루빈은광선치료또는교환수혈시비정상적으로증 가한다. 5. Dry film 방법에서의빌리루빈분류 Kodak 은모든빌리루빈분리를위한순차적인 과정을 dry film 기술로개발했다. Film slide 법에서 빌리루빈을알부민으로부터분리하기위하여 dyphylline Triton X-I00 을채택했다. 모든빌리루빈의종류는 diazonium salt 4-(N-carboxymethyl sulfamyl)-benzenediazonium hexafluorosphate 와반응하여 azo dye 를생성하며이것은 520nm 에서 absorbance 피크를나타낸다. 착색료 copoly(styrene-/n-vinylbenzyl-n-n-dimethylbenzylammonium chloride / divinuylbenzene) 에 의하여반사파장이최고에달하게하고 diazobilrubin 의발색을증대시킨다. 발색된색조는투명한필름을통하여 ref1ectom- Fig.8. Unconjugated and glucuronidated bilirubin 그림 9 는 a, β, r bilirubin(bu and glucuronidated form) 을측정하는데사용하는 BuBc 슬라이드 를나타낸것이다. 발색제와반응한후 mono- 와 diglucuronidated bilirubin (Bc) 가 nm 에서반사를보이는 반면 Bu 는 nm 에서최고의반사값을보 인다. Mordant-enhanced bilirubin pigment 를사용 하여분광계를직접 \} 용하였을때발색제의흔재하에 Bu가 endogenous bilirubin 의약두배의반 사를갖는다. [TBIL] - 47-

When serum, urine, or CSF come into contact with these dry chemical layers, the reaction occurs. The chemical reactions thus incorporated into the slides are similar to those used in traditional wet chemistries. There are two basic types of slides used by the Kodak Ektachem 700 analyzer. Colorimetric ( CM) slides measure analyte concentration or activity by light reflectance. The intensity of light reflectance is preportional to the color density formed. Potentiometric(PM) slides measure electrolyte concentration by the potential difference between the sample and referenced electrodes. The top layer of a colorimetric slide is the spreading layer. A drop of undiluted sample is metered onto this layer of the slide. The term metered indicates that a precisely measured amount of sample is placed onto work, which allows the liquid in the specimen to penetrate through to the other layers. Its porous capillary network assures uniform distribution of the sample volume over the slide area. The structure of the spreading layer traps large molecules such as lipids, proteins, and hemoglobins. This eliminates interference from these substances and creates a type of protein-free filtrate. The reagent layer contains the reagents necessary for the chemical reactions to occur. These reagents include enzymes, buffer, and catalysts. Often several different reactions must occur, making it necessary to separate and control each reaction sequence. This is made possible by constructing the slide with serveral reagent layers. The indicator layer of the colorimetric slide contains a dye or similar indicator to produce a colored complex formed is the slide by the analyzer. This method eliminates potential errors caused by initial sample dilution. The spreading layer is a porous capillary net- in proportion to the analyte concentration. The support layer is the bottom layer of the slide. It is made of a clear plastic, onto which all other layers are applied. The purpose of the support layer is to allow light to pass through, so that the colored complex can be measured. (2) New concepts in serum bilirubin mesurement. For nearly 70 years, laboratorians have used the terms 'direct bilirubin' to mean bilirubin conjugated to glucuronic acid and 'indirect bilirubin' to mean unconjugated bilirubin. Van den Bergh described the measurement of bilirubin in serum in 1913 the 'direct' and 'indirect' tests were described shortly there after. Direct-reacting bilirubin has always been thought to consist solely of bilirubin molecules esterfied to one or two molecules of glucuronic acid. These forms are water soluble, are cleared by the kidneys, and react at once with diazotized sulfanilic acid. Indirect bilirubin is the unconjugated analyte ; is bound primarily to albumin, and reacts with diazotized sulfanilic acid only in the presence of ac- celerators such as methanol, which is used in the Malloy-Evelyn method, or a mixture of caffeined and sodium benzoate, as employed in the Jendrassik -Grof procedure. The terms direct and indirect bilirubin are now deeply embedded in the practice of laboratory and clinical medicine and have served reasonably well in the differential diagnosis of hyperbilirubinemias. However, it is now known that 'direct' bilirubin is an ill-defined quantity. Furthermore, cases of an increased serum concentration of conjugated bilirubin with little or no bilirubin in the urine have been descrbed-a finding that contradicts the prevalent notion that conjugated bilirubin should always appear in the urine when it is present in abnormal a mounts in the serum.
