1 대한영상의학회지 2010;63: 천대퇴동맥스텐트골절발생의위험요인및임상적연구 : Prote ge GPS 스텐트 1 이다운 김재규 정혜두 허태욱 임남열 오현준 장남규 2 최수진나 3 목적 : 천대퇴동맥스텐트 (Prote ge GPS) 골절의발생빈도를평가하고, 스텐트골절과원인인자및임상양상의연관성에대해알아보고자하였다. 대상과방법 : 천대퇴동맥에발생한동맥경화성폐쇄또는협착을 Prote ge GPS 스텐트삽입으로치료하였던환자 38명중스텐트모양의추적관찰이가능하였던 17 명을대상으로하였다 ( 남자 15명, 여자 2명, 평균연령 72.8세 ). 총 17개의천대퇴동맥에 43개의스텐트가삽입되었고, 병변의평균길이는약 26.4 cm (15-50 cm) 이었다. 시술에사용되었던혈관조영기기를이용하여전후, 측면단순영상을획득후골절을평가하였으며, 동시에색도플러초음파, 카테터혈관조영술또는 CT혈관조영술을이용하여스텐트내부혈류를평가하였다. 스텐트의길이, 위치및수등에따른골절발생률의차이를분석하였으며골절과개통률의차이도알아보고자하였다. 결과 : 스텐트의골절은 43개의스텐트중 14개 (30.2%) 에서발생하였다. 10개의스텐트 (71.4%) 가천대퇴동맥의상부 1/3, 4개의스텐트 (28.6%) 가하부 1/3에서골절이발생하였다. 단일 strut 골절이 10예 (71.4%), 2개이상의 strut 골절이 4예 (28.6%) 있었다. 병변및삽입된스텐트의길이가길수록스텐트골절의발생빈도가높았다 ( 각각 p=0.021, 0.012). 관절과가까이스텐트가삽입될수록골절발생빈도가높았으며오금동맥까지연장하여스텐트가삽입될수록하부 1/3에골절이잘발생하였다 (hip joint; p=0.01, knee joint: 0.029, insertion in popliteal a.: 0.015). 스텐트개수와골절간의유의한상관관계는보이지않았으며 (p=0.126), 골절여부에따른혈관개통률은의의있는차이를보이지않았다 (p=0.44). 결론 : 천대퇴동맥에삽입했던 Prote ge GPS 스텐트는추적기간에비교적많은골절이발생하였고, 병변과스텐트의길이및관절과의거리와유의한상관관계를보였다. 골절여부와개통률사이에의미있는관계는없었다. 천대퇴동맥 (superficial femoral artery, 이하 SFA) 의동맥경화성협착또는폐쇄의치료방법으로풍선카테터를이용한경피적경혈관성형술 (percutaneous transluminal angioplasty, 이하 PTA) 은높은성공률을보이고있다. 하지만, 병변의협착범위가길거나완전폐색인경우, 석회성협착, 한쪽으로치우친병변, 그리고 PTA 후발생한동맥벽의박리등은재발률과합병증의발생률이높은것으로알려져있다 (1-3). 최근에는혈관의개통률향상을위해 PTA와함께스텐트삽입을이용한치료가시행되고있으며 3년개통률이 1 전남대학교의과대학영상의학교실 2 화순전남대학교병원영상의학교실 3 전남대학교의과대학외과학교실이논문은 2010년 4월 26일접수하여 2010년 8월 2일에채택되었음 % 이상으로높은것으로보고되고있다 (2, 4, 5). 그러나천대퇴동맥자체의해부학적, 혈류역학적특징과스텐트의형태학적변화때문에, 스텐트삽입을이용한치료의유용성은논란이되고있다 (2, 3, 6). 최근보고들에따르면천대퇴동맥에삽입된스텐트자체의골절이비교적흔하게발생하며, 재협착의중요한원인이될수있다고한다 (7-9). 하지만, 대부분이다양한병변의길이와여러종류의스텐트를대상으로한연구로써비교연구에제한이있었다. 본연구는단일스텐트를이용한연구로써 15 cm 이상폐쇄병변을대상으로하여골절의위험인자및재협착과의연관성에대하여알아보고자하였다.

2 이다운외 : 천대퇴동맥스텐트골절발생의위험요인및임상적연구 대상과방법연구대상 2006년 8월부터 2008년 8월까지 CT 및혈관조영상천대퇴동맥에국한되어폐쇄병변을보인환자중 Prote ge GPS 자가확장성니티놀스텐트 (ev3, Minneapolis, MN, USA) 를삽입한 38명중추적관찰기간이 3개월이상, 병변의길이가 15 cm 이상, 그리고투시촬영및단순촬영을이용해스텐트모양의추적관찰이가능한 17명을대상으로하였다. 남녀비는 15:2이었으며평균연령은 72.8세 (58-89세) 였다. Rutherford-Becker category의분류에의한환자들의임상증상은간헐성파행 3명, 휴식기동통 8명, 하지괴저 6명이었다. 시술스텐트일차삽입기준은완전폐쇄, 90% 이상의협착또는동맥벽의박리가있는경우이며, 이차삽입기준은먼저 PTA 를시행한후, 남아있는협착이 30% 이상이거나동맥벽의박리가의심되는경우였다. 17명의환자에서총 17개의천대퇴동맥에 43개의스텐트를삽입하였다. 시술의기술적인성공여부는스텐트내경이 30% 이상의협착부위없이완전히확장되고, 조영검사상병변하방으로혈류가잘유지되는것으로하였으며, 시술직후모든환자에서혈관의확장과병변하방으로잘유지되는혈류를확인할수있었다. 기술적으로스텐트삽입이실패한경우는없었다. 임상적으로는 Rutherford- Becker category가최소한단계이상상승하는것으로평가하였다. 추적조사추적관찰기간은재발한경우에는재협착이진단된날로하였고재발하지않은경우에는마지막검사일로하였으며 6개월에서 29개월로평균 12.9개월이었다. 추적관찰은혈관조영기기인 Multistar T.O.P (Siemens, Erlangen, Germany) 를이용하여스텐트를전후, 측면에서촬영하여골절양상을분석하였으며추가로색도플러초음파, 카테터혈관조영술, 또는 CT혈관조영술을이용하여스텐트내부의혈류를평가하였다. 골절의분류는 Cardiovascular Institute of the South (CIS) 에서발표한 stent fracture classification system의분류법을따랐다 (Fig. 1)(7). 모든환자는시술후아스피린 (325 mg/ 일 ) 과 Ticlid (500 mg/bid) 를 3개월간복용하였다. 재협착은임상적으로재발이의심된경우에추적혈관조영술상치료받은동맥의직경이 30% 이상감소하였거나, 색도플러검사상 PSVR (peak systolic velocity ratio) 이 2배이상되거나도플러파형이폐쇄소견을보인경우로정의하였다 (10). 분석환자의연령, 성별, 병변및스텐트의길이, 스텐트의수와스텐트골절의위험을분석하였다. 관절과가까이스텐트가위치할수록골절위험이클것이라는가정하에혈관조영술시대퇴골두 (femoral head) 와대퇴골과간함요 (intercondylar fossa) 를경계지표로이용하였으며이들과스텐트와의간격을 Type I Type II Type III Type IV Type I A single strut fracture only Type II Multiple single nitinol stent fractures that can occur at different sites Type III Multiple nitinol stent fractures resulting in complete transverse linear fracture, but without stent displacement A B Fig year-old man with right leg claudication. 대한영상의학회지 2010;63:513-518 측정하였다. 상부 1/3 골절의위험요인으로총대퇴동맥에삽입여부및대퇴골두와스텐트간의거리를분석하였고하부 1/3 골절의위험요인으로오금동맥에삽입여부및대퇴골과간함요와스텐트간의거리를분석하였다.

3 대한영상의학회지 2010;63: 측정하였다. 상부 1/3 골절의위험요인으로총대퇴동맥에삽입여부및대퇴골두와스텐트간의거리를분석하였고하부 1/3 골절의위험요인으로오금동맥에삽입여부및대퇴골과간함요와스텐트간의거리를분석하였다. 통계총 43개의스텐트를대상으로스텐트골절과위험요인과의관계를 Logistic regression analysis를이용하여알아보았고골절과재발률을 Kaplan-Meier method 및 log-rank test를이용하여분석하였다. 결과병변의평균길이는 26.4 cm (15-50 cm) 이였다 TransAtlantic Inter-Society Consensus (TASC) 에의한병변의분류는 Type C 8명, Type D 9명이었으며, Type D 병변은모두오금동맥까지병변이확장되어있었다. 일차스텐트삽입동맥은 8개, 이차스텐트삽입동맥은나머지 9개였다. 1개의동맥에평균 2.53 개의스텐트가삽입되었으며 (1 개 : 1명, 2개 : 7명, 3개 : 8명, 4개 : 1명 ), 천대퇴동맥에삽입된스텐트의길이는평균 27.5 cm (8-38 cm) 이었다. 17개의동맥중 11개의동맥에서천대퇴동맥전체에걸쳐스텐트가삽입되었고, 1개의동맥에서중부 1/3, 5개의동맥에서상부와중부 1/3에스텐트가삽입되었다. 삽입된 43개스텐트중 14개 (30.2%, 11/17명환자 ) 에서골절이발생하였다. 14개골절중 10개 (71.4%) 는 1형단순골절 (single strut fracture), 4개 (28.6%) 는 II형다중단순골절 (multiple single fracture) 이었다 (Figs. 2, 3). 특히 10개 (71.4%) 의골절이 Fig. 3. 4 이다운외 : 천대퇴동맥스텐트골절발생의위험요인및임상적연구 천대퇴동맥의상부 1/3에서발생하였으며, 4개 (28.6%) 가하부1/3에서발생하였다. 스텐트골절발생의위험요인을분석한결과는표 1과같다. 병변및삽입된스텐트의길이가길수록스텐트골절의발생빈도가높았으나 ( 각각 p = 0.021, 0.012) 삽입된스텐트의수와의유의한연관관계는없었다 (p = 0.126). 오금동맥에연장하여삽입될수록, 대퇴골과간함요와가까이삽입될수록하부골절이잘발생하였다 (p = 0.015, 0.029). 대퇴골두와가까이삽입될수록상부골절이잘발생 (p = 0.012) 하였으나총대퇴동맥삽입여부와골절발생빈도와는통계학적으로유 의한상관관계가없었다 (p = 0.402). 2개의스텐트에서스텐트겹침부위에골절이발생하였으며 (Fig. 2) 나머지 12개의스텐트에서골절부위와관계없이재협착이발생하였다 (Fig. 3). 전체 17명의환자중 7명의환자에서시술후증상의호전 (Rutherford- Becker category가 1 단계이상상승 ) 을, 3명의환자에서는증상이유지되었으며, 5명의환자에서절단수술을, 2 명의환자에서우회로조성수술을받았다. Kaplan-Meier 방법을통해분석한 6개월누적개통률은골절이없었던환자에서 50%, 골절이발생한환자에서 45.5% 로골절이없으면개통률이비교적높았으나이들사이에연관관계는없었다 (p = 0.44) (Fig. 4). 고 찰 Fig. 4. Primary stent patency rates for fractured and non-fractured stents (p=0.440). 천대퇴동맥은가장긴말초동맥으로주변근육및관절의움직임에의하여다양한힘을받게되며이에의하여재협착이빈번히발생하고, 골절또한잘발생할것으로생각한다. Sirolimus Coated Cordis Self-expandable Stent (SIROCCO) trial (9) 에서스텐트골절이발생할수있음을처음으로보고하였고이후이에관한몇몇연구들이발표되고있다. 최근발표된연구들에서스텐트의길이가길수록, 삽입한스텐트의수가많을수록골절이잘발생하고, 스텐트의골절양상이심할수록재협착의빈도역시높아진다고보고하였다 (7, 8, 10). 스텐트골절이발생할수록재협착이잘발생한다고보고 Table 1. 5 대한영상의학회지 2010;63: 되고있지만이에대하여논란이있다 (11-13). 본연구에서사용된 Prote ge GPS 자가확장성니티놀스텐트는 intracoil 스텐트로횡축으로는방사강도 (radial strength) 를종축으로는유연성을가져주변근육에의한압박, 구부림, 비틀림, 신장, 수축등에의한스텐트의변형을최소화할수있게제작되었다. 실제로 Allie 등 (7) 에의한연구에따르면 intracoil (ev3, Plymouth, MN, USA) 스텐트에서골절이발생하지않았으며, 여러스텐트를이용한 Scheinert 등 (11) 에의한연구에서병변의길이가 8 cm에서 16 cm 미만의병변에서 42.4%, 16 cm 이상에서 52.0% 의높은골절발생률을보였던것과달리이연구에서는 30.2% 로비교적낮았다. 본연구에서스텐트골절은병변및스텐트의길이가길수록빈번하게발생하였으나삽입된개수가많아중첩될수록축성경직 (axial stiffness) 이증가하여골절이잘발생한다는이전에제기되었던연구결과 (11) 와달리스텐트숫자와의관련성은없었다 (p = 0.126). 실제로 14개의스텐트골절중스텐트겹침부위에서골절은두개의스텐트에서발생하였고 (Fig. 2) 재협착은골절부위에관계없이발생하였다 (Fig. 3). Scheinert 등 (11) 에따르며스텐트골절이천대퇴동맥에고르게발생하였으나본연구에서 13개의골절중 10개 (71.4%) 가상부 1/3에분포하였으며이는 1개를제외한나머지동맥에서모두상부 1/3에스텐트가삽입되었기때문으로생각한다. 관절과가까이스텐트가삽입될수록상부와하부 1/3에골절이발생할위험이컸다. 또한, 오금동맥에연장하여스텐트가삽입될수록하부 1/3 골절의발생위험이컸다. 총대퇴동맥내스텐트삽입여부와상부 1/3 골절의발생여부는통계학적으로유의한상관관계를보이지않았다 (p = 0.40). 골절과재협착간의관계에대하여다양한보고가있지만본연구에서 Kaplan-Meier 방법을통해분석한 6개월누적개통률은골절이없었던환자에서 50%, 골절이발생한환자에서 45% 로골절이없으면개통률이비교적높았으나통계적으로유의하지않았다 (p = 0.440). 이는본연구에서 type III 이상의심각한골절이없었으며병변이길어골절에관계없이재협착발생률이높았기때문으로생각한다. 실제로삽입된스텐트의평균길이가 Schlager 등 (8) 은 13.9 ± 1.8 cm, Scheinert 등 (11) 은 15.7 ± 5.9 cm였던반면본연구에서는평균 27.5 cm로길었고, 1명의환자를제외한나머지 16명의환자에서추적기간에재협착이일어났다. 하지만, 본연구에서와같이길게삽입된스텐트에서스텐트의숫자와골절발생과의연관성및골절과재협착과의관계에대한보고가없어이에대한추가적인비교연구가필요할것으로생각한다. 본연구는 17명의작은수의환자를연구대상으로하여각각의요인들간에통계적으로의의있는결과들을도출해내기 어려웠으며, 충분한장기간의추적검사가이뤄지지못한제한점이있다. 결론적으로천대퇴동맥에삽입했던 Prote ge GPS 스텐트는추적기간에비교적많은골절이발생했고, 발생빈도는병변및스텐트의길이와유의한상관관계를보였으며관절과가까이스텐트가삽입될수록골절의발생빈도는높아진다. 골절과재협착간의유의한상관관계는보이지않으나삽입된스텐트의길이에따른추가적인연구조사가필요할것으로생각한다. 참고문헌 1. Materials and Methods: Of the 38 patients who underwent treatment with Prote ge GPS stenting due to arterial occlusions on the superficial femoral artery, 17 also underwent a clinical analysis. Forty-three stents were inserted in the 17 superficial femoral arteries, ranging between 15 and 50 cm in length, with a mean treated length of 26.4 cm (15-50 cm). A fracture was evaluated by taking a PA and lateral simple radiography, as well as a follow-up evaluation accompanied with a CT angiography, DSA, and a color Doppler sonography. The examination involved the assessment of the difference between bone fractures due to length, placement, and frequency. Results: Fractures occurred in 13 of 43 stents (30.2%). A total of 10 (71.4%) occurred in the upper third, compared to 4 (28.6%) in the lower third of the superficial femoral artery. In addition, 10 stents (71.4%) had a single strut fracture, whereas 4 (28.6%) had multiple strut fractures. A stent fracture occurred more frequently when the stents and lesions were longer (p=0.021, 0.012) and the stents were inserted near the joint. However, there was no significant relationship between stent numbers and the fractures (p=0.126). When the stents were inserted along the popliteal artery, a stent fracture occurred more frequently in the lower third of the artery. The stent fractures did not significantly influence the patency rate of the stented artery (p=0.44) Conclusion: Prote ge GPS stents in the superficial femoral artery revealed a considerable number of fractures and the fracture frequency showed a significant relationship with the length of stents and lesions. The closer stent insertion was to the joints, the more frequently fractures occurred. There were no evident significant relationships between the presence of stent fractures and the patency of the stented arteries.
