1 大韓超音波第 9 券第 4 號 2007 Vol. 9 No. 4, December 2007 종설 임신중내시경의안전성 순천향대학교의과대학천안병원산부인과학교실 김윤숙 The Safety of Diagnostic Modality in Pregnancy: Endoscopy Yun-Sook Kim, M.D. Department of Obstetrics & Gynecology, Soonchunhyang Chunan Hospital, College of Medicine, Soonchunhyang University, Chunan, Korea 서론 1) 내시경검사는많은검사기관과병원에서손쉽게행해지고있는위험도가적은진단방법이다. 그러나임신중위장내시경의안전성과효과에대한연구는그리많지않다. 임신중태아와모체에대한위험도는임신주수, 검사시행의적응증, 모체의기저질환등에따라많은차이가있다. 임산부의경우전향적연구를하는데제한점이있어위험도가정확히알려져있지않으므로검사여부의결정은개인마다큰차이가있다. 따라서산과의사와내시경을시행하는의사상호간의협력이매우중요하다. 검사결정에있어첫째태아, 둘째임산부, 셋째임상적인상황의긴박성, 넷째다른사용가능한대체방법유무, 다섯째검사를분만후까지안전하게미룰수있는상황인지를꼼꼼히따져야한다. 우선임신일삼분기에검사를시행할때는약물과방사선노출의위험성이있으므로팀접근이필요책임저자 : 김윤숙 하다. 임신후반기에는커진자궁으로인해복강및골반내장기의위치에변화가올수있어대부분임산부검사에익숙치않은내시경을시행하는의사들이힘들어할수있다. 이렇게검사가익숙치않아발생할수있는문제점으로검사시간이연장되고, 정맥으로진정제투여량이증가하며, 더잘볼수있게하기위한가스주입량이증가하면서임산부의불편감이증가하고, 임산부와태아모두에게해를줄수있다. 검사받는자세는커진임신자궁에의한 vena caval compression을최소화하기위해 supine position을피하고, lateral 자세를취해야하고시행의사는여기에익숙해져있어야한다. 따라서임산부에게는 prudently, rapidly, accurately 하게검사를시행해야한다 내시경의구성 본론 194

2 상황이므로모체의저혈압이오면혈류량의감소가일어나게된다. 이러한상황에서태아가사에의한서맥이오고, 모체에투여한 narcotics와 sedative에의해부정맥이올수있다. 이러한이론적인 suboptimal outcome에도불구하고, 임신중내시경을받은임산부에게서태어난태아에서내시경을받지않은임산부에서태어난임산부에비해 fetal wastage 나 morbidity 가더높다는증거는없다. 게다가 83명의여러가지적응증으로 upper endocopy를시행받은임산부의 case-control study에서유도분만, still birth, 아프가점수에있어서일반인과비교하여차이가없었다. 또한 Amerian College of Gastroenterology에서 3,000명을대상으로한 mailed survery에의하면임신중 73명의 upper endoscopy와 13명의 colonscopy를시행받은환 2. 태아에대한위험내시경을시행하기위한전투약에의해태아에게는잠재적발달장애를유발할수있고, 임산부에게내시경을함으로서조기진통을유발시키거나, mechanical trauma를줄수있다. 그러나아직까지이러한 data를증명할만한자료는거의없는실정이다. 더욱이이러한시술의경우는 prospective, randomized 연구를도덕적인입장에서진행하기가어렵다. 태아는또한내시경동안투여하는약물의생리적효과에의해저산소증, 저혈압이유발되어유산, 사산, 저체중아, defined congenital abnormality, neurologic developmental delay를유발할가능성을가지고있다. 태아는산소해리곡선상으로볼때모체 PO 2 가 60 mmhg 이상을유지하는동안 protected 되어 sleep portion을유지한다. 또한저혈압상태의임산부에게는보상적인혈관수축이일어나자궁혈류감소가일어난다. 자가조절을할수있을정도의많은 capacity를가지고있지못한 자에서임신에어떤 adverse effect도증명하지못했다 년 10개의 medical center에서임신중 48개의 flexible sigmoidoscopy와 8개의 colonoscopy를시행한결과를보고하였으나태아에게이러한 procedure가해를주었다는증거를찾지못하였다. 지금까지 published 된자료에의하면임신중 endoscopy가태아에게해를준다는것을증명하지는못했으나안전하게시행하기위한 guideline이필요하다. 따라서 Amerian Society of gastroenterology (ASGE) 에서는 2005년에 ASGE guideline: guidelines for endoscopy in pregnant and lactating women 을정하였다. 8 임신초기에노출시에다음과같은 physical factor 가태아에게해를줄수있다. physical factor에는 fluoroscopy 중의 radiation exposure, 용종제거수술중에사용하는 electrical current, hot biopsy, electrocoagulation이있다. 임신중에는생리적으로담즙의 supersaturation, biliary hypomotility가흔히발생한다. 불행히도이러한현상으로 biliary sludge와 Table 1. ASGE Guidelines for endoscopy in pregnant and lactating women (1995) ASGE Guidelines for endoscopy in pregnant and lactating women (1995) 1. 가능하다면분만후까지검사를연기하고, 안되면임신일삼분기이후로검사를연기한다. 2. 임신과관련된합병증이발생할가능성이있으므로검사전반드시검사계획단계부터산부인과의사를참여시켜야한다. 3. fetal well-being은검사직전에반드시확인을하고, 가능하다면검사직후에도 reevaluation을한다. 4. 고위험상황에있는임산부에서는 intraprocedural fetal monitoring을가능하면시행하도록한다. 5. hypoxia의상황을최소화한다 - maternal sedation을최소화한다. - nasal canula로 O 2 를공급한다. - supine position을피하고, left lateral position을취한다. Risks of endoscopy to the mother Risk aspiration hypotension delayed diagnosis and treatment bowel and uterine trauma Precipitating factor abdominal distension inferior vena cava compression, anemia reluctance to employ tests and procedures, concern regarding administration of medications, difficulty examining abdomen, atypical presentations distorted anatomy resulting from gravid uterus gallstone 이형성되고이러한현상이 biliary colic, cholecystitis, 심지어 pancreatitis를유발한다. diagnostic imaging으로안전한초음파와 MRI를시행하는것이좋지만진행중인 cholelithiasis를치료하고 biliary drainage를시키기위해서는 endoscopic retrograde cholangiopancreatography (ERCP) 동안 fluoroscopy의사용이필요하다. radiation에의한 teratogenesis의 level은 5-10 rad이다. 태아에게방사선의조사를최소로하기위해서는임신자궁을가리기위한 abdominal shielding이가장중요하고, 다음으로시간을최대한빨리하는것이중요하다 Electrocautery cutting과 cauterizing이안전하게시행될수있으나양수가 electrical current를유발할수있다. 외부에 grounding pad를놓아야하며 bipolar current만사용할수있다. colon polyp을제거할때는대부분응급이라기보다는 elective로시행하므로분만후로미루는것이좋다. 4. 모체에대한위험임산부는임신으로인한 physiological, pregnancy-related 라는말로진단이늦어져위장계통 의증세에대해진단이늦어지고예후가나쁠수있다. 더욱이커진자궁으로인해압통을유발하는부위의변화가올수도있고, 이로인해의사에게혼란을줄수도있다. 따라서산과의사와 endoscopist는임산부에서발생할수있는 potential hazard에대해충분히숙지하고있어야한다. 임산부에서임신자궁이 inferior vena cava를눌러서 venous return이감소하여기립성저혈압이발생할수있으므로 prone과 supine position은피해야한다. 그리고임신한자궁이장의 lumen을눌러서내시경검사시의 landmark 가잘안보일수있다. 따라서내시경검사시혼란을초래하여시간이더많이걸리고잘보이게하기위해더많은 air 를넣어특히 colonoscopy 시에장비의 looping (doubling back) 을초래할수있어임산부에게 abdominal distension과 discomfort를유발할수있다. 또한 colonoscopy시에 manual external abdominal compression을시행할경우자궁에서멀리시행해야한다. 이러한여러내시경과관련된합병증을최소화하기위해적절한적응증이되는경우에만내시경을시행해야한다. 196

Contraindications for endoscopic procedures Obstetric condition 1. imminent delivery 2. abruptio placenta 3. eclampsia 또한분만이급박한상태나산과적인응급상황에서는가능하면내시경을시행하는것은피해야한다. 임산부에서의내시경은경험이풍부한내시경시술의사와장비가잘갖추어진상태에서 nasogastric tube에의해위내용물을모두제거하고, 전날가급적이면금식을한후시행한다. 임산부에서는 nasal oxygen을투여하면서시행하는것이좋다. 예방적항생제투여는반드시필요한것은아니지만 biliary sepsis가의심되는경우나 endocarditits의예방을위해적응증이되는경우에투여한다. 5. Indications for endoscopic procedures during pregnancy 임산부에서내시경시행여부의결정은산과의사, 내시경시행의사, 환자가적응증에대해충분히토의후에결정해야한다. Esophagogastroduodenoscopy (EGD) 는임신중가장많이시행되는 procedure이다. 임신중 nausea 와 vomiting은약 90% 의임산부에서호소하는증세다. 6,10,14 증세가여러약물치료에도불구하고호전이없고, 심한출혈을동반하는경우 peptic ulcer, gastric outlet obstruction 여부를감별하고, bleeding source 부위의지혈을위해시행할수있다. 지혈을위한 cautery, injection, hemoclip, band는임신중 안전하게사용할수있다. 임신중 portal hypertension이의심되는모든임산부에서는임신이 variceal bleeding을더증가시키므로즉시 EGD를시행해야한다. 4 Percutaneous endoscopic gastrostomy (PEG) 가임신중요구되는경우는거의없다. 시술시에자궁천공의가능성이있으므로시행시바늘을적어도복부초음파를보면서자궁상방 5 cm 이상에서삽입해야한다. 7,9 Sigmoidoscopy는임신중안전하게시행할수있다. 임신중상기시술이조기진통을유발하거나선천적기형을유발하지는않는다. Hematochezia 를호소하거나 anorectal 또는 sigmoid abnormality 가의심되는경우, ulcerative colitis가있거나, severe unexplained diarrhea가있는경우 tap water enema, 또는 colon cleansing agent 사용후 mucosa를적절히볼수있다. 13 Colonoscopy는 sigmoidoscopy보다 invasive하지만임신중안전하게시행할수있다. 극심한설사나계속적인출혈이있는경우, malignancy 가의심되는경우시행할수있다. colonoscopy는임신중단순히 colon cancer의가족력이있거나 bowel habit change, 단순한 abdominal pain 같은 elective indication 으로임신중시행하지는않는다. polyethylene glycol (PEG) (category C) 를이용한 colon cleansing이임신중가장많이사용되며, sodium phosphate solution (category C) 도사용되어질수있으나위험이있는환자에서 electrolyte abnormality 를유발할수있다. 3 1) Endoscopic retrograde cholangiopancreatography (ERCP) gallstone이임산부의약 8% 에서보일수있다. uncomplicated cholelithiasis의경우 postpartum 까지연기가될수있고, 때때로임신중 cholecystecyomy 가시행되어질수도있고이수술은임신중두번째로많이시행되는 nonobstetric surgery 이다. 그러나 choledocholithiasis가임신중 cholangitis와 biliary pancreatitis의위험성때문에응급수술이필요하기도하다. 때때로 obstructive jaundice, cholangitis 심지어 pancreatitis가첫증상이될수 197

Fluoroscopy를이용하여 sphincterotomy, stone removal, duct stenting을하는경우에도가능한한 빨리시행하여 radiation exposure를최소화하고 procedure-related pancreatitis의위험을최소화해야한다. 6. Sedating agents and antibiotics commonly used during endoscopy: implications during pregnancy 불행하게도 adequate, well-controlled study는 198

Bipolar electrocautery is preferred over monopolar; the grounding pad needs to be positioned to minimize current through the amniotic fluid 없다. 임신중 endoscopy 시에사용되는대부분의약은 category B 또는 C이다. Category D는가능한한피하고, Category X는절대사용하면안된다. sedation은가능한최소로하고만약 deep sedation 이필요하다면마취과의사의도움을받는다. 5 최근에가장많이사용되는약으로다른약보다 short-acting이고, 빠르게 recovery가일어난다. 그러나 respiratory depression이일어날수있다. 임신중에안전하게사용할수있는약이나일삼분기사용에관한기록이거의없다. 1) Meperidine (Category B) Meperidine은안전한약이다. 이약은 morphine 만큼 fetal blood-brain barrier를통과하지는않으므로분만중에도사용이가능하다. 그러나 near term에너무고용량을사용시에는신생아에서 respiratory depression을유발할수있다. 일시적으로 fetal beat-to-beat variability의감소를유발할수있으나 poor prognostic indicator는아니다. 임산부의내시경중최소용량을사용해야하며, 최대 75 mg 까지사용할수있다. 과량사용하여 respiratory depression, hypotension이발생시에 reversal agent 인 naloxone (Category B) 을사용해야한다. 이약은 placenta를빠르게통과하며 teratogenesis와관련이없다고증명이되었다. 2) Fentanyl (Category C) 임신시에소량사용시안전하게사용될수있다. seizure history가있는환자에서 rapid onset을보여안전하게사용될수있다. 3) Propofol (Category B) 4) Benzodiazepine (category D) diazepam은지용성 benzodiazepine으로모체에투여시빠르게 fetal circulation에나타난다. 쥐를대상으로한연구에의하면 cleft lip과 palate를유발할수있다고되어있고, human에게도비슷한기형을유발할수있다는보고와그렇치않다는보고가팽팽히맞서있고다른사용가능한약이있으므로임신중에사용하지말아야한다. midazolam 은수용성 benzodiazepine으로내시경중에가장많이쓰이는약이다. 비록태반을통과하여 fetal level이측정될수는있으나 fetal circulation에축적되지는않으며 congenital abnormalities 와연관이없다. neonatal respiratory depression을유발할수있으므로 meperidine 이불충분한경우주의하여저용량을사용한다. 과량을사용한경우 reversal agent인 flumazenil (Category C) 을사용해야한다. 이약은 placenta 를빠르게통과하며아직기형에대한보고는없다. 7. 7 Videocapsule endoscopy는 gastrointestinal tract 의 mucosa를보는방법으로 sedation, abdomen의 mechanical pressure가없다. 그러나임신중의안전성에관한연구가아직없다. MRI를이용한 virtual colonoscopy는 CT보다임신중에안전하고, colorectal neoplasia를볼수있는 screening tool이다. 그러나아직임신중안전성에관한연구가없다. 2 결론 임신중적응증이되는환자에서내시경은안전하게사용되어질수있는 procedure이다. 따라서적응증이되는환자의경우환자의동의하에최소의 sedation으로전문 endoscopist가빠르고정확하게시행할수있으며, 앞으로 sedation과 radiation의 risk가적어 fetal teratogenensis의위험도가적은대체방법에대한계속적인연구가필요한실정이다. 참고문헌 1.
