Primary pigmented nodular adrenal dysplasia (PPNAD) is a cause of Cushing syndrome, independently of adrenocorticotropic hormone. A 21-year-old man was referred for evaluation with multiple spinal compression fractures and adenomas affecting both adrenal glands. Plasma and urinary cortisol levels were elevated and were not suppressed by the administration of a high dose of dexamethasone. The patient was diagnosed with Carney complex manifesting as Cushing syndrome due to PPNAD with multiple pigmented skin lesions. Left adrenalectomy was performed because the nodule on the left adrenal gland was markedly larger than the other nodules. Subsequently, right adrenalectomy was also performed. Pathological analysis revealed that the adrenal glands contained multiple dark brown and black nodules. We describe herein our recent experience of a case of Carney complex manifesting as Cushing syndrome due to PPNAD, with lentigines on the lips, and review the published literature on this subject. PPNAD의육안소견은피질의색소침착성미세결절이있는것을특징으로한다. 저자들은다발성척추골절로내원한쿠싱양외형의젊은남자에서양측부신의일차성색소침착성결절성부신피질이형성증및입술의색소침착을관찰하여 Carney 복합체로진단하였고, 양측의부신절제술을통한조직학적검사로확진된 1예를경험하여문헌고찰과함께보고하는바이다. 증례환자 : 21세, 남자주소 : 허리통증, 월상안현병력 : 상기환자는내원 6년전부터얼굴붉어짐있었고, 약 2년전부터얼굴부종보였다. 내원수개월전부터허리를굽히거나펼때심해지고, 마치뼈가빠져나가는듯한허리통증있어시행한척추의방사선사진상흉추 11, 12번, 요추 2, 3, 5번의다발성압박골절이있었다. 쿠싱양외형으로검사한 24시간소변유리코티솔상승보여, 복부전산화단층촬영시행하였고, 양측부신의다발성결절및결절성증식소견보여이에대한평가및치료위해내원하였다. 과거력 : 2년전군입대당시공황장애, 조울증등으로입원치료하였고, 조기제대판정받았다. 3달여간의정신과입원중 risperidone 등의약물복용하였으나이후자의중단하였다. 사회력 : 흡연력, 음주력없음. 가족력 : 아버지는환자가 5살때교통사고로인한뇌출혈로사망하였다. 환자의여동생은특이병력없었고, 환자의아버지와 2명의삼촌및 1명의고모그리고할머니의얼굴에피부색소침착이있었다. 특히환자의삼촌중한명은 얼굴에심한피부색소침착으로사회생활이어려운정도였고, 정신분열병의병력이있었다 (Fig. 1). 신체검사소견 : 내원당시혈압 110/70 mmhg, 맥박 99회, 호흡수 18회, 체온 36.5 였다. 신장 160 cm, 체중 56 kg로신체질량지수 21.8 kg/m 2 이었다. 월상안으로양볼에홍조가뚜렷하였고, 입술에색소침착이관찰되었다 (Fig. 2). 중심부비만및뚜렷한 buffalo hump 소견보였고, 색소침착의소견및자색선조는보이지않았다. 척추는후만형으로변형을보였다. 흉부진찰에서심잡음및폐음의이상소견없었고, 복부진찰에서특이소견없었다. 양하지의점상출혈소견이있었고, 양발꿈치의궤양성병변이있었다. 방사선소견 : 흉부방사선소견상심비대및폐병변의소견은보이지않았고, MRI T1 강조영상에서 T11, T12 edema 소견과기저의오래된골절에동반된최근의골절소견이보였다. 그외 L2, L3, L5, T4, T5, T8, T9 척추에서도오래된골절소견을보였다. 부신의자기공명영상에서왼쪽부신에 15 mm의선종이보였고, 그아래 10 mm 크기의결절성비대혹은선종이관찰되었다. 오른쪽부신에서도 5 mm 크기의결절성비대혹은선종의가능성이있는결절이관찰되었다 (Fig. 3). 뇌하수체자기공명영상에서는특이소견없었다. 이중에너지방사선흡수법 (DEXA, LUNAR R ) 을이용하여골밀도측정결과, L1-4의 T-score 및 Z-score가 -4.0으로골다공증을진단할수있었다. 검사실소견 : 내원당시말초혈액검사에서백혈구 9,150 /mm 3, 혈색소 11.3 g/dl, 혈소판 176,000/mm 3 였다. 생화학검사에서 AST/ALT 17/34 IU/L, ALP/LDH 336/433 IU/L, BUN/ Creatinine 7.9/0.44 mg/dl 총단백 5.4 g/dl, 알부민 3.4 g/dl, Figure 1. Na mmol/l, K 4.07 mmol/l, Cl mmol/l, Calcium 8.6 mg/dl, phosphorus 3.2 mg/dl, 무작위혈당 95 mg/dl이었고, 요검사는특이소견없었다. 내분비학적검사소견 : 기저혈장 ACTH 농도는오전 8시 0.75 pg/ml, 오후 4시 1.34 pg/ml (10-60 pg/ml) 였고, cortisol 농도는오전 8시 25.4 ug/dl (5-25 ug/dl), 오후 4시 22.8 ug/dl (2-12 ug/dl) 였다. 증가한 cortisol 에의해 ACTH 억제소견관찰할수있었다. 24 hr 요중유리 cortisol 배설량은 ug/24 hr (21-85 ug/24 hr) 이었고, 24 hr 요중 17-OHCS 배설량은 2.74 mg/24 hr ( mg/24 hr) 이었다. 저용량과고용량덱사메타손억제검사결과혈장및소변의 cortisol과요중 17 OHCS 는억제되지않았다 (Table 1). 덱사메타손의되먹이기기전에의한억제가되지않는부신종양에의한쿠싱증후군을진단할 Table 2. (D) Cells within the nodules were relatively large and most contained compact eosinophilic cytoplasm with abundant granular brown pigment (H&B stain, 400). 좌측은 2개의조직으로나누어적출되었으며 7.3 g의무게에약 cm의크기였고, 우측은 8.5 g의무게에 cm의크기였다. 육안소견상양측부신은 2 mm 부터 21 mm까지의장경을가지는피질내결절을다수가지고있으며대부분의결절은검은색소침착을보였다. 현미경소견상결절은특징적인색소침착을보이는세포로구성되어있었다. 각각의세포는약간의핵크기의증가를보이나뚜렷한이형성은관찰되지않았다 (Fig. 4). 치료및경과 : 환자는일차성색소침착성결절성부신피질이형성증과피부색소침착을포함하는 Carney complex 로진단되어부신절제술을고려하였다. 양측부신절제술이 원칙이나우선부신종양이큰좌측부신절제술이후경과를보기로결정하였다. 좌측부신절제술후시행한 75 g 경구당부하검사에서 112 mg/dl로감소함을볼수있었다 (Table 2). 이후우측부신절제술시행하였다. 현재 Hydrocortisone 20 mg 아침식후, 10 mg 저녁식후복용, fludrocortisone 0.1 mg 2일에한번복용하며외래에서경과관찰중이다. 고찰 Carney complex 는점액종 ( 심장, 피부, 유선 ) 과피부와점막의자반증, 그리고쿠싱증후군, 성조숙증, 말단비대증, 말초 -S242-

5 - 배민정외 6 인. 다발성척추골절로내원한 Carney Complex 1 예 - 신경종, 고환의종양등을발현하는질환군으로이들중두가지이상을만족하는경우로정의한다 [1]. 이질환은다발성내분비선종 (MEN, multiple endocrine neoplasia) 또는자반증 (lentiginosis), McCune-Albright syndrome (MAS) 과유사한증상을나타내나, 염색체이상부위에차이를보인다 [2]. Carney complex는 2p16 또는 17q22-24 염색체의이상으로 Protein kinase A type I-alpha regulatory subunit (PRKAR1A) 유전자의암호화부위 (coding region) 의돌연변이로알려져있다 [3]. 최근에 PRKAR1A 돌연변이없이 2q31-2q35 위치의 phosphodiesterase 11A4 (PDE11A4) 유전자의돌연변이가보고되고있고, 이러한 PRKAR1A와 PDE11A 모두 camp 신호전달체계를조절하여다양한내분비종양에관여하고있다. Carney complex 환자전체의 45%, Carney complex 환자중쿠싱증후군을가지고있는환자의 80% 에서 PRKAR1A 유전자의돌연변이가있는것으로보고되고있다 [4]. 이러한 Carney complex는드문질환으로 NIH-Mayo Clinic 및 Cochin center에의하면약 500예가보고되고있다 [5]. 환자는다발성골절로내원한젊은남자로쿠싱증후군의심하에호르몬검사및복부전산화단층촬영을시행하였다. 혈청코티솔은상승소견을보였고, ACTH 는억제되어있으며덱사메타손억제검사에서억제되지않는부신피질호르몬의과분비를보였다. 방사선검사상양측부신에서부신피질결절을발견할수있었고, 뇌하수체자기공명영상에서이상소견을보이지않았다. 쿠싱증후군이있는환자에서골다공증및그에의한골절의빈도는높다. 이는코티솔의뼈에대한영향으로서, 코티솔은뼈의형성은감소시키고, 뼈의재흡수는영향이없거나증가시키는작용을한다. 뼈의여러세포의분화와활성에직접적인영향을미치기도하고, 골모세포의활성화에영향을미치는사이토카인에영향을미치기도한다. 또한골모세포와골세포의수명은단축시키는반면파골세포의수명을연장시키는역할을하면서골다공증의발생을늘린다. 이환자에서는가족성의색소침착성피부병변이있었다. 이러한피부병변은빠른진단에도움을줄수있는데, 특히심장점액종과같이생명을위협하는합병증을빠른진단을통해예방이가능하다. 이러한피부의색소침착은 Carney complex가있는환자의반수이상에서발견할수있고, 주로얼굴및중심부에서볼수있고앞이마볼, 코, 특히 입술, 각막에흔하다 [6]. 본증례의가족성색소침착성피부병변으로보아, 다른가족들의 Carney complex 가능성도고려가필요한상태로, 가족들의내분비검사및추적검사가필요하다. 특히나심장점액종은생명에직접적인영향을줄수있으므로가족들의심장초음파시행이권고된다 [7]. 심장점액종은주로젊은연령에서발생하고, 우심방에서의발생빈도가높으며수술후에도재발률이높다. 이환자의심초음파는정상이었고, 추후가족의심초음파역시시행해야할것이다. Carney complex가있거나 Carney complex의유전적요인 (PRKAR1A gene mutation carrier) 이있는환자에서는증상에대한주기적인추적이필요하다. 현재추적관찰의주기에대해서입증된권고는없는실정이나모든증상에대한추적관찰은적어도일년에한번이루어져야한다. 심장점액종은주요한질환으로 Carney complex 환자에서적어도일년에한번심장초음파를통해검사가필요하고, 점액종이있었던환자에서는 6개월에한번추적검사가필요하다 [8]. PPNAD에의한쿠싱증후군의원칙적인치료는양측부신절제술이다. 하지만드문예에서는현성쿠싱증후군의 PPNAD 환자에서일측부신절제이후장기간의경과관찰후에도재발하지않은보고가있다 [8]. 본증례의환자는좌측부신결절의크기가더커서우선좌측부신절제술을시행하였고, 경과관찰하기로하였다. 수술후시행한덱사메타손억제검사에서정상범위로억제되지않는양상으로 (Table 3) 이후우측부신절제술을시행하였다. 단측부신절제술이후재발하여반대측부신절제술을계획한예는 Choi 등의증례에서확인할수있다 [9]. 이에치료선택시양측부신절제술을우선고려하는것이필요하리라생각된다. Liu의증례중아전부신절제술을시행한한환자에서수술 2년이후갑상선종양이발견된것을볼수있었다 [10]. 환자는다발성척추골절에대하여수술적치료하지않고지지요법중이고, 골다공증으로 bisphosphonate 제제를복용중이다. 수술적요법으로좌측부신절제술이후우측부신절제술단계적으로시행하였고, 향후가족에대한평가가진행될예정이다. 요약 Carney complex는피부의색소침착과여러양성종양 ( 점액종, 내분비종양 ) 이상염색체우성으로표현되는질환이다. 저자들은다발성척추골절로내원한젊은남자에서쿠싱양외형을관찰하여시행한검사상 PPNAD 발견하였고, 환자의입술에색소침착을관찰하여 Carney complex 진단하였다. 이에좌측부신절제술이후우측부신절제술단계적으로시행하고조직학적으로확진된예를문헌고찰과함께보고하는바이다. 중심단어 : Carney complex; 쿠싱증후군 ; 색소침착 REFERENCES 1.
