We investigated the frequency and distribution of unexpected red cell antibodies that were detected in cancer patients at National Cancer Center (NCC) and we compared our results with the previously published data. Methods: From January 2001 to June 2009, 56,660 sera of the cases from NCC were screened and 197 sera were identified with using the Ortho BioVue System (Ortho-Clinical Diagnostics, Raritan, USA) and the conventional tube method. In case of the presence of autoantibody, the ZZAP method was performed to distinguish alloantibody from autoantibody. Results: 759 cases (1.34%) showed positive results out of all 56,660 cases that underwent unexpected antibodies screening. Among them, unexpected antibodies were identified in 197 cases. The most frequently detected antibody was anti-le a in 62 cases (31.47%), followed by anti-e in 32 cases (16.24%) and anti-le b in 18 cases (9.14%). Unidentified antibodies were detected in 43 cases (21.83%). In the cases with a previous history of transfusion at the NCC and the screening results were altered from negative to positive, anti-e was the most frequently detected antibody (8/30 cases, 26.67%), and this included the mixed antibodies. Conclusion: Compared with the previous reports, this study showed that the frequency and distribution of the unexpected red cell antibodies of cancer patients were not different from those of general patients. Moreover, there was no apparent difference of frequency of the unexpected red cell antibodies among the diagnosed cancers. Our research may provide data for the frequency and characteristics of red cell antibodies because we targeted only cancer patients. 2 서론 비예기항체는검사를실시하기전에는항체의존재와종류를알수없으므로불규칙항체라고도하며, ABO 및일부 P 혈액형항체이외의대부분의혈액형항체들이이에속한다. 1) 항체의종류에따라서자연발생적으로생기는경우도있지만대부분의비예기항체는임신이나수혈등을통하여다른항원에노출되어생기는면역항체이다. 이러한면역항체는급성및지연성용혈성수혈부작용을일으키는원인이되고, 이러한사례는다수보고되었다. 2,3) 따라서수혈전비예기항체선별및동정검사는매우중요하며수혈이필요한환자에게수혈로인한부작용을최소화하고수혈의안전성을높이기위해수혈을시행하는의료기관중비예기항체선별검사를실시하는기관이증가하고있다. 비예기항체선별및동정검사방법은시험관법, micro-plate법, 원주응집법이있으며이중원주응집법이기존의시험관법을점점대체해가고있는추세다. Ortho Biovue System (Ortho-Clinical Diagnostics, Raritan, USA) 을이용한원주응집법은적혈구와항체를반응시켜원침하여 glass beads를통과하는정도를이용한것으로응집이일어난적혈구는 column의상부에남지만, 응집이일어나지않은적혈구는 column의하단에가라앉는다. 이검사의장점으로는검사과정중에세척과정이없고, 적은검체양으로검사가가능하며, 결과판독이용이하다는점을들수있다. 4) 국내에서발표된대부분의연구결과들은 Diamed-ID System (Diamed, Murten, Switzerland) 을이용한연구결과이고, 적혈구비예기항체의빈도및분포중암환자만을대상으로연구한결과는아직까지는없었다. 이에저자들은 2001년부터 2009년 6월까지국립암센터에내원하여암환자로진단되어수혈이예정된환자에서관찰된비예기항체의빈도및분포특성을분석하고다른질환을포함하고있는기존의연구결과와비교하였다. 대상및방법 1. 연구대상 2001년 1월부터 2009년 6월까지국립암센터에서임상적으로수혈이예정된환자중비예기항체선별검사가의뢰된 56,660건을대상으로하였다. 대상자중비예기항체선별검사결과가양성으로판정되어, 임상에서검사가의뢰된 197건의비예기항체동정검사에대한성별, 나이, 질환군, 수혈기왕력을의무기록을통하여후향적으로조사하였다. 수혈기왕력은국립암센터내원전기왕력이의무기록상불분명한예가많아국립암센터에서실시된것으로한정하였다. 2. 검사방법비예기항체선별검사및동정검사는 Ortho Bio- Vue System (Ortho-Clinical Diagnostics, Raritan, NJ, USA) 을이용한원주응집법으로시행하였고, 원주응집법을이용한비예기항체동정검사로항체를동정하지못한경우에는추가로시험관법을이용하였다. 그리고비예기항체선별검사가양성이면서이전에항체가동정된환자중이전과동일한비예기항체가동정된경우에는동정빈도에서제외하였다. 1) 비예기항체선별검사비예기항체선별검사는 LISS 40μL씩을 Ortho BioVue AHG Cassette에분주한후선별용세포인 Selectogen I, II 각각을카세트에 10μL씩분주하

3 대한수혈학회지 : 제 20 권제 2 호 고환자혈청 40μL을첨가하였다. 이를 37 o C에서 15분간반응시킨후 5분간원침하여응집정도에따라각각음성부터 4+ 까지기록하여결과를판독하였다. 2) 비예기항체동정검사비예기항체동정검사는비예기항체선별검사와같은방법으로 11가지혈구패널로구성된 Resolve Panel A를사용하였고, Resolve Panel A로명확하게동정되지않은경우에는 Resolve Panel B 또는 Resolve Panel C Untreated를사용하여검사를진행하였다. 검사결과의판독은비예기항체선별검사와동일하였다. 패널혈구와반응하여응집강도가약한 Rh, Kidd 및 Lewis 계통의항원에대한항체에대해서는 ficin 효소로처리된 Resolve Panel C Ficin Treated를이용하여추가동정검사를시행하였다. 패널혈구이외에자가대조적혈구를이용하여자가항체를감별하였다. 자가대조가양성인경우에는위양성을배제하기위해서원주응집법 (Ortho BioVue AHG Cassette) 을이용하여직접항글로불린검사를시행하였고, 항체용출검사중하나인 ZZAP법 5) 을이용하여자가항체와동종항체를구별하였다. 시험관법은 Resolve Panel A를사용하여실온식염수법, 알부민가온법그리고항글로불린법을이용하여검사를시행하였다. 항글로불린검사시약은다가항글로불린시약 (Millipore, Livingstone, U.K.) 을사용하였다. 결과 1. 비예기항체의빈도및환자분포비예기항체선별검사총 56,660건중선별검사양성은 759건 (1.34%) 이었다. 연도별선별검사에 서양성이나온빈도는각각 2001년 15건 (1.27%), 2002년 50건 (1.59%), 2003년 67건 (1.69%), 2004년 55건 (1.05%), 2005년 90건 (1.31%), 2006년 154건 (1.72%), 2007년 147건 (1.47%), 2008년 95건 (0.88%), 그리고 2009년 1월부터 6월 30일까지 86건 (1.32%) 으로시행건수의증가와상관없이비슷한비율로관찰되었다 (Table 1). 비예기항체선별검사양성건수중임상에서동정검사가의뢰된경우는 197건이고, 성별은남자 100명 (50.76%) 과여자 97명 (49.24%) 이었으며, 평균연령은 58.5세 ( 범위 10 88) 였다. 이중수혈기왕력이있는환자와없는환자는각각 45건 (22.84%) 과 152건 (77.16%) 이었다. 수혈기왕력이있는환자의평균적혈구수혈량은 8단위 ( 범위 1 87) 였다 (Table 2). 비예기항체동정검사가실시된환자를질환군별로살펴보면 Table 3과같다. 2. 비예기항체의종류및분포 비예기항체동정검사가이루어진 197건에대한성상을분석하면가장많은빈도의계열은 Table 1. Duffy계열은항-Fy b 가 1건 (0.51%), Kidd계열은항-Jk a 가 2건 (1.02%) 동정되었다. MNSs계열은항-M이 5건 (2.54%), 항-S가 1건 (0.51%) 동정되었고, 그외에항-P 1 가 4건 (2.03%), 항-I가 1건 (0.51%), 비특이적한랭항체가 1건 (0.51%) 그리고고빈도의심항체가 1건 (0.51%) 씩각각동정되었다. 자가항체만있는경우는 3건 (1.52%) 이었고, 자가항체와동종항체가공존하고있는경우는 4건 (2.03%) 이었다. 정확하게동정되지않은비예기항체는 43건 (21.83%) 이 Table 3. 6 었다. 수혈기왕력이있는환자군에서가장빈도가높은비예기항체는항-E가 10건 (5.08%) 으로가장높았고, 수혈기왕력이없는환자군에서는항-Le a 가 58건 (29.44%) 으로가장높은빈도를나타내었다 (Table 4). 3. 수혈기왕력에따른비예기항체선별검사의주기적인양상 비예기항체동정검사가실시된 197건의주기적인비예기항체선별검사의성상을분석하면처음에는선별검사가음성이었지만양성으로바뀐성상이 46건 (23.35%) 이었고, 이중에서수혈기왕력이있는경우와없는경우가각각 30건과 16건이었다. 선별검사가처음에는음성이었지만이후양성그리고다시음성으로바뀐성상은 13건 Table 5. (6.60%) 이었으며, 수혈기왕력이있는경우와없는경우는각각 8건과 5건이었다. 4. 수혈에의한비예기항체성상 비예기항체선별검사가음성에서양성으로바뀌어진수혈기왕력이있는 30건중복합항체를포함하여항-E가 8건 (26.67%) 으로가장많았고, 그다음으로는동정되지않은비특이적항체 7건 (23.33%), 항-Le b 6건 (15.79%) 순이었다 (Table 5). 고찰 비예기항체의빈도및종류는연구대상군, 연구대상의혈액형빈도, 대상연구집단의혈액형빈도, 유전자빈도, 검사방법, 그리고판독자의능력등에따라결과에큰차이를보이는것으로알려져있다. 6) 국내에서시험관법이나 micro-plate법으로시행한비예기항체양성률은 0.33% 에서 1.57% 내외로보고되었고, 7-11) Diamed-ID System (Diamed, Murten, Switzerland) 을이용한원주응집법에서비예기항체양성률은 0.46% 에서 1.2% 까지다양하게보고되었다 ) Ortho BioVue system을이용한본연구에서는비예기항체양성률이 1.34% 로시험관법이나 micro-plate법에의해보고된빈도범위와비슷했으나, Diamed-ID System을이용한원주응집법의빈도범위보다는다소높았다. 본연구에서가장높은빈도로동정된비예기항체는항-Le a 가 62건 (31.47%), 항-E가 32건 (16.24%) 순이며, 이연구결과는동정된비예기항체중항- E가가장높은빈도로나타나는 Diamed-ID System을사용한대부분의원주응집법과는상반된결과를나타내었다 ) 국내에서보고된비예기항체빈도및분포는대부분 DiaMed-ID System을이용한후향적연구

7 대한수혈학회지 : 제 20 권제 2 호 결과들로서 Ortho BioVue System을이용한비예기항체빈도및분포와서로비교하여분석하기에는아직까지는어려운측면이있다. Ortho BioVue system을이용한 Lim 등 16) 의연구결과를살펴보면 383건중동정된비예기항체는항-Le a 가 88건 (22.98%) 으로가장많은빈도를나타내었고, 그다음으로는항-E가 42건 (10.97%) 으로두번째순위였다. 이는본연구결과에서동정된비예기항체순위와도일치하였다. 이전의 Lee 등 17) 의연구결과에의하면양성검체에서 Ortho Biovue System이더자주 +/ 반응을보인다는연구결과가있어, 한랭항체검출률에서두제품간의차이가있다고생각되지만확실한원인을알아내기위해서는향후두회사의제품간에더많은연구가진행되어야한다고사료된다. Shin 등 18) 의연구결과에의하면 DiaMed-ID System인 LISS/Coombs와 NaCl/Enzyme을같이사용하면각각단독으로사용했을때보다더많이비예기항체를동정할수있다고보고하였다. 본연구에서도패널혈구와반응하여응집강도가약한 Rh, Kidd 및 Lewis 계통의항원에대한항체에대해서는 ficin 효소로처리된 Resolve Panel C Ficin Treated를사용하여추가동정검사를시행하여역가가낮은비예기항체를검출할수가있었다. 이전에보고된연구결과에의하면암환자들은적혈구항원에대한동종면역발생률이낮은데, 그원인은악성종양화과정에서면역계손상과항암화학요법또는방사선요법등의치료로인한면역반응이억제되었기때문이라는연구결과가있다 ) 또한 Schonewille 등 20) 의연구결과에따르면강한항화학요법의치료를받는혈액종양환자들은치료를받지않는환자들에비해적혈구항원에대한동종면역발생률이낮고, 처음동정된항체중 40% 이상의항체가다시검출되지 않았다고한다. 그리고이연구에서수혈후가장많이동정된항체는항-E, 항-K, 항-c 순이었다. 국내에서보고된연구에서수혈후가장높은빈도로발생하는면역항체는항-E이며, 23) 이는본연구에서수혈후비예기항체선별검사가음성에서양성으로바뀐 30예중복합항체를포함하여항-E가 8예 (26.67%) 로가장높은순위를보이는연구결과와도일치하였다. 국외보고에서발생빈도가높다고알려진항- K는 20) 심각한신생아용혈성질환이나수혈부작용을일으킬수있는임상적으로매우의미있는동종항체로서주로수혈이나임신에의해생성되고국내에서는발생빈도가드물게보고되며, 12,16) 본연구에서는동정되지않았다. 그원인은적혈구항원의면역원성은일정하더라도민족에따라항원양성률이다르게나타나므로백인인경우에는 K 항원양성자가 9% 이나한국인중 K 항원양성자는 0.5% 이기때문에백인에비해상대적으로수혈이나임신에의해항-K가생성되는일이매우드물기때문으로보인다. 1) 자가면역성용혈성빈혈을가진환자에대한수혈은용혈에대한환자의반응정도, 면역성용혈의병인, 그리고용혈을초래하는항체의종류등에따라결정된다. 임신이나수혈의경험이없는환자들은자가항체이외에동종항체를추가로가지고있을가능성이적으므로흡착시험을실시하지않고도적합혈액을선택할수있으나자가면역성용혈성빈혈을가지고있는 647명의환자중 209명 (32.30%) 의환자에서동종항체가발견되었다는연구결과등으로미루어보아 24) 다회수혈자나다산부에서는적합혈액선택시동종항체의공존을염두에두어야한다. 1) 본연구에서도항체용출검사중하나인 ZZAP법 5) 을이용하여자가항체와공존하는동종항체를검출하였다. 자가항체존재시대부분의적혈구와자가항체

8 가반응하기때문에용혈성반응을일으킬수있는동종항체와자가항체의구별이어렵다. 이에대부분의혈액은행에서는자가항체보다반응역가가낮은최소부적합적혈구제제를환자에게수혈하고있는데, 보다환자에게적합한혈액을찾기위한노력이지속적으로필요하다고사료된다. 본연구결과는 DiaMed-ID system을이용하여국내에보고된비예기항체의빈도및분포연구결과 12,23) 와는차이가있는결과를나타내었지만, Ortho BioVue system을이용한연구결과와는상당히비슷한결과를보여주었다. 16) 그러나, 이전의연구결과와비교할때암환자와다른질환을포함한환자간비예기항체의빈도나성상이다르지않았고, 암종간의빈도차이도뚜렷하지않은양상을보였다. 국내에서는암환자만을대상으로한연구가없었기때문에본연구는암환자에대한비예기항체빈도나성상에대한기초자료로서이용될수있을것으로사료된다. 요약 배경 : 비예기항체는급성및지연성수혈부작용을일으키는원인이되므로수혈전비예기항체선별및동정검사는매우중요하다. 저자들은국립암센터에서적혈구비예기항체의빈도및분포를조사하고기존의연구결과와비교하였다. 방법 : 2001년 1월부터 2009년 6월까지 Ortho BioVue System (Ortho-Clinical Diagnostics, Raritan, USA) 과시험관법을이용하여본원에선별검사가의뢰된 56,660건을대상으로검사를시행하였고, 197건을동정하였다. 자가항체가존재할경우에는 ZZAP법을이용하여자가항체로부터동종항체를구별하였다. 결과 : 선별검사가시행된 56,660건중 759건 (1.34%) 에서양성을보였고, 197건에서비예기항체가동정되었다. 가장많은빈도로검출된비예기항체는항-Le a 62건 (31.47%), 항-E 32건 (16.24%), 그리고항-Le b 18건 (9.14%) 순이었다. 정확하게동정되지않은항체는 43건 (21.83%) 이었다. 비예기항체선별검사가음성에서양성으로바뀐수혈기왕력이있는 30건중복합항체를포함하여항- E가 8건 (26.67%) 으로가장높은빈도로동정되었다. 결론 : 이전에보고된연구결과와비교할때암환자와다른질환을포함한환자간비예기항체의빈도와성상이다르지않았다. 더욱이암종간의빈도차이도뚜렷하지않은양상을보였다. 암환자만을대상으로한본연구는비예기항체빈도나성상에대한기초자료로서이용될수있을것으로사료된다. 참고문헌 1.
