1 비뇨기과영역에서 18 F-FDG PET/CT 의임상적역할 The Clinical Roles of 18 F-Fluorodeoxyglucose Positron Emission Tomography/ Computerized Tomography for Urological Disease Kwang Ho Ryu, Ho Suck Chung, Seung Il Jung, Taek Won Kang, Dong Deuk Kwon, Kwangsung Park, Soo Bang Ryu From the Department of Urology, Chonnam National University Medical School, Gwangju, Korea Purpose: We evaluate the clinical roles of 18 F-fluorodeoxyglucose positron emission tomography/computerized tomography ( 18 F-FDG PET/CT) for diagnosing disease in the urogenital tract, and we compared this with the other established radiologic and pathologic diagnoses. Materials and Methods: From June 2006 to June 2007, the total number of subjects who underwent 18 F-FDG PET/CT was 4,438. The mean patient age was 57.4±7.6 years and the ratio of males to females was 1.28:1. During the study period, except for 152 patients who had been given a diagnosis of urologic tumor, 614 (14.3%) healthy subjects and 3,672 (85.7%) patients with non-urologic tumors were enrolled. The results of detecting urologic disease by 18 F-FDG PET/CT were compared with the results of detecting urologic disease by conventional imaging techniques and the postoperative histopathological diagnoses. Results: With including 147 healthy subjects and 251 non-urologic tumor patients, 398 (9.3%) urologic diseases were detected on 18 F-FDG PET/CT. Diseases of the kidney, adrenal and prostate were frequently found (215, 95 and 52 patients, respectively). A thorough examination was indicated for 153 (3.6%) of the patients as a result of positive findings that suggested possible tumor. A total 93 urologic cancers were confirmed, and the overall positive predictive value of 18 F-FDG PET/CT was 60.7%. The positive predictive value for adrenal, kidney, bladder and prostate cancer were 87.7%, 73.3%, 57.1% and 14.0%, respectively. Conclusions: 18 F-FDG PET/CT was not superior to conventional imaging for making the diagnosis of urologic disease. But 18 F-FDG PET/CT was more predictive for adrenal and renal tumor than for bladder and prostate tumor. So, urologic tumor that is incidentally detected on 18F-FDG PET/ CT, and especially adrenal and renal tumor, should be closely evaluated. PET/CT는 PET의낮은공 775

2 776 대한비뇨기과학회지 : 제 49 권제 9 호 2008 간해상력으로인한한계를 CT 영상으로극복하였을뿐만아니라, 다양한암의조기진단, 치료계획수립, 재발조기발견등여러장점을갖고있다 F-FDG PET/CT는최근에관심이집중되고있는새로운핵의학적진단장치로암의독특한대사적특성을이용하여생화학적, 기능적, 해부학적영상을획득한다. 그러나비뇨기계영역에있어서 18 F- FDG PET/CT의역할은아직확실하게규명되어있지않다. 저자들은본원에서 18 F-FDG PET/CT를시행하여비뇨기계이상소견을보인환자를분석하여비뇨기계영역에서 18 F-FDG PET/CT의임상적역할에대해알아보고자하였다. Characteristics of the patients who underwent 18 F-FDG PET/CT No. of patients 4,286 Male:Female (ratio) 2,406:1,880 (1.28:1) Mean±SD* (years) 57.4±7.6 Purpose of PET/CT Health examination (%) 614 (14.3) Non-urologic tumor follow-up (%) 3,672 (85.7) *SD: standard deviation, 18 F-FDG: 18 F-fluorodeoxyglucose positron emission tomography, PET: positron emission tomography, CT: computerized tomography 대상및방법 2006년 6월부터 2007년 6월까지 18 F-FDG PET/CT를시행한 4,438명중비뇨기계종양환자를제외한 4,286명을대상으로하였다. 전체환자 4,286명의평균나이는 57.4±7.6세 (2-91) 로나타났으며, 남녀비는 1.28:1 (2,406명:1,880명) 로남자환자가더많았다. 18 F-FDG PET/CT에서우연히비뇨기계이상소견을보인환자를추적하여영상학적또는병리학적검사결과와최종비뇨기과적진단을비교분석하였다. 비뇨기계이상소견에대해수술적검사나치료가시행되지않은환자는기존의진단및추적검사방법인단순흉부촬영, 배설성요로조영술, 복부초음파, 경직장전립선초음파, 복부전산화단층촬영, 골주사검사, 뇌자기공명영상등의영상학적검사결과와비교하였다. 비뇨기계이상소견에대해수술적검사및치료가시행된환자는최종병리학적조직검사를 18F-FDG PET/CT 결과와비교하였다. 환자들은 DST PET/CT (Discovery ST PET/CT, General Electric Medical Systems, Milwaukee, USA) 를이용하여영상을획득하였다. 18 F-FDG PET/CT를시행하기전에 6시간이상금식하였으며, 검사전물을 1리터섭취하였다. 8.1MBq/ kg의 18 F-FDG를정맥주사하고 50분이경과한후에환자의머리에서허벅지중간까지감쇠보정용 CT (Helical, 8slice, 120Kvp, 80mAs, 3.79mm slice thickness) 영상을얻었다. 이때시행한 CT에서는방사선조영제를사용하지않았다. 이후환자를움직이지않도록하고 frame당 3분씩 CT 스캔과동일한범위의감마선방출영상 (emission scan) 을 PET를이용하여얻었다. 획득된 PET 데이터는 CT 데이터를이용하여감쇠보정한후 OSEM (128x128 matrix, 3.27mm slice thickness, subset: 21, iteration: 2, post-filter: 5.14 FWHM (mm), loop-filter: 4.69 FWHM (mm), diameter: 60cm) 을이용하여재구성하였다. 결과전체 4,286명의대상환자중에서특별한증상이나질환이없이건강검진을위해시행한환자는 614명 (14.3%) 이었고, 타과종양으로인해 18 F-FDG PET/CT를시행한환자는 3,672명 (85.7%) 으로나타나, 건강검진등의선별검사목적보다타과질환이있는경우에 2차적인목적으로 18 F-FDG PET/CT를시행하는환자가많았다 (Table 1). 연령대별로는 60대 (30.1%) 가가장많았으며 50대 (25.2%), 40대 (20.9%) 순이었다. 비뇨기과진료및검사를시행하고 18 F-FDG PET/CT 결과판독에서비뇨기계이상소견이나타난환자수는 4,286명의환자중에서총 398명 (9.3%) 이었다. 건강검진에서비뇨기계질환이나타난경우는 614명중 147명 (23.9%) 이었으며, 타과종양이있었던환자에서비뇨기계질환이나타난경우인 3,672명중 251명 (6.8%) 이었다. 비뇨기계질환이나타난환자 398명중신장질환이 215 명으로가장많았으며, 부신질환 95명, 전립선질환 52명, 방광질환 16명, 요관질환 15명, 기타 5명순이었다 (Table 2). 질환별로는신낭종이 142명으로가장많았고, 부신종양이 81명, 전립선종양이 50명으로나타나비뇨기계질환이우연히발견된경우가많음을알수있었다. 비뇨기계종양이발견된환자를장기별로분석한결과전체 153명의환자에서비뇨기계종양이의심되는소견이나타났으며, 부신종양과전립선종양외에도신장종양 15명, 방광종양 7명으로확인되었다 (Table 3). 비뇨기계종양이의심되었던 153명은건강검진에서발견된환자가 614명중 21명 (3.4%), 타과종양의추적검사에서발견된환자가 3,672명중 132명 (3.6%) 으로나타났다. 18 F-FDG PET/CT 판독에서비뇨기계종양이의심되었던 153명의환자를대상으로, 추적검사에서실제로비뇨기계악성종양이진단된환자는 93명으로 60.7% 의높은양성예측률을보였다 (Table 4). 장기별로는전이성부신암이 88.1%,

The positive predictive value of 18 F-FDG PET/CT Positive Tumor Cancer* predictive value (%) Adrenal tumor Metastatic tumor Primary tumor Renal tumor Bladder tumor Prostate tumor Total *: compare to radiologic or pathological finding, 18 F-FDG: 18 F- fluorodeoxyglucose positron emission tomography, PET: positron emission tomography, CT: computerized tomography Beyer 등 2 에의해처음으로소개된 PET/CT는 PET의생화학적, 기능적영상정보와 CT의해부학적구조영상정보를결합하여 PET 또는 CT 단일검사의한계를극복할수있게되었다. 현재까지가장많이이용되는 PET 또는 PET/CT의방사선추적자는포도당유도체인 18 F-FDG로악성종양세포에서포도당이용률이증가하는점을검사에활용하게된다. 1 우리나라에는 2003년이후 PET/CT가점진적으로도입되었고, 검사건수는해마다기하급수적으로증가하고있는추세이다. 최근일본에서는 2005년한해동안건강인 50,558명을대상으로시행된 FDG-PET (32,970명) 와 PET/CT (17,588명) 를분석하여암선별검사로서의 performance profile 을보고하였다. 3 이연구결과에따르면대상환자 9.8% 에서암이의심되었으며, 500명 (1.14%) 에서암이진단되어악성종양에대한 PET의전체적인양성예측률은 29.0% 였다. 비뇨기계질환은전립선암 47명, 신장암 13명, 방광암 4명, 고환암 1명으로조사되었다. 또한 PET/CT가 PET보다발견율, 민감도, 양성예측률에서통계학적으로의의있는

4 778 대한비뇨기과학회지 : 제 49 권제 9 호 2008 결과를보였다고보고하였다. 저자들의연구에서도부신암을제외하였을때, 악성종양이의심되었던 72명중 22명에서암이진단되어비슷한수준의양성예측률 (30.6%) 을보였다. 18 F-FDG PET/CT의가장대표적인적응증은악성종양으로서조기진단, 양성및악성의감별, 치료를위한병기결정, 재발진단, 치료효과판정, 치료후예후판정등임상진료의전영역에서활용되고있다. 비뇨기계악성종양에대해서 18 F-FDG PET/CT는그역할이재발진단, 치료효과판정, 원격전이발견또는병기재설정등으로아직까지매우제한적인상태이고, 수술전병기결정을위한검사로인정받지못하고있는실정이다. 근래에는암에대한일반인들의인식이높아지면서건강검진에서 18 F-FDG PET/CT를이용하는것에관심이집중되고있다. 본원에서도 18 F-FDG PET/CT가도입된 2006년이후 1년간건강검진을위해시행한환자가 614명에달했으며, 23.9% 에서우연히비뇨기계이상소견이발견되었다. 비뇨기계종양이의심된환자는 153명에달했으며, 93명 (2.17%) 에서실제로악성종양이진단되었다. 흔히비뇨기계영역으로분류되지않는부신은거의모든종양환자에서주변림프절과함께전이가흔한장기이다. 정상부신은 18 F-FDG PET 단독검사보다 18 F-FDG PET/CT에서정확하게보이는것으로알려져있으며, 4 암환자에서부신전이가의심될경우 18 F-FDG PET/CT가양성및악성감별에도우수한것으로보고되고있다. 5 Blake 등 6 은부신암이의심된환자에서 18 F-FDG PET/CT를통해민감도 100%, 특이도 93.8% 로정확도높게부신암을진단할수있었다고보고하였다. 이러한결과를바탕으로 18 F-FDG PET/CT는원발성및전이성부신암의구별없이일차적진단에유용한검사가될것으로생각한다. 저자들의연구에서도 18 F-FDG PET/CT는전이성부신암과원발성부신암모두에서매우높은양성예측도를보였다. 특히전이성부신암에서의의있게높은결과를보인것은폐암, 대장암등과같은타과종양환자에서타장기전이소견이함께부신으로의전이가동반되었기때문이다. 신장암은 1991년도 18 F-FDG 섭취증가가보고된이후, 18 F-FDG PET를이용한여러연구가시행되었다. 신장암과관련된 18 F-FDG PET/CT 단독연구는없으나, 신장암의진단이나병기결정을위해서는 18 F-FDG PET보다 CT가현재까지는유용한검사인데, 이는신장암을대상으로소변으로배설되는 18 F-FDG 추적자를사용하는것이많은한계점을지니고있기때문이다. 일부연구에서신세포암에대한 18 F-FDG PET의표준화섭취계수 (standardized uptake value; SUV) 로 4.6의결과를제시하기도하였으나, 대다수연구에 서신장암의진단에대한 18 F-FDG PET의특이도는비슷한결과를보이지만민감도가낮은것이단점으로지적되었고, 다른영상학적검사에비해우수한결과를보이지못했다. Kang 등 7 은신세포암이의심되는 66명의환자를대상으로 18 F-FDG PET의민감도를 60% 로보고하였고, Aide 등 8 은신종물이있는 35명의환자를대상으로 47% 의낮은민감도를보고하였다. 그러나 18 F-FDG PET/CT를이용한본원에서는신장종양이의심되었던 15명의환자중 11명에서신장암이진단되어, 18 F-FDG PET보다우수한양성예측률 (73.3%) 과민감도 (68.8%) 를보였다. 18 F-FDG PET는신장암의진단에서인정받지못하고있으나, 전이병변진단, 치료효과판정, 추적검사또는병기재설정등에대해서는기존의영상학적검사에비해우수한결과를보여향후대체검사법으로까지기대되고있다. 9,10 실제로 Lee 등 11 은신종물환자에서기존의병기결정방법과 18 F-FDG PET의결과를비교하였을때, 기존의연구와같이신세포암원발부위진단에서민감도가낮고특이도는높았으나, 신세포암의전이병변진단에서는민감도, 특이도, 정확도모두높은결과를보였다고보고하였다. 또한 Martínez 등 12 은신세포암과관련되어 2004년까지보고된 18 F-FDG PET 연구들을분석한결과, 신세포암의병기재설정과전이부위의진단에서높은민감도와특이도를보고하기도하였다. 18 F-FDG PET 또는 PET/CT는 18 F-FDG가소변으로배설되기때문에일반적으로방광암진단을위해유용한검사가될수없다 F-FDG를사용한저자들의연구에서도방광종양감별및진단의한계를극복할수없었고, 낮은양성예측률을보였다. 방광암과관련된최근의연구들은신장암과비슷하게조기진단보다는원격전이, 재발평가등에관심이모아지고있으며, 실제로 18 F-FDG PET와 18 F-FDG PET/CT는림프절전이발견및수술전병기설정과원격전이진단에서우수한결과들을보였다. 9,13-15 최근에는 18 F- FDG 방사선추적자의한계를극복하기위해소변으로배설되지않는추적자들 ( 11 C-methionine, 11 C-choline 등 ) 을이용한연구가진행되고있다. 16 Ahlstrom 등 17 은 11 C-methionine을이용하여대부분의방광암을발견하였다고보고하면서, 11 C-methionine의섭취량과방광암분화도의상관관계를제시하였다. 11 C-choline을사용한 Gofrit 등 18 의연구에서도 18명의진행성이행세포암환자모두방광암이진단되었다. 또한방광암의림프절전이여부에있어서도 11 C- choline PET/CT가 CT보다우수한것으로확인되었다. 19,20 그러나이러한우수한결과들에도불구하고, 11 C-choline은반감기가너무짧아일상적인검사가쉽지않고, 잦은합성이필요하다는단점을극복해야한다. 전립선암에대해 18 F-FDG PET의진단율은높지않으며,

5 유광호외 : 비뇨기과영역에서 18 F-FDG PET/CT 의임상적역할 779 양성전립선비대증과감별이어렵다고보고되고있다. 21,22 18 F-FDG PET/CT 역시전립선암진단을위해유용하지못한것으로알려져있다. 본연구에서도전립선종양이의심되었던 50명의환자는거의대부분전립선염및양성전립선비대증으로진단되어비뇨기계종양중에서가장낮은결과를보였다. 현재전립선암에서 18 F-FDG PET와 PET/CT의역할은치료후국소재발여부를결정하는데있다. 특히 prostatespecific antigen (PSA) 이증가하였지만기존의 CT 또는골스캔에서이상부위를찾아내지못한경우에유용하다. 23 전립선암병기결정에있어매우중요한주변림프절전이는원발병변에섭취증가없이 18 F-FDG PET에서나타날수있고, 이는전이림프절에서의높은증식속도때문일것으로추측하고있다. 21 또한골전이의진단은골스캔으로충분하다고생각되었으나, 골수에국한된전이병변은 18 F-FDG PET 가더예민하게찾아낼수있다. 근래에는방광암과마찬가지로전립선암에서 PET/CT의방사선추적자로 18 F-FDG를사용하는것보다 11 C-choline, 18 F-fluorocholine, 11 C-acetate, 18 F-fluoride 등이효과적일것으로기대되고있다. 9 일부 11 C-choline PET를이용한연구에서전립선암원발병변진단에기존의 MRI보다정확도가높았다는보고가있으나, 24 아직까지 11 C-choline, 18 F-choline PET/CT 역시전립선암의일차적진단검사가될수없다는의견이지배적이다. 25,26 18 F-FDG PET와 PET/CT가가장유용한종양은고환암이다. 18 F-FDG PET는고환암의조기진단, 수술후잔류종양진단, 재발진단및치료에대한반응에서기존의영상학적검사에비해우수한것으로입증되었다. 13,27 고환암의치료방침을결정하는데 18 F-FDG PET가매우유용하며, 검사결과에따라 57% 의환자에서치료방침이바뀌었다는보고도있다. 28 비록저자들의연구대상에서고환암환자가없어그결과를비교할수없으나, 18 F-FDG PET와 PET/CT는고환암에서우선적인영상학적검사가될것으로기대한다. 아직까지 18 F-FDG PET/CT는비뇨기계악성종양에대한선별검사가될수없으며, 비뇨기과질환을진단하거나건강검진목적으로 18 F-FDG PET/CT를사용하는것은제한점이많다고생각한다. 또한기존의영상학적진단검사에비해경제적인측면에서효율적인방법이라고할수없으며, 기존의비뇨기계종양환자들의추적검사에서도 18 F-FDG가소변으로배설되기때문에그유용성은제한적이라고생각한다. 실제로일본의연구결과에서도비뇨기계질환의비율과악성종양환자수는매우극소수로확인되어, 비뇨기계질환을위해 PET/CT가보편적으로이용되는것은현재까지많은한계점을갖고있다고생각한다. 그러나최근에는많은일반인들이비뇨기계질환뿐만아니라전신질환 에대한선별검사로 PET/CT를고비용에도불구하고건강검진목적으로시행하고있으며, 악성종양환자에서는 PET/ CT의보험적용으로인한경제적혜택으로추적검사에시행하는경우가늘고있다. 그결과우연히발견된비뇨기계이상으로비뇨기과를내원하는환자는꾸준히증가하고있으므로, 임상진료에활용할수있기위해서는 18 F-FDG PET/CT에대한비뇨기과의사로서적절한인식이필요하다. 향후 PET/CT는 18 F-choline, 11 C-acetate 등의새로운방사선추적자를상용화하여각각의비뇨기계질환에대한정확성을향상시켜이러한한계점을극복하는방향으로발전해나가야할것이다. 또한다양한생체현상을영상화할수있는 PET/CT의장점을고려해볼때, 미래에는종양의저산소영역, 혈관신생등구체적인치료목표를영상화하고, 세포치료또는유전자치료경과를추적하는데활용하여종양환자에서생존율과삶의질향상에기여하리라생각한다. 결 18 F-FDG PET/CT는비뇨기계영역에서진단및건강검진목적의영상학적검사가될수없다. 그러나 18 F-FDG PET/ CT에서우연히비뇨기계이상소견을보인환자는지속적으로늘어나고있으며, 부신및신장종양이의심되는환자에대해서는방광종양과전립선종양에비해상대적으로높은양성예측률을보이므로 CT, MRI 등의기존영상검사를포함하여부가적인진단검사를적극적으로시행해야될것으로생각한다. 론 REFERENCES 1.
