We investigated HER2 overexpression and amplification and their concordance rate in Korean gastric adenocarcinomas. Methods : Tissue microarrays were constructed with 232 gastric adenocarcinoma samples. We performed immunohistochemistry (IHC), silver-enhanced in situ hybridization (SISH) and fluorescence in situ hybridization (FISH) for HER2. Results : IHC was negative in 94.8% (218/232), equivocal in 1.7% (4/232) and positive in 3.5% (8/232) of cases. HER2 protein expression was heterogeneous in 75% (9/12) of IHC 2+/3+ cancers. Gene amplification was observed in 6.5% (15/230) by SISH and the same 15 cases were also FISH-positive. We observed HER2 amplification in 1.4%, 27.3%, 25%, and 100% of IHC 0, 1+, 2+, and 3+ gastric adenocarcinomas, respectively. The concordance rate between IHC and SISH results was 95.7%. Conclusions : HER2 overexpression and amplification were less frequent in gastric adenocarcinomas than breast carcinomas. Compared to breast carcinoma, (1) there may be IHC-negative but gene amplification-positive cases for HER2 and (2) frequent intratumoral heterogeneity of IHC for HER2 in gastric adenocarcinomas. Key Words : Stomach neoplasms; HER2, human; In situ hybridization, fluorescence; Silver; In situ hybridization 위암은우리나라에서가장흔히발생하는암으로, 년동안암으로진단받은환자들을대상으로발생률을조사한결과, 남자의경우전체암발생중 1위를차지하였고여자에서는 3위를차지하였다. 1 이러한위암의 5년생존율은꾸준히증가하고있지만, 년동안진단받은위암환자들의 5년생존율은약 61.2% 로초기에발견된조기위암은비교적예후가좋으나, 진행성위암은항암화학요법이나방사선치료를시행한다하더라도예후가좋지않았다. 또한 2007년암으로인한전체사망가운데위암의연령표준화사망률이남자는 3위, 여자는 2위였다. 1,2 Human epidermal growth factor receptor 2 (HER2) 는침윤성유방암에서중요한예후인자인동시에예측인자이며, 유방 암의 20-25% 에서유전자증폭이관찰되고그결과 HER2 단백과발현이나타난다. 3 HER2 유전자의증폭또는과발현이있는유방암환자는나쁜예후를나타내지만, HER2 단백에대한단클론항체인 trastuzumab (Herceptin, Genentech, South San Francisco, CA, USA) 을이용한표적치료의대상이된다. 4 이러한 HER2 유전자의증폭또는과발현은유방암외에도난소암, 전립샘암, 대장암, 췌장암, 위암등에서도보고되고있는데, 3,5-8 최근 HER2 양성인진행성위암환자에게 trastuzumab을기존항암제 (5-fluorouracil 또는 capecitabine과 cisplatin) 와병용투여하였더니기존항암제만투여한군보다환자의생존기간이유의하게증가하였다는다기관임상 3상연구 (ToGA trial) 결과가발표됨으로써, HER2의상태가유방암뿐만아니 493

2 494 김애리 배정민 김세원외 2 인 라위암에서도중요한치료예측인자가되고있다. 8,9 환자의조직에서 HER2 유전자의상태를검사하는방법으로는면역조직화학염색 (immunohistochemistry, IHC) 을이용하여 HER2 단백과발현을검사하는방법과형광제자리부합법 (fluorescence in situ hybridization, FISH) 이나발색제자리부합법 (chromogenic in situ hybridization, CISH) 을이용하여유전자증폭을검사하는방법등이있다. 위암에서는주로 IHC 를이용한연구가이루어졌는데, HER2 단백과발현율은연구자마다매우다양하게보고되고있다. 9,10 국내에서시행한연구에서위암의 HER2 과발현 (IHC 2+/3+) 빈도는 15.9% 와 22.6% 였으며, 유전자증폭은 3.8% 와 7.7% 에서관찰되었다. 11,12 이에저자들은최근국내에서 trastuzumab을 HER2 양성인진행성위암환자에게사용하는것이승인됨에따라조만간모든진행성위암을대상으로 HER2 유전자검사가시행될가능성이있다고판단하였다. 따라서이제까지유방암에서 HER2 유전자검사를시행했던경험을바탕으로위암조직을대상으로 IHC, FISH, 그리고유전자증폭을검사하는새로운방법인은제자리부합법 (silver-enhanced in situ hybridization, SISH) 을이용하여 HER2 과발현및유전자증폭의빈도를조사하여보았다. 그리고현재유방암에적용하고있는 HER2 검사의흐름도 13 가위암에서도적용가능한지, 또한유방암에서와마찬가지로 SISH가유전자증폭을관찰하는데 FISH를대체할수있는검사법 이될수있는지에대해서도알아보고자하였다. 재료및방법연구대상 2000년 1월부터 12월까지일년동안영남대학교의과대학부속병원과대구파티마병원에서위샘암종으로진단받고근치적절제술을시행한 232명의환자조직을이용하여연구를수행하였으며, 연구대상환자의임상병리학적특성은 Table 1에정리하였다. 환자의조직은 10% 중성포르말린에고정된후파라핀에포매된조직으로통상적인병리조직검사가끝난뒤병리과에보관되어있던검체를이용하였으며, 각환자당병변의대표파라핀블록 1개와림프절전이가있는경우는림프절블록 1개를선정하여조직배열블록 (tissue microarray, TMA) 을제작하였다. Quick-Ray TM (Unitma, Seoul, Korea) 기구를이용하여증례당직경 2 mm 조직 3개를파내새로운파라핀블록에심었으며, 림프절전이가있는경우는병변에서 2개, 림프절에서 1개의조직을얻었다. 결과적으로 232명의환자조직에서총 13개의 TMA 블록을제작하였으며, 각블록에서 4 mm 두께의연속절편을얻어 hematoxylin & eosin, IHC, SISH를시행하였고 FISH는 2 mm 두께의조직절편을사용하였다. 3 HER2 Status in Gastric Adenocarcinomas 495 anti-rabbit antibody와은a (silver acetate), 은B (hydroquinone), 은C (H2O2) 가들어있어이들을조직에적용시키면은이온 (Ag+) 이 hydroquinone에의해금속은원자 (metallic silver atoms) 로환원되어 probe가결합된부위에침착하는데이반응을 HRP 기질인 H2O2 (silver C) 가유도한다. 발색반응후 Ventana hematoxylin II를이용하여대조염색을시행하고, 광학현미경 600배시야에서증례당약 20개의암세포에서나타나는발색신호를세었다. 형광제자리부합법 FISH는 PathVysion HER2 DNA probe kit (Abbott/ Vysis, Abott Park, IL, USA) 를이용하여자동화장비 (Discovery, Ventana Medical Systems) 로시행하였다. 각검사단계를간략하게정리하면다음과같다. 즉 2 mm 두께의조직절편을 EZ Prep TM (Ventana Medical Systems) 을이용하여탈파라핀한후, RiboMap kit (Ventana Medical Systems) 의 RiboPrep TM reagent를 37 에서 32분간그리고 RiboClear TM reagent를 37 에서 16분간전처리하고, 37 에서 40분간 protease 3 (Ventana Medical Systems) 와반응시켰다. 그후 20 ml의 PathVysion LSI HER2/CEP probe를적용시킨뒤커버슬립을덮고 75 에서 6분간변성시킨다음 37 에서 12시간유지하여부합반응을시켰다. 또커버슬립을제거하고 Ribo- Wash TM bulk reagent (Ventana Medical Systems) 로세척한후, RiboMap kit의 RiboFix TM reagent로고정을한다음말렸으며, 20 ml의 4,6-diamidino-2-phenylindole (DAPI) 로핵막염색을한다음판독할때까지슬라이드는 -20 에서보관하였다. 그리고결과판독을위해형광현미경 1,000배시야에서 DAPI/ Spectrum Green/Orange에대한단필터와삼중필터를이용해각증례당약 20개의암세포에서형광신호를세었다. 판독기준검사결과판독은다른검사결과를모르는상태에서한명의병리의사가판독하였다. 이때 SISH와 FISH는적어도 20개의암세포를관찰하였고, 종양의불균질성 (tumor heterogeneity) 이있다고판단되면형광또는발색신호가높은곳에서관찰하였다. IHC와 FISH의결과판독은 American Society of Clinical Oncology (ASCO)/College of American Pathologists (CAP) 기준 13 에따랐으며, 이는 Table 2에기술하였다. 단, IHC 의경우조직배열블록을만들어연구를시행하였기때문에종양의 30% 미만에서강양성을보이더라도 3+ 로판독하였으며, Hofmann 등 17 이제시한판독기준을참조하여샘구조의내강쪽이염색이안되더라도나머지세포막이강양성으로염색된경우는 3+ 로판독하였다. SISH 결과판독은제조사인 Ventana Medical Systems ( Table 2. NA, not available; LN, lymph node; Amp, amplification. SISH를이용하여 HER2 유전자의증폭유무를검사한결과, 232예위암환자조직중 230예에서결과를얻을수있었는데, 그결과 213예 (92.6%) 는음성이었으며, 2예 (0.9%) 는불확실 (equivocal) 하였고 15예 (6.5%) 에서는유전자증폭이관찰되었다. HER2 유전자증폭은 IHC 0인군의 1.4% (3/207), 1+ 군의 27.3% (3/11), 2+ 군의 25% (1/4), 그리고 3+ 군의 100% (8/8) 에서관찰되었다 (Table 4). 한편유전자증폭이관찰된 15예가운데 IHC 0/1+ 인증례의 SISH ratio는 였고, IHC 2+/3+ 인증례의 SISH ratio는 였다. 또한 7예에서뭇염색체 (polysomy) 가관찰되었는데, 이중 1예는불확실 (SISH ratio, 1.9) 하였고 6예는음성으로판독된증례였다. 뭇염색체로나타난 7예의 IHC 결과는 2예가 0, 4예가 1+, 그리고나머지 1예가 2+ 였다. SISH 결과를 IHC 결과와비교하였을때결과일치율 (concordance rate) 은 95.7% (220/230) 였으며, IHC 에서 2+ 인증례를제외하고음성또는양성인증례만을대상으로하였을때 IHC와 SISH의결과일치율은 96.9% (219/226) 였다. Table 4. Comparison between immunohistochemistry (IHC) and silver-enhanced in situ hybridization (SISH) results for human epidermal growth factor receptor 2 status IHC FISH 와 SISH 결과의비교 FISH는 IHC 2+/3+ 인증례와 SISH에서유전자증폭이관찰된 IHC 0/1+ 증례에서만시행하였다. 그결과 22예에서 FISH 를시행하였으며, 15예에서 HER2 유전자증폭이관찰되었는데이들은 SISH에서도증폭이관찰된예였다 (Fig. 2). 한편 SISH 에서불확실 (SISH ratio, 1.9) 로판독되었던 1예가 FISH에서음성 (FISH ratio, 1.7) 으로나온것외에나머지 21예의결과는 SISH 결과와일치하였다 (Table 3). SISH 결과를 FISH 결과와비교하였을때결과일치율은 95.5% (21/22) 였다. HER2 상태와임상병리학적인자들과의관련성 HER2 단백발현과유전자증폭유무에따른임상병리학적인자들 - 성별, 종양의위치, 조직학적분화도, 침윤의깊이, Lauren 분류, 림프절전이상태 - 과의관련성을살펴보았을때 HER2 단백발현 (IHC 2+/3+) 은 Lauren 분류의장형 (intestinal type) 에서유의하게높았다 (p=0.04). 또한 SISH로측정한 HER2 유전자의증폭은고분화샘암종에서유의하게높게관찰되었으며 (p=0.005), 장형에서미만형이나혼합형보다높았으나통계학적으로유의한차이는아니었다 (Table 5). 조기위암과진행성위암으로나누어 HER2 단백발현과유전자증폭의빈도를조사하였을때두군사이의차이는없었다 (p>0.05). 고 1986년에처음으로위암에서 HER2 유전자증폭과단백과발현이보고되었고, 18,19 이후많은연구자들이위암을대상으로연구를시행하여다양한빈도의 HER2 단백과발현 (6-53.4%) 과유전자증폭 (16-27%) 을보고하였다. 8,10 최근위암을대상으로시행한전향적임상 3상연구 (ToGA trial) 에서 HER2 양성유방암환자에게표적치료약물로사용하던 trastuzumab을 HER2 양성 (IHC 3+ 또는 FISH 양성 ) 인위암환자에게기존에사용하던항암제와함께투여했을때기존항암제만투여한환자에 찰 SISH Negative Equivocal Positive Total Negative (0) (90) (1+) (4.8) Equivocal (2+) (1.7) Postitive (3+) (3.5) Total 213 (92.6) 2 (0.9) 15 (6.5) 230 (100) Values are presented as number (%).

NS, not specific; SRC, signet ring cell carcinoma; LN, lymph node. 판독기준도 Kim 등 12 이 HercepTest 판정기준을따른데반해저자들은 ASCO/CAP 기준을이용하였으며, IHC 3+ 에해당하는염색강도를보일때 TMA라는점을감안하여종양세 포의비율에상관없이양성으로판독하는등약간차이가있었다. 이처럼사용한항체나판독기준의차이가있음에도불구하고 IHC 3+ 와유전자증폭비율이두연구에서비슷하게나온것을보면 HER2 양성위암의비율은유방암에비해현저히낮음을알수있었다. HER2 IHC는대부분유방암을대상으로시행된연구이긴하지만 FISH와높은결과일치율을보여주었다 그러나일반적으로조직에서 HER2 상태를측정할때는단백발현을관찰하는 IHC보다유전자증폭을직접관찰하는 FISH가좀더나은방법으로인식되고있다. 이러한인식의가장큰이유는 IHC 결과가조직고정액의종류, 고정시간, 항원노출방법, 사용한일차항체의종류에따라영향을받으며, 결과판독도주관적일수있기때문에실험실간또는검사자간결과의차이가크기때문이다. 24 이에반해 FISH는결과가좀더객관적이고증폭정도를정량화할수있다는장점이있지만검사에사용되는시약이매우비싸고결과판독을위해서고가의형광현미경과필터가필요하며, 형광신호가급속히퇴색한다는단점을가지고있다. 반면 SISH는가장최근에개발된제자리부합법 (in situ hybridization, ISH) 으로발색신호를광학현미경에서관찰하기때문에 FISH에비해조직형태를알아보기쉽고, 검사방법이자동화되어있어검사에소요되는시간이 FISH나 CISH에비해짧기때문에당일검사및판독이가능하며검사결과의일관성을유지할수있다는장점이있다. ASCO/CAP는잘인증된검사를시행한다면 HER2 표적치료제의이점을예측하는데있어 IHC나 ISH 중어느검사가더우위라고볼수없으며, 새로운검사는이전의인증된검사와비교했을때 95% 이상의결과일치율을보여야한다고하였다. 13 이에저자들은유방암을대상으로한이전연구 16 에서 IHC, FISH, SISH를이용하여 HER2 상태를측정한결과서로다른두검사간결과일치율이 95% 이상임을확인하였고, 동일한방법을위암에적용하여이들검사법이위암의 HER2 측정에도서로높은결과일치율

8 500 김애리 배정민 김세원외 2 인 을보이는지알아보고자하였다. 저자들의연구결과에의하면위암에서 IHC와 SISH의결과일치율은 95.7% 였고, SISH와 FISH를모두시행한예의결과일치율은 95.5% 로나타나어느검사를시행하더라도위암에서 HER2 상태를잘반영할수있을것으로생각되었다. 그러나위암에서 IHC와 FISH 결과를비교한연구를살펴보면 HER2 유전자의증폭은 IHC 0/1+ 를보인증례의 % 에서관찰되었고, 25,26 ToGA trial에서도 IHC 0/1+ 인위암의 23% 에서유전자증폭이관찰되었다. 27 또한본연구에서 IHC 0인종양의 1.4% 와 1+ 종양의 27.3% 그리고 2+ 종양의 25% 가유전자증폭을나타냈는데, 이는유방암을대상으로동일한방법으로 HER2 유전자상태를측정한연구결과 16 와차이를보이는것이다. 유방암에서는 IHC 0/1+ 였던모든증례에서유전자증폭이관찰되지않았다. 유방암에서는 HER2 상태를검사할때, IHC를일차적으로시행하여 0 또는 1+ 인경우 HER2 음성유방암으로, 3+ 인경우 HER2 양성유방암으로판정하고 IHC에서 2+ 로판독된경우에만유전자증폭유무를검사하는것을권장하고있다. 13 하지만위암에서는유방암과동일한접근을하기가곤란할것으로생각된다. 왜냐하면유전자증폭이있음에도불구하고 IHC에서음성 (0/1+) 으로판정되어표적치료대상에서제외될증례들이있기때문이다. 우리나라에서도최근유럽에이어 trastuzumab 이 HER2 양성인진행성위암환자에게사용하는것이승인됨에따라치료대상환자군선별을위한가장효과적인 HER2 검사법을찾을필요가있다. 이를위해서는 IHC 0/1+ 이면서유전자증폭이있는군에서의 trastuzumab 투여효과와 IHC 3+ 이면서유전자증폭이있는군에서의 trastuzumab 투여효과를비교하여, HER2 단백발현과유전자증폭중어느것이더치료효과를잘반영하는지를밝히는것이우선이다. 왜냐하면두군모두 trastuzumab에효과가있다면일차검사법으로 IHC를사용하는것은문제가있기때문이다. 현재진행되고있는 ToGA trial에서이와관련한해답을얻기를기대한다. 위암에서 HER2 단백발현이유방암에비해불균질하게나타난다는것은다른연구자들도보고하였지만, 17,26 저자들의연구에서도 IHC 3+ 증례의 25% 에서만 TMA에포함된모든조직에서균질한염색양상을보였고, 나머지증례는부위마다발현정도의차이를보였다. 증폭이관찰된 15예중 2예 (13.3%) 에서원발종양은 IHC와 SISH에음성이었으나, 림프절에전이된종양세포만양성을나타내림프절전이가있는위암에서 HER2 검사를시행할때전이병소도포함시켜야할지아니면광범위한부위의원발종양병소에서 HER2 검사를시행해야할지에대해서는좀더많은증례를대상으로한연구가필요할것으로생각된다. 한편저자들의연구에서 HER2 단백발현 (IHC 2+/3+) 은 Lauren 분류의장형에서유의하게높게나타났고, 유전자증폭은고분화샘암종에서높게나타났는데이러한결과는다른연구에서도동일하게나타났으며, 위식도문합부의종양이위에서 발생한종양에비해 HER2 발현이높다고알려져있다. 8,27 그러나위암에서 HER2의단백발현또는유전자증폭이예후에미치는영향에관해서는여러연구들이일관된결과를보여주지못하였다. 8,10,11,25 이는특히 HER2 단백발현의경우염색방법이나판독기준등이통일되지않아연구자들마다과발현군으로분류한위암이이질적인집단일가능성이높기때문이다. Hofmann 등 17 이유방암에서사용하는 HER2 IHC 판독기준을근거로위암에적용가능한판독기준수정안을제시하였는데, 이를적용한 ToGA trial에서 IHC와 FISH 간결과일치율이 87.5% 로유방암에비해다소낮게나타나, 27 앞으로위암의 HER2 IHC 판독기준에대한논의와검증이필요할것으로생각된다. 결론적으로저자들은 230예의위암조직에서 HER2 발현및유전자증폭을조사한결과, HER2 과발현 (IHC 3+) 은전체종양의 3.5%, 유전자증폭은 6.5% 에서나타나위암은유방암에비해 HER2 변이빈도가낮음을알수있었다. 또한 HER2 발현음성 (IHC 0/1+) 인위암의일부에서도유전자증폭이발견되므로위암에서 trastuzumab 투여대상환자선별을위해서는현재유방암에적용되는검사흐름도와는다른접근이필요하며, 특히유전자증폭을검사하는방법인 SISH와 FISH가서로높은결과일치율을보여주므로, SISH나 FISH 중각검사실환경에적합한방법을선택하여시행하면될것으로생각된다. 참고문헌 1.
