3 김재홍 임만빈 이창영 김일만 일시적 결찰술를 시행하고 동맥류가 발생된 부위를 조사한 견이 있었다(Table 1). 이중에서 다른 2례의 증례를 예시 바, 동맥류는 사라지고 뇌량주위동맥 자체의 약 2/3가 소 하고자 한다. 실되어 있어 이 부위를 절재하고 양 끝을 서로 문합하였다. 증 례 5 22세 남자로 1997년 2월 13일 교통사고로 타병원에 입 이 과정에서 뇌량주위동맥의 일시적 결찰 기간은 3시간이 었다. 이후 환자는 계속 반 혼수상태로 지내다가 사망하였 고 추적 시행한 뇌전산화단층촬영상 대뇌부위에 뇌경색소 원 가료중 반복적인 심한 비출혈이 발생되어 자기공명사진 A C B Fig. 1. 가성동맥류의 수술적 가료 (이하 RI라 칭함) 촬영을 시행한 바 우측 내경동맥의 해 경동맥을 폐쇄시켰다. 시술후 28일에 시행한 뇌혈관촬영상 면정맥동 부분에서 기원하고 접형동으로 돌출된 가성 동맥 혈류가 천측두동맥을 통하여 중대뇌동맥으로 흐르는 것이 류가 의심되어 1997년 5월 21일 본원으로 전원되었다. 신 확인되었고(Fig.

로 뇌혈관촬영을 시행한 바 내경동맥의 해면정맥동 부분에 서 내측으로 접형동내로 돌출된 가성동맥류가 발견되었다 (Fig. 1A). 내경동맥을 풍선(balloon)으로 폐쇄하고 10분 을 경과한 바 환자는 우측 반신의 마비가 발생되어 천측두 동맥-중대뇌동맥 문합술를 시행하고 가성동맥류를 폐쇄하 기로 계획하였다.

이후 환자는 뇌혈관촬영실 로 이동시켜 3개의 분리풍선을 이용하여 가성동맥류와 내 증 례 6 17세 남자로 간질발작을 주소로 내원하였다. 과거 10개 월전 두부외상으로 좌측 두정부 및 후두부에 뇌경막외 혈 종과 뇌량 부위에 뇌내혈종이 있어 본원에서 수술하고 외 래에서 추적관찰중인 환자였다.

4 가성동맥류의 수술적 가료 (이하 RI라 칭함) 촬영을 시행한 바 우측 내경동맥의 해 경동맥을 폐쇄시켰다. 시술후 28일에 시행한 뇌혈관촬영상 면정맥동 부분에서 기원하고 접형동으로 돌출된 가성 동맥 혈류가 천측두동맥을 통하여 중대뇌동맥으로 흐르는 것이 류가 의심되어 1997년 5월 21일 본원으로 전원되었다. 신 확인되었고(Fig. 1B), 좌측 내경동맥을 혈관촬영한 바 전교 경학적 검사상 특별한 이상소견은 없었고, 양안의 시력도 통동맥을 통하여 우측 전대뇌동맥 및 중대뇌동맥이 혈류공 정상이었다. 1997년 5월 22일 우측 내경동맥을 선택적으 급을 받으나 가성동맥류는 조영되지 않았다(Fig. 1C). 로 뇌혈관촬영을 시행한 바 내경동맥의 해면정맥동 부분에 서 내측으로 접형동내로 돌출된 가성동맥류가 발견되었다 (Fig. 1A). 내경동맥을 풍선(balloon)으로 폐쇄하고 10분 을 경과한 바 환자는 우측 반신의 마비가 발생되어 천측두 동맥-중대뇌동맥 문합술를 시행하고 가성동맥류를 폐쇄하 기로 계획하였다. 1997년 5월 23일 천측두동맥-중대뇌동 맥 우회술를 시행하고자 마취를 시행하였다. 이때 동맥류 가 파열되어 다량의 비출혈이 발생되었다. 급히 경부 경동 맥를 압박하고 비강에 packing을 시행후 내경동맥을 노출 시킨 다음 내경동맥 결찰을 준비하여 두고 천측두동맥-중 대뇌동맥 문합술을 시행하였다. 이후 환자는 뇌혈관촬영실 로 이동시켜 3개의 분리풍선을 이용하여 가성동맥류와 내 증 례 6 17세 남자로 간질발작을 주소로 내원하였다. 과거 10개 월전 두부외상으로 좌측 두정부 및 후두부에 뇌경막외 혈 종과 뇌량 부위에 뇌내혈종이 있어 본원에서 수술하고 외 래에서 추적관찰중인 환자였다. 뇌전산화단층(이하 CT라 칭함) 촬영상 대조강화되는 둥근 모양의 종괴가 대뇌겸에 붙어 우측 전두엽 부위의 대뇌반구간 열구에 위치하고 있 었다(Fig. 2A). 뇌량주위동맥(pericallosal artery)을 초선 택적으로 시행한 뇌혈관촬영상 부중심동맥(paracentral artery)에서 기원하는 동맥류가 발견되었고(Fig. 2B) 이는 CT 촬영상 보이는 소견을 참조한 바 혈전화된 외상성 동맥류로 A B C D Fig. 2. 김재홍 임만빈 이창영 김일만 추정되었다. 동맥류가 발생된 부중심동맥은 대뇌의 운동영 endocarditis)에 기인한 세균성 동맥류였고, 2례는 수술후 역의 혈류를 공급하므로 혈관내 수술방법으로 이 동맥 및 동맥류가 발생한 예였는데 1례는 간질 수술후 뇌막염에 기 동맥류를 폐쇄시키지 못하고 수술을 시행하기로 하였다.

5 김재홍 임만빈 이창영 김일만 추정되었다. 동맥류가 발생된 부중심동맥은 대뇌의 운동영 endocarditis)에 기인한 세균성 동맥류였고, 2례는 수술후 역의 혈류를 공급하므로 혈관내 수술방법으로 이 동맥 및 동맥류가 발생한 예였는데 1례는 간질 수술후 뇌막염에 기 동맥류를 폐쇄시키지 못하고 수술을 시행하기로 하였다. 수 인된 세균성 동맥류였고, 1례는 중대뇌동맥 동맥류를 결찰 술은 전신마취하에서 대뇌반구간 열구로 접근하였다. 동맥 후 aspergillus의 감염에 기인된 진균성 동맥류였다(Fig. 3). 류는 혈전화되어 딱딱하였고 크기는 직경이 약 3cm 정도로 동맥류의 발생부위는 중대뇌동맥에 3례, 후대뇌동맥에 1례 측정되었다. 동맥류의 경부는 없었고 부중심동맥의 파열된 였다. 중대뇌동맥에 발생한 예중 1례는 2개의 동맥류를 가 부위에서 기원되었다. 동맥류 경부의 결찰이 불가능하여 동 졌던 예로써 중대뇌동맥 전체 동맥류 수는 4개였고, 3개의 맥류를 절제하고 10-0 나일론 사를 이용하여 절단된 부중 동맥류는 말초부위에, 1개는 수평분절에 발생된 예였다. 후 심동맥의 단 과 단(end-to-end)을 봉합하고 그 부위를 대뇌동맥에 발생되었던 예는 P1-P2 접합부에 발생한 예 Sundt clip으로 보강시켰다. 수술후 환자는 운동력 약화를 였다. 수술방법은 동맥류를 적출하였던 예가 2례, 결찰하였 보이지 않았고 술후 시행한 뇌혈관촬영상 동맥류는 보이지 던 예가 2례였다. 1례에서만 양호한 결과를 얻었고 2례는 않고 혈류는 연결 봉합한 부중심동맥의 부위를 지장없이 통 사망하였다. 심내막염에 기인된 예는 수술후 1개월 반 및 4 과하는 소견을 보였다(Fig. 2C). 절제된 동맥류의 병리소 개월후에 타부위에 새로운 동맥류가 발생되어 출혈를 일으 견은 동맥류내 혈전이 형성되고 섬유조직으로 형성된 동맥 켜, 처음 수술후의 경과는 양호하였으나 1례는 반신부전마비 류 벽이 관찰되었다(Fig. 2D). 가 발생되었고 1례는 사망하였다. 다른 사망한 1례는 asp- 2. 세균성 및 진균성 동맥류 진균성 동맥류는 4례로써, 남자 및 여자가 각 각 2례였고, 연령은 7세에서 63세였다. 2례는 세균성 심내막염(bacterial ergillus에 기인한 진성 진균성 동맥류였다(Table 2). 1례 의 증례를 예시하고자 한다. 증 례 4 7세 남아로 1998년 3월 7일 학교에서 수업을 받던중 갑작스런 두통과 좌측 반신 마비가 발생되어 1998년 3월 9일 타병원을 거처 본원에 전원되었다. 신경학적 검사상 환 자의 의식상태는 가면상태였고, 좌측 반신마비가 관찰되었 다. 과거력상 어려서부터 심장병을 앓았다는 병력이 있었 고 심음 청취상 잡음이 청취되었다. 심초음파도상에서 심 실중격결손과 승모판폐쇄부전이 발견되었다. 입원시 시행 한 CT 촬영상 소량의 뇌지주막하 출혈이 좌측 실비안구에 서 관찰되었고 우측 내포(internal capsule) 부위에 뇌경색 을 암시하는 저음영 병소가 관찰되었다. 조영제 주입후 시 행한 CT 촬영상 좌측 실비안 구에서 원형상의 조영 증강 Fig. 3. 수술후 환자의 신경학적 상태는 술전 상태와 비교하여 변 함이 없었으나, 발열이 계속되어 심내막염을 치료하기 위하 여 소아과로 전과하였다.

소실되었으나(Fig. 4D, 4E) 처음 수 술시 관찰되지 않았던 기저동맥 분기부에 새로운 동맥류가 312 관찰되었고(Fig. 4F) 환자는 회복하지 못하고 사망하였다. 3. 원인 미상의 동맥류 원인미상 예들은 6례로 남자가 5례, 여자가 1례였고, 연 령은 40일에서 52세로 다양하였다.

F Right vertebral angiogram, A- P view, showing a new aneurysm at the bifurcation of basilar artery. 소형의 동맥류(Fig. 4B)가 발견되었으나, 기저동맥 분기부 에는 동맥류가 존재하지 않았다(Fig. 4C). 동맥류들을 1998 년 3월 17일 결찰하였다. 먼저 전신마취하에서 pterional 로 접근하여 좌측 거대 동맥류를 3개의 straight clip으로 결찰한 다음 다시 환자의 위치를 좌측으로 돌린 후 우측 동 맥류를 결찰하였다. 수술후 환자의 신경학적 상태는 술전 상태와 비교하여 변 함이 없었으나, 발열이 계속되어 심내막염을 치료하기 위하 여 소아과로 전과하였다. 이후 소아과에서 치료하던 중 1998 년 4월 20일 갑작스런 두통을 호소하면서 환자는 혼수상 태가 되어 CT 촬영을 시행한 바 뇌지주막하출혈이 발견되 어 뇌혈관조영술을 시행한 바 결찰술을 시행하였던 중대뇌 동맥 동맥류들은 모두 소실되었으나(Fig. 4D, 4E) 처음 수 술시 관찰되지 않았던 기저동맥 분기부에 새로운 동맥류가 312 관찰되었고(Fig. 4F) 환자는 회복하지 못하고 사망하였다. 3. 원인 미상의 동맥류 원인미상 예들은 6례로 남자가 5례, 여자가 1례였고, 연 령은 40일에서 52세로 다양하였다. 발생부위는 총경동맥 분지부에 2례, 내경동맥의 원위부가 폐쇄되고 근위부 내경 동맥 및 추골동맥 근지(muscular branch)에 동맥류가 각 각 발생하였던 예가 1례, 두개강외 골내부(intraosseous portion) 추골동맥에 발생하였던 예가 1례, 중대뇌동맥과 후 대뇌동맥의 말초부위에 발생하였던 예가 각각 1례였다. 총 경동맥에 발생한 예중 1례는 동맥류 절제후 복재정맥을 patch 이식으로, 1례는 간치(interposition) 이식으로 치료하 였다. 추골동맥 동맥류는 근위부는 분리풍선으로, 원위부는 clip으로 폐쇄시겨 포착(trapping)으로 치료하였다. 원위부 J Korean Neurosurg Soc/ Volume 30 / arch, 2001

D Gross photograph of the aneurysm showing intra-aneurysmal thrombus surrounded by thick aneurysmal wall. 있다20)29). 해면정맥동내의 내경동맥 손상시는 해면정맥동 내경동맥에 발생시 주요 증상은 비출혈(epistaxis)이다4)7)15). 내 정맥들이 동시에 손상되면 경동맥-해면정맥동 루(car- 수상후 1 내지 3개월 후에 나타나는데 이는 접형동 주위 골 otid cavernous fistula)가 발생하고 정맥계의 손상이 동반 이 동맥류에 의하여 미란이 되어야 하기 때문이다4). 재출혈 4) 되지 않은 예들에서는 동맥류가 형성된다. 원위부의 전대 시는 사망률이 높기 때문에, 재출혈하기 전에 진단하여 치료 뇌동맥에 발생시는 대뇌겸(cerebral falx)에, 중대뇌동맥에 하는 것이 중요하다4)7). 두부손상의 병력, 편측 시력상실 및 발생시는 접형골 융기(sphenoid ridge)에 손상되어 주로 비출혈이 존재시 가능성이 높기 때문에4)15) 대조강화 CT 촬 발생한다2)29). 영이나 뇌혈관촬영을 시행하여 확인하는 것이 필요하다4). 314 J Korean Neurosurg Soc/ Volume 30 / arch, 2001
