Focal inflammatory reaction without Helicobacter pylori (H. pylori) infection is thought to be the characteristic pathologic findings suggesting Crohn s disease in the stomach. Yet gastric involvement of Crohn s disease has not been studied in Korea. We studied the endoscopic and pathologic findings of patients with Crohn s disease in the stomach by taking biopsies. Methods : Thirty patients with Crohn s disease who underwent gastroduodenoscopy followed by biopsies were included in the study. The pathology of the gastric biopsy specimens and the presence of H. pylori were evaluated. Results : Among 30 cases, 22 cases (73.3%) were H. pylori negative and 8 cases (26.7%) were H. pylori positive. For the H. pylori negative cases, all but one cases showed pit abscess and focal lymphocytic collections in the antrum. Granulomas were found in 6 cases (20%) and they were exclusively located in the antrum. Conclusions : In the stomach, pit abscess and focal lymphocytic collections that are not associated with H. pylori infection are the characteristic pathologic findings found in Crohn s disease. Key Words : Crohn s disease; Lymphocytes collection; Granuloma; Helicobacter pylori 크론병은위장관의어느부위라도침범할수있는만성염증성장질환이다. 구강에서항문까지전체소화기계에나타날수있지만주로소장및회장말단부와결장에자주발생한다. 그러나크론병이상부위장관계를침범한다는사실이 1940년에알려진이후현재까지상부위장계의크론병예가계속증가하고있다. 1-2 크론병이상부위장관계를침범하는비율은보고자마다차이를보여 5-70% 까지다양하다. 특히상부위장관계가운데위의유문부에서내시경적, 조직학적이상소견이가장흔히발견되는것으로보고되고있다. 2-4 그외십이지장구 (duodenal bulb) 와십이지장고리 (duodenal loop) 에서도병변이발생한다. 그러나식도나위의분문부, 그리고공장에서발생하는비율은극히드물다. 2 위에서발생하는크론병의진단은크론병의침범정도를알수있다는점, 중간대장염 (intermediate colitis) 환자에게서발생하는궤양성대장염과크론병을감별할수있게해준다는점, 그리고대장내시경을권고하여미만성복통증세를보이는환 자에게서발생한염증성장질환을진단하고감별하는데도움을준다는점에서중요하다. 5 상부위장관계에서크론병의진단은증세, 내시경소견및점막생검소견그리고방사선소견으로이루어진다. 2,6,7 크론병은대장및소장의만성염증성질환으로증상이있지않는한위생검을하는경우는흔하지않다. 그러나임상증상을분명하게설명하지못하는소아환자의경우에는위생검을통하여육아종의형성을보고크론병을진단할수있다. 본연구는임상적으로확진된크론병환자중에서위생검을실시한 30명을대상으로하여임상증상, 위생검의특징적인조직학적소견을살펴보고, Helicobacter pylori (H. pylori) 의유병률을알아보며, 위내시경소견과환자의증상을비교분석해봄으로써위내시경이 1년에한번씩연중행사로이루어지는우리나라에서크론병의특징적인위생검의병리소견을도출해보고자한다. 269

2 270 유창영 이보인 최규용외 6 인 대상환자 재료와방법 1991년 1월부터 2005년 8월까지가톨릭대학교성모자애병원을내원한환자들가운데임상증상및내시경소견, 그리고결핵약제치료후의반응등을추적관찰하여크론병으로확진한환자중다음과같은소견을보인 30명을연구대상으로하였다. 1) 대장내시경소견상만성염증장질환에부합되는병리소견, 즉형질세포의심한침윤, 틈새궤양 (fissuring ulcer), 정상점막과병변의혼재및육아종을보이며, 2) 상복부증상이있어서위내시경생검을시행한환자. 대장내시경실시후위내시경을바로실시한예가 14예였고, 16예는치료과정중실시하였다. 추적이가능한환자들에게이번연구의취지를상세히설명하고동의를구하였으며연구에관하여본원윤리위원회의승인을얻었다. 성별에따라분류하면여자 12명 (40%), 남자 18명 (60%) 이었으며, 평균연령은 29.4세였다 ( 범위 15-48). 3예는임상적합병증등의원인으로장절제술을시행받았다. 조직학적관찰 파라핀포매조직을 4 m 두께로박절하여헤마톡실린에오 신염색을실시한후현미경관찰을실시하였다. 위생검은분문 부와유문부에서각각병리학적소견을보면서기술하였는데이중염증세포의침윤정도, 염증의활성도, 호산구침윤, 부종, 미란, 궤양, 움염 (cryptitis) 등의조직학적소견을자세히기술하였다. H. pylori의감염여부및정도는변형된 Sydney 체계에기초하여분석하였다. 8 결 과 총 30 명의환자에게서다양한증상이관찰되었다. 이중상부 Table 1. H. pylori 는 8예 (26.7%) 에서양성이었고 22예 (73.3%) 에서는음성이었다. 특이하게도 H. pylori가양성인 8예중 3예는체부에서만 H. pylori가관찰되었고유문부에서는관찰되지않았다 (Table 3). 전체 30예중 20예는분문부에서도생검을실시하였는데, 분문부에서비특이적인병리조직소견을보인경우가 6예, 경미하거나중등도의호산구가발견된예가 9예, 미란이발견된예가 3예였다. 그리고 H. pylori에감염되지않았음에도 6예에서상피와의오목 (pit) 에염증및농양형성 (pit abscess) 과림프구의부분적인침윤이관찰되었다. 상피와 (foveolar epithelium) 내에침윤한림프구의수는대부분예에서상피와세포 100개당 10개를넘지않았다. 분문부에서육아종의형성은관찰되지않았다. 30예모두유문부를생검하였는데이중육아종의형성은 6예에서발견되었고, 미란이 3예에서발견되었다. 또한중등도이상의위축이 8예에서발견되었다. 대부분예에서 H. pylori 감염과상관없는오목내염증및농양형성과림프구의부분적인집합이관찰되었다. 그리고상피와내에침윤한림프구의수는대부분인 27예에서상피와세포 100개당 2-12개였다. 전체 30예중오직두예에서만 100개의상피와세포당각각 24개, 28개의림프구침윤을보여림프구성위염과감별해야할필요가있었다. 그러나이들두예에서는내시경검사결과편평한미란이관찰되었고병리학적으로는육아종이관찰되어서림프구성위염의가능성은떨어졌다. 조직학적으로분문부에비특이적병리소견을보인 6예를보면, 내시경검사결과 2예에서홍반이관찰되었고 2예에서는미란이관찰되었다. 이들 6예모두유문부에서는미란및홍반이관찰되었다. 이들 6예의유문부조직소견을보면, 2예에서는움와농양과국소적림프구침윤이함께관찰되었고, 1예에서는국소적림프구침윤이, 그리고 1예에서는움와농양과육아종이관 Table 2. Histopathologic findings of the stomach in Helicobacter pylori negative patients with Crohn s disease Serial no. of case Sex Age (years) Fundus Focal lymphocytic collection Pit abscess Granuloma Antrum Focal lymphocytic collection Pit abscess Granuloma 1 F M F F F M F M M M F M M M M M M M F M M F Total No of cases (%) (22.7) (22.7) (0) (68.2) (72.7) (22.7) 고찰상부위장관계에서크론병의진단은증세, 내시경소견및점막생검소견그리고방사선소견으로이루어진다. 2,6-7 여러문헌에따르면상부위장관크론병의주된증상으로는식욕부진, 삼킴곤란, 명치고통, 구역및구토, 체중감소등이있으며, 내시경소견으로는홍반, 점막비후, 미란, 궤양, 자갈모양의변화등이있어서본연구의결과와일치된소견을보여주고있다. 내시경소견으로십이지장이나식도에아프타양미란이보일때크론병을의심할수있다고하지만, 이러한소견을볼수있는예는드물기때문에 9 크론병을진단하기위해서는병리조직학적검사가반드시필요하다. Oberhuber 등 10 은크론병이발생했을때나타나는위의특이한병리학적소견으로국소성의활동성위염 (focally enhanced gastritis) 이라는용어를처음으로사용하였다. 이는국소적인 CD3+ 림프구, 조직구그리고과립구의침윤을의미하는명칭이다. 보고한바에따르면이러한조직학적소견이 H. pylori 음성인크론병환자에게서 97.5% 의예측률 (positive predictive value) 을보인다고하였다. 그러나최근의또다른연구는국소성의활동 성위염이크론병의특징적인조직학적소견이아니라고반박하고있다. 11 물론만성염증성장질환때위에서특징적인병리소견이더욱두드러지게나타나는소아환자의예가적게포함되어있어서이러한결과가나왔을수도있다고시인하고있지만, 결론적으로역시국소성의활동성위염을크론병과연관시켜과도한진단을내리는일을경계하고있다. 그러나최근의여러보고들은 H. pylori 음성인사람에게서특징적인병리학적소견인국소적인염증세포의침윤이위장생검조직에서나타나면크론병을감안해야한다고주장하고있다. 1,5,12 여러문헌에나타난크론병의위장점막병리학적소견으로는국소성의활동성위염, 육아종의형성, 국소적인선염 (glandulitis), 선농양 (glandular abscess), 움염, 호산구의침윤, 국소적인급성및만성위염등이있다. 1,10,13 이러한소견은특히위상피오목의기저부나목부위에서현저하게나타난다. 물론어떤보고자는국소적인염증소견으로궤양성장염과크론병을감별할수는없다고주장하기도한다. 14 일부문헌은육아종의형성하나만을크론병진단기준으로삼기도한다. 4 그러나위에서육아종이형성되는비율은 30-40% 정도에지나지않기때문에국소적인염증반응이상부위장관계의크론병에서더일반적으로관찰되는소견으로보고있다. 2,15

4 272 유창영 이보인 최규용외 6 인 이번연구는연구대상이주로성인으로구성되었지만국소적림프구침윤과오목의농양형성 ( 또는움와염 ) 이기존의소아를대상으로한보고와유사한비율로나타났다. 또한본연구에서는성인및소아에서육아종이형성되는비율이별다른차이를보이지않았다. 따라서 H. pylori 음성인환자에게서관찰되는분문부혹은유문부의움와농양 ( 혹은움와염 ) 과국소적인림프구의집락형성은크론병을시사하는특징적인소견으로볼수있었다. 16 H. pylori는감염률이일반인보다크론병환자에게서다소낮다는보고도있고 3,17-19 환자와대조군에서비슷한비율로나타난다는보고도있다. 10 한보고에따르면, 위장에서크론병을진단할때특징적인위장의국소적염증반응과함께 H. pylori의낮은감염률이두가지주요요건이된다고하였다. 9 본연구결과크론병환자의감염률이 26.7% 로한국인의평균감염률인 50% 보다낮았다. 20 H. pylori가크론병의진행에영향을미친다는보고가있는반면, 3,21 최근들어상부위장관계의크론병유병률이증가하고있는일본에서는 H. pylori 감염이크론병의진행과관련이없는것으로보고되고있다. 17 따라서 H. pylori 감염과크론병의연관관계에대해서는앞으로더욱활발한연구가필요하리라고본다. 육아종은위장점막의병리학적소견가운데크론병을진단하는결정적인소견이지만, 이육아종의빈도는보고자마다달라 7-68% 까지다양하게보고하고있다. 1,4,10 대장의만성염증성장질환에서육아종이발견되는빈도는대체적으로 21-37% 정도다. 22 이러한육아종은환자의나이가어릴수록, 그리고유병기간이짧을수록더잘관찰된다고한다. 4 따라서최근자료들에서는크론병환자에게 ( 특히어린환자에게 ) 일반적인상부위장관내시경검사와함께위점막생검을하도록권하고있다. 그러나병변이국소적으로발생하기때문에조직채취에따른오차가발생할여지가있다. 따라서위내시경으로생검을할때는체부, 유문부그리고위각 (gastric angle) 부위에서각각조직을얻어야한다. 9 본연구에서육아종은 20% 에서관찰되었고유문부에국한되어있었다. 위의생검상육아종이관찰되는경우에는 H. pylori 등여러감염인자들에의한위염과감별해야하는데, H. pylori는위장점막에미만성염증을유발하고심지어는육아종을형성하기도하여크론병과감별이필요할때가있다. 23 이럴경우 H. pylori 에대한치료후다시특징적인조직학적소견이있는지살펴보아야한다. 5 치료후에도이러한조직학적소견이계속보인다면크론병을의심해서대장내시경을권유할필요가있다. 1 정리하면, 크론병환자의상부위장관내시경소견으로는미란이가장흔하였고, 분문부및유문부에서 H. pylori의감염과상관없이움와내염증및농양형성과림프구의부분적인집합이관찰되었다. 위생검에서 H. pylori의감염률은 26.7% 로한국인의평균감염률보다낮았고, 증상은상복부동통이제일흔하다는것을알수있었다. 따라서 H. pylori에감염되지않았는데도분문부혹은유문부에부분적인림프구침윤과움와농양 이관찰되는경우육아종이존재하지는않는지찾아봐야할것이다. 특히나이가젊은사람에게서체중감소등의증상이함께나타날경우에는대장내시경을권할필요가있다. 참고문헌 1.
