1 대한방사선의학회지 1994 : 30(3): 간엽발육부전의방사선학적소견 1 김종민 김소선 허진도 조영덕 김선희 2 목 적 : 간엽의발생부전은매우드문질병으로서좌우엽어디서나발생할수있다. 저자들은 3 예의 간우엽발생부전과 2 예의간좌엽발생부전을경험하여이들의방사선학적소견을알아보고자하였다. 대상및방법 : 1992 년 7 월부터 1993 년 2 월까지우상복부통증등으로내원하여초음파, CT 등으로진 단된 3 예의간우엽발생부전과 2 예의간좌엽발생부전환자를대상으로하였으며, 이들중 2 예에서자 기공명영상을시행하였고, 2예에서담도촬영술을, 그리고 1 예에서디지탈감영혈관촬영술을실시하 였다. 결 과 : 간우엽발생부전의밤사선학적소견은우측문맥분지가보이지않고담낭의우측에간조직 이없는것이고, 간좌엽발생분전의소견은좌측문맥분지가관찰되지않고담낭의좌측에간조직이 없으며간원삭렬 (Iigamentum teres) 이보이지않는것등이다. 보조적으로둘이하의간정맥이관찰되 는소견등이있다. 결론 : 방사섣검사에서비정상적인간형태가보이면, 문맥분지, 담낭의위치, 간원삭렬의유무그 리고간정맥의숫자등을관찰함으로써간엽발육부전의진단을할수있겠다. 서 간엽의발육부전은드문질병으로서좌우엽어느쪽이나 발생가능하며좌엽발생부전의숫자는정확하게통계되지 않았지만드문질환이고 (1-3) 우엽의경우세계적으로 31 예보고되었고좌엽의경우보다더욱희귀하다 (4-9). 감별 을요하는질병으로는간경화, 간문부담도암등으로인한 간엽의위축, 그리고간엽절제후의소견등이있다. 그리고 담도계질환, 문맥성고혈압, 다른선천성질환, 비특이성 담냥염, 그리고위염전등이이질환과자주동반된다. 최 근저자들이우엽발생부전 3 예와좌엽발생부전 2 예를경 험하였기에이에대한 CT, MR, US, DSA 등의방사선학 적인소견을알아보고자하였다. 료응 - 대상및방법 1992 년 7 월부터 1993 년 2 월까지우상복부통증등을주 소로내원하여 CT 등으로진단된 5 명을대상으로하였고 1 고신대학교의과대학진단방사선과학교실 2 메리놀병원방사선과 이논문은 1993 년 6 월 14 일접수하여 1993 년 11 월 4 일에채택되었음 조직이없였다 (Fig. 1c). 간원삭렬은존재하면서좌엽을 - 이들중초음파, 디지탈감영혈관촬영술 (digi tal subtrac- tion angiography), 자기공명영상 (magnetic resonanceimage), 방사성동위원소겸사등을경우에따라시행하였다. 간엽발생부전의방사선학적인소견 3로간엽구분의기준 (1 0) 이되는문맥과간정액의분지, 담낭의위치, 간원삭렬 (l igamentum teres) 의유무등의소견과각간엽에나타나는형태학적인양상을분석하였다. 이들 5명의동반된임상진단과발육부전의간엽등은다음표 (Tab le 1) 에기술되어있다. 결과우측간엽발생부전 3예의경우, 그중 1 예 (case 1) 의디지탈감영혈관촬영술에서문맥조영시우측분지가전혀보이지않으면서좌측분지는현저하게커져보였고총간동맥조영상두개의분지즉좌간동액의내외측분지와외측분지에서기시하는담낭동맥이관찰되었다 (Fig. 1a, b). CT에서 3 예모두문맥의우측분지가보이지않고좌측분지만관찰되는소견을보였고, 간내담관역시우측분지는보이지않고좌측분지만관찰되었다 (Fig. 1c, d). 정상적으로좌우간엽사이에있어야하는담냥은간의우측에서복벽과의사이에위치하여담낭의우측에는간

대한방사선의학회지 1994: 30(3) : 511-516 내외측분절로나누고있었다. 간엽혹은분절의형태학적인소견으로, 좌엽은대상성으로크기가증가되었고복부의중앙선을넘어크게위치하였고 2예에서는내측분절 ( me dial segment) 이, 다른 l 예에서는외측분절이상대적으로약간커져있었다 (Fig.2).

2 대한방사선의학회지 1994: 30(3) : 내외측분절로나누고있었다. 간엽혹은분절의형태학적인소견으로, 좌엽은대상성으로크기가증가되었고복부의중앙선을넘어크게위치하였고 2예에서는내측분절 ( me dial segment) 이, 다른 l 예에서는외측분절이상대적으로약간커져있었다 (Fig.2). 3예모두에서미상엽은존재하고내측분절에부착되면서크기는좌엽의분절보다훨씬작았다 (Table 1). 이에반하여좌측간엽발생부전의 2예의 CT, MRI등의소견에서문맥의좌측분지가보이지않고우측분지만관찰되었고 ( Fig. 3), 담냥은 1 예에서는간하부 (subhepatic) 에 (Fig. 4a), 다른 1 예에서는간의좌측에서위 (stomach) 와의사이에위치하였고담낭의좌측에는간조직이존재하지않았다. 간좌엽을내외측분절로나누는경계가되는간원삭렬은 2예모두에서관찰되지않았다 (Fig. 3, 4). 간엽의형태학적인소견으로, 2예모두우엽의경계가복부의중앙선을념지않아서우엽발생부전경우와는달리상대엽이대상성으로커져있지않았다. 미상엽의경우, 1예에서는우엽의좌측면에서돌출하여문액과하대정맥사이를지나면서하대정맥을갈고리모양으로돌아나왔고 (Fig. 3) 다른 l 예에서는문맥과하대정맥이인접하면서 t r a b c Fig. 1. The left medial segment 01 the liver( M) is larger than the left lateral one(l). ligamentum teres are absen t. 미상엽은전혀관찰되지않아발생부전의소견을보였다 (Table 1) (Fig. 4b). 에담냥이위치하고간원삭렬이존재하지않았고물이하의 간정맥이관찰되었다. 그리고간엽, 간절구분의기준이되는간정맥은좌우간 엽부전모두에서둘이하로관찰되었고, 두개의간정맥이 각각같은높이에서하대정맥으로유입되는경우 ( Fig. 5), 고 찰 다른높이에셔각각유업되는경우, 두개의간정맥이하대 정맥에들어가기전에먼저만나하나로된후유업되는등 다양한소견을보였는가하면때로특히 CT 에서간정맥이 간엽의발생부전은매우드문질병으로서좌우엽어디서 나발생할수있으나좌엽의발생부전이더많다고하지만 (4, 5), 좌엽의경우누적통계가없어서정확한숫자의파악 잘보이지않는경우도있어간정맥을진단의기준에포함 이어렵다 (1-3). 그런데간우엽의발생부전에대한논문은 시키는데에어려움이있었다. 이상에서, 우엽부전은우측문맥이보이지않고담낭의우측에간조직이없고간원삭렬이존재하며룰이하의간정맥이관찰되는소견을보였으며, 좌엽부전에서는좌측문 1870 년부터 년까지 6 예가보고되었지만이들의경우임 상적인정보가충분하지못하였기에 19 59년 ( 4) 부터정렵되 어보고된 19 예이후에 Randin 등이 년에 5 예를추가발 표하였다 (6). 그리고계속해서 1, 2 차례씩더하여전세계적 맥이보이지않고담냥의좌측에간조직이없거나간하부 으로 3 1 예가보고되어있으나 (7-9) 국내에서는아직발표가 Table 1. Ultrasonogram demonstrates only two hepatic veins draining into the inferior vena cava 없었다. 간우엽발생부전의발생기전은우측간문맥의형성부전으 로생기거나흑은태생 3주경앞장관 (fo r egu t ) 에서돌출되 어나오는내배엽상피로구성된원시간 ( primiti ve li ve r ) 이 원시횡격막 (se ptum tr a nsve r sum ) 을관통하는과정에서상 호유발 ( mu t u a l induct ion ) 착오를일으켜형성된다고하 였다 ( 6, 8 ). R andin 등 ( 6) 은 5 예의보고에서 CT, 초음파등 의소견으로대개좌엽의내측절 ( m e di a l segme n t) 이정상 인반면외측절이아주커지고미상엽은다양하게나타내어 3 예에서보이지않았는데이는우측, 좌측혹은동시에양측 간문액의혈액을받기때문에커지기도하고없어지기도할 수있다고하였다. 또한담냥은좌엽내측절의뒤, 우측횡격 막의아래즉간상부 (s upr ahe pati c) 에위치하였으나이는진단에보조적인소견으로횡격막성내장전위 (11), 이소성담냥, 간경변증 (1 2 ) 등과감별하여야한다. 우측간정맥은정상에서간우엽의전후절의경계가되는데우엽발생부전시는형성되지않아보이지않는것이진단에도움이되나만약간정액의관찰이불확실할때는간의다른해부학적인구조물인간열 (he p at i c fissure), 담냥, 간원삭렬등으로진단할수있다. 또한혈관촬영술을이용하여문맥을조영하면좌측분지는잘보이나우측분지는아주작거나관찰이않되는소견을보인다고하였다. 이질환과자주동반되는질환으로서담석증 ( Fi g. 1, 5), M i rizzi 증후군, 담냥암, 총담관의섬유성협착등의담도계 b. 5 김종민외 간엽발육부전 질환, 또한문맥압항진증, 다른선천성질환, 위염전, 우측 횡격막미형성으로안한횡격막탈장, 장회전이상등이있 으며 (4, 8), 감별질환드로서간경변증, 간문부담관암, 엽 상절제후의간등이있다 (5, 13, 14). 간좌엽발생부전은우엽의경우보다는비교적흔히보 고된다고하지만역시매우드문질환으로알려져있다 ( 1 5). Belton 등 (2) 은 2 예를보고하면서 CT, 초음파의소견 으로담낭좌측에간조직이보이지않는것이주요소견이 고겸상인대 (falciform ligament) 와간원삭렬이관찰되지 않으면진단에도움이된다고했다. 감별해야할질병으로 괴사후성간경변증 (postnecrotic cirrhosis), 담도폐쇄증, 포충증 (hydatid disease) 등이있다 (2, 15). 결론적으로, 간우엽의발생부전에대한방사선학적인 소견을살펴보면우측문맥분지가보이지않는것이가장 중요하고보조적으로닫냥의우측에간조직이없고간원 삭렬이존재하며둘이하의간정맥이관찰되는것등이있 으며, 간좌엽발생부전의소견은좌측문맥분지가보이지 않고, 그외에담냥의좌측에간조직이없고간원삭렬이존 재하지않으며물이하의간정맥이관찰되는것등이있다. ;ξl I그 고 C 그 l!:" 헌 3. We report five cases (three cases of right lobar agenesis and two cases of left lobar agenesis) and discuss the radiologic findings of this congenital anomaly. Materials and Methods: Between July, 1992 and February, 1993, three cases of right lobar agenesis and two cases of left lobar agenesis of the liver were diagnosed by means of computed tomography(ct) and/or sonography. MR imging was performed in two patients, cholangiography in two, and digital subtraction angiography in one Results : The main findings of right lobar agenesis of the liver were nonvisualization of the right portal vein and absence of liver tissue to the right of gallbladder. The findings of left lobar agenesis were nonvisualization f left portal vein, absence of liver tissue to the left of the gallbladder, and absence of ligamentum teres. The ancillary finding of the lobar agenesis was visualization of less than three hepatic veins.
