2 김시형 외: 혈액투석도관 유치에 의한 의인성 상대정맥 천공의 혈관내 치료 었다(Fig. 1D). 파란색으로 표시된 유출부쪽의 정맥용 관내로 2.7-F 미세도관(Progreat Terumo, Tokyo, Japan)을 삽입 하여 종격동내의 누출로를 직경 10 mm 코일과 8 mm 코일 (Tornado, Cook, Bloomington, U.S.A.) 각각 10개를 사용하 여 색전하였다. 코일 색전 후에 시행한 상대정맥 조영술에서 대부분의 누출로는 막혔으나 천공된 상대정맥 입구부위에서 여전히 조영제의 혈관 밖 유출이 보여 0.5 ml N-butyl cyanoacrylate(histoacryl, Braun, Melsungen, Germany) 2개 를 사용하여 추가로 색전을 시행하였으며 조영제의 혈관 밖 유출이 보이지 않아서 성공적인 시술로 확인되었다(Fig. 1EG). 시술 2일 후 시행한 혈액 검사에서 헤모글로빈은 9.5 g/dl 로 증가하였으며 CT에서 색전물질이 잘 집적되어 있고 우측 흉곽 내의 혈흉과 종격동 내의 혈종이 일부 감소하였다(Fig. 1H). 환자는 시술 후 안정된 활력 징후를 보였으며 심와부 통 증도 소실되어 의뢰된 병원으로 이송되었다. 고 찰 항암화학요법이나 혈액투석, 장기간 항생제 치료, 비경구 영 양요법과 같은 만성질환의 관리및 치료법의 발달에 의해 장기 A B C D E Fig. 1. 중심정맥 도관의 설치시 도관 의 끝은 상대정맥과 우심방의 이행부에 위치시키는 것이 좋 다. 짧으면 도관의 끝이 상대정맥의 벽에 접촉하여 염증 반응 을 일으켜 혈전이나 협착 등이 생길 가능성이 커지고, 너무 길 어 우심방에 있으면 드물지만 자극에 의한 심벽의 손상과 혈 전을 일으킨다(6).

F G H 간 사용할 수 있는 정맥 경로에 대한 요구가 증가함에 따라 중심정맥 도관의 설치와 유지는 중요하며, 영상 유도하에 시 행하는 중심정맥 도관 유치술은 안전하고 실패율이 낮고 도관 을 가장 적당한 곳에 위치시킬 수 있으며 시간 및 비용 효율 이높다는 것이 입증되어 있다. 중심정맥 도관의 설치시 도관 의 끝은 상대정맥과 우심방의 이행부에 위치시키는 것이 좋 다. 짧으면 도관의 끝이 상대정맥의 벽에 접촉하여 염증 반응 을 일으켜 혈전이나 협착 등이 생길 가능성이 커지고, 너무 길 어 우심방에 있으면 드물지만 자극에 의한 심벽의 손상과 혈 전을 일으킨다(6). 터널식 중심정맥 도관의 유치를 위한 전통 적인 정맥경로는 쇄골하정맥을 주로 이용하여 왔으나, 도관이 정맥을 따라 진행할 때 제 1늑골을 지나는 부위에서 염력 (Torque)을 받게 되고 심박도에 따라 도관이 정맥과 닿는 부 위에서 기계적 자극을 정맥벽에 가해서 합병증을 일으킨다. 따 라서 최근에 외과적 방법보다 영상의학적인 시술로 터널식 중 심정맥 도관을 유치할 때 우심방까지 거의 직선으로 연결되는 우측 속목정맥을 주로 이용하며 정맥 혈전이나 협착의 빈도가 낮다고 보고된다(1, 6-8). 터널식 중심정맥 유치 중에 발생한 의인성 상대정맥 천공은 매우 드문 합병증이며, 좌측 쇄골하정맥을 이용한 경우 발생 한 상대정맥 천공에 의한 다량의 혈흉이 Florescu 등(1)에 의 해 최근 1예가 보고되었으며, Morita 등(2)에 의해 우측 속목 정맥(Internal Jugular Vein)을 통해 폐동맥 도관 삽입 중 상 대정맥 천공에 의한 다량의 혈흉이 1예 보고되었다. 그 외 의 인성 상대정맥 천공으로 보고된 예들로는 좌측 쇄골하정맥을 이용하여 상대정맥에 Trapease(Cordis, Florida, U.S.A.) 필터 를 넣은 후 발생한 심장탐폰 1예, 인접한 종격동의 악성종양 으로 눌린 상대정맥에 Wall Stent(Boston Scientifics, Calway, Ireland) 유치 후에 발생한 지연성 상대정맥 천공으로 말미암 은 심장탐폰 1예, 정맥포트의 장기간 유치로 인해 도관의 말 단부가 지속적으로 정맥벽에 자극성 미란을 가하여 발생한 지 연성 상대정맥 천공이 1예 등이다(3-5). 본 증례는 외과적인 방법에 의해 우측 쇄골하정맥의 천자를 통한 터널식 혈액투석용 중심정맥 도관의 유치 중에 발생한 의인성 천공으로 투시하에서 시술이 이루어지지 못하여 발생 하였다. 쇄골하정맥, 특히 좌측을 이용할 경우 거의 직각에 가 깝게 상대정맥으로 들어가므로 이 부위에서 확장기나 분리 제 거(Peel-away)피포가 진입할 때 저항이 발생할 수 있으며, 이때 무리한 조작시 유도철사가 꺾이면서 상대정맥 천공이 발 생할 수 있다. 그러므로 투시를 통해 유도철사를 하대정맥까 지 깊이 삽입하는 것이 유도철사의 힘이 없는 원위 부위가 꺾 이면서 확장기나 분리 제거(Peel-away)피포에 의한 상대정 맥 천공의 위험성을 줄일 수 있다. 또한, 유도철사를 하대정맥 까지 깊이 삽입하였더라도 확장기나 Peel-away 피포를 삽입 할 때 저항이 느껴지면 투시를 통해 각이 진 부위에서 유도철 사를 통해 상대정맥 하방으로 내려가는지를 확인하여야 한다. 그러나 가능하다면 우측 속목정맥를 통해 중심정맥 도관을 설 치하는 것이 상대정맥 천공을 예방하는데 더욱 좋을 것이다. 상대정맥 천공시 치료법은 대부분의 보고에 의하면 전신 마 취하에 개흉술을 시행하여 종격동 및 흉강의 혈종을 제거하 고, 동시에 도관을 뽑아내며 손상된 부위의 상대정맥을 봉합 하는 것이다. 본 증례는 환자가 다발성 림프절 전이를 지닌 위 암 환자이며 동시에 만성 신부전증에 의한 크레아티닌의 지속 49

The treatment of this complication requires an early diagnosis and open thoracic surgery. Herein, we report a patient with hemothorax due to the perforation of the superior vena cava during the placement of a tunneled hemodialysis catheter via the right subclavian vein which was successfully treated by embolization by way of a coil and histoacryl.
