1 과오종과유사한폐상피모양혈관내피종 -1 예보고 - Pulmonary Epithelioid Hemangioendothelioma Mimicking Hamartoma -A case report- Epithelioid hemangioendothelioma, originating from the vascular endothelium, is a very rare and low-grade malignancy. World-wide, about 50 cases of pulmonary epithelioid hemangioendothelioma have been reported. This is more common in female and is usually shown as multiple nodules in both lung fields. A 41-year-old male, who had suffered from right pleuritic chest pain for 3 months, was initially diagnosed as adenocarcinoma under bronchofiberscopic biopsy. At that time, the stage of tumor according to the TNM staging was IIIa. He received bronchoscopic biopsy again during follow-up period and it was diagnosed as hamartoma. 수술시야에서 흉막이 심하게 비후되어 있었 막통이 악화되고 호흡곤란이 발생하였다. 이후 흉수가 증 으며 우중엽과 우하엽은 심한 경화, 용적감소 그리고 환 가되어 진단적 흉강천자술을 시행하였고 기관지경검사를 기장애를 보였고, 특히 우중엽이 심하였다. 전 폐에 걸친 다시 시행하였다.

Also shows multiple nodules in the parenchyme of left lung. 로 보고되었다. 임상적 병기는 IIIa (T2N2M0)로 진단되었고 시사하는 것으로 수정 보고되었다. 이에 과오종에 대한 환자가 대증치료만 원하여 외래에서 경과 관찰하였다. 처 치료목적으로 우측 후측방개흉술을 통하여 폐절제술을 음 샘암종으로 진단되고 약 13개월이 경과한 후 우측 흉 시도하였다. 수술시야에서 흉막이 심하게 비후되어 있었 막통이 악화되고 호흡곤란이 발생하였다. 이후 흉수가 증 으며 우중엽과 우하엽은 심한 경화, 용적감소 그리고 환 가되어 진단적 흉강천자술을 시행하였고 기관지경검사를 기장애를 보였고, 특히 우중엽이 심하였다. 전 폐에 걸친 다시 시행하였다. 흉수의 체액분석(fluid analysis) 결과는 ph 심한 유착으로 틈새박리(fissure dissection)가 불가능하였으 7.744, 포도당 121 mg/dl, 총단백질 4.2 g/dl, 젖산탈수소 며 폐문부위의 심한 유착으로 혈관 및 기관지의 박리가 불 효소(lactate dehydrogenase) 294 U/L, adenosine deaminase 가능하여 전폐절제술을 시행하였다. 횡격막부위의 흉막 (ADA) 8 IU/L 그리고 백혈구 3,425/μL(림프구 95%)였고, 역시 심하게 비후되어 있었으며 주위의 흉벽 및 횡격막과 세포검사(cytology)결과는 class I이었다. 기관지경검사에서 박리가 불가능할 정도로 강하게 유착되어 있어서 완전절 는 처음과 같이 돌출된 종괴에 의한 우중엽기관지의 완전 제가 불가능하였다. 적출된 우폐의 기관지벽은 매우 비후 폐쇄가 관찰되어 생검을 시행하였고 과오종으로 보고되 되어 있었고 폐실질에서는 약 mm 크기의 다발성 었다. 생검조직병리검사결과가 상이하게 보고되어 이전의 결절들이 관찰되었으며 우중엽기관지의 완전폐쇄를 관찰 모든 생검조직병리표본(slide)들을 재검토하였고 과오종을 할 수 있었다(Fig. 2A). 조직학적으로 결절의 중심부는 유

조직병리검사에서 상피모양혈관내피종은 기관지주위 처럼 보였다(Fig. 2B). 종양세포는 상피모양세포로서 중등 침범과 혈관주위침범을 보이며 상엽, 중엽 그리고 하엽에 도의 비정형성을 보이며 흔하게 세포질내 공포를 가지고 모두 침범되어 있었다.

(D) The tumor cells show strong cytoplasmic immunoreactivity for anti-factor VIII related antibody (Immunohistochemical stain, 100). 리질(hyaline)이 풍부하고 세포가 거의 없는 반면, 주변부 에는 음성반응을 보였다. 광학현미경소견과 면역조직화학 는 다수의 종양세포가 연골을 닮은 점액유리질기질 검사를 통해 폐 상피모양혈관내피종으로 확진할 수 있었 (myxohyalinized matrix) 사이에 있는 양상을 보여 과오종 다. 조직병리검사에서 상피모양혈관내피종은 기관지주위 처럼 보였다(Fig. 2B). 종양세포는 상피모양세포로서 중등 침범과 혈관주위침범을 보이며 상엽, 중엽 그리고 하엽에 도의 비정형성을 보이며 흔하게 세포질내 공포를 가지고 모두 침범되어 있었다. 병변은 중간기관지까지 퍼져 있었 있어 샘암종과의 감별을 필요로 하였다(Fig. 2C). 면역조 으며 벽측흉막(parietal pleura)에도 침범이 있었고, 또한 국 직화학검사를 시행하였고 종양세포는 내피표지자(endo- 소림프절(6/12)에서도 전이된 상피모양혈관내피종이 관찰 thelial marker)인 anti-cd 31 antibody, anti-vimentin antibody, 되었다. 흉수에서는 종양세포가 관찰되지 않았다. 전신화 anti-factor VIII related antibody에 양성반응을 보였으며(Fig. 학요법을 권유하였으나 환자가 이를 거부하여 대증치료 2D) 상피표지자(epithelial marker)인 anti-cytokeratin antibody 를 하면서 외래추적관찰 중이다
