A 72- year- old man admitted with dyspnea and a mass lesion in the left lower lobe on chest roentgenogram and chest computed tomogram(ct). Large cell carcinoma of the lung was diagnosed by percutaneous needle biopsy. Also gastroduodenoscopy was conducted for evaluation of gastrointestinal symptoms, such as indigestion and epigastric discomfort. Large cell carcinoma of duodenum was diagnosed by biopsy of duodenal polyp, which was considered to be metastasized from the primary lung cancer. Palliative radiation therapy was performed, but he died 2 months after diagnosis. 반대편 폐문부와 이학적 소견 : 내원 당시 환자는 만성 병색을 보였고, 기관 분지부, 식도 주위의 종격동 림프절이 커져 있었고, 활력 징후는 혈압이 100/80 mmhg, 맥박이 108 회/분, 좌측 흉막삼출과 함께 흉막에는 결절성 병변을 보였다 호흡수는 34 회/분, 체온은 37.2 이었다. 흉부 청진상 (Figure 2).

2 g/dl, 혈소판 407,000/mm3 이었고, 생화학적 검사상 알부민이 2.7 g/dl로 감소되어 있었다. 소변 검사, 간기능 검사는 정상이었고, 대변 잠혈 반응은 양성이었다. 단순 흉부 X- 선상 좌하폐야에 경계가 불분명한 종괴 음영과 경화성 병변이 있었고, 이로 인하여 심장과 횡격막 음영의 소실되었으며, 양측 폐문부의 림프 절 종대가 관찰되었다.

2 Bo Ra Yun, et al : A case of metastatic large cell carcinoma of the duodenum from the lung 에서 치료를 받았으나, 호흡곤란이 심해지고 식욕부진 전산화단층촬영(CT )상 좌하폐엽에 약 6 5cm 크기의 및 소화 불량이 발생하여 본원으로 전원되어 입원하였 변연이 불규칙하고 분엽상을 보이는 비균질한 저밀도의 다. 과거력상 20년 전에 폐결핵으로 치료받은 병력 이외 종괴가 관찰되었고, 종괴는 폐중심부로 파급되어 하폐정 에는 특이 병력은 없었다. 맥의 침범을 보였다. 종괴의 주변부에는 석회화된 결절 가족력 : 특이 병력 없음 과 함께 지저분한 조영증강을 보였다. 반대편 폐문부와 이학적 소견 : 내원 당시 환자는 만성 병색을 보였고, 기관 분지부, 식도 주위의 종격동 림프절이 커져 있었고, 활력 징후는 혈압이 100/80 mmhg, 맥박이 108 회/분, 좌측 흉막삼출과 함께 흉막에는 결절성 병변을 보였다 호흡수는 34 회/분, 체온은 37.2 이었다. 흉부 청진상 (Figure 2). 양측 하폐야에서 악설음이 청진되었다. 복부 및 하지소 견은 특이소견이 없었다. 치료 및 경과 : 제 3병일째 환자는 소화불량과 식후 심와부의 불편감을 호소하여 위내시경 검사를 시행하였 검사 소견 : 내원 당시 말초 혈액 검사상 백혈구 으며, 검사 결과 위축성 위염과 함께 십이지장 제 2부위 18,100/ mm3, 헤모글로빈 11.2 g/dl, 혈소판 407,000/mm3 이었고, 생화학적 검사상 알부민이 2.7 g/dl로 감소되어 있었다. 소변 검사, 간기능 검사는 정상이었고, 대변 잠혈 반응은 양성이었다. 단순 흉부 X- 선상 좌하폐야에 경계가 불분명한 종괴 음영과 경화성 병변이 있었고, 이로 인하여 심장과 횡격막 음영의 소실되었으며, 양측 폐문부의 림프 절 종대가 관찰되었다. 양측 상폐야는 과거에 앓은 결핵에 의한 석회화와 섬유화로 선상음영과 결절성 병변을 보였 고, 좌측 폐야에 흉막삼출이 동반되었다(Figure 1). 흉부 Fig ure 2. T umor cells are same histologic features w ith lung tumor(h&e, 100). 에 cm 크기의 용종이 관찰되어 조직생검을 시행 뼈 등의 순이며, 본 증례와 같이 원발성 폐암이 십이지 하였다(Figure 3). 제 5병일째 흉부 단층 촬영하에 경피 장으로 전이된 경우는 매우 드문 것으로 되어 있다. 소 적 폐 침생검을 실시하였다. 폐 침생검 병리조직 소견으 장의 종양은 전체 소화기 종양의 1 5%에 불과하며1), 로 종양세포는 비교적 크고 타원형의 큰 핵과 크고 뚜렷 십이지장에서 발견되는 암종은 전이에 의한 경우보다는 한 핵소체를 가진 미분화성의 세포로써 대세포 폐암으 십이지장 자체에서 원발성으로 발생하는 경우가 대부분 로 진단하였다(Figure 4). 위내시경 검사로 시행한 십이 이다6). 또한 위장을 포함한 상부 위장관으로의 전이성 지장 용종 조직 소견으로 폐 병리조직에서 보였던 것과 악성 종양은 사후 부검 결과까지 포함하여 1 4%로 보 동일한 대세포암세포가 관찰되어(Figure 5) 원발성 대세 고되고 있다2). Menuck 등7)은 1010예의 부검 결과, 위에 포성 폐암의 십이지장 전이임을 확인할 수 있었다. 서 발견된 전이성 악성 종양의 유병율은 1.7%라고 하여, 환자는 고식적 방사선 치료를 두차례 시행하였으나 오심 및 호흡곤란, 전신 허약감 등의 증상 악화로 방사 위에서의 전이성 악성 종양 발생은 매우 드문 것으로 생 각된다. 원발성 폐암의 위장관 전이를 보고한 경우가 드물지 선 치료는 중단하였고, 보존요법만 하는 상태로 퇴원 후 않게 있다. Burbige 등8)은 147명의 폐암 환자의 부검 결 2개월만에 사망하였다. 고 과, 18명에서 위장관 전이를 관찰 할 수 있었다고 하였 찰 다. Antler 등3)은 8,243예의 부검에서 423예에서 폐암을, 원발성 폐암에 의한 전이는 국소 전이, 혈행성 전파, 이 중 58예(14%)에서 위장관 전이를 확인한 바 있으며, 림프성 전이, 드물게 기관지를 통한 전이 등이 있는데, 종 흔히 전이되는 부위로는 식도의 중간 부위(47%), 소장 양의 조직형에 따라 다를 수 있다. 특히 소세포암은 조기 (19%), 위장(9%)순 이었고, 그 외 혼합된 경우도 약 에 전신으로의 전이가 흔한 편이어서 혈관 침범을 쉽게 25%이었다고 하였다. 그러나 보고자마다 발생 부위의 일으켜 혈행성 전이를 잘 하는 것으로 알려져 있다4). 빈도는 약간의 차이가 있는 것 같다. McNeill 등9)은 폐 Ihde 등5)에 의하면 소세포암의 완치 수술 1달 후에 사망 암 환자 431예의 부검에서 46예(10.7%)가 소장으로의 한 환자의 부검 결과, 약 70%에서 전이가 존재한다고 하 전이를 가지고 있었으며, 이 중에서 6예만이 전이 증상 였다. 전이가 흔히 되는 부위로는 간, 뼈, 골수, 림프절, 을 보였다고 하였고, Ryo 등10)은 1,635명의 폐암 환자에 중추 신경계 등으로 전이되지만, 국소 혈류량이 많은 비 서 수술 및 부검을 통하여 위장관 전이는 30명(1.8%)에 장, 심장, 골격근이나 위장관은 드문 것으로 알려져 있 서 발견되었고, 이 중 소장 전이는 1.1%이었다고 보고한 다. 반면에 비소세포암의 경우는 부신, 뇌, 간, 폐, 그리고 바 있다
