1 Splenic Abscess in Gachon Medical School, Gil Medical Center Sang Tae Choi, M.D., Jung Nam Lee, M.D., Keon Kuk Kim, M.D., Min Chung, M.D. and Seung Yeon Ha, M.D. 1 Departments of Surgery and 1 Pathology, Gachon Medical School, Gil Medical Center, Incheon, Korea Purpose: A splenic abscess is a rare disease, but appears to be increasing in frequency. The key to successful treatment is early diagnosis, effective antibiotics treatment and surgical management when needed. This study was intended to clarify the clinico-pathological and radiological findings of splenic abscesses from their therapeutic aspects. Methods: Between Jan and Dec. 2003, the outcomes of 8 splenic abscess patients were retrospectively analyzed. Results: The male to female ratio was 6:2, with a mean age of 57 years, ranging from 34 to 70 years. Predisposing conditions were identified in 7, including diabetes mellitus, steroid medication, chronic alcoholism, infective endocarditis and lymphoma. As chief complaints, fever/chills was present in 2, left upper quadrant pain/tenderness in 4, diffuse abdominal pain in 1 and dyspnea in 1, with leukocytosis found in 6 (75%). Ultrasonography or computed tomography detected left pleural effusion, splenomegaly and splenic abscess in 7 (88%). The solitary to multiple ratio was 2:6. An emergency laparotomy for peritonitis detected a solitary abscess rupture in 1. The original site of infection was detected in 5, including endocarditis, dental abscess, urinary tract infection and pancreatitis in 2. Blood cultures were positive in 3 (43%), with Escherichia coli in 1 and Streptococcus viridans in 2. 3 (75%) of 4 specimens had positive culture results, including Pseudomonas, Streptoccus viridans and Enterococcus. 1 (25%) had pathogens identical to bacteria isolated form the blood culture. 1 was identified as mixed infection, with actinomycosis. A splenectomy was performed in 5, including 2 with a distal pancreatectomy, intraoperative fine needle aspiration in a lymphoma and endoscope-guided aspiration in a gastrosplenic fistula. One lymphoma patient, with multiple abscesses, died of aspiration pneumonia during chemotherapy. Conclusion: A splenic abscess is rare, and failure of early diagnose and institution of treatment is fatal. Although patients have various underlying diseases, a splenic abscess can develop arising from a dental abscess in a healthy man. Percutaneous drainage may not be suitable for multiple or hilar locations; therefore, a splenectomy, with appropriate antibiotics, is the definitive treatment. A less-invasive treatment modality can be considered, taking into account the patient s clinical situation from an immunological aspect for preservation of the spleen. 2 180 한국간담췌외과학회지 제 9 권 제 3 호 2005 ꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏꠏ 환자들을 후향적으로 연구하여 임상적인 특징, 방사선 및 병리학 소견, 치료 방법과 결과에 대해 알아보았다. 방 법 가천의대 길병원에서 1993년 1월에서부터 2003년 12월까 지 비장농양으로 진단을 받았던 8명의 환자들의 병록지와 병리조직검사를 조사하여 후향적 연구를 시행하였다. 일차 적으로 복부 초음파와 전산화단층촬영을 통하여 비장농양 이 의심되었던 경우에 임상양상과 일치여부를 알아보았다. 비장농양 진단은 추가로 혈액 배양 검사에서 양성으로 나 온 경우, 수술도중 비장조직의 괴사를 유발하는 감염증이 보이거나 비장 피막내 염증성 삼출물에서 균 배양검사 혹 은 조직 검사로 증명이 된 경우, 그리고 비장 혹은 비장내 추출물에서 조직학적으로 농양이 확인된 경우로 제한하였 다. 이들 환자의 병력과 임상적인 특징, 방사선학적 진단, 미생물학적인 검사, 치료방법과 그 결과에 대해 알아보고 관련 문헌을 고찰하였다. 결 과 8예의 평균 연령은 57세(34 70)이었고, 남녀 비는 6 2였 다. 이들에 대한 자세한 임상적인 자료들은 Table 1에서 볼 수 있다. 처음 임상증상이 발현하여 진단된 시점까지의 기 간은 평균 100일(10일 1년)이었다. 2명(25%)의 환자에서 만 발열과 오한을 주된 증상으로 호소하였고, 복통(63%)은 5명의 환자에서 호소하였는데 농양 파열로 전반적인 복통 을 호소한 1예를 제외하고 주로 좌측 상복부와 옆구리에 국한되었고, 1예에서는 흉수로 인한 호흡곤란을 호소하였 a 다. 수술 전 흉부사진상 흉수가 7예(88%), 복부 전산화단층 촬영이나 초음파에서 비장종대소견과 비장농양이 진단된 경우는 7예(88%), 이중에서 단발성은 2예이고(Fig. 1a) 다발 성은 6예였다(Fig. 1b). 혈액검사상 백혈구 증가증은 6예 (75%)에서 나타났으며 농양의 원인으로 의심되는 원발 감 염부위는 5예(63%)에서 확인되었는데 심내막염이 1예, 요 로감염이 1예 치아 농양이 1예, 췌장염이 2예였다. 7예 중 3예(43%)가 혈액에서 균이 동정되었는데 Escherichia coli가 1예, Streptococcus viridans가 2예였다. 술 후 4예 중 3예 (75%)가 농양에서 Pseudomonas, Streptoccus viridans, Enterococcus가 각각 동정되었는데 술전 혈액과 일치한 경우 는 1예(25%)였고 1예에서 Actinomycosis 감염에 보이는 황 색 과립이 보여 혼합 감염을 시사하였다(Fig. 2). 선행요인으로는 당뇨나 관절염으로 스테로이드 복용 병 력이 있는 경우가 2명, 만성 음주병력과 췌장염이 3명, 비호 지킨성 림프종이 1명, 심한 대동맥 판막 협착증이 1명이었 고 나머지 1명은 평소 건강하였고 기저 질환 병력이 없었다 (Table 1). 치료는 수술적 처치로 비장절제술을 5예에서 시행하였 는데, 2예에서는 췌장 미부가 같이 포함되어 췌미부절제술 을 같이 시행하였다. 한 예는 비호지킨성 종양으로 병기 결 정을 위해서 개복하에 조직 검사하면서 흡입 후 배액술을 시행하였다. 보존적 처치로 1예에서는 항생제만으로 호전되었 고 비장농양과 위기저부 사이에 누가 형성된 1예에서 내시 경을 통하여 흡입술을 시행하였다(Fig. 3b). 경피적 배액술 을 2예에서 시행하였는데 다발성이고 비문부에 위치하여 실패하였고 비장절제술을 추가로 시행하였다. 수술한 5예에서 병리 조직 검사 결과 비장의 최장경이 평 균 12.2 cm (10 16)였고 농양의 크기는 다발성인 경우 가 b Fig. 1. Computerized tomography shows large calcified lesion containing multiple air pocket, edematous gastric wall thickening and gastrosplenic fistula tract (a), Gastrofibroscopy shows draining pus through gastrosplenic fistula in thickened fundic fold (b). 류의 감염이 49 68%에 해당한다고 하였다.1,10,11,14-16 본 연 구에서도 위와 같이 8예 중 3예(38%)가 혈관을 통한 직접적 인 전파를 보였다. 심내막염은 비장 농양의 10 20%를 나 10,15-18 타내는 가장 빈번한 감염의 경로이다. 인접 감염원으로부터의 직접 전파되는 경우는 6 15%를 2,10,15 신장주위 농양, 크론씨 대장염, 대장암, 천 차지하는데, 공성 위암, 괴사성 췌장염, 게실염, 그리고 간주위 횡경막하 14,21-23 본 연구에서도 췌장염으로 인해 비장 종양 등이 있다. 내 췌장 미부에 농양이 있었던 경우가 2예(25%)있어 췌미 부 절제술을 같이 시행하였다. 외상은 이전에는 비장농양 의 가장 흔한 원인이었지만 수술 술기의 발달로 최근에는 1 드문 것으로 되었다. 겸상적혈구증이나 백혈병 같은 혈액 질환에서도 비장농양이 보고되는데 비장 내 미세경색을 유 발한 후 비장구조가 파괴되어 발생하며 비장동맥 색전술 같은 방사선학적 중재적 시술 후에도 유사 기전으로 발생 12,17 할 수 있다. 최근 면역결핍증도 비장 농양의 주요인으로 대두되고 있 는데, 장기이식, 약물치료, 스테로이드 투여와 후천성 면역 10,15 그 결핍증 환자가 증가함에 따라 더욱 중요해지고 있다. 러나 특정한 기저 질환이 없는 건강한 환자에서도 발생할 수 있는데 본 연구에서도 평소 아주 건강하고 치아 농양만
