An IMT cn occur in vriety of loctions, including the lung, orit, protid, pleur nd stomch. However, they hve rrely een encountered in the liver. An IMT of the liver most often presents in young dults. The clinicl presenttions of heptic IMT vry: symtomtic, fever, dominl pin, plple mss, vomiting, ostructive jundice, nemi nd heptosplenomegly. Their rdiogrphic ppernce nd clinicl presenttion mke them extremely difficult to distinguish from mlignnt neoplsm, preopertively. Therefore, surgicl mngement is used when mlignncy cn not e excluded preopertively or when the IMT produces iliry ostruction. We encountered 31-yer-old mle ptient who is still live for 38 months following resection for symtomtic IMT. 이러한 병리학적 소견으로 염증성 근육 섬유 모세포 종의 원인으로 과거에는 간문맥을 통한 상행감염성에 대한 의심 3,5,8 대부분 보고에서 미생물은 발견되 이 제기되어왔으나, 지 않았다. 간세포암과 달리 염증성 근육 모세포 종은 조직 학적으로 간문맥 침윤이 발견되지 않아 감별진단하는 데 4 도움이 되기도 한다.

(d) CD34 stining revels high vsculrity of this tumor ( 100). 구별이 가능하다.4 조직학적으로는 형질구 증식, 다양한 수 의 호중구와 림프구, 육아성 반응과 관련된 방추형 세포와 섬유지질로 구성되어 있으며, 초자화 된 교원질에 나선형 4,5,8 으로 염증성 세포가 침윤되어 있는 소견이 특징적이다. 이러한 병리학적 소견으로 염증성 근육 섬유 모세포 종의 원인으로 과거에는 간문맥을 통한 상행감염성에 대한 의심 3,5,8 대부분 보고에서 미생물은 발견되 이 제기되어왔으나, 지 않았다. 간세포암과 달리 염증성 근육 모세포 종은 조직 학적으로 간문맥 침윤이 발견되지 않아 감별진단하는 데 4 도움이 되기도 한다. 치료방법은 수술적 절제와 항생제와 전신성 스테로이드 제제를 이용한 보존적 방법을 병행하면 6,9 10 서 추적 관찰하기도 한다. Okmoto 등 은 보존적 방법으 로 치료 후 62명 중 14명 (22.6%)의 환자에서 염증성 근육 모세포 종이 소실되는 것을 보고하면서, 정확한 방사선학 적 방법이나 생검 등으로 정확한 진단이 이루어진다면 대 부분의 환자들이 단순 관찰이나 보존적 방법으로 치료될 6,9 11 수 있다고 주장하였다. 그러나, Kneko 등 은 종양이 간 문부를 침윤하여 폐색성황달이 생긴 환자에서 33개월 동안 의 경피적 담즙 배액술 후 종양의 변화가 없어서 수술적으 로 절제를 시행한 환자를 보고하였다. 대부분의 경우 술전 감별진단이 명확하지 않아서 수술적 절제를 시행하는 경우 가 많다. 염증성 근육 모세포 종은 병리학적으로 양성 종양 으로 수술적 절제 후 재발의 빈도가 낮으며, 자연적으로 소 실되거나 보존적 방법으로 치료되기도 하여 간의 악성 종 6 12 양에 비해 좋은 예후를 보인다. 그러나 Coffin 등 은 종양 이 절제된 49명의 환자중 12명(24%)의 환자에서 재발하였 으며 종양이 크거나 종양이 절제하기 힘든 곳에 위치해 있 을 때 더 자주 재발한다고 보고하였다. 본 연구에서는 부분 간 절제술 후 전신적 증상과 빈혈이 소실되고, 38개월간 재 발의 증후는 없이 추적 관찰 중인 31세 남자의 간에서 발생 한 염증성 근육 모세포 종의 치료 1예를 보고하는 바이다.
