Its etiology is unknown and its clinical outcome is unpredictable. There is no generally accepted therapeutic strategy because of its rarity and the variable natural course between hemangioma and angiosarcoma. We report a case of a 64-year old woman who underwent hepatic resection due to epithelioid hemangioendothelioma in the right lobe that progressed to extrahepatic metastases of the bone, pleura, and peritoneum 22 months later. However, after resection there was no primary hepatic recurrence. 종양의위치는대부분에있어서다발성으로간의양엽을모두침범하며진단시폐, 복막, 임파선그리고뼈가가장흔한간외전이병소이다. 2 병의진행은다양한임상경과를보이며, 빈도가낮고다양한성격을띠는종양의특성상아직확립된치료는없다.

(D) Peripheral enhancement was washed out in the delayed phase. 며, 간에서의발생률은 100,000명당 0.1명미만이다. 4 간상피모양혈관내피종은원인이뚜렷이밝혀져있지않으며, 성인여성에서호발하고, 가장흔한임상증상은우상복부통증, 간비대그리고체중감소이다. 종양의위치는대부분에있어서다발성으로간의양엽을모두침범하며진단시폐, 복막, 임파선그리고뼈가가장흔한간외전이병소이다. 2 병의진행은다양한임상경과를보이며, 빈도가낮고다양한성격을띠는종양의특성상아직확립된치료는없다. 국내문헌에서도간상피모양혈관내피종이몇차례발표되긴했지만, 5-8 간절제술후원발병소인간에재발없이간외전이를보인 경우는아직까지보고된바없다. 이에저자들은간절제술후간내재발없이, 간외전이를보인간상피모양혈관내피종 1예를경험하여이를보고하고자한다. 사례 64세여자가 7년전발견된간종괴로추적관찰해오다가최근들어크기가증가하여내원하였다. 최근 6개월간 5 kg의체중감소가있었으며, 가끔씩쥐어짜는듯한복부통증이반복되었다. 과거력상특이소견은없었으며가족력상동생이폐암이었다. 내원시활력징후는혈압이 110/70 mmhg, 맥

박은분당 78회, 호흡은분당 20회, 체온은 36.8 로안정적이었다. 공막에황달소견이나촉지되는경부림프절은없었다. 흉부청진에서심음및폐음은정상이었으며, 복부검사상압통은없었으며, 촉지되는종괴나장기도없었다. 말초혈액검사에서백혈구 7,200/µL, 혈색소 11.9 g/dl, 혈소판 404,000/µL였다.

1 g/dl, alkaline phosphatase (ALP) 70 U/L, gamma glutamyltranspeptidase (GGT) 37.9 U/L였다. 간염바이러스표지자는 HBsAg 양성, anti-hbs 음성, anti-hcv 음성이었다. 암표지자검사에서 alpha-fetoprotein 0.3 ng/ml, PIVKA-II 11.

(D) Epithelioid hemangioendothelioma involving the parietal pleura of the chest wall (H&E stain, 100). 박은분당 78회, 호흡은분당 20회, 체온은 36.8 로안정적이었다. 공막에황달소견이나촉지되는경부림프절은없었다. 흉부청진에서심음및폐음은정상이었으며, 복부검사상압통은없었으며, 촉지되는종괴나장기도없었다. 말초혈액검사에서백혈구 7,200/µL, 혈색소 11.9 g/dl, 혈소판 404,000/µL였다. 생화학검사에서 aspartate aminotransferase (AST) 24 U/L, alanine aminotransferase (ALT) 23 U/L, 총빌리루빈 0.47 mg/dl, 총단백 6.8 g/dl, 알부민 4.1 g/dl, alkaline phosphatase (ALP) 70 U/L, gamma glutamyltranspeptidase (GGT) 37.9 U/L였다. 간염바이러스표지자는 HBsAg 양성, anti-hbs 음성, anti-hcv 음성이었다. 암표지자검사에서 alpha-fetoprotein 0.3 ng/ml, PIVKA-II 11.0 mau/ml, carcinoembryonic antigen 2.68 ng/ml, carbohydrate antigen (CA) U/mL였다. 흉부X선소견은정상이었다. 복부전산화단층촬영 (computed tomography, CT) 에서간의 7번분절에 cm 크기의난원형의저음영종괴가관찰되었으며, 종괴내부에석회화가관찰되었다 ( 그림 1). 폐CT에서는다수의육아종이관찰되었고, 골주사에서골전이소견은없었다. 대장내시경소견에서특이소견없었으며, PET-CT (positron emission tomography-ct) 에서는간종괴외에다른전이소견은관찰되지않았다. 담관암의심하에간우엽절제술을시행하였으며, 수술소견상종괴는경계가비교적분명하였으나횡경막과유착되어있어서종괴를박리하였다. 크기는 cm으로엷은갈색을띠었으며, 내부

However, there was no intrahepatic recurrence after hepatic resection. 에는노란빛을띠는엷은갈색의석회화가관찰되었다 ( 그림 2A). 현미경검사상종괴는침윤성성장을보이며경계가명확하지않았다. 종양의중심부에심한유리질화, 석회화와괴사가관찰되었으며그주변부에는섬유화가관찰되었다.

이와같은세포질내공포 (vacuole) 는 alcian blue와 PAS 염색에음성을보였다. 가지돌기세포는상피모양세포보다덜관찰되었으며세포질내공포도거의관찰할수없었다. 유사분열은 거의관찰되지않았으며핵다형성은심하지않았다. 림프관이나혈관내로의침윤은보이지않았다.

However, there was no intrahepatic recurrence after hepatic resection. 에는노란빛을띠는엷은갈색의석회화가관찰되었다 ( 그림 2A). 현미경검사상종괴는침윤성성장을보이며경계가명확하지않았다. 종양의중심부에심한유리질화, 석회화와괴사가관찰되었으며그주변부에는섬유화가관찰되었다. 조직소견상종양세포는상피모양혹은가지돌기 (dendrite) 모양이었으며, 상피모양세포는호산성세포질과거품핵 (vesicular nucleus) 을보이며핵소체는뚜렷하지않았다. 반지세포 (signet ring cell) 구조를보이는종양세포는세포질내에투명공간을가지고있었다 ( 그림 2B). 이와같은세포질내공포 (vacuole) 는 alcian blue와 PAS 염색에음성을보였다. 가지돌기세포는상피모양세포보다덜관찰되었으며세포질내공포도거의관찰할수없었다. 유사분열은 거의관찰되지않았으며핵다형성은심하지않았다. 림프관이나혈관내로의침윤은보이지않았다. 면역조직화학염색에서세가지혈관내피세포표지자중 CD31( 그림 2C) 과 Factor VIII 관련항원에양성을, CD34 에음성을보였다. 또한 vimentin, actin, S-100에양성을보였으며, 감별진단을위해시행한 pancytokeratin (CKMNF116), epithelial membrane antigen, alpha-fetoprotein, hepatocyte specific antigen, thyroid transcription factor-1, alpha-1-antitrypsin, HMB45, chromogranin에는음성을보였다. 환자는수술후외래에서추적관찰하던중, 수술후 22개월째에우측둔부통증을호소하였고, 이에시행한 PET-CT와골주사에서우측장골에전이

5 정승원외 8 인. 간내재발없이간외전이를일으킨간상피모양혈관내피종 소견이관찰되었으며 ( 그림 3A), 1개월뒤에는요추 3번과 4번에도전이를보였다 ( 그림 3B, 3C). 환자는 2주에걸쳐서우측장골에총 30 gray의방사선치료를받았으며, 요추 3번과 4번에도방사선치료를시행받았다. 방사선치료후환자는증상의완화를보였고 3개월뒤시행한 PET-CT상우측장골골전이병변의호전이관찰되었다. 그러나골전이 4개월뒤호흡곤란을주소로내원하였고, 검사상우측흉수소견과함께폐CT에서우측흉강의다발성결절을동반한전이소견이의심되었으며, 복부 CT상에서도복강내림프절전이소견이의심되었으나, 원발병소인간내재발소견은관찰되지않았다 ( 그림 3C). 환자는우측흉수의원인규명을위해서흉막유착술및조직검사를시행하였고, 조직검사에서상피모양혈관내피종의흉막전이소견이관찰되었다 ( 그림 2D). 복수검사에서도뚜렷한핵을가진비전형세포들과함께혈복강소견을보였다. 환자는보존적치료를받던중수술후 31개월째호흡부전으로사망하였다. 고찰간상피모양혈관내피종은진단시영상소견상전이성간암이나담관암종등으로오인되기쉽다. 본증례의경우도수술전영상소견상담관암종의심하에, 조직검사없이바로수술을시행하였으나, 수술후조직검사에서간상피모양혈관내피종으로진단되었다. 본환자의경우 B형및 C형간염바이러스항원이모두음성이었고, 암표지자검사에서 alpha- fetoprotein, PIVKA-II, CEA, CA19-9 등도모두정상소견이었으며, 폐CT, 골주사, 대장내시경, PET-CT 등에서도특별한소견을보이지않아간세포암종, 전이성간암, 담관암종등을의심할만한소견은없었다. 그러나 CT 및 MRI에서문맥기와지연기에서종괴변연부의조영증강및소실이관찰되어, 담관암종의심하에수술을시행하였고, 최종적으로는간상피모양혈관내피종으로진단된경우이다. 306명의간상피모양혈관내피종환자들을대상으 로조사한연구에서종괴의간내위치를보면, 다발형 (multifocal) 이 87.3%, 단일형 (unifocal) 이 12.7% 였으며, 다발형 (multifocal) 이면서양엽을모두침범한경우가전체의 81% 로가장많았다. 그리고다발형 (multifocal) 이면서우엽이나좌엽만침범한경우가각각 5.3%, 1% 였고, 본증례의경우처럼단일형이면서우엽만침범한경우는전체의 8.2% 였다. 2 간상피모양혈관내피종은초기에는결절형병변을보이나종양이진행되어크기가커지면병변이융합되고종종혈관침범을동반하는괴상형 (massive or diffuse) 병변으로발전한다. 9 또한많은경우에서병변이간실질의주변부에존재하며캡슐까지인접하는경우가많다. 본증례의경우도 cm 크기의큰종괴로서간실질의주변부에위치했고피막까지인접해있었다. 간상피모양혈관내피종은초음파소견상대부분의경우주위실질과비교해서저음영을나타내며, 종양내부에석회화를동반하기도한다. CT 소견에서는대부분변연부조영증강과종괴의내부가저음영을보이며, MRI상에서는주로 T1강조영상 (T1-weighted image) 에서저음영을보이고 T2강조영상 (T2-weighted image) 에서고음영을보인다. 2 본증례에서도초음파에서저음영의종괴로내부에석회화를동반하였으며, CT에서는저음영소견을보였고 MRI상에서는 T1강조영상에서저음영을, T2강조영상에서고음영을보였다. 간상피모양혈관내피종은조직학적으로특징적인상피모양, 가지돌기모양, 혹은중간모양의세포로구성되어있으며, 전자현미경검사에서세포질내봉입체 (intracytoplasmic inclusions) 를보이는혈관내피세포기원의성장패턴을보인다. 종양의혈관학적특성으로인해 Factor-VIII 관련항원과 CD31 및 CD34 에대한면역화학염색에서양성을보이면확진에도움이된다. 또한전자현미경검사에서구조적으로잘발달된고유판 (lamina propria), 포음소포 (pinocytotic vesicle) 그리고 Weibel- Palade bodies가관찰될수있다. 10 본증례의경우비록 CD34 면역화학염색에는음성을보였으나 CD31 과 Factor VIII 관련항원에는양성을보여상피모

Mehrabi 등이 286명을대상으로치료방법을조사한결과, 간이식이 128명 (44.8%) 으로가장높았으며, 치료없이경과를관찰한경우가 71명 (24.8%), 항암치료또는방사선치료가 60명 (21%), 간절제가 27명 (9.4%) 이었다. 치료성적은 1년과 5년생존율에있어서간절제가 100% 와 75%, 간이식이 96% 와 54.5%, 항암치료또는방사선치료가 73.3% 와 30% 그리고치료받지않은경우가 39.3% 와 4.5% 였다. 2 치료방법에있어서는이론적으로는간절제가치료를위한첫번째선택이지만, 실제대개의경우에병변이다발성이고, 종양의위치상수술이용이하지않은경우가많다. 또한고식적절제는종양의특성상절제후간증식성장인자에의해서종양세포가증식하는경향이있어서추천되지않고있다. 11,12 현재까지발표된증례가많지않으므로아직까지정해진치료지침은없지만, Mehrabi 등에의하면양엽을산발적으로침범하고간외전이가없는경우간이식을시행하고, 간외전이가있다면간이식시행또는간이식후항암화학요법또는간동맥항암색전술이나항암화학요법, 방사선치료등을시행하며, 한개의엽만침범한경우에는간외전이가없다면간절제술을시행하고간외전이가있다면양엽을침범한경우처럼간이식치료또는간이식과항암화학요법의혼합치료또는간동맥항암색전술이나항암화학요법, 방사선치료등을시행한다. 2 또한진단시에간외전이가있는경우, 간이식을시행하더라도간외전이가없는경우와비교하여생존에영향을미치지않으므로수술의금기증이아니라는보고들도있어서, 1,13,14 간외전이를동반한경우에도간이식을시행하고있으며, 종양이광범위하게간을침범한경우와급속히진행되는간부전의경우간이식이유일한치료법이다. 15 전이시에항암치료, 간동맥항암화학색전술, 방사선치료등을시행해볼수도있으나, 이들의효과는제한적이며특히간병변에대한일차치료로는적합하지않다. 10,11 간상피모양혈관내피종의예후에관한국내연구를살펴보면, 김등 7 이보고한 7예의 치료법에서, 1예는간좌엽절제술을시행하였고, 1 예는 1회의항암화학요법을시행하였으며, 나머지 5예는특별한치료없이경과관찰하였다. 그리고평균 24.5개월의추적기간동안병의진행은없었다. 9 본증례의경우수술후 22개월과 23개월째에원발병소인간의재발없이우측장골과요추 3번과 4번의전이소견을보였다. 대개뼈에일차적으로발생한상피모양혈관내피종의경우절제술이일차치료이지만, 수술이불가능한경우나다발성골전이가된경우에는일반적으로고식적방사선치료를시행한다. 16 간상피모양혈관내피종의골전이에대해서항암화학요법의시도가있었으나효과를보지못했다. 17,18 본환자의경우우측장골과요추 3번과 4번의골전이병변에대해서방사선치료를시행하고호전을보였다. 그러나수술후 26 개월째호흡곤란을동반한우측흉수가발견되었고, 조직검사상에서흉막의전이소견이관찰되었다. 복수및복부CT상에서는복강의전이소견도관찰되었다. 환자는보존적치료를시행받던중수술후 31개월째사망하였다. Lauffer 등 10 의보고에의하면간상피모양혈관내피종으로진단받은 100명의환자들의 5년생존율은무치료군을포함하여 55.5% 였다. 그러나간상피모양혈관내피종의다양한임상경과와드문발생빈도로인해아직까지정확한예후와치료방침은없는실정이다. 향후축적된전체환자들의자료정리와분석을통해서종양의자연경과와치료방침이정해질것으로생각된다. 요약간상피모양혈관내피종은혈관에서기원하는종양으로간에서발생하는가장드문육종중의하나로서저등도에서중등도의악성도를가지고있다. 아직까지원인은알려져있지않으며예후또한예측하기어렵다. 발생빈도가낮고, 불균일한특성, 다양한임상경과등으로현재까지정해져있는치료지침은없다. 저자들은 64세여자환자에서우엽에발생한간상피모양혈관내피종을간절제술을시
