The purpose of this study is to investigate the clinical features and treatment outcomes of mucosal malignant melanoma of the nasal cavity and paranasal sinuses. Subjects and Method:Eighteen patients who were diagnosed and treated as mucosal malignant melanoma of nasal cavity and paranasal sinuses from 1995 to 2004 were included. Male to female ratio was 10 to 8. Their ages ranged from 43 to 87 years with median of 62 years. The medical records were reviewed retrospectively with respect to presenting symptoms, stage, treatment modalities and outcome. Result: The most common symptoms were epistaxis and nasal obstruction. The mean duration of symptom was 4.2 months. Commonly involved sites were middle turbinate, inferior turbinate, maxillary sinus and ethmoid sinus. Eight cases belonged to stage I, nine cases to stage II and only one case to stage III. Among them, seventeen patients underwent surgery with or without radiotherapy and immunotherapy. Of the seventeen patients who underwent surgery, twelve had recurrence and the mean period for recurrence was 8.2 months. During the follow-up periods, five had distant metastases. Five-year survival rate was 27.2% and the survival rates according to age, sex, tumor size and stage were not statistically significant. Conclusion:Mucosal malignant melanomas of the nasal cavity and paranasal sinuses developed commonly in the 6th to 8th decades. Epistaxis and nasal obstruction were the most common presenting symptoms. Almost all cases belonged to the stage I or II and the mainstay of the treatment was surgery. However, it was associated with frequent recurrences and a poor prognosis. 2 홍성룡외 tutional review board) 의심의를통과하여, 1995년 1월부터 2004년 12월까지본병원에서비강및부비동의점막형악성흑색종으로치료받은 18명의환자를대상으로의무기록을후향적으로분석하였다. 남자는 10명, 여자는 8 명이었으며, 연령범위는 43세에서 87세로중위연령은 62 세였고, 이중 15명 (83.3%) 의환자가 50대에서 70대에해당하였다. 경과추적기간은 4개월부터 89개월로평균 32 개월이었다. 연령, 성별, 초기증상, 병기, 수술방법등에따라생존율을분석하여임상적으로의미있는자료를얻고자하였다. 통계적분석은 SPSS V12.0(SPSS Inc., Chicago, IL) 을이용하여, Kaplan-Meier 방법으로생존율을구하였고, Log-Rank 법으로유의성을분석하였으며, p-value가 0.05 이하인경우에통계학적으로유의하다고판단하였다. 결 과 환자들이호소하는초기증상으로는비출혈및비폐색이가장많았다. 그외에비루및비내동통, 협부종창등이있었다 (Table 1). 증상의발생부터진단까지는평균 4.2 개월이소요되었다. 종양의침범부위에대한조사는전비경검사및비내시경소견을기본으로하였으며, 부비동전산화단층촬영및부비 Table 1. Involved sites at presentation Involved sites (%) Middle turbinate 9 (50.0%) Inferior turbinate 8 (44.4%) Maxillary sinus 8 (44.4%) Ethmoid sinus 8 (44.4%) Septum 6 (33.3%) Nasal floor 4 (22.2%) Skull base 3 (16.7%) Orbit 2 (11.1%) Vestibule Eustachian tube area Superior turbinate The numbers are not mutually exclusive 동자기공명영상소견을반영하였다. 이에따른침범부위는, 중비갑개가 9예로가장많았으며, 하비갑개, 상악동, 사골동이각각 8예씩이었다. 그외에도비중격이 6예, 비강저가 4 예, 두개저가 3예, 안와가 2예에서침범되었다 (Table 2). 종양의장경은 1 cm에서 6 cm로평균 3.14 cm였다. 최대크기가 3 cm보다큰경우는 7예였고, 3 cm보다작은경우는 11예였다. 2003년에 Thompson 등 3) 이제안한비강및부비동의점막형악성흑색종의병기분류에따르면, 1기에는 8예, 2기에는 9예, 3기에는 1예가해당하였으며 4기에해당하는환자는없었다 (Table 3). 초치료로, 총 18예중 17예에서수술을시행하였다. 이중 2예에서는수술후 interferon-α 300만단위를 6회투여받는면역치료를시행하였고, 1예는수술후방사선치료를시행하였다. 또한 1예에서는수술후면역치료와방사선치료를추가로시행하였다 (Table 4). 수술방법은내측상악절제술이 5예, 두개안면절제술, degloving 접근법에의한광범위절제술및내시경을이용한광범위절제술이각각 Table 3. 3 비부비동의점막형악성흑색종 3예씩이었다. 그외상악전절제술 2예및부분적비절제술 1예가포함되어있었다. 두개안면절제술을받은 1예는양측경부림프절전이가있는 3기로양측경부림프절청소술을함께시행받았다 (Table 5). 수술을거부한 1예에서는면역치료만을시행하였으나, 종양은치료에대해반응하지않았다. 수술을시행한 17예중 12예에서재발하였다. 9예에서는국소재발을하였고, 2예에서는경부림프절전이에의한재발을하였으며, 1예에서는대뇌로의원격전이가발견되었다. 초치료의종료부터재발까지는평균 8.2 개월이소요되었다. 국소재발을한 9예중 6예에서구제수술을시행하였으며, 이중 3예에서다시국소재발을하였고 1예에서경부림프절전이에의한재발을하였으며 1예에서는요추전이가발견되었다. 다시국소재발한 3예중 2예에서구제수술을시행하였으며, 이중 1예에서국소재발을하였으며 1예에서소뇌전 Table 6. Five-year survival rate was 27.16%. 이가발견되었다. 또한경부림프절전이가발견된 1예에서구제수술을시행하였으나이후폐전이가발견되었다. 한편, 초치료후경부림프절전이가발견되었던 2예중 1예에서구제수술을시행하였으나이후장골전이가발견되었다. 전체적으로, 추적관찰중, 18예중 5예에서원격전이가발견되었으며, 초치료의종료부터원격전이의진단까지는평균 32.8 개월이소요되었다. 이들의 5년생존율은 27.16% 이었다 (Fig. 1). 병기에따른 5년생존율은 1기가 50%, 2기가 18.52% 였으나, 통계적으로유의하지않았다 (Table 6). 또한 60세이후에진단된환자에서의 5년생존율은 40% 인반면, 60세이전에진단된환자에서는 0% 였으나, 통계적으로유의하지않았다. 한편성별에따른 5년생존율도, 남자 20%, 여자 43.75% 로, 통계적으로유의하지않았다. 종양의장경에따른 5년생존율도, 3 cm 미만인경우 25%, 3 cm 이상인경우 25.71% 로, 통계적차이가없었다. 고찰 점막형악성흑색종이피부형악성흑색종에비해예후가불량한이유로는, 첫째, 점막형이잠행성으로발생하고, 둘째, 세포분열의빈도가높아급속히성장하며, 셋째, 혈관및림프계의침습빈도가높고, 넷째, 증상이비특이적이어서조기발견이어려우며, 다섯째, 침윤정도를정확히결정하기어렵고, 여섯째, 도약병변 (skip lesion) 이다수존재하는경우가많아수술적치료시전체를다제거하지못할가능성이있다는점등을들수있다. 5) 비강및부비동의점막형악성흑색종은대부분 50세이후에발생하며, 남자에서여자보다약간더호발하는것으로알려져, 본연구의결과와큰차이가없었다. 6-10) 임상증상도, 본연구에서와마찬가지로, 비폐색및비출혈이가장흔하며그외에비루, 두통, 후비루등의비특이적인증상을보일수있는것으로알려져있다. 9-11) 점막형악성흑색종에있어서, 병변의범위와침윤정도를평가하기위한영상진단수단으로자기공명영상이가장선호된다. 비강및부비동의점막형악성흑색종은, T2 강조영상에서는저신호강도를보이며, T1 강조영상에서는등신호강도를보이고, Gadolium 으로조영한 T1 강조영상에서는고신호강도를보인다. 한편, 호발부위도비중격, 비강측벽, 중비갑개, 하비갑개, 상악동, 사골동순으로알려져있어, 본연구의결과와크게다르지않았다. 6,7,10,11) 병리조직학적으로점막형악성흑색종은원형또는난원형의핵및크고뚜렷한핵소체를가지고있고, 핵대세포 Korean J Otolaryngol 2006;49:

4 홍성룡외 질의비율이높으며, 유사분열지수 (mitotic index) 가높은것이특징이다. 종양세포내의멜라닌색소를증명하면확진할수있으나, 최근에는면역조직화학염색을이용하여확진한다. 이중 S-100 단백은멜라닌세포의분화표지자로, 흑색종에매우민감한것으로알려져있으며, HMB-45 는흑색종관련항원 (melanoma associated antigen) 을인지하는표지자로, 흑색종에매우특이적인것으로알려져있다. 12,13) 그외에도 melan-a, tyrosinase 등의표지자가사용되고있다. 13) 이러한면역형광염색을이용하면멜라닌색소가없고다른미분화암종들의병리소견과유사한무멜라닌성흑색종의진단에서도오류를범하지않을수있다. 11,12) 피부형악성흑색종에서와달리, 점막형악성흑색종에서는국소병변인경우를 1기, 국소림프절전이가있는경우를 2기, 원격전이가있는경우를 3기로분류하는 Ballantyne 의병기분류를일반적으로사용하고있다. 14) 본연구의대상환자군에서도위의병기분류를이용한다면, 1기에 17예, 2 기에 1예가해당된다. 그러나본연구에서는, 이병기분류를좀더세분하기위해, 국소병변을원발부위의종양이침범한해부학적부위의숫자에따라하나일때와두개이상일때로재분류하여각각 1기와 2기로정의한 Thompson 등 3) 의병기분류를사용하였다. 치료에는수술적치료, 방사선요법, 항암화학요법이있으나, 근치적수술요법이가장우선적으로고려된다. 원발병소의광범위한절제가다른치료요법들과비교해국소재발, 국소림프절전이, 그리고생존율에서좋은성적을보이기때문이다. 11) 그러나비강및부비동의악성흑색종은국소림프절전이및원격전이의빈도가낮으므로, 림프절전이가없으면예방적경부곽청술은실시하지않는다. 방사선치료는수술후종양이남아있을가능성이있을경우, 국소절제가불가능한경우, 재발한경우에고식적인요법의의미로사용되고있으나, 술후보조방사선요법이국소조절을증가시키고무병생존율을증가시킨다고하여충분한안전역확보유무와관계없이실시되어야한다는주장도있다. 10,15,16) 한편항암화학요법은원격전이가있거나재발한경우에사용될수있으며, decarbazine 을가장많이사용한다. 최근에는 BCG, 인터페론등의면역요법도사용되나국소재발및국소림프절전이, 생존율에있어서도움이되지는못하고있다. 본연구에서도면역요법을 2예에서시행하였으나, 효과에대해서는더연구가필요하다. 치료실패의주원인은경부림프절전이보다국소재발이다. 16) 비강및부비동의점막형악성흑색종은, 해부학적구조상진단이늦어원발암이진행된경우가많고, 도약병변이많아종양을완전절제하는것이어렵기때문인것으 로생각된다. 17) 본연구에서도치료를마친 17예중 12예에서재발을하였으며, 이중 9예는국소재발이었다. 그러나국소림프절전이에의한재발도, 초치료후 2예에서발생하였으며, 국소재발에대한구제수술후에도 1예에서발생하였다. 비강및부비동의점막형악성흑색종에있어서, 성별은예후에영향을주지않는다고생각되고있으나, 연령에대해서는다소논란이있다. 연령은예후에영향을주지않는다는보고와, 10) 60 세이후에발생한경우에 60 세이전에발생한경우보다예후가나쁘다는상반된보고가있다. 3,18) 한편 Thompson 등 3) 은종양의크기가클수록예후가나쁘다고보고하였다. 본연구에서는성별, 연령, 종양의크기모두예후와관련이없었다. 5년생존율이 1기에서는 75%, 2기에서는 62.5%, 3기와 4기에서는 0% 로전체 5년생존율이 42.6% 라는보고가있다. 3) 그러나다른보고에서는 5년생존율을 28% 라고하여, 본연구의결과와비슷하였다. 19,20) 결 론 비강및부비동에발생하는점막형악성흑색종은대개 50대에서 70대에발생하며, 비출혈및비폐색을주증상으로하는경우가많다. 대개 1기또는 2기에해당하며, 면역치료및방사선치료도시도되고있지만현재까지는수술이치료의주를이루고있다. 그러나이종양은재발을잘하며예후는극히불량하다. 따라서, 비출혈및비폐색이주증상인경우철저한비내시경검사를통해조기진단을하고조기에광범위절제수술을하는것이국소재발과치사율을감소시키는데에도움이될것으로생각된다. 중심단어 : 악성흑색종 비강 부비동.
