1 대한구강내과학회지 Vol. 34, No. 1, 2009 구강편평태선환자에서발생한암종의증례보고및문헌고찰 원광대학교치과대학구강내과학교실 임현대 이유미 편평태선은작고편평한다각형의구진을피부나점막에보이는만성염증성질환으로편평태선을유발하는원인은명확하지는않으나, 피부나점막에서의항원변화가세포매개성면역반응을유도하여발생하는것으로생각되어진다. 구강편평태선은초기에는양성병소로생각되었으나세계보건기구에의해전암병소로분류되었다. 구강편평태선이악성화되는단계는명확하게밝혀지지않았으며, 편평태선등의만성염증성질환은구강암발생에있어일반적인위험요인 ( 예 : 흡연, 음주 ) 과관계없이악성발현을보인다. 구강편평태선의악성전이는여러문헌에서논란을보이나, 0.5% 에서 5% 까지보고되었다. 구강편평태선의악성전이는특정임상적양상, 과각화나미란성에서더높은비율로발생한다. 구강편평태선의악성전이는예후가불량하므로예후의개선과조기진단을위하여임상가는일년에최소일회에서 2 회까지추적조사를해야한다. 본증례보고에서는중년의여성에게서십수년간지속된구강편평태선에서발생한편평상피암종을살펴봄으로써, 임상가가유념해야할구강편평태선의악성변이에대한고찰및진단시와추적조사시에고려사항을고찰하자고한다. 주제어 : 구강편평태선, 악성전이, 편평상피암, 추적조사 1)Ⅰ. 서 편평태선은작고편평한다각형의구진을피부나점막에보이는만성염증성질환이다. 구진은붉은빛을띠는자주색이고구진표면에격자모양또는환상모양의회백색선조가존재하는데이를 Wickham's striae 라부른다. 주로호발부위는신체의접히는부분예를들어손목사타구니같은부분이며보통대칭으로발생한다. 편평태선은구강점막에때때로침범하며피부병소단독으로혹은구강점막단독으로발생하기도하며피부병소와구강점막병소가같이발생하기도한다. 교신저자 : 이유미전라북도익산시신용동 원광대학교치과대학구강내과학교실전화 : Fax: marusae@wonkwang.ac.kr 원고접수일 : 원고수정일 : 심사완료일 : * 이논문은 2008 년도원광대학교교비지원에의해수행됨. 론 편평태선을유발하는원인은명확하게알려지지않았으나면역학적기전에서기인하는것으로제시되고있으며, 피부나점막에서의항원변화가세포매개성면역반응을유도하여발생하는것으로생각되어진다. 이런항원은약물, 아말감수복물 1) 과세균성이나바이러스성감염등이며면역억제제를포한한다. 원발쓸개관간경화증, 만성활동간염, 궤양대장염, 중증근육무력증, 가슴샘종과관련되어발생하는편평태선유발이이러한자가면역질환적인병인론을암시하고있다. 편평태선은모든종족과모든지역에서발생하며여성이남성보다많이발생하고주로 30 대에서 60 대의호발연령을지닌다. 2) 구강점막에서의임상적발현은매우다양구진형, 망상형, 위막형, 위축형, 미란형그리고수포형으로발생하며협점막이가장호발하는부위이다. 3-5) 구강편평태선의악성으로전환가능성은여러문헌에서논란중이며, 1869 년에 Wilson 이처음보고한이후초기에는양성병소로생각되었으나세계보건기구에의해전암병소로분류되었다. 구강편평태선의악성으로전환은문헌에따라 0.5% 에서 5% 6) 까지보고되었고편평태선이악성화되는단계는명확하게밝혀지지않았다. 49

2 임현대 이유미 편평태선의악성전이에대해서는여러의견이분분하며이는구강편평태선, 편평양이형성그리고진성악성종양사이에존재하는병리학적진단적차이에있어편평태선과관련되어발생한편평상피암과단독으로발생한편평상피암사이에명확하게존재하지않는것도한가지이유이다. 7) 편평태선을포함한만성염증성질환은구강암발생에있어흡연이나음주같은일반적인위험요인과관계없이악성변이를보인다. 편평태선은다른암성변이에서보여주는것처럼활성화된염증세포가 cytokine network 를통해발암요인으로작용하는것과유사하게구강상피형성이상과다른만성염증이암발생과관련이있는것으로기대된다. 편평태선에서편평상피암의발생에대한보고가이루어져있기는하나십여년간치료에반응하지않는편평태선병소가지속되었고바로인접부위에서편평상피암이발생하였기에편평태선의치료에있어악성전이의평가에도움이되고자본증례를보고하는바이다. 1. 주소및병력 Ⅱ. 증례 40 세의여자환자가구취를주소로 1994 년 7 월 13 일처음내원하였으며, 약 3 년전에도비슷한증상을보였으나자연소실된병력이있다고하였다. 병소의발병전에장기간의스트레스가지속되었고음식물섭취시에통증과잇솔질시쉽게출혈이된다고하였으며양측협점막에붉은점막과회백색표면을보였다. 99 년 2 월 1 일치은과협점막에과민성을주소로재내원하였고뜨거운것과매운것에양측구치부와협점막에민감함을호소하였다. 치은에서쉽게출혈소견을보였고양측협점막에미란성, 홍반성과과각화성을보였다. 그후십수년간증상에정도의차이는존재하였지만회백색의심한부육성괴사성병소가궤양과함께지속되었다. 생리시나육체적피로, 정신적스트레스시에증상이심해지는경향을보였고협점막의반흔조직으로인한경직으로인해개구량이 25 mm 내외로유지되었다. 칫솔의접근이어렵고병소상태가청결유지가어려워양측구치부구강내위생이불량하였다 (Fig. 1) 년좌측상악결절에지름 1cm 정도두께 1~2 mm 정도의분홍빛의외방성병소를 2 주전발견하였 다고내원하였으며촉진시쉽게출혈소견을보였다. 2. 치료및경과 predinisolone 투여와스테로이드국소도포시행주관적증상감소하였으나육안적소견은계속존재하고증상은경감하나병소는소실되지않고유지하였고병소에서출혈소견계속존재하였다. 내원시에 Co2 레이저도포와증상의심화정도에따라 predinisolone 을투여내지는 steroid gaggling 을 2000 년까지실시하였다. 그후장기간의스테로이드복용에따른부작용과환자의복용에대한소극적대응으로구강내위생을위하여과산화수소수와생리식염수로구강내세척을주기적으로실시하였고병소는여전히지속되었다 년 7 월좌측하악제 2 대구치협측에통증을동반한부종으로절개및배농을실시하였고 2003 년개구량개선을목적으로양측협점막에 Z-plasty 실시하였으며술후 35 mm 까지개선되었으나그후개구량은 25 mm 내외로유지되었다 2007 년 colchine 을처방하였으나오랜투약으로인해약복용을규칙적으로하지않았으며 3 개월간부정기적으로복용후중단하였고병소는여전히지속되었다. 구강위생개선을위하여구강세정기사용을권하여사용하였다 년 11 월 20 일좌측상악결절에지름 1 cm 두께 1~2 mm 출혈이쉽게되는분홍색의외방성병소가생겨서내원하였으며생검을위하여구강외과로의뢰하였고생검결과 well to moderately -differentiated squamous cell carcinoma 로판명되었고 Whole body scan 에서원거리전이는보이지않았다. 술전에실시한협점막부위생검결과는편평태선으로판명되었고 2008 년 12 월 18 일편평상피세포암종제거를위한수술과방사선요법을실시하였다. 3. 검사소견및진단 biopsy : Squamous cell carcinoma Lt maxillary tuberosity well to moderately-differentiated biopsy : lichen planus(fig.2) Lt. upper second molar vestibular area Diffeuse band-like lymphocytic infiltration of subepitherial stroma and ulcerative epithelium noted. 50

H & E (x 400) Keratin pearl with squamous tumor cells Ⅲ. 총괄및고찰 구강백반증, 구강점막하섬유증, 홍색판이주요구강편평상피암의구강전암병소로고려되며, 유전자이상을지닌이수성홍색판증과백색판증은다수의종양발생과높은사망률을보인다. 8-10) 이러한만성염증을지닌영역의아래집단인편평태선은다른외부적위험요소예를들어흡연이나음주와무관하게악정전이의잠재적위험에의평가를포함하여야한다. 편평태선은만성염증성질환으로악성으로의변이는 0.5~5% 정도를보이며특히중년의여성에게서악성변이는추적조사를고려해야한다. 본증례는약십수년간의지속된난치성편평태선병소를지닌환자에게서구강편평상피암이발생하여임상적의의를지닌증례라하겠다. 환자는편평태선에대한투약에도불구하고십수년간증상의악화와완화를반복하여경험하였고비록완화된경우에도부육성의괴사된조직을지니고있었으며작은자극예를들어칫속질에도쉽게출혈성향을보이고있었다. 환자는 51

4 임현대 이유미 양측협점막의반흔조직으로인하여조직의탄력성을소실하여개구제한을지니고있었고이에칫솔의접근이용이하지않아양측구치부의퇴축된치은에과도한치태침착을보였고이런구강위생불량이병소의진행에어떠한영향을미쳤는지는생각해볼문제이다. 이러한편평태선의악성변이에대한연구는 1910 년 Hallopeau 가기술한이후로여러문헌에서보고되었으나 Krutchkoff 와동료들은 1950 년에서 1976 년동안의구강편평태선에서발생한악성종양을보고한 223 증례중단지 15 증례 (7%) 만충분한기준을갖추었다고하였다. 악성변이를인정하지못하는대부분의증례들은초기구강편평태선의진단이정확하지않았고일부에서는편평태선병소부위와떨어진부위에서구강암이발생되었었다. 11,12) Van der Meij EH 등은 1977 년부터 1999 년사이보고된 98 증례중 33 증례만받아들일수있다고하였다. 13) Van der Meij EH 등은수정된세계보건기구의구강편평태선 (OLP,oral lichen plannus) 및구강태선양반응 (OLL, oral lichenoid lesion) 진단기준에따른 173 명 ( 남자 60 명, 여자 113 명, OLP 의추적조사결과 ( 추적조사기간 6.6 에서 72 개월평균 31.9 개월 ), 1.7%( 남 2 명여 1 명 ) 에서구강점막의편평세포암종이발견되었다고보고하였다. 7,14) 이와같이악성변이에관한여러사람들의다양한견해는구강편평태선에관한정확한진단기준을다시명확하게할필요성과유전학적기전에대한연구의필요성을말해주고있다. 편평태선의악성전이에대한 C 형간염바이러스의역활, 이형접합의상실, p53 체계의변화기타여러가지의유전적변성에대해서연구가활발히이루어지고있다. 16,17) p53 tumor suppressor 는많은종양의발현에중요한역할을하며구강편편태선에서의악성발현에도역시작용하는것으로보인다. Candida albicans 도또한 carcinogen N-nitrosobenzylmethylamine 형성을촉진하는것으로가설되어졌다 ) 편평태선과관련되어발생한편평상피세포암은편평상피암단독발생하는것보다암발생의원발부위에서재발이더많았으며 6) 구강편평태선을가진여자환자의 50% 에서두경부에서의암발생이대조군에비하여 50% 높다고하였다. Mignogna 등은구강편평태선에서발생한편평상피세포암 21 증례에서악성변이의위험도는편평상피세포암단독으로발생한경우보다높았고또한구강암의높은이차또는삼차 전이와경부임파절전이로인하여불량한예후를지닌다는것을발표하였다. 15) 이런문헌고찰에서살펴보듯이악성변이가비록높은비율은아니더라도편평태선을지닌환자에게는연 2 회이상의추적조사가필요하며특히위축형, 미란형, 궤양형환자에서는정기적추적조사가중요하며, 임상가는악성전이의가능성을최소 6 년간추적조사할것이추천된다. 22,23) 편평태선의악성전이의진단은초기의임상및조직학적진단의정확성을기초로하여임상경과에따라주기적추적조사를통하여평가할수있다. 비록편편태선의악성전이가 0.5~5% 의낮은비율을보이지만편평태선을대하는임상가는악성전이를염두에두지않을수없다. 본증례에서와같이중년여성에서발병한편평태선에서악성전이의비율이상대적으로높을것을염두에두고임상가는주기적평가를통해악성전이를고려해야만한다. 참고문헌 1. 6 임현대 이유미 - ABSTRACT - Case Report of Squamous Cell Carcinoma arising in an Oral Lichen Planus and Literature Investigation Hyun-Dae Lim, D.D.S M.S.D., You-Mee Lee, D.D.S,Ph.D. Department of Oral medicine, School of Dentistry, Wonkwang University Lichen planus is a relatively common chronic inflammatory disease involving the skin and mucous membranes showing small flat polygonal papules. The accurate etiology is unknown but it's suggested that cell-mediated immune response to an induced antigenic changes in skin or mucosa. Oral lichen planus was regarded as an benign lesion but oral lichen planus was classified as premalignant lesion by WHO criteria. It was not known that progress of malignat transmmission in the the patient with oral lichen planus, and chronic inflammatory disease including oral lichen planus showed malignacy in oral cancer unrelated common risk factors(ex: tabacco, alcohol). Although malignant development in the patient with oral liche planus was various greatly in the literature, from 0.5% upward to 5%. It has been reported that a specific clinical type of oral lichen planus, hyperkeratotic or erosive had a higher chance of transformation into an squamous carcinoma. Clinician has to follow-up check of at least one or two visit per year to detect of malignancy of oral lichen planus and improved prognosis with squamous cell carcinoma. At this case with the middle aged women with squamous cell carcinoma developed from oral lichen planus of more than a decade of persisting, we try to discuss the malignacy of oral lichen planus and cosideration with follow-up. 김범수 Analysis of Outcomes after Resection of Sarcomatous Hepatocellular Carcinoma Purpose: Sarcomatous hepatocellular carcinoma (HCC) is rare. Therefore, the clinicopathologic characteristics and prognosis
