Recently, newer physiologic functions for vitamin D have been identified. Several studies suggest vitamin D plays a vital and complex role in immune system function and regulation. Asthma is one of the most common chronic diseases worldwide and has been increasing in prevalence over the last decades. Common risk factors for both asthma and vitamin D deficiency, such as an urbanized, westernized lifestyle and obesity have led to a hypothesized link between asthma and vitamin D. The purpose of this review is to present the evidence for a role of vitamin D in asthma. D의결핍이천식발생에영향을 천식은소아에서가장흔한만성알레르기호흡기질환 으로전세계적으로빈도가증가하고있다. 1) 천식환자의증 가에대해서는생후초기에미생물노출이이후면역형성 에영향을준다는위생가설등이제안되고있으나 2) 위생가 설의경우에비만이천식환자에미치는영향과미생물에 노출되기쉬운저소득수준도시가정에서증가된천식유 병률을설명하는데에는부족하였다. 따라서이러한현상을 설명할수있는기전을찾기위한노력이있어왔으며이러 한노력과정중천식유병률이적도에서멀리위치한국가 에서높게나타나고 3) 최근 30년간 D 결핍이증가하 는것을관찰할수있었다. 4) 또한비만에서 D의결 핍이증가하고 5) 도시거주자에서 D의결핍이증가 접수 : 2012 년 9월 17 일, 승인 : 2012 년 9월 19일책임저자 : 이주석, 경상남도창원시마산회원구합성동 50 성균관대학교의과대학삼성창원병원소아청소년과 Tel : 055) Fax : 055) ljs @lycos.co.kr D 대사와생리작용 인체내에서 D는대부분을인체내에서생합성을 통해서이루어지며초기 D 전구물질인 7-dehydro- cholesterol 이피부내에서 ultra- violet B의영향을받아 cholecalciferol 로바뀌게된다. 이후 cholecalciferol 은간 에서 25-hydroxy vitamin D3 [25(OH)D] 로바뀌게되고 25(OH)D 는칼슘 -인평형을조절하는부갑상선호르몬의 영향을받아콩팥에서 1.25-dihydroxy vitamin D3 [1.25 (OH)D] 로바뀌게된다. 그러나 25(OH)D 의일부는콩팥 이아닌다른장소에서 hydroxylation 이일어나며이러한 과정을통해여러가지면역작용에관계하게된다. 7)

2 Ju Suk Lee : Vitamin D and Asthma D 결핍증의역학 D 의피부내생합성은피부의멜라닌양, 나이, 인체내지방, 위도, 계절, 야외활동시간, 선크림의사용 유무등에의해영향을받게된다. 8) 인체내 D는대 부분피부내생합성을통해이루어지지만일부는곡류, 유 제품, 생선등의섭취를통해얻어지기도한다. D의 결핍, 부족혈중농도는 25(OH)D 농도를기준으로 <20 ng/ml (50 nmol/l), ng/ml (50 70 nmol/l) 로 정의된다. 9) 이농도는골격의형성, 유지에대한효과를기 준으로정해졌으며현재 D의면역계통의효과를포 함하는비골격계영향을고려한부족, 결핍농도는정해지지 않았다. D 의하루요구량은보고마다차이가있지만, 미국 Institute of Medicine s Committee 에서는 1세이상 소아에서 600 IU를권장하고있고 10) 미국소아과학회에서 는출생후부터청소년시기까지 400 IU를권장하고있 다. 11) 성인을대상으로한국내의한보고에서 D의 부족은 48.2%, 결핍은 7.9% 로국내에서도부족, 결 핍이많이있으리라추론된다. 12,13) D와인체면역과의관계 D와감염에대항하는숙주면역에주요한역할을 하는것은널리알려져있으며 D의면역조절작용은 대식세포, 수지상세포등을포함하는각세포에서혈중에 서세포내로이동한 25(OH)D 의 hydroxylation 을통해 이루어진다. 14) 세포내에서활성화된 1.25(OH)D 는세포 내의 D 수용체인 vitamin D receptor (VDR) 과결 합하여선천면역, 후천면역에관계하게된다. 선천면역의 경우 antimicrobial peptide 의형성을유도하는것으로알 려져있는데, 세균감염시대식세포내에서 25(OH)D 가 1.25(OH)D 로바뀌게되고활성화된 1.25(OH)D 는 anti- microbial peptide 가 coding 된세포내의유전자를활성화 시켜결과적으로 cathelicidin 을생성하게한다. 7,15) 또한비 타민 D는 Junction 유전자를활성화시켜인체내생체장 벽을강화시킨다. 16) D는 T 세포의활성화과정에직 접영향을미쳐후천면역반응을조절하기도하고 antigen presenting cell 의기능에영향을미쳐후천면역반응을조 절한다. 실험실연구에서 D는 CD4+T 세포와 interleukin (IL)-2, granulocyte macrophage colony stimulating factor, interferon- gamma 와같은 Th1 사이토카인을 억제하는것으로알려져있으나 17,18) Th2 사이토카인에대 한역할은일정치않다. D가 T 세포에서 IL-4 의생 성을억제하는것으로알려져있고, VDR 결핍생쥐의경우 IL-13, immunoglobulin E (IgE) 의증가가보고되고있 다. 19,20) D의면역조절작용은 IL-10 분비조절 T 세포의분화유도를통해이루어진다. 21) 또한 D는 Th2 면역반응과관계되는수지상세포의 Ox40L 를억제 함으로써 transforming growth factor- β 양성조절 T 세 포의생성을증가시킨다. 22) D와천식과의관계 D와천식과의연관성에있어서많은연구가있어 왔으나천식과 D의연관성에대해서는일관된결과 가나오지않았다. 이에대한이유로는각연구의연구설계, sample 크기, D 분석방법의차이에의한다고추론 된다. 핀란드성인을대상으로했던단면연구에서는신생아 시기의 D 투여가이후천식발현을높인다고보고 하였으나신생아시기의 D의측정이이루어지지않 았고 23) 미국흑인을대상으로한연구에서는 D의부 족이천식과유의한상관관계를보고하였다. 24) 비록추적 관찰기간이짧기는하지만출생코호트연구를살펴보면 임신시산모의 D 섭취가이후출생아의천식빈도 를감소시키는것을관찰할수있었다. 25) 그러나영국소아 를대상으로실시한연구에서는임신후반기산모의높은 혈중 D 농도가 9세때출생아의천식빈도를증가시 킨다고보고하였으나 70% 의출생아가추적관찰동안소실 되었다. 26) 그외대부분의출생코호트연구들에서는임신 시산모의 D 부족상태가출생아의천식빈도를증 가시킨다고보고하고있으나 % 까지상당수의출 생아가추적관찰기간동안소실되었다. 25,27-30) 이상의연 구결과로아직 D와천식발생과의관계가명확하지 는않지만적어도임신시산모의 D 상태가이후출 생아의천식발현과연관있다고추정된다. D는천식의발생뿐아니라천식의상태악화와 연관성이있다고보고되고있다. Costa Rica 에서천식을 가진 616명소아를대상으로한연구에서혈중 D 농도가낮을수록총 IgE, 호산구수, 병원입원빈도, 항염 증약물사용빈도, 기관지과민성이증가한다고보고하였고 31) 1,024 명의경증혹은중등증지속성천식소아를대상

3 이주석 : D 와천식 으로한북미연구에서도 D 농도가낮을수록응급실 방문빈도가증가한다고보고하였다. 32) 이직접적으 로천식발작의예방효과가있는지를알아보는임상시험 에서도 D (500 IU/d) 를고식적천식치료와같이투 여하였던군에서천식발작빈도가감소한다고보고되었 다. 33) 이탈리아소아천식환자를대상으로한연구에서비 타민 D의혈중농도와운동유발성천식의빈도사이에음 의상관관계가있음이보고되어 D가부족한군에서 적절한 D의투여는운동유발성천식의빈도를줄일 수있을것으로생각된다. 34) 천식이환율에서 D의역할 혈중 D 농도의증가는바이러스감염의감소, 스 테로이드반응성의증가, 아토피의감소, 폐형성및발달의 촉진을통하여천식이환율을감소시키는것으로추론된다. D 는대식세포의활성화, antimicrobial peptide 의증가등을통하여바이러스감염으로부터기도를보호하 는역할을한다. 167명의일본학동기소아를대상으로한 연구에서겨울기간동안 D (1,200 IU/d) 를투여한 경우에 influenza A의감염빈도가감소하였다고보고하였 다. 35) D 농도가낮으면천식발생과관련있다고알 려진 respiratory syncytial virus 감염의빈도와감염중 증도가증가한다는보고가있어 36) D는천식환자의 천식증상발현뿐만아니라천식발생에도어느정도관여 할수있으리라추론된다. IL-10 은조절 T 세포에서생성되는사이토카인으로 Th1, Th2 면역반응을억제한다고알려져있다. 스테로이 드는조절 T 세포의 IL-10 분비를향상시켜천식과같은 Th2 면역반응을억제시키는것으로알려져있는데 D도조절 T 세포의 IL-10 분비를향상시키는것으로알려 져있으며스테로이드와 D를같이투여하는경우에 는각각투여하는경우에비해조절 T 세포에서 IL- 10 분비 가보다더증가하는것으로알려져있다. 37) D는스 테로이드투여후조절 T 세포에서 IL-10 의분비를증가시 키지못하는스테로이드저항성천식환자에서도스테로이 드와 D를같이투여하는경우에조절 T 세포에서의 미있는 IL-10 의분비증가를야기할수있다고보고되고 있다. 38) D의스테로이드반응성에대한역할은스테 로이드저항성유무에관계없이일어난다고생각된다. D 의감소는천식뿐아니라비염, 아토피역시감 소시키는것으로알려져있어 25,32) D의감소가천식 외다른알레르기질환의발현을촉진하고이후이러한알 레르기환자에서천식의발생을추론해볼수있다. D 가감소하는경우에성인폐기능의감소를관찰할수있 었으며 39) 동물실험에서 D가결핍된모체에서태어 난경우에페기능감소와페조직의변화가관찰되어 40) 비 타민 D 는페의형성, 발달에주요한역할을한다고추론된 다. 2형당뇨환자에서 D의결핍이관찰되고 41) 혈중 D의감소와체지방증가의연관성이보고되고있는 데 42) 이는 D의결핍이체중증가를담당하는뇌하수 체를자극하여비만이발생한다는제안이있으나 43) 아직까 지정확한기전은밝혀져있지않다. 천식과비만과의관계 를고려해볼때 D의투여가천식환자의체중감소 를통하여천식조절에영향을미칠수있으리라짐작된다. D의세포내작용은 VDR 을통해서이루어지는 데 VDR 유전자의변이와천식과의관계가보고되고있어 천식과 D 결핍을설명할수있는공통유전자가있 으리라추론된다. 44) 천식과 결 론 D의관계에대해최근많은보고가이루 어지고있고현재로서는 D의조절 T 세포에의한작 용으로천식발생, 천식증상악화를설명하고있다. 특히 비만이동반된조절되지않는천식환자에서 D의결 핍을고려해볼수있으리라생각된다. 참고문헌 1.
