1 Journal of the Korea Academia-Industrial cooperation Society Vol. 17, No. 12 pp , ISSN / eissn 이종열, 유선희 * 건국대학교생물공학과 A study on how lifestyle and shampoo habits affect middle-aged men with seborrheic Scalp especially office workers Jong-Youl Lee, Seon-Hee You * Department of Bioengineering, Konkuk University 요약본연구는지루성두피염발생빈도가높은것으로알려진 30~50대남성 150명을대상으로생활습관과샴푸습관이지루성두피에어떠한영향을미치는지알아보고자하였다. 그결과지루성두피의주요원인과관련이있다고알려진두피가려움증과과다유분, 두피뾰루지, 두피붉은증상에대하여샴푸시기와잦은피로감은모든증상에서통계적으로유의한영향을미치는것으로조사되었으며 (p<0.01), 샴푸시기, 잦은피로감, 규칙적인출 퇴근, 스트레스민감, 헤어스타일제품사용유 무와같은생활과샴푸습관은지루성두피에영향을미치는것으로확인되었다. 연구결과를통해사무직군무종사자들의생활과샴푸습관이지루성두피의발생원인과통계적으로유의한것으로확인되었으며, 특히샴푸습관과샴푸의불완전한세정및모발건조에의한습진이지루성두피발생의일차적인원인이되고, 불규칙적인출 퇴근과잦은피로감, 만성스트레스가지루성두피의이차적악화원인이되고있음을알수있었다. Abstract This study surveyed 150 men aged 30 to 50, a demographic which seborrheic dermatitis is common. The time of day one shampoos and recurrent fatigue were found to have a statistically significant impact on symptoms contributing to the main causes of seborrhea, including itchiness, excessive oil, acne, and redness on the scalp (p<0.01). Study results confirm lifestyle and shampoo habits, such as the frequent sense of fatigue, a routine work schedule, susceptibility to stress, the time of day one uses shampoo, and whether hair styling products are used, affects seborrheic scalp disease. This study found seborrheic scalp disease for office workers primarily resulted from shampoo habits, rashes were predominantly caused by a failure to rinse shampoo and dry hair completely, while secondary aggravating factors included irregular work schedules, recurrent tiredness, and chronic stress. Keywords : Hair, Life Styles, Seborrheic Scalp, Habits, Total Analysis 1. 서론 자신을돋보이는목적으로이용하기도한다 [1]. 하지만잦은화학제품의사용과잘못된생활습관등으로인하현대인들의생활수준과미적가치에대한관심도가크여두피의생리적기능이많이손상되며, 두피지루성피게증가하면서자신의이미지개선을위해끊임없이변부염, 지루성탈모증의원인이되기도한다. 화를시도하고있다. 특히모발의변화는이미지개선에두피의지루성피부염은비듬과가려움증을동반하사용되는손쉬운방법중의하나로새치, 백모등의모발며, 피지선의기능이증가되는부위에많이발생된다. 그의색을변화시켜자신의외모에대한결점을감추고원인은확실하게규명되지않았지만주로피지선의활성 * Corresponding Author : Seon-Hee You (Konkuk Univ.) 2 한국산학기술학회논문지제 17 권제 12 호, 2016 이증가하여과다한피지분비, 피부에상재하는호지성인말라세지아글로보사 (Malassezia globosa, M. globosa) 이스트의과다증식, 세포매개성면역이상등이주요한것으로알려져있다 [2,3]. 특히말라세지아레스트릭타 (Malassezia restricta) 와 M. globosa가지루성피부염에발병한피부에서제일많이발견되며 [4], M. globosa 이스트가분비하는리파아제로 [5], 피지구성물인트리글리세라이드와에스테르를분해하여 [6], 피부자극원이되는리놀산, 올레산이만들어진다. 이지방산이각질을투과하는정도는개인별로감수성이있어서발병여부가결정되며, 피부장벽파괴, 지방산의투과, 염증유발이지루성피부염의주요원인기작으로보고되고있다 [7]. 지루성두피염의개선을위하여주로사용되는레티노이드 (retinoid) 는비타민 A의천연또는합성유도체로서생체내활성형태인레티노인산 (retinoic acid) 으로변화하여, 다양한작용으로여러피부질환에사용되고있지만, 레티노이드피부염 (retinoid dermatitis) 이라고불리는홍반, 인설, 건조, 작열감, 소양감등이부작용을나타내는일종의자극성접촉피부염이나타나는경우가있다 [8]. 이러한부작용을개선하기위하여지루성두피의염증개선효과 [9], 천연추출물을활용한두피개선효과 [10,11,12,13,14] 등최근천연물질을활용한지루성두피개선연구가보고되고있다. 또한지루성두피염은생활습관중스트레스를많이받고, 수면시간이 6시간이하에서발병률이더욱심해지는것으로보고되고있으며 [15], 이민호 [16] 의두피의효율적방안연구에의하면자신의두피에맞는샴푸제를선택하고, 올바른샴푸방법인식을습득하여 [17], 잘못된샴푸습관을교정해야한다고보고되고있다 [18]. 따라서본연구에서는두피모발의문제성질환중지루성두피염발생빈도가높은것으로알려진 30~50대중장년층의남성 [19,20] 중한정된실내공간에서비슷한생활을하는사무직군무종사자들을대상으로두피가려움증과과다유분, 두피뾰루지, 두피붉은증상이발생하는지루성두피에생활습관과샴푸습관이어떠한영향을미치는지알아보고자하였다. 샴푸습관을대표하는변수로는총 3개의독립변수를상정하였으며, 생활습관총 7개의독립변수, 종속변수총 5개로선정하였다. 2. 연구방법 2.2 설문조사 연구대상및기간본연구는두피염발생빈도가높은 30~50대남성중한정된실내공간에서비슷한생활을하는사무직군무종사자들의생활과샴푸습관이지루성두피에어떠한영향을미치는지에대한관계를알아보고자충남 A 부대에근무하는남성 150명을대상으로 2015년 11월 23일부터 11월 30일까지설문조사를실시하였다. 조사방법은조사대상자가직접설문지를작성하는자기기입식설문지법을사용하였으며, 총 150부를최종분석자료로사용하였다 설문연구내용본연구는지루성두피유형이나타나는원인을파악하기위하여, 일반적인특성 4문항, 두피와관련한문항 14문항, 생활습관 15문항, 샴푸습관 9문항으로총 42문항을통해조사하였으며, 수집한 150명에대한설문내용을바탕으로지루성두피에생활습관과샴푸습관이어떠한영향을미치는지에대하여확인하고자하였다. 2.3 통계분석방법본연구는다음의분석프로세스를통해 SPSS 20.0(Armonk, NY, IBM corporation) 으로실시하였다. 첫째, 빈도분석 (frequency analysis) 을통해조사대상의인구통계학적변인과샴푸습관대한현황을파악하였다. 둘째, 본연구에서고려된두피상태및생활습관에대한기술통계량 (descriptive statistics) 을도출하였다. 셋째, 지루성두피에영향을주는변수로샴푸습관과생활습관관련 9개의변수를독립변수 (independent variable) 로정의하고지루성두피에어떠한영향을주는지검증하기위해다중회귀분석 (multiple regression) 을실시하였다. 지루성두피에결정적인영향을미치는생활습관과샴푸습관이무엇인지규명하였다. 3. 연구결과및고찰 3.1 연구대상자의일반적특성본연구결과조사대상자의직업은 군인 150명 702

3 (100%) 으로, 근무기간이 16~20 년이내 44 명 (29.3%), 21~25년이내 41명 (27.3%), 11~5년이내 37명 (24.7%), 6~10년이내 19명 (12.7%), 26년이상 9명 (6.0%) 순으로조사되었으며, 연령은 30세 ~34세 19명 (12.7%), 35세 ~39세 57명 (38.0%), 40세 ~44세 65명 (43.3%), 45세 ~49세 9명 (6.0%) 으로조사되었다. 조사대상자의학력은 대졸 80 명 (53.3%), 석사 65 명 (43.3%), 박사 5명 (3.3%) 으로확인되었으며, 결혼여부는 미혼 7명 (4.7%), 기혼 143명 (95.3%) 으로대부분이기혼인것으로나타났다. 월소득은 400~500만원미만 51명 (34.0%) 로가장많이나타났으며, 그다음으로 500~600만원미만 41명 (27.3%), 300~400만원미만 37명 (24.7%), 200~300만원미만 19명 (12.7%), 600만원이상 2명 (1.3%) 순으로조사되었다 (Table 1). General characteristics of respondents Item n % Job Soldier years years Service years Period years more than 26 years Age 30 ~ 34 years ~ 39 years ~ 44 years ~ 49 years Edu cation Graduation from University Graduation from Master Graduation from Doctorate Marital Status Monthly income Single Married < 200 ~ 300 million won < 300 ~ 400 million won < 400 ~ 500 million won < 500 ~ 600 million won million won < 샴푸습관에관한빈도분석본연구에서고려된사무직군무종사자들의평소샴푸습관에대한특성파악하고자빈도분석 (frequency analysis) 을통하여 Table 2와같이살펴보았다. 조사대상자들의샴푸하는시기의경우, 아침 과 수시로 가각 65 명 (43.4%) 로가장많이나타났고, 저녁 20 명 (13.3%) 순으로나타났다. 샴푸횟수 의경우, 2 회 75 명 (50.0%), 1회 55명 (36.7%), 3회 15명 (10.0%), 하지않는다 5명 (3.3%) 순으로나타났다. 샴푸소요시간은 3 분 55 명 (36.7%), 2 분 50 명 (33.3%), 1 분 35 명 (23.3%), 4 분 과 5 분 각 5 명 (3.3%) 순으로나타났다. 샴푸종류의경우모발상태와관계없이 정상모발용 40명 (26.7%), 손상모발용 35명 (23.3%), 지성용, 기타 각 25명 (16.7%), 비누 20명 (13.3%), 중건성용 5 명 (3.3%) 순으로사용하는것으로확인되었다. 샴푸하는물의온도의경우, 따뜻한물 75명 (50.0%), 미지근한물 50명 (36.7%), 찬물 20명 (13.3%) 로나타났다. 샴푸하는방법으로는 두피를손가락지문을이용해마사지하듯이문지른다 50명 (33.3%), 두피와모발을함께문지른다 45명 (30.0%), 두피를손끝으로문질러깨끗이씻어낸다 35명 (23.3%), 두피를문지르지않고모발을집중적으로문지른다 20명 (13.3%) 순으로나타났다. 헹굼방법의경우 샤워기를걸고양손으로헹굼 70명 (46.7%), 오른손에샤워기를잡고왼손으로헹굼 50 명 (33.3%), 왼손에샤워기를잡고오른손으로헹굼 23 명 (16.7%), 바가지에물을떠서부우면서한손으로헹굼 5명 (3.3%) 순으로나타났다. 샴푸후건조방법으로는 타올로말린다 65명 (43.3%), 타올과드라이기를병행한다 35명 (23.3%), 온풍드라이기 30명 (20.0%), 그리고 냉풍드라이 와 선풍기로말린다 각 10명 (6.7%) 로나타났다. 건조정도는 적당히말린다 75명 (50.0%) 와 완전히말린다 55 명 (36.7%) 로조사되었다. Research results of scalp condition factors n = 150) Scalp condition M SD Scalp urtication Whether or not the scalp is oily Do you get rashes on your scalp often Do you have a lot of dandruff Is your scalp clean Does your scalp have symptoms of red Do you feel scalp heat Scalp is hard rather than soft Do you spend time for your scalp health Do you get treatment from a professional shop for your scalp health Do you buy and use specific products for your scalp health Do you massage your scalp Do you feel the need for a professional management of scalp health Have allergic reactions to the scalp 두피상태와생활습관에대한조사본연구에서고려된두피상태와생활습관의기술통계량 (descriptive statistics) 을 Table 3과 Table 4와같이살펴보았다. 본연구에적용된기술통계량들은위치측도 (local measurement) 인평균 (mean), 퍼짐의측도 (dispersion measurement) 인표준편차 (standard deviation) 에대해서살펴보았고, 정규분포로부터왜곡된정도를나타나는왜도 (skewness), 정규분포의평균을중심으로얼마나뾰족한봉우리를보이는가를첨도 (kurtosis) 로확인하였다. 두피상태변인에대한조사결과조사대상자들의두피관리에대한전문적인관리가필요하다고느끼고있으며 (M=3.67, SD=1.20), 두피에유분량 (M=3.20, SD=1.14) 과뾰루지 (M=3.00, SD=1.19) 가자주생기는편으로확인되었다. 생활습관에대한변인조사결과규칙적인식사 (M=4.00, SD=0.97) 를하는것으로조사되었으며, 피로를자주느끼며 (M=3.83, SD=0.90), 잦은샤워 (M=3.67, SD=0.91), 컴퓨터사용으로인한목과어깨뭉침 (M=3.63, SD=1.17), 주로야근을한다 (M=3.43, SD=1.02) 의순으로확인되었다. 5 성두피상태를종속변수로가정한다음회귀분석을실시한결과, 모형은유의수준 0.01에서유의하였다 (p<0.01). Table 5와같이샴푸시기와 (p<0.01), 잦은피로감 (p<0.01) 은지루성두피에유의수준 0.01에서통계적으로유의한영향을미치는것으로확인되었으며, 규칙적인출 퇴근 (p<0.05), 스트레스민감 (p<0.05), 헤어스타일제품사용유 무 (p<0.05) 는두피붉은증상에유의수준 0.05에서통계적으로유의한영향을미치는것으로확인되었다. 이와같은결과를통해샴푸시기, 잦은피로감, 규칙적인출 퇴근, 스트레스민감, 헤어스타일제품사용유 무와같은생활과샴푸습관은지루성두피에영향을미치는것으로확인됨에따라본연구의가설이검증되었다. 4. 결론본연구는스트레스, 피로감, 출 퇴근, 수면시간등의일상생활습관과샴푸시기, 방법, 건조방법등의샴푸습관이두피가려움증, 두피과다유분, 두피뾰루지, 두피붉은증상이발생하는지루성두피에영향을미칠것이라는문제의식을갖고이를검증하기위하여연구가시작되었다. 본연구결과연구의첫째, 연구대상자들 150명을대상으로표본을구성한결과직업은 군인 150명 (100%) 으로, 복무기간이 16~20년이내 44명 (29.3%), 연령은 40세~44세가 43.3% 로가장높았으며, 대부분기혼으로, 군복무기간은 16~20년 29.3%, 학력은대졸 53.3%, 월소득은 400~500만원 34.0% 가가장높은것으로조사되었다. 둘째, 연구대상자들의샴푸습관에대해조사한결과샴푸시기는아침과저녁에응답자가 43.3% 로조사되었으며, 1일샴푸횟수는 2회 50.0%, 1회 36.7% 순으로조사되었다. Yoon[24] 의연구에의하면두피, 모발손상의주요원인으로스트레스가가장많은영향을미치는것으로보고되고있으며, Lee[23] 는스트레스와불규칙적인생활습관이지루성두피와탈모에영향을미치는것으로보고되고있다. 본연구의결과들을종합해볼때사무직군무종사자들의생활과샴푸습관이지루성두피의발생원인과관련이있는것으로확인되었다. 특히잦은샴푸습관과샴푸의불완전한세정및모발건조에의한습진이지루성두피발생의일차적인원인이되고있으며, 불규칙적인출 퇴근과잦은피로감, 만성스트레스가지루성두피의이차적악화원인이되고있음을알수있었다. 이와같은연구결과를통해지루성두피의연구에대한기초자료가될수있을것으로사료되며, 향후지루성두피에대한지속적인관심과연구가필요할것으로생각된다.
