Our aim was to investigate the expression of cyclin D1, CK7, CK20 and vimentin in 138 papillary thyroid cancer and relationship with clinicobiological factors. We also tried to evaluate the value of those as prognostic factors. Methods: We performed an immunohistochemical assay for cyclin D1, CK7, CK20, and vimentin in 138 papillary thyroid cancer tissue specimens. The correlation between these factors and the clinicobiological parameters was analyzed. Results: In 138 papillary thyroid carcinoma specimens, the expression of cyclin D1, CK7, CK20 and vimentin was seen in 118 cases (85.5%), 133 cases (96.4%), 2 cases (1.4%) and 119 cases (86.2%), respectively. In our cases, there was a significant association among the expressions of cyclin D1, gender and lymph node metastasis. However, no obvious correlation was found between those proteins, age, tumor size, tumor amount, lymph node status and the TNM stage. Conclusion: Our results suggested increased cyclin D1 expression was seen in female gender and it may be a useful marker for evaluating lymph node metastasis. However, the clinical utility of cyclin D1, CK7, CK20 and vimentin in thyroid cancer patients has to be further defined by prospective studies with larger sample sizes. (Korean J Endocrine Surg 2012;12:92-97) 책임저자 : 김권천, 광주시동구필문대로 , 조선대학교병원외과 Tel: , Fax: kckim@chosun.ac.kr 접수일 :2011 년 11 월 21 일, 수정일 :2012 년 4 월 11 일, 게재승인일 :2012 년 4 월 12 일이논문은 2009 년도조선대학교학술연구비의지원을받아연구되었음. 92 Key Words: Papillary thyroid cancer, Cyclin D1, Cytokeratin 7, Cytokeratin 20, Vimentin 중심단어 : 유두상갑상선암, Cyclin D1, Cytokeratin 7, Cytokeratin 20, Vimentin Department of Surgery, Chosun University College of Medicine, Gwangju, Korea 서 Cyclin D1은염색체 11번에위치하는 cyclin D1 유전자의단백질산물로세포주기중 G1/S기의이행에관여하는데, cyclin D1이 cyclin dependent kinase 4 (CDK4) 또는 CDK6와복합체를이루어활성화되면 retinoblastoma 단백을인산화시켜세포를 S기로이행하게한다 (Fig. 1).(1) 그러므로, cyclin D1이유전자증폭이나과발현이될경우세포증식을촉진하는것으로알려져있다.(2) 갑상선암에서 cyclin D1 의역할은잘알려져있지는않지만유두상갑상선암의면역조직화학검사에서 cyclin D1의발현이 60% 이상에서관찰되어지고있다 (Fig. 2).(3,4) Cytokeratin (CK) 은중간미세섬유단백질의 complex family로서 20개이상의다른폴리펩티드로구성되어있고주로양성및악성상피세포에서발견된다. 20종의 cytokeratin 이있으며, 조직의종류에따라또는세포의분화및성장에따라발현되는종류가특이하다.(5) 정상세포들이암으로변화되어가더라도그고유의중간미세섬유의특이성을간직하기때문에원발병소의진단과분화및성장정도의척도로서유용한가치를지닌다 (Fig. 3, 4).(6,7) Vimentin은주로연부조직에서관찰되는중간단섬유 family의구성원으로 cytoskeleton의주요요소이며 mesoderm 기원의조직에대한표지자및유육종의면역조직화학표지자로이용되고있다. 종양세포의국소침범, 전이의능력과 vimentin의연관성과관련하여여러연구들이있는것으로알려져있다 (Fig. 5). 현재여러종양에서이러한 cell-cycle regulator에대한연구가이루어지고있으나유두상갑상선암에서 cyclin D1을 론

Cyclin D1 immunoreactivity is present in papillary carcinoma (Cyclin D1, H&E, 100). 제외한 CK7, CK20 및 vimentin의 발현에 대한 연구는 부족 하다. 따라서, 본 연구에서는 유두상 갑상선암에서 Cyclin D1의 역할에 대한 재조명, CK7, CK20 및 vimentin의 발현과 유두상 갑상선암 환자의 상관관계를 분석하여 예후인자와 2008년 1월부터 2010년 8월까지 유두상 갑상선암으로 갑 상선 절제술을 시행한 환자 중 면역조직화학검사를 시행한 138명의 환자를 대상으로 하였다. 대상군에서 남자가 18명, 여자는 120명이었으며, 대상 환자들의 평균 연령은 49.3세 였고 연령 분포는 19세부터 82세였다. 환자의 자료는 임상 기록지, 병리학 진단 기록지를 근거로 후향적으로 연구하 였으며, 환자의 성별, 종양의 개수 및 크기, 림프절 전이 유 무, TNM stage 등으로 구분하였다.

3 94 대한내분비외과학회지 : 제 12 권제 2 호 ) 면역조직화학적염색및결과의해석 10% 중성완충포르말린에고정시킨조직의파라핀블록을 4 6μm 두께로연속절편후 60 o C 항온기에서 60분간가열후 xylene으로 3 5분간 3회침수하여파라핀을제거하였다. 항원성을증진시키기위해 ph 6.0 citrate buffer에넣고 micro-oven에서 20분가열후내인성과산화효소의활성을억제시키기위하여 3% 과산화수소수를사용하여 10분간처리후 PBS (phosphate buffer solution) 에씻고일차 및이차항체의비특이적결합을막기위하여정상염소혈청을가한후실온에서 10분간반응시켰다. 일차항체로실온에서 1시간반응후 PBS에세척, 이차항체로 20분반응후 PBS에세척후 avidin-biotin complex (ABC) chromogen 에서발색후대조염색은 Mayer Hematoxylin에서 3 5초발색후수세한다음봉입후검경하였으며고배율 ( 400) 시야에서관찰하여핵이나세포질에서발현된경우를양성으로판독하였다. 3) 통계적분석 Social Science program 10.1 version (SPSS Inc. Chicago IL) 의통계프로그램을이용하였다. 임상자료와 Cyclin D1, CK 7, CK20과 Vimentin의발현과의관계를 Chi-squire 방법과 Fisher's exact test를사용하였으며. P value는 0.05 미만일때통계적으로유의하다고하였다. 결 과 Fig. 5. Tumor cell diffuse cytoplasmic reactivity for vimentin (Vimentin, H&E, 100). 138예의유두상갑상선암환자에서 cyclin D1, CK7, CK20 및 vimentin의발현은각각 118예 (85.5%), 133예 (96.4%), 2예 (1.4%) 그리고 119예 (86.2%) 에서관찰되었다. 성별에따른각각의발현정도나 TNM stage 기준에따라나이를 45세미만인군 (44예) 과 45세이상인군 (94예) 으로나누었으며종양의개수가 1개인경우와 2개이상인경우에서각각의발현정도를비교하였을때이들임상적인자들의유의성은관찰되지않았다. 4 김호진외 : 유두상갑상선암에서 Cyclin D1, Cytokeratin 7, Cytokeratin 20, Vimentin 의발현과예후인자의연관성분석 95 크기를미세유두상갑상선암의크기와관련하여 1 cm 미만인군 86예와 1 cm 이상인군 52예로구분하였을때, cyclin D1, CK7, CK20, vimentin의발현이 1 cm 미만인군에서각각 72예, 84예, 0예, 75예, 1 cm 이상인군에서는각각 46 예, 49예, 2예, 44예에서보였고통계적유의성은없었다. TNM stage에따른분류에서각각 stage I-102, 97, 1, 86예, II-3, 3, 0, 2예, III-32, 32, 0, 30예, IV-모두 1예였으며이의통계적유의성은없었다. 림프절전이는 55예 (39.9%) 였으며 Cyclin D1 발현이되었을경우더많은림프절전이를보였고통계적으로유의하였으나 (P=0.050) 나머지 CK7, CK20, vimentin의발현과림프절전이와의관련성은통계적으로유의성을보이지않았다 (Table 1). 고찰유두상갑상선암은갑상선에서발생하는가장흔한암으로예후는좋은것으로알려져있으나일부에서는미분화암으로퇴행성변화를보여예후가나빠지기도한다. 최근분자생물학적연구방법의발달로특정유전자나생물학적표지자로예후및질환의진행및침범도등의평가에도움이되는것으로알려져있으나, 이들인자를단독으로적용하여예후및질환의진행정도를예측하는것은아직명확히확립되어있지는않다. 따라서, 본연구에서는유두상갑상선암에서 cyclin D1, CK7, CK20과 vimentin의발현을알아보고, 이들의발현이성별, 나이, 종양의개수, TNM stage, 림프절전이등의예후인자와연관이있는지를알아보고자하였다. Cyclin D1은세포주기에관여하여세포성장을촉진시키고초기에갑상선종양의형성에기여하는것으로알려져있다. 여포암이나미분화갑상선암에비해유두상갑상선암에서 cyclin D1의발현이 80% 정도로더많은비율을보이고있다.(8) Wang 등 (9) 은 cyclin D1이갑상선종양의발전과예후에서중요한역할을한다고하였고, Goto 등 (10) 은고분화암에서 32.0% (39/122) 가양성으로, 저분화암에서 36.8% (7/19) 가양성으로발현되어여포상선종의 3.0% (1/33) 발현에비해유의한차이를보였다고하였다. 또한 cyclin D1이종양형성에기여하고림프절전이를평가할수있는표지자라고보고하였다.(3,4,11,12) 본연구에서 cyclin D1의발현은 118예 (85.5%) 로다른연구와유사한결과를보였으며림프절전이를보인 55예중 51예 (92.7%) 에서발현되었고림프절전이가없는 83예중 67예 (80.7%) 에서발현 (P=0.050) 을보였으며이는 cyclin D1 이림프절전이여부를평가하는표지자로서의역할을보고한연구 (3,4,11,12) 의결과와부합한다. 또한림프절전이여부가국소재발과관련이있고예후에영향을미치므로 cyclin D1이유두상갑상선암에서예후인자로서의가치를지님을알수있었다. 유두상갑상선암환자에서예후인자로서 cyclin D1의발현의연관성을알아보기위해갑상선암의진행정도와상관성을분석했을때암의크기가클수록 cyclin D1의발현이증가한다는 Park 등 (13) 의보고와는상이한결과를보였으나이는대상환자의크기가적어예후인자로의유용성을규명할수없다고기술하였기때문에예후인자로의가치평가는더많은환자에서의연구가필요할것으로생각된다. 또한본연구에서유두상갑상선암환자에서여성에서 106예 (88.3%) 로남성의 12예 (66.7%) 에서보다더높은비율을보였고통계학적으로유의성 (P=0.015) 을보였으나남성환자의수자체가많지않기때문에더많은수의남성모집단을이용한연구가필요할것으로생각된다. 갑상선암의병기에따른 cyclin D1의발현변화는뚜렷이관찰되지않았다. Cytokeratin은다양한종류의원발성및전이성암에서발견이되고있으며여포상및유두상갑상선암에서 CK7이강한 immunopositivity를보이나 CK20은단지약하고국소적인 immunopositivity를보인다는보고가있었고,(14) 마찬가지로다른연구에서 CK7은정상갑상선조직, 유두상갑상선암, 여포암과저분화암에서 100% 양성발현을보였으나, 미분화암에서는발현되지않았으며, CK20은 41예의유두상갑상선암중 1예에서약한양성발현 (weak immunoreactivity) 을보였다는동일한보고가있었다.(15) 그리고 CK7과 CK20의발현과갑상선종양에대한여러연구에서 CK7은유두상갑상선암, 여포성암, 그리고대다수의미분화암과저분화암에서발현되었으나 CK20은발현되지않는것으로보고가되었다.(16-18) Schmitz-Winnenthal 등 (19) 은분화및미분화갑상선암에서종양조직과혈액에서의 RT-PCR을이용한 CK20의검출과예후와의관련성을연구하였는데 32예중 15예 (47%) 에서 CK20이검출되었다고하였는데이로보아유두상갑상선암에서 CK20의 mrna는만들어지나단백질로의발현이안되는것으로추정된다. 본연구에서도유두상갑상선암에서 CK7은대부분양성발현이었지만 Fonseca 등 (15) 의연구에서와같이 CK20은 2 명 (1.4%) 에서양성발현을보였는데예후인자와의연관성은없었지만 Fonseca 등은 cytokeratin의발현에대해이는아마도미토콘드리아의높은비율과관련되어있으며비특이적발현이라는견해를주장하고있어서추후더많은예에서의연구가필요할것같다. Vimentin은주로연부조직에서관찰되어지는중간단섬유 family의구성원으로 cytoskeleton의주요요소이다. McInroy 등 (20) 은 vimentin발현은결장암과유방암에서암세포의침습성및이동성과관련있다고보고하였다. Bennett 등 (21) 은미분화종양세포일수록 vimentin의발현도가높다고보고하였고미분화갑상선암에서 vimentin의발현도가높은것으로보고하였다. Vasko 등 (22) 은유두상

5 96 대한내분비외과학회지 : 제 12 권제 2 호 2012 갑상선암조직및림프절에서 vimentin의발현과 vimentin mrna 검출에대한연구를하였는데, vimentin mrna와단백발현은모든유두상갑상선암과림프절에서나타났으나정상갑상선조직에서는검출되지않았다고하였다. Vimentin의면역조직화학적염색강도는주위조직침범 (P=0.005), 림프절전이 (P=0.004) 및다발성 (P=0.013) 등과관련있으나, 종양의크기, 환자의나이및성별등과는상관관계가없었다고하였다. Yamamoto 등 (23) 은 vimentin은정상갑상선조직 (65%) 을포함한잠재암 (97%) 에서발현되며, 원격전이와상관없이모든유두상갑상선암에서발현을보이나정상세포보다는종양세포에서더강한발현을보인다고하였다. Yagi 등 (24) 도미세유두상갑상선암과 3cm 보다큰유두상갑상선암의비교에서 vimentin의발현은차이가없다고보고했다. 위연구들에서살펴보면 vimentin과예후인자와의상관관계에대해서는여전히일관된결과가없는것같다. 본연구에서는 vimentin은 138명중 19명을제외한대다수환자의조직에서발현되었으나예후인자와의상관관계는없었다. 결 본연구에서유두상갑상선암환자에서 cyclin D1과성별및림프절전이와의상관성을알수있었으나, CK7, CK20, vimentin의발현과임상적예후인자들과의상관성은보이지않았다. Cyclin D1의발현은여성갑상선암환자의경우에서높았으나 cyclin D1을포함한 CK7, CK20, vimentin의모든염색에서남성유두상갑상선암환자의수가적어예후인자로서의가치를평가하기에어려울것으로생각된다. 하지만 cyclin D1의발현과림프절전이와는상관성을지니고있었고또한림프절의전이는국소재발과연관이있고예후에영향을미치므로 cyclin D1의발현이예후인자로서가치가있음을확인할수있었다. 그러나본연구에서대상환자수가적고성별과관련하여서도남성환자의비율이너무적다는점이연구의맹점이라고생각되고향후더많은수의모집단을대상으로연구가필요할것으로생각된다. 론 REFERENCES 1) Kang YK, Kim WH, Jang JJ.
