Autoimmune pancreatitis, sclerosing cholangitis, and retroperitoneal fibrosis are representative presentations of IgG4 related autoimmune disease. Herein, we describe 2 patients (40-years-old woman and 47-years-old man) diagnosed with pulmonary involvement of IgG4-related autoimmune disease. The patients were admitted for an evaluation of the lung mass or multiple lung nodules found on chest radiography. Surgical lung biopsies were performed and pathologic finding revealed lymphoplasmacytic sclerosing inflammation with numerous IgG4 positive cells. The patients had elevated serum total IgG and IgG4 levels. Treatment consisted of high dose methylpredinisolone (1 mg/kg/day) and demonstrated good responsiveness. However, one patient experienced 2 relapses while being tapered off of steroid treatment. IgG4 연관폐질환은국내에서는아직보고가없는데, 저자들은최근 IgG4 연관폐질환으로확인된환자 2예를경험하였기에문헌고찰과함께보고하는바이다. 증례증례 1 환자 : 여자, 40세주소 : 내원 2개월전에발견된다발성폐결절현병력 : 내원 2개월전전신위약감으로연고지병원에서입원치료중촬영한단순흉부사진및흉부전산화단층촬영 (computed tomography, CT) 에서오른쪽폐에다발성결절이발견되었다. 경피적폐생검을 2차례시행하 359

방사선학적소견 : 단순흉부방사선사진에서는우폐상엽부위에다발성결절이관찰되었고, 흉부 CT에서는주로우상엽과우중엽에공기기관지조영상 (air-bronchogram) 을동반한다발성결절이관찰되었다 (Figure 1A, B). 진단및임상경과 : 입원 12일째비디오흉강경을이용하여우상엽및우중엽의결절에대하여조직검사를시행하였다.

혈청 total IgG는 1,700 mg/dl ( 참고치, 700 1,600 mg/ dl), IgG4는 1.35 g/l ( 참고치, 0.06 1.21 g/l) 로상승되어있었다. 환자는 IgG4 와연관된자가면역성질환의폐침범으로진단되었고, methylprednisolone (1 mg/kg) 을일 Figure 1.

2 JW Yoo et al: Immunoglobulin G4 related lung disease 였으나진단이불명확하여본원으로전원되었다. 과거력 : 내원 7년전결핵성흉막염으로치료후완치된병력이있었고, 내원 6개월전우상엽과좌상엽에폐결핵이의심되어항결핵제를복용중이었다. 가족력 : 특이소견없었다. 사회력 : 비흡연자이다. 신체검사소견 : 혈압은 98/68 mmhg, 호흡은 20회 / 분, 맥박은 90회 / 분, 체온은 36 o C였다. 가슴청진상특이소견없었고, 심박동은규칙적이었고, 심잡음은들리지않았다. 검사실소견 : 전체혈구계산에서혈색소 14.3 g/dl, 백혈구 10,800/mm 3 ( 중성구 71.3%, 림프구 15.3%) 혈소판 430,000/mm 3 이었다. C-reactive protein 은 9.3 mg/dl ( 참고치, mg/dl) 이었고, 류마티스인자 (rheumatoid factor), 항핵항제 (antinuclear antibody) 및항호중구세포질항체 (antineutrophil cytoplasmic antibody) 등은음성 이었다. 방사선학적소견 : 단순흉부방사선사진에서는우폐상엽부위에다발성결절이관찰되었고, 흉부 CT에서는주로우상엽과우중엽에공기기관지조영상 (air-bronchogram) 을동반한다발성결절이관찰되었다 (Figure 1A, B). 진단및임상경과 : 입원 12일째비디오흉강경을이용하여우상엽및우중엽의결절에대하여조직검사를시행하였다. 조직검사상우상엽의결절은괴사성육아종소견으로결핵에합당하였고, 우중엽의결절은 lymphoplasmacytic sclerosing inflammation 소견과함께면역염색시다수의 IgG4 염색세포가관찰되었다 (Figure 2A, B). 혈청 total IgG는 1,700 mg/dl ( 참고치, 700 1,600 mg/ dl), IgG4는 1.35 g/l ( 참고치, g/l) 로상승되어있었다. 환자는 IgG4 와연관된자가면역성질환의폐침범으로진단되었고, methylprednisolone (1 mg/kg) 을일 Figure 1. (D) IgG4 immunostaining showed immunoreactive plasma cells ( 100). 주일간 투여하고, prednisolone을 30 mg q 12 hr 유지용 량으로 퇴원하였다.

증례 2 환 자: 남자, 47세 주 소: 내원 1개월 전 발생한 기침 현병력: 내원 1개월 전부터 발생한 기침 및 복통으로 타 병원 내원하여 촬영한 단순 흉부 사진상 우종격동 종괴 가 의심되어 본원으로 내원하였다.

(D) IgG4 immunostaining showed immunoreactive plasma cells ( 100). 주일간 투여하고, prednisolone을 30 mg q 12 hr 유지용 량으로 퇴원하였다. 퇴원 후 외래에서 시행한 흉부 CT에 서 다발성 결절 소견은 호전되었다. 증례 2 환 자: 남자, 47세 주 소: 내원 1개월 전 발생한 기침 현병력: 내원 1개월 전부터 발생한 기침 및 복통으로 타 병원 내원하여 촬영한 단순 흉부 사진상 우종격동 종괴 가 의심되어 본원으로 내원하였다. 과거력: 내원 1년 전 외부 병원에서 부폐렴성 흉막염으 로 치료받은 적이 있었고, 입원 중 장 유착이 발생하여 수술을 시행받았다. 가족력: 특이 소견 없었다. 사회력: 35갑년의 흡연력이 있었다. 신체검사 소견: 혈압은 118/67 mmhg, 호흡은 18회/분, 맥박은 92회/분, 체온은 36.4oC였다. 가슴 청진상 특이 소 견 없었고, 심박동은 규칙적이었으며 심잡음은 들리지 않 았다. 검사실 소견: 전체혈구 계산에서 혈색소 12.4 g/dl, 백 혈구 14,600/mm3 (중성구 72.5%, 림프구 21.2%, 호산구 3 1.4%), 혈소판 478,000/mm 이었다. 간기능 검사에서 aspartic acid transaminase는 41 IU/L (정상: 0 40 IU/L), alanine transaminase는 63 IU/L (정상: 0 40 IU/L)으로 361

4 JW Yoo et al: Immunoglobulin G4 related lung disease 상승되어있었다. 항핵항체는양성으로역가가 1:160으로상승되어있었고, 항이중 DNA 항체및항호중구세포질항체는음성이었다. 방사선학적소견 : 단순흉부방사선사진에서우폐하부에경계가불분명한폐경화가관찰되었고, 흉부 CT에서우중엽에종괴및우상엽과양하엽에다발성결절이관찰되었다 (Figure 1C, D). 진단및임상경과 : 입원 2일째우중엽종괴에대해경피적폐생검을시행하였고, 조직소견상기질화폐렴이관찰되었다. 입원 13일째비디오흉강경으로우하엽결절에대해조직검사를시행하였다. 폐조직검사결과형질세포육아종 (plasma cell granuloma) 이관찰되어염증성가성종양 (inflammatory pseudotumor) 으로진단하였고퇴원후외래에서치료결정하기로하였었다. 퇴원약 8개월, 1년뒤외래에서촬영한추적흉부 CT에서병변의악화가관찰되었으나, 이후외래추적관찰이나치료가이루어지지않았다. 외래추적관찰이이루어지지않은지 3년후, 내원 5일전발생한호흡곤란, 발열, 화농성가래로타병원경유하여폐렴의심하에본원호흡기내과에입원하였다. 환자는내원한달전천공을동반한급성담낭염으로타병원에서치료받은병력이있었다. 환자는경험적항생제치료후폐렴에대한임상증상은호전되었으나, 타병원에서촬영한흉부 CT상경화및결절성병변이과거에비해악화된것을확인하였다. 악성종양의가능성을배제하기위해폐병변에대한조직검사를고려하던중, 4년전본원에서절제했던폐조직에서다수의 IgG4 염색세포가관찰되어 IgG4 연관자가면역질환으로진단하였다 (Figure 2C, D). 당시혈청 total IgG는 2,400 mg/dl (700 1,600 mg/ dl), IgG4는 1.56 g/l ( g/l) 로상승되어있었다. Methylprednisolone (1 mg/kg) 을열흘동안투여하였고, 경구용 methylprednisolone 을 40 mg qd로유지하여퇴원하였다. 외래에서 methylprednisolone 을줄여나가던중 2차례재발이발생하여스테로이드를증량하였고, 이후호전되었었다. 고찰 IgG4와연관된질환으로자가면역성췌장염뿐만아니라 6, 경화성담도염, 경화성침샘염, 후복막강섬유화등에서도 IgG4 양성인형질세포들의침윤이보고되면서 IgG4 연관자가면역성질환이있는것이알려졌다 1,3,5. 이질환 의특징은혈청 IgG4 농도상승, 조직소견상 IgG4 양성형질세포침윤이동반된경화성염증소견, 스테로이드에대한양호한반응이등이다. IgG4 연관자가면역질환폐침범의양상은, 자가면역성췌장염등다른장기에서폐침범을동반 7,8 혹은폐질환단독으로서는가성종양 2, 비특이적간질성폐렴 4 등도보고되었다. IgG4 연관자가면역질환의폐침범에관한대규모역학연구는없으나증례보고들에따르면, 진단당시나이는평균 59세, 남자의비율이많은편이다 9. 증상은대개비특이적이거나무증상으로흉부방사선상에서종괴혹은결절로발견되는경우가많다 2,3,10. 그외흉통, 혈담, 기침, 미열등을동반할수있는것으로보고되어있다 2,9. 혈청학적검사상 C-반응단백및 total IgG, IgG4 수치가상승할수있다 2. 이질환에특이적인자가항체는현재알려져있지는않다. 방사선학적으로는흉부 CT에서공기기관지조영이동반된폐포경화소견 (consolidative lesion), 간유리음영 (groundglass opacity), 벌집모양 (honeycomb appearance) 이동반된그물모양의음영 (reticular shadow) 등이보인다 4,10. Inoue 등 9 은 13명의 IgG4 연관자가면역질환의폐침범의방사선학적형태를종괴형태를포함한단독결절형 (solitary nodular type), 원형의간유리음영 (round-shaped ground glass opacity), 폐포간질성 (alveolar interstitial), 그리고기관지혈관성 (bronchovascular) 등크게 4가지로분류하였다. 본두증례에서는공기기관지음영이동반된다발성결절형태를보였다. 치료는스테로이드를사용하며치료에대한반응이좋은것으로알려져있다 3-5,8. 스테로이드에반응이없는경우에는면역억제제를추가하기도한다 10. 결론적으로, 본증례는국내에서도환자가단독종괴및다발성결절등으로내원시감별질환으로 IgG4와연관자가면역질환의폐침범을고려해야할필요가있음을보여주었다. 참고문헌 1.
