Several tumor markers have been studied because the pleural fluid cytology and pleural biopsy specimens do not provide a diagnosis in a high percentage of malignant effusions. In an attempt to overcome this limitation, procalcitonin and C-reactive protein (CRP) in pleural effusions and serum, which are known to be inflammation markers, were measured to determine if they can differentiate an exudate from trasndate as well as the diverse causes of exudative pleural effusion. Methods: 178 consecutive patients with pleural effusion (malignant 57, tuberculous 51, parapneumonic 31, empyema 5, miscellaneous benign 7, transudative 27)were studied prospectively. The standard parameters of pleural effusion and measured serum and pleural procalcitonin were examined using in immunoluminometric assay. The level of CRP in serum and pleural fluid was determined by turbidimetric immunoassay. Results: The pleural procalcitonin levels in the exudate were significantly higher than those in the transudate, 0.81±3.09 ng/ml and 0.12±0.12 ng/ml, respectively (p=0.007). The pleural CRP levels were significantly higher in the exudate than the transudate, 2.83±3.31 mg/dl and 0.74±0.67 mg/dl, respectively (p<0.001). The pleural procalcitonin levels in the benign effusion were significantly higher than those in the malignant effusion, 1.15±3.82 ng/ml and 0.25±0.92 ng/ml, respectively (p=0.032). The pleural CRP levels were significantly higher in the benign effusion than in the malignant effusion, 3.68±3.78 mg/dl and 1.42±1.54 mg/dl, respectively (p<0.001). The pleural procalcitonin levels in the non-tuberculous effusion were significantly higher than those in the tuberculous effusion, 1.16±3.75 ng/ml and 0.13±0.37 ng/ml, respectively (p=0.008). Conclusion: Measuring the level of procalcitonin and CRP in the pleural fluid is helpful for differentiating between transudates and exudates. In addition, it is useful for differentiating between benign and malignant pleural effusions. 2 SH Kim et al: Procalcitonin and C-reactive protein in pleural effusions 혈증등에서유의한증가를보이는것으로알려진 procalcitonin 농도측정이새로운지표로서가능성이있음이제시되었다 3,4. 흉수의감별에이러한염증지표들이유용한지조사하기위하여환자의흉수와혈청에서 CRP와 procalcitonin의농도를각각측정하였고, 여출액과삼출액의감별과함께삼출성흉수의다양한원인들에대한감별의유용성을기존에사용되던지표들과비교하여확인하였다. 대상및방법 1. 대상 2005년 10월 1일부터 2006년 9월 30일까지연세대학교원주의과대학원주기독병원호흡기내과로흉수가관찰되어입원하였거나입원중흉수가발생한환자와협진으로의뢰되었던총 198명을전향적으로연구하였다. 이중에서흉수의원인질환으로한가지의정확한진단이가능하였던 178명을분석하였는데, 남자 125명 (70.2%), 여자 53명 (29.8%) 이었고, 평균연령은 62세였다. 2. 방법가. 검체의분리및일반적검사흉수에대하여흉강천자를시행하여흉수의 ph, 총적혈구및백혈구수, 백혈구감별계산, 총단백, 알부민, 포도당, lactate dehydrogenase (LDH), adenosine deaminase (ADA) 등을측정하였고, 결핵균도말및배양검사와세균에대한도말및배양검사를시행하였으며, 세포학검사를시행하였다. 시행이가능한경우에흉막생검을시행하였다. 동시에흉수를얻은즉시검체를 10분간 3,000 rpm 으로원심분리하여세포성분은침전시키고상층액을분리한후검체측정전까지 -70 로냉동보관하였다. 혈청에서의측정치분석을위하여채혈을하였으며흉수와동일한방법으로처리하였다. 나. 환자군의분류일반적으로문헌에서받아들여지고있는진단기준의정의에근거하여각각의흉수의원인군을정의하였다 5-7. 결핵성흉수는흉막생검에서건락성육아종소견을보이거나, 흉수나흉막생검조직에서 Mycobacterium tuberculosis(m. tuberculosis) 배양양성을보인경우, 또는 M. tuberculosis에대한객담배양양성이면서삼출성흉수가있을때, 흉수에서단핵구가우세하면서 ADA치가 45 U/L 이상일때로정의하였다. 부폐렴성흉수는암이나다른흉수를야기할만한다른질환이없으면서흉부 X선검사에서폐침윤을보이고화농성객담이있는급성열성질병이있는경우에동반되는흉수로정의하였다. 농흉은흉수의육안적검사에서농이관찰되거나세균학적도말및배양에서양성일때로정의하였다. 악성흉수는흉막생검이나흉수의세포학검사에서암세포가규명된경우이거나이미암으로진단된환자에서흉수를동반할수있는다른질환이진단되지않은경우로정의하였다. 여출성흉수로분류된경우는원인질환으로울혈성심부전, 만성신부전, 간경화가있었다. 다. CRP 측정 CRP 측정은대상환자의혈청과흉수에서면역측정법 (turbidimetric immunoassay)(cobas Integra800, Roche, Basel, Switzerland) 으로측정하였다. 라. Procalcitonin 측정 Procalcitonin 측정은대상환자의혈청과흉수에서 immunoluminometric assay (Brahms Diagnostica, Berlin, Germany) 로측정하였다. 먼저측정하기전에모든시약과검체를실온으로유지하였다. 제공되는 6개의표준액과대조액, 측정할환자의흉수검체를각각 50 μ L씩튜브에넣고약 30분간실온에서반응시켰다. 반응시킨후, 추적자를각각 200 μl씩넣고짧게흔들어서섞어준다음빛을차단하여실온에서약 2시간동안 300 rpm으로진탕하여반응시켰다. 반응후에 4회세척하고남아있는용액을완전히제거하였다. Germany) 에튜브를하나씩넣어값을측정하였으며, 환자의혈청검체도동일한방법으로시행하였다. 마. 통계분석윈도우용 SPSS 12.0을사용하여서로다른두군간의평균값은독립표본 T 검정으로비교하였고, 통계학적으로유의한차이를보이는측정값에대하여 ROC (receiver operating characteristic) 곡선분석을통해민감도와특이도가높게유지되는결정치를구하였다. 측정값은평균값 ± 표준편차로나타내었으며, p값이 0.05 미만일때통계적으로유의하다고판정하였다. 연구는연세대학교원주의과대학임상시험위원회의승인하에이루어졌다. 결과 1. 원인에따른흉수의 procalcitonin과 CRP 의측정값정확한진단이가능하였던대상환자 178명의흉수에대하여 Light's criteria에따라삼출액과여출액으 로구분하였을때, 삼출액은 151명 (84.8%), 여출액은 27명 (15.2%) 이었다. 다시대상환자 178명에대하여진단기준에따라악성흉수 (57명), 결핵성흉수 (51명 ), 부폐렴성흉수 (31명), 농흉 (5명), 여출성흉수 (27명) 와기타원인의양성흉수 (7명) 로나누었을때 Table 1과같았다. 흉수 procalcitonin은농흉에서 3.10±3.31 ng/ml 로가장높았으며, 흉수 CRP도농흉에서 8.60±6.08 mg/dl로가장높았다. 2. 삼출액과여출액에서의 procalcitonin 과 CRP 의측정값총 178명의대상환자를삼출액과여출액으로구분하여흉수와혈청에서의 procalcitonin과 CRP를각각측정하였고, 혈청에대한흉수의 procalcitonin 및 CRP의비를각각계산하여비교하였다 (Table 2). 삼출액과여출액에서의흉수 procalcitonin은각각 0.81±3.09 ng/ml, 0.12±0.12 ng/ml, 혈청 procalcitonin은각각 0.90±3.58 ng/ml, 0.15±0.12 ng/ml로 355

Specificity and sensitivity value for pleural fluid CRP were 96.3% and 45.0% at the 2.06 mg/dl cut-off value. 모두삼출액에서통계학적으로유의한증가소견을보였다 (p=0.007, p=0.011). 삼출액과여출액에서의흉수 CRP는각각 2.83±3.31 mg/dl, 0.74±0.67 mg/dl, 혈청 CRP는각각 8.24±8.83 mg/dl, 3.41±3.50 mg/dl 로모두삼출액에서통계학적으로유의한증가소견을보였다 (p<0.001, p<0.001). Procalcitonin 및 CRP 각각의혈청에대한흉수에서의측정값의비는모두삼출액에서증가되어있는경향을보였으나통계학적으로유의한차이를보이지는않았다. 흉수 CRP 및흉수 procalcitonin의측정값이여출액과비교하여삼출액에서통계적으로매우유의하게증가되어있어서, 이들측정값에대한 ROC 곡선분석을하였다. 흉수 CRP 측정값이 2.06 mg/dl 보다높을때, 특이도 96.3%, 민감도 45.0%, 양성우도비 (positive likelihood ratio) 12.16, 음성우도비 (negative likelihood ratio) 0.57, 양성예측치 (positive predictive value) 98.6, 음성예측치 (negative predictive value) 23.9로삼출성흉수의감별에도움이될수있는것으로관찰되었다 (ROC 곡선하면적 =0.752, p=0.0001, Figure 1). 그러나흉수 procalcitonin에대한 ROC 곡선분석은 ROC 곡선하면적이 0.521로 (p=0.7295) 가치있는결정치를찾지못하였다. 4. 악성흉수와양성흉수에서의 procalcitonin과 CRP 의측정값총 151명의삼출액환자에서악성흉수와양성흉수 ( 결핵성흉수, 부폐렴성흉수, 농흉, 기타원인의양성흉수포함 ) 로구분하여흉수와혈청에서의 procalcitonin과 CRP 측정값을각각비교하였고, 혈청에대한흉수의 procalcitonin 및 CRP의비를계산하여비교하였다 (Table 3). 악성흉수와양성흉수에서의흉수 procalcitonin은각각 0.25±0.92 ng/ml, 1.15± 3.82 ng/ml, 혈청 procalcitonin은각각 0.19±0.68 ng/ml, 1.33±4.47 ng/ml 로양성흉수및혈청에서통계학적으로유의한증가소견을보였다 (p=0.032, p=0.017). 혈청에대한흉수 procalcitonin비는각각 356

PCT and CRP in patients with malignant and benign effusion Variables Malignant (n=57) Benign (n=94) p value Pleural PCT (ng/ml) Serum PCT (ng/ml) PCT ratio (pleural/serum) Pleural CRP (mg/dl) Serum CRP (mg/dl) CRP ratio (pleural/serum) 0.25± ± ± ± ± ± ± ± ± ± ± ±0.35 * p value<0.05, tuberculous effusion, parapneumonic effusion, empyema, other benign effusions * * < * < * ±74.43, 1.38±1.42였다. 또한악성흉수와양성흉수에서의흉수 CRP는각각 1.42±1.54 mg/dl, 3.68± 3.78 mg/dl, 혈청 CRP는각각 4.28±4.56 mg/dl, 10.64±9.89 mg/dl 로양성흉수및혈청에서통계학적으로유의한증가소견을보였다 (p<0.001, p<0.001). 혈청에대한흉수 CRP비는각각 0.47±0.43, 0.46±0.35 였다. 5. 악성흉수와양성흉수의감별을위한흉수 procalcitonin 및 CRP 의유용성 원인의양성흉수포함 ) 으로구분하여흉수와혈청에서의 procalcitonin과 CRP를각각측정하였고, 혈청에대한흉수의 procalcitonin 및 CRP의비를각각계산하여비교하였다 (Table 4). 결핵성흉수와비결핵성흉수에서의흉수 procalcitonin은각각 0.13±0.37 ng/ml, 1.16±3.75 ng/ml, 혈청 procalcitonin은각각 0.09±0.30 ng/ml, 1.31± 4.35 ng/ml 로비결핵성흉수및혈청에서통계학적으로유의하게증가되었다 (p=0.008, p=0.007). 혈청에대한흉수 procalcitonin비는각각 1.67±1.72, 6.85± 흉수 procalcitonin 및흉수 CRP의측정값이악성흉수에서보다양성흉수에서통계적으로매우유의하게증가되어있어서, 이들측정값에대한 ROC 곡선분석을하였다. 흉수 CRP 측정값이 2.28 mg/dl보다높을때, 특이도 86.0%, 민감도 53.2%, 양성우도비 3.79, 음성우도비 0.54, 양성예측치 86.2, 음성예측치 52.7로양성흉수의감별에도움이될것으로판단되었다 (ROC 곡선하면적 =0.729, p=0.0001, Figure 2). 흉수 procalcitonin 측정값은 0.11 ng/ml 보다높을때, 특이도 84.2%, 민감도 40.4%, 양성우도비 2.56, 음성우도비 0.71, 양성예측치 80.9, 음성예측치 46.2로양성흉수의감별에도움이될수있을것으로판단되었다 (ROC 곡선하면적 =0.591, p=0.0522, Figure 2). 6. 결핵성흉수와비결핵성흉수에서의 procalcitonin 과 CRP 의측정값총 151명의삼출성흉수환자에서결핵성흉수와비결핵성흉수 ( 악성흉수, 부폐렴성흉수, 농흉, 기타 Figure 2. Light's criteria에서측정하는세가지항목을살펴보면, 혈청에대한흉수에서의총단백의비가 0.5 이상일때특이도 84%, 민감도 86% 로삼출액을감별할수있고, 혈청에대한흉수에서의 LDH 비가 0.6 이상일때특이도 82%, 민감도 90% 로삼출액을감별해낼수있으며, 흉수 LDH가혈청 LDH 참고범위상한값의 2/3 이상일때특이도 89%, 민감도 82% 로여출액에서삼출액을감별해낼수있다. 그러나이기준에의하여도삼출액과여출액의감별은완전하지못하여몇가지보완적인측정지표들이제안되었다 9,10. 흉수와혈청에서각각콜레스테롤을측정하여판단하는지표가그중에하나로, 흉수콜레스테롤이 60 mg/dl 이상일때특이도 92%, 민감도 54% 로삼출액을감별할수있으며, 혈청에대한흉수의콜레스테롤비가 0.3 이상일때특이도 81%, 민감도 89% 로삼출액을감별해낼수있다 9,10. 삼출액의감별에있어서민감도가비교적높은 Light's criteria는다른지표들에비해상대적으로특이도가 떨어진다. 이러한이유로임상적으로는여출액이의심되지만 Light's criteria에의하면삼출액으로판단되는경우가있다. 이를보완하기위해보다높은특이도를보이는지표를제안하게되었는데, 그중에한방법으로흉수알부민보다혈청알부민이 1.2 g/dl 이상증가된소견을보일경우에여출액으로판단할수있고이경우특이도 92%, 민감도 87% 의감별력을보인다 10. 본연구에서삼출액에서유의한증가를보인흉수 CRP의경우 2.06 mg/dl 보다높을때특이도 96.3%, 민감도 45.0% 로삼출액을감별할수있었다. 이것은흉수콜레스테롤을측정하여판단하는지표와비슷한수준의감별력을보이는것으로, 삼출액과여출액의감별에있어서 Light's criteria에보완적으로사용할수있을것으로판단된다. 다른연구에서는흉수 CRP가 1.0 mg/dl 이상일때특이도 87.5%, 민감도 82.0% 로, 또다른연구에서는동일한결정치로특이도 74%, 민감도 74% 의감별력으로삼출액을감별할수있다고보고하였다 11,12. 흉수 procalcitonin도여출액에서보다삼출액에서통계학적으로유의한증가가관찰되었다 (p=0.007). 하지만흉수 procalcitonin 측정값의분포가넓어서 ROC 곡선분석을통한의미있는결정치를확인하지는못하였다. 흉수및혈청에서 procalcitonin과 CRP 의측정값이삼출액에서모두유의하게증가되어있으면서각각의혈청에대한흉수의측정값의비는유의한차이를보이고있지않았다. 또한결과에기술하지않았지만, procalcitonin (r=0.899, p<0.001) 및 CRP (r=0.736, p<0.001) 의흉수와혈청에서의측정값이각각서로양의상관관계를보이고있었다. 이러한사실들은 procalcitonin과 CRP가흉수에서증가되는기전 358

CRP와더불어 1989년 calcitonin의전구물질로알려지기시작한 procalcitonin은 년에처음감염표지자로소개된이후 14, 많은연구들을통해세균성감염이나이로인한패혈증등에서증가되는것이관찰되었다 3,4. Procalcitonin은 116개의아미노산으로구성된약 13 kd의분자량을가지는단백질로아직까지어느세포에서어떠한기전으로만들어지는지잘알려져있지않다. 하지만 CRP의분비를촉진시키는것으로알려진국소적인염증에관여하는시토카인 interleukin-6 (IL-6), tumor necrosis factor-alpha (TNF-α) 등이 15,16 procalcitonin이증가되기전에이들시토카인들이먼저증가된다는보고가있어 17, 추후흉수에서이러한시토카인들을측정하여흉막의국소적인염증의결과로인한 procalcitonin 및 CRP의증가가능성을관찰하고 procalcitonin의분비기전을확인하는것이필요하다. 악성흉수의감별진단은흉막의세포학검사나흉막생검을통해서이루어지고세포학검사는악성흉수환자의 60% 가양성으로보고되는정도이며 18, 흉막생검을추가하였을때에도그진단율을많이높이지는못하고있다. 흉강경을이용하는경우에는악성흉수환자의약 95% 에서진단이가능하지만모든병원에서시행이가능하지않으며 19, 비교적침습적인방법으로시행이용이하지는못하다. 악성흉수감별에보완적으로사용될수있는표지자들에대한연구가다양하게있었으며, 대개가암표지자를사용한연구이다. 가장대표적인것이 carcinoembryonic antigen (CEA) 로가장특이도가높은표지자로알려져있으나민감도는결정치에따라약 29-77% 로다양하게보고되고있다 20. 본연구에서악성흉수와양성흉수의감별을위해측정한 procalcitonin과 CRP 측정값이악성흉수에비하여양성흉수에서모두통계적으로유의하게증가된소견이관찰되었다. 흉수 CRP의경우측정값이 2.28 mg/dl 이상일때, 특이도 86.0%, 민감도 53.2% 의감별력으로양성흉수를감별할수 있었으며, 흉수 procalcitonin의경우측정값이 0.11 ng/ml 이상일때, 특이도 84.2%, 민감도 40.4% 의감별력으로양성흉수를감별할수있었는데, 이것은민감도에있어서 CEA의감별력과비슷한수준으로흉수 procalcitonin 및 CRP는 CEA와더불어악성흉수감별에보완적인지표로서유용할것이다. 특히림프구가우세한삼출액의감별에있어서악성흉수와결핵성흉수의감별이어려울때가있는데, 본연구에서악성흉수및결핵성흉수환자 108명에대하여흉수에서의 procalcitonin과 CRP를따로분석하였을때흉수 CRP만이결핵성흉수에서통계적으로유의한증가를보였다 (p=0.003). CRP의측정은다른표지자들의측정보다신속하며, 저렴한가격으로측정이쉬우므로감별지표로서의그유용성이더높을것으로판단된다. 결핵성흉수는우리나라에서흉수의가장흔한원인으로알려져있으며확진된결핵성흉수에서도결핵균배양의양성률은 20-30% 에불과하고흉막생검에서건락성괴사를동반하는육아종성병변을관찰하는경우도 50% 를넘지않아임상에서결핵성흉막염을진단하는데어려움이있다. 이에대한감별을위해다양한표지자들의연구가있었으며, interferon (INF)-γ, ADA, IL-18 등이그예이다 21,22. 본연구에서흉수 procalcitonin 측정값이결핵성흉수보다비결핵성흉수에서통계적으로유의한증가를보였는데 (p=0.008), ROC 곡선분석을통해측정값이 0.07 ng/ml 이상일때특이도 74.5%, 민감도 54.0% 로비결핵성흉수감별에도움이될수있으며이때의 ROC 곡선하면적이 (p=0.0036) 이었다. ROC 곡선하면적이 INF-γ 가 1.000, ADA가 0.958로보고한것과비교하면 22 특이도와민감도에서떨어져있으나보완적으로사용될수있는가능성이있겠으며, 이에대한추가연구가필요할것으로판단된다. 흉수의감별을위해염증지표로알려진 CRP와 procalcitonin을흉수에서측정하는것은삼출액과여출액의감별뿐아니라삼출성흉수의다양한원인들에대한감별에보조적으로사용하기에유용할것으로판단된다. 359

8 SH Kim et al: Procalcitonin and C-reactive protein in pleural effusions 요 연구배경 : 흉수의감별을위해염증지표로알려진 procalcitonin과 CRP를흉수와혈청에서각각측정하여, 삼출액과여출액의감별과삼출성흉수의다양한원인들에대한감별에유용한지확인하고자하였다. 대상및방법 : 2005년 10월 1일부터 2006년 9월 30 일까지흉수로입원하였거나입원중에흉수가발생하였던총 198명을전향적으로연구하였으며, 원인질환으로한가지의정확한진단이가능하였던 178명을 분석하였다. 악성흉수 57명, 결핵성흉수 51명, 부폐렴성흉수 31명, 농흉 5명, 기타원인의양성흉수 7명과여출성흉수 27명이었으며, 대상환자의혈청과흉수에서 procalcitonin과 CRP를포함한측정값을비교분석하였다. 결과 : 흉수 procalcitonin과 CRP는여출액과비교하여삼출액에서유의한증가를보였고, 흉수 CRP가 2.06 mg/dl 이상일때특이도 96.3%, 민감도 45.0% 로삼출액을감별할수있었다. 악성흉수와비교하였을때에도흉수 procalcitonin과 CRP는양성흉수에서유의한증가를보였고, 흉수 CRP가 2.28 mg/dl 이상일때특이도 86.0%, 민감도 53.2% 로, 흉수 procalcitonin이 0.11 ng/ml 이상일때특이도 84.2%, 민감도 40.4% 로양성흉수를감별할수있었다. 또한흉수 procalcitonin은비결핵성흉수에서유의한증가를보였다. 결론 : 흉수에서 CRP와 procalcitonin을측정하는것은삼출액과여출액의감별뿐아니라삼출성흉수의다양한원인들감별등에보조적으로사용하기에유용하다. 약 참고문헌 1.
