1 2011 연세대학교의과대학소화기학연수강좌 하부위장관질환의최신가이드라인 연세대학교의과대학내과학교실 홍성필 생활습관과식생활의서구화, 비만인구의증가로최근국내에서대장직장폴립, 대장직장암, 염증성장질환등의하부위장관질환이급격히증가하고있다. 이에따라하부위장관질환의진단및치료에대한관심이높아지고있으며대장직장폴립을진단하고동시에치료할수있는대장내시경의시행건수도증가하고있다. 그러나아직까지대장내시경의적응증이나선별검사, 추적검사에대한국내의진료가이드라인이정립되어있지않아임상에서경험적으로이루어지는것이많은실정이다. 이에따라저자는대장내시경등하부위장관질환의진단법에대한서구의최신가이드라인을알아보고자한다. 대장내시경적응증의최신가이드라인 대장내시경은대장직장폴립이나대장직장암의조기발견을위한선별검사로널리사용되고있다. 그외의대장내시경적응증은다음과같다 (Table 1). 대장조영술이나그외영상검사에서충만결손이나협착이의심되는경우, 위장관출혈병소를찾기위해검사하는경우 ( 혈변, 상부내시경상출혈병소가없는흑색변, 대변잠혈반응검사양성 ), 원인을알수없는철결핍성빈혈, 염증성장질환의진단및활동성평가, 원인을알수없는설사, 수술시병변을찾기위한경우 ( 폴립절제부위, 출혈병소 ), 혈관이상, 궤양, 폴립절제술후출혈을치료하기위한경우, 이물질제거, 급성비독성거대결장및구불창자꼬임의감압술, 협착부위의풍선확장술, 종양에의한협착혹은출혈의고식적치료, 종양위치표시등이다. 미국소화기내시경학회 (ASGE) 에서는평균위험군에서대장내시경이선별검사로사용되는경우 50세이상에서 10년주기로시행하도록권고하고있다. 1 최근의연구결과에의하면초기대장내시경검사결과정상이었던평균위험군에서 5년후에추적대장내시경을시행한경우진행성대장직장종양의발견율이낮았으며구불결장경을이용한폴립절제술의예방효과에관한연구에서도 10년과 16년주기의추적내시경의예방효과가유지됨을보였다. 2,3 비록이러한권고에대한직접적인데이터는부족한실정이나상기의연구결과등을토대로현재가이드라인에서는평균위험군에서 10년주기의대장내시경검사가권고되고있다. 주요한가족력이있는경우는선별대장내시경검사의시행연령을낮추도록권고하고있는데가족성증식성용종증의가족력이있는경우 25세부터 2년주기로대장내시경을시행받거나대장직장암이진단된나이보다 5년일찍시행받도록권고하고있다. 40세이후부터는매년대장내시경을시행받아야한다. 60세이전에대장직장암을진단받은가족력이있는경우는진단연령보다 10년일찍대장내시경을 5년주기로받아야하며선종이발견된경우는 3년주기로받도록권고하고있다 (Table 2). 37

Marking a neoplasm for localization 대장직장폴립및대장직장암선별검사법의최신가이드라인 대장직장암은전통적으로서구에서가장흔한암중하나였으나최근아시아일부국가에서생활습관의서구화로인해발생률이증가하고있고국내에서도대장직장암의유병률은급격히증가하고있다. 북미에서대장직장암의연령보정유병률은인구 10만명당 44.4명으로 3번째로호발하는암이며암사망율에서도 2위에해당한다. 4 한국중암암등록사업에의하면 2006년에서 2007년국내에서대장직장암의발생률은남성에서인구 10만명당 43.5명, 여성에서 23.4명으로남녀에서모두암발생순위 3위로증가하였다. 대장직장암에의한암사망율에서도급격히증가하고있는데대장직장암의암사망율은 35% 증가하였으며남녀모두에서 4위에해당한다. 5-7 따라서대장직장암의사망률을감소시키고대장직장폴립을발견하기위한선별검사가중요하다 (Table 3, 4). 1,8 대변잠혈검사는대장직장암이나 1 2 cm 이상크기의대장직장폴립에서나오는대변내혈액을찾아내기위해고안된검사법으로비교적간단하고저렴하여대장직장암의선별검사법으로널리사용되고있다. 대변잠혈검사법에는 guiaiac 검사법 (gfobt) 과면역화학검사법 (FIT) 이있으며과거 guaiac 검사법을주로사용하였으나 1970년대에민감도를높인면역화학검사법이처음으로제안되었으며, 9 guaiac 검사법중민감도를높인 high-sensitive guaiac 검 38

CT 대장조영술은 가상대장내시경 이라고도불리며, CT 영상기법의놀라운발전에힘입어가는절편의영상획득이가능해지고컴퓨터영상그래픽기술을이용한삼차원내시경기법을통해대장내부를관통하며들여다볼수있게됨에따라가능해진검사이다. 2003년 Pickhardt 등에따르면무증상평균위험군에서 10 mm 이상크기 의진행성샘종에대한 CT 대장조영술의환자별검출민감도는 94%, 특이도는 96% 를보였다. 13 최근 2 개의메타 분석연구를종합하여보면폴립검출에대한 CT 대장조영술의환자별민감도와특이도는 10 mm 이상크기의폴립에대해서는각각 83 85% 와 92 97% 였으며, 6 9 mm 크기의폴립에대해서는 60 70% 와 90 93% 였다. 14,15 결과를종합해보면대장직장암과폴립의선별검사법으로 CT 대장조영술은대장내시경과비슷한결과를보여 1차선별검사로추전되고있으며정상인경우 5년마다시행받도록권유하고있다. CT 대장조영술에서 6 mm 이상의폴립이발견되는경우는대장내시경을시행받도록권유하고있다. 구불결장경검사는수면이필요하지않고외래에서시행할수있어대장직장암과폴립의선별검사로널리시행 39

Guidelines for screening for the early detection of colorectal cancer and adenomas for average-risk women and men aged 50 years and older by ACS and USMTF 되고 있다. 영국에서 55세에서 64세 사이의 약 112,000명의 대조군과 57,000명의 연구군을 대상으로 평균 11.2년 동안 추적한 무작위대조군 연구에서 구불결장경검사가 대장암의 발생률과 그로 인한 사망률을 유의하게 감소시켰 16 그러나 약 2 5% 정도의 환자는 원위부 샘종 없이도 근위부에 진행성 종양이 있을 수 있으므로 주의하여야 다. 17 한다. 결론적으로 비장만곡부나 항문연에서 40 cm까지 관찰하는 구불결장경 검사는 선별검사로 추천되며 원위부 대장에 샘종이 발견되는 경우 대장내시경을 시행받도록 추천되고 있다. 검사 결과 정상인 경우 5년 주기로 구불결 장경 검사를 시행받도록 하고 있으며 gfobt나 FIT와 같이 시행하기도 한다. 대장내시경검사는 전제 대장의 점막을 직접 관찰할 수 있고 동시에 폴립절제술 등의 치료도 시행할 수 있어 선 별검사법으로 널리 시행되고 있다. 특히 효과적인 대장내시경검사를 위해서는 대장정결이 중요하다. 아직까지 대 장내시경 검사가 대장직장암의 발생률이나 사망률을 감소시킨다는 전향적 무작위 대조군 연구는 없다. 그러나 환 자-대조군 연구나 코호트 연구 등을 보면 대장내시경검사는 대장직장암의 발생률과 사망률을 50% 정도 감소시키 40

2011 연세대학교의과대학소화기학연수강좌 는것으로알려져있다. 18-20 폴립절제술을시행한경우는대장직장암의발생률을 80 90% 까지감소시킨다, 그러나이와같은대장암관련사망률감소효과는우측대장에서좌측대장보다낮게나타나는것으로알려져있으며, 18,21 이에대한추가적인연구가필요하다.

6 2011 연세대학교의과대학소화기학연수강좌 는것으로알려져있다 폴립절제술을시행한경우는대장직장암의발생률을 80 90% 까지감소시킨다, 그러나이와같은대장암관련사망률감소효과는우측대장에서좌측대장보다낮게나타나는것으로알려져있으며, 18,21 이에대한추가적인연구가필요하다. 결론적으로대장내시경검사는 50세이상평균위험군에서선별검사로추천되고있으며결과가정상인경우 10년주기로시행받도록권유되고있다. 대장직장폴립절제후추적대장내시경검사의최신가이드라인 대장직장폴립절제를받은환자는폴립재발에대해대장내시경을이용한주기적인감시검사 (surveillance) 가필요하다 최근대장직장암선별검사로대장내시경검사가널리이용되면서대장직장폴립의진단과절제가빠르게증가하고있다. 25,26 이로인해폴립절제후추적검사수요가가파르게증가하고있지만폴립절제후감시는선별대장내시경검사보다대장직장암예방효과가작아제한된의료자원을효율적으로사용하고, 불필요한감시검사로인한의료비용과합병증을감소시키기위해서적절한간격의효과적인폴립절제후감시전략이필요하다. 27,28 폴립절제후진행성샘종이발생할위험이낮은저위험군은기준대장내시경에서 10 mm 이하, 2개이내의저등급이형성의관샘종으로정의할수있다 (Table 5). 1993년 National Polyp Study에서폴립절제후 1년째및 3년째추적검사를시행받은군과 3년째한차례추적검사를시행받은군에서양군모두 3.3% 라는낮은진행샘종발생률을보인이후이를토대로폴립절제후적절한감시검사시기를 3년으로제시하였다 년 Lieberman 등이발표한저위험군에서감시대장내시경검사간격에관한무작위대조연구결과에의하면 10 mm 미만의관샘종을제거했던환자에서기준대장내시경 2년후및 5년후에두차례검사를시행하였던군과 5년후한차례검사를시행하였던군에서진행성샘종의발생률이각각 5.5%, 6.4% 로의미있는차이가없었다. 30 이러한연구결과를토대로최근미국의폴립절제후감시가이드라인에서는저위험군의경우기준대장내시경 5년후에감시대장내시경검사를시행할것을권고하고있다. 폴립절제후진행성샘종이발생할위험이증가하는고위험군은초기검사에서 10 mm 이상크기의샘종, 융모 / 관융모샘종, 고등급이형성샘종과 3개이상의샘종으로정의할수있다. 결론적으로기준대장내시경에서 1 cm 이상, 3개이상, 고등급이형성, 융모성이있는샘종이있는고위험군에서는 Table 5. 8 2011 연세대학교의과대학소화기학연수강좌 3년후추적대장내시경검사를권유하며 1개혹은 2개의 1 cm 이하관상샘종이있는저위험군에서는 5년뒤추적대장내시경검사를권유하고있다 (Table 6, Fig. 1). 1 cm 이하, 5개이내의구불결장직장과형성폴립은대장직장암의위험성을증가시키지않으므로정상으로간주한다. 그러나 1 cm 이상이거나 5개이상의과형성폴립이발견된경우는 5년후감시대장내시경검사를권유한다. 특히구불결장보다근위부대장에서 5개이상의과형성폴립이있으면서 2개이상이 1 cm 이상이거나전체대장에 30개이상의과형성폴립이관찰되는과형성폴립증환자나그러한환자의직계가족은매년대장내시경검사를받아야한다. 2 3 cm 이상의무경성폴립의경우분할절제될가능성이놓고재발률이 28% 에이를정도로높아 3 6개월뒤추적대장내시경검사가추천된다. 결 론 현재국내에서는다학제기반으로대장직장암및폴립의선별검사법및폴립절제술후감시대장내시경검사에대한가이드라인이재정중이다. 국내임상현실에맞는가이드라인이재정된다면좀더올바른시술이이루어질수있을것이다. 참고문헌 1.
