1 B-IV. 소장캡슐내시경완전정복 Room B 아주대학교의과대학소화기내과학교실 Case Study I: Common, but Important Findings in Small Bowel CE Sung Jae Shin Department of Gastroenterology, Ajou University School of Medicine, Suwon, Korea 서론 2000년초까지소장질환의진단에있어방사선검사나내시경검사의유용성은매우제한적이었다. 그러나캡슐내시경 (video capsule endoscopy, VCE) 및이중풍선소장내시경 (double balloon enteroscopy, DBE) 등이임상에적용된 2000년이래소장질환의진단적결함을줄이는데크게기여하고있다. 1,2 특히, 캡슐내시경은기존의소장검사법인영상의학적검사나소장내시경검사와비교하여검사에따른환자의고통이없고, 간편하며, 소장병변발견에유용한검사법으로알려져있다. 이러한캡슐내시경의적응증 (Table 1) 으로는원인불명의위장관출혈, 크론병, 소장종양, 선천성폴립등이 Table 1. Capsule Endoscopy Contraindications (1) cardiac devices pacemaker/defibrillator (relative) (2) major abdominal surgery less than 6 months (relative) (3) any swallowing disorder (4) non compliance (5) previous history/suspected small bowel obstruction (6) pregnancy 의심되는경우이며, 금기로는 (Table 2) 연하곤란, 장폐색, 복부수술, 복부와골반부에방사선치료병력, 소장협착이확인된크론병인경우캡슐저류의위험이있으므로캡슐내시경전에캡슐내시경이통과가가능한지반드시확인한후시행한다. 1 3 이글에서는이러한캡슐내시경의적응증중캡슐내시경시흔히볼수있는질환에대하여알아보고자한다. 본론 1. 원인불명의위장관출혈 원인불명의위장관출혈이란위내시경과대장내시경검사로원인을알지못하는위장관출혈이반복적, 혹은지속적으로발생하는경우를지칭하는데, 위, 대장내시경검사에서출혈병변을찾지못하면가급적캡슐내시경검사를빠른시간내에시행하는것이좋다. 출혈속도가 1분당 0.5 ml 이상인대량의위장관출혈인경우출혈부위를확인하는데혈관조영술이도움이되나그렇지않은경우캡슐내시경이소장의출혈부위를찾는데도움이된다. 4 7 원인불명의위장관출혈의가장흔한원인질환은혈관이형성증 (angiodysplasia), Dieulafoy 병변, 정맥확장증 (venous ectasia), 림프관확장증 (lymphangiectasia) 등혈관성질환으로알려져있으며그외원인질환으로종양, 궤양, 게실증등이있다. 8 1) Angiodyplasia (synonyms: vascular ectasia, Angioectasia, telangiectasia, arteriovenous malformation) 혈관이형성증은위장관에서모세혈관이점막혹은점막하에확장된것으로원인불명의위장관출혈의가장많은원인으로알려져있다 (Fig. 1). 9,10 이러한혈관이형성증의출혈 120 Korean Society of Gastrointestinal Endoscopy

A B 과연관된요인으로는유전적, NSAIDs 및항응고제의복용등과관련이있다고알려져있으며, 또한, 나이가증가할수록, 혈액투석 (hemodialysis), 심부전 (heart failure), 대동맥협착증 (aortic stenosis) 그리고 von Willebrand`s syndrome, 방사선치료를받은경우증가한다.

한연구에의하면 Dieulafoy`s lesion이발견된 10개의예에서 9개가공장에있었으며, 1개가회장에관찰되었다고보고하였는데, 활동성출혈이아닐경우에는캡슐내시경으로병변의발견은어려우나출혈시다량의출혈을동반할수있으므로주의를요한다.

A B 과연관된요인으로는유전적, NSAIDs 및항응고제의복용등과관련이있다고알려져있으며, 또한, 나이가증가할수록, 혈액투석 (hemodialysis), 심부전 (heart failure), 대동맥협착증 (aortic stenosis) 그리고 von Willebrand`s syndrome, 방사선치료를받은경우증가한다. 11,12 2) Dieulafoy's lesion Dieulafoy's lesion은점막하에큰직경을가지는돌출된병변 (protruding large caliber submucosal artery) 으로재출혈이흔하며보통동맥의크기는 2 mm 이상이다 (Fig.2). 이병변은위에가장흔하며소장에는드문것으로알려져있다. 한연구에의하면 Dieulafoy`s lesion이발견된 10개의예에서 9개가공장에있었으며, 1개가회장에관찰되었다고보고하였는데, 활동성출혈이아닐경우에는캡슐내시경으로병변의발견은어려우나출혈시다량의출혈을동반할수있으므로주의를요한다. 13 2) Venous Ectasia or Varix 큰정맥확장증은캡슐내시경상에서간혹관찰되나작은 Fig. 4. Capsule endoscopic view showing the diffusely edematous intestinal mucosa covered with enlarged whitish villi. 정맥의경우는관찰하기가어렵다. 그러나이러한정맥확장이간경화와관련되어있을경우는출혈의잠재적위험이있으므로주의깊은관찰이요구된다 (Fig. 3). 14 3) 림프관확장증 (Lymphangiectasia) 림프관확장증을동반할경우소장의 villi는부어 (swollen) 보이며, 점막에 white spots 형태를띠게되는데이러한원인은 chylomicrons이쌓이게되어 lymphatic capillary 가막혀확장이되기때문이다 (Fig. 4). 보통의경우감염, 크론병같은염증, 복강내종양및방사선소장염등이있을때 2차적으 제 52 회대한소화기내시경학회세미나 121

(B) Multiple epithelial mass with fungoid growth, diagnosed histologically as lymphoma. protectin과 CECDAI보다 더 높은 상관관계를 보이는 것으로 19 보고 되었다. 크론병을 의심할 수 있는 캡슐 내시경 소견은 일반적으로 궤양(ulceration), 발적(erythema) 점막 부종(mucosal edema) 및 협착 (stricture)를 포함하는 데, 이는 일반적으로 비특이적 인 소견이다(Fig. 5). 정상인에서도 10 15%에서는 경미한 상 기 병변이 관찰된다고 알려져 있으며, 또한, NSAIDs를 복용하 는 경우 점막의 미란이 약 2/3에서 발견된다고 알려져 있기 때 문이다. 따라서 NSAIDs에 의한 손상과 감별하기 위하여 캡슐 내시경 시행 약 1개월 전부터는 이러한 약물의 중단이 필요하 20 다. 소장의 아프타 병변/미란/궤양은 크론병과 NSAID 유발 장염 외에도 림프종, chronic nonspecific multiple ulcer of 21 the small intestine와 같은 다양한 질환과 감별이 필요하다. 15 로 나타날 수 있다. 3. 소장 종양 (Small bowel tumor) 2. 크론병 캡슐내시경은 크론병 진단이 의심되는 경우나, 혹은 질환 의 활성도 및 범위를 예측하는데 도움을 준다. 이러한 캡슐내 시경은 소장촬영, ileo colonoscopy, push enteroscopy 및 CT enterocolysis에 비해 진단율이 우수하다고 알려져 있다. 정상 캡슐내시경 검사 결과는 활동성 크론병을 배제할 수 있 16 는 높은 음성 예측도(negative predictive value)를 보인다. 이미 크론병이 진단된 환자에서 캡슐내시경 검사는 점막치유 를 평가하여 치료 방침을 결정하는 데 도움이 될 수 있으며, 질병 활동성을 평가할 수 있다. 캡슐내시경 검사에서 크론병 의 활동성을 평가하는 척도로 루이스 점수(Lewis score)와 캡슐 내시경 크론병 활동 지수(capsule endoscopy Crohn s dis17,18 ease activity index, CECDAI)를 이용할 수 있다. CECDAI 는 최근에 타당성이 검증되었으며, 루이스 점수는 대변 cal- 122 대부분의 소장 종양은 빈혈과 원인 불명의 위장관 출혈을 22 동반하며, 뒤늦게 복통 및 체중 감소를 동반할 수 있다. 대 부분의 소장 종양은 선암, 신경 내분비 종양, 림프종, GIST (gastrointestinal stromal tumor) 등과 같은 악성 종양과 혈관 종, 과오종, 선종, 지방종 같은 양성 종양, 그리고 전이암 등이 발생할 수 있다(Fig. 6). 캡슐내시경의 도입으로 인해 소장 종 양이 새롭게 조명되었는데, 소장 촬영에 비해 진단율이 높으 며, MRI와 비교해서도 좀 더 작은 소장의 종양을 발견할 수 있다고 알려져 있고, 캡슐내시경 시행환자의 6 9%에서 소장 종양이 발견됨이 보고 되고 있다. 최근에는 위양성을 피하기 위해서 캡슐내시경에서 점막 돌출(mucosal bulge)를 종괴와 감별하기 위해서 최근 Smooth Protruding Index on Capsule Endoscopy (SPICE score)가 개발되었으며, 대상 환자가 적기 는 하지만 SPICE score 2 경우 소장 종양 진단에 대한 민감 Korean Society of Gastrointestinal Endoscopy

도 83% 와특이도 89% 로보고하였다. 23 4. 선천성폴립증후군 (Inherited polyposis Syndrome) 캡슐내시경은가족성용종증후군 (Familiar polyposis syndrome) 혹은 Peutz Jeghers syndrome 등과같은용종증후군의진단에유용한수단이다 (Fig. 7).

캡슐내시경검사는또한소장폴립이호발하는 Peutz Jeghers 증후군환자에서도소장조영술에비해더좋은진단능력을보였는데, 소장조영술의경우약 12.5% 의환자에서용종이발견된반면, 캡슐내시경에서는약 27.5% 에서 발견되었고발견된용종의개수또한조영술의평균 12개에비해 44개로많았다. 5.

4 도 83% 와특이도 89% 로보고하였다 선천성폴립증후군 (Inherited polyposis Syndrome) 캡슐내시경은가족성용종증후군 (Familiar polyposis syndrome) 혹은 Peutz Jeghers syndrome 등과같은용종증후군의진단에유용한수단이다 (Fig. 7). 24,25 27 십이지장용종을가지고있는가족성용종증후군환자에서캡슐내시경을시행한환자의약 24 57% 에서공장및회장에용종이발견되었다고알려져있다. 그러나십이지장은캡슐이매우빠르게통과하기때문에위음성을보일가능성이높기에주의를기울여야한다. 캡슐내시경검사는또한소장폴립이호발하는 Peutz Jeghers 증후군환자에서도소장조영술에비해더좋은진단능력을보였는데, 소장조영술의경우약 12.5% 의환자에서용종이발견된반면, 캡슐내시경에서는약 27.5% 에서 발견되었고발견된용종의개수또한조영술의평균 12개에비해 44개로많았다. 5. NSAIDs induced small bowel injury 비스테로이드소염제에의한소장병증의진단을위해서는첫째, 비스테로이드소염제복용력이있어야하고, 둘째, 내시경적미란, 궤양혹은 diaphragm like stricture 등이관찰되어야하며, 셋째, 이러한약물의중단에의해임상증상의호전이있어야하며, 마지막으로이러한증상을일으킬수있는염증성장질환, 종양및감염성질환등의배제가필요하다. 28 Maiden 등 29 은캡슐내시경에의한소장병변을 reddened folds, denuded area, red spot, mucosal break, blood 등으로분류하였고 (Fig. 8), declofenac 을 2주간복용한 40명의환자에서 mucosal breaks 등의병변이 16명 (40%) 에서관찰됨을보고하였다. Graham 30 은캡슐내시경소견을 red spots, small erosions, large erosions and ulcer 등으로분류하였는데, 지속적으로비스테로이드소염제를복용한환자 21명중 13명 (62%) 에서소장점막병변이발견되었다고하였다. diaphragm like stricture는비스테로이드소염제에의한점막손상의특징정인소견으로소장점막의반흔흔적이며경우에따라서장폐쇄를보일수있다. 조직학적으로점막하섬유화소견을보이나고유근층이손상되지않아장폐쇄시이중풍선소장내시경을이용하여풍선확장술시행시천공등의위험은낮다고알려져있다. 31,32 결론 Fig. 7. 5 장질환의진단에많은발전을가져왔으며, 매우안전하고간편히시행할수있다. 이러한캡슐내시경에있어가장유용한질환은원인불명의위장관출혈, 소장크론병과종양이의심되는경우, NSIADs 등에의한소장손상등이지만, 다른소장질환에서도캡슐내시경의유용성에대한자료가축적되고있어캡슐내시경은적응증은확대될것으로생각된다. 추후캡슐내시경은화질의선명성, 소화관내에서조작 (manipulation), 조직검사등이가능한방향으로발전되리라생각한다. 참고문헌 1.
