Several studies have shown that patients with RA have a 2~3x increased risk of developing lymphomas as compared with the general population. TNF-α antagonist therapy might be independently associated with an increased risk of lymphoma; however, other studies have shown that the risk of lymphoma is increased in RA patients with severe disease activity rather than the treatment per se. We experienced a 65-year-old woman who developed Etanercept associated malignant lymphoma with rheumatoid arthritis. (Korean J Med 77:S1342-S1346, 2009) Key Words: Etanercept; Malignant lymphoma; Rheumatoid arthritis 서론류마티스관절염은관절활막의염증을특징으로하는만성염증성질환으로, 많은다른질환들과연관되어있으며, 특히악성림프종발생위험이일반인구의 2~3배높은것으로알려져있다 1,2). 특히최근류마티스관절염의증상호전및질병의활성도완화에탁월한효과가증명된종양괴사인자-알파길항제 (Tumor necrosis factor-alpha antagonist) 와악성림프종과의관련성에대한많은연구가진행중이다 3,4). Brown 등은 infliximab과 etanercept로치료한류마티스관절염환자에서악성림프종발생률을각각 100,000명당 6.6 명과 19명으로보고하였다 5). 그러나류마티스관절염환자에서 TNF-α 길항제치료와악성림프종발생간의상관관계를증명할명확한증거는아직부족한상태이며 2), 국내보고는드물다 6). 본저자는 Etanercept으로치료한류마티스관절염환자에서발생한악성림프종 1예를경험하여보고하고자한다. mizoribine 50 mg/day) 및 methylprednisolone 8 mg/day으로치료받던중내원 1년전부터관절증상악화되어 TNF-α 길항제 (Etanercept) 치료를시작하였다. 치료시작당시환자는조조강직이 60분이상지속되었으며압통관절수는 37개, 종창관절수 32개, 적혈구침강속도 12 mm/hr, C-반응단백 2.

과거력및가족력 : 20년전당뇨, 고혈압및만성 B형간염으로진단받고, 인슐린, 항고혈압제및항바이러스제제 (lamivudine) 복용중이었다. 신체검사소견 : 내원당시활력징후는혈압 115/61 mmhg, 체온 36.0, 맥박 130회 / 분, 호흡수 20회 / 분이었다. 심음은동성빈맥이었고, 잡음은청진되지않았으며, 양쪽하부호흡음이감소되어있었다.

A case of Etanercept -associated malignant lymphoma - 증 례 환자 : 65세, 여자주소 : 전신쇠약감, 가슴답답함현병력 : 내원 6년전부터시작된다발성관절통으로본원에서류마티스관절염으로진단받고 NSAID (celecoxib 400 mg/day), DMARD (methotrexate 10 mg/week, mizoribine 50 mg/day) 및 methylprednisolone 8 mg/day으로치료받던중내원 1년전부터관절증상악화되어 TNF-α 길항제 (Etanercept) 치료를시작하였다. 치료시작당시환자는조조강직이 60분이상지속되었으며압통관절수는 37개, 종창관절수 32개, 적혈구침강속도 12 mm/hr, C-반응단백 2.2 mg/dl, 류마티스인자 163 IU/mL, 항CCP항체음성이었다. 외래에서경과관찰중압통및종창관절수각각한개로크게호전양상보여 etanecept 치료유지하던중내원 1개월전부터시작된전신쇠약감, 가슴답답함을주소로응급실내원하였다. 과거력및가족력 : 20년전당뇨, 고혈압및만성 B형간염으로진단받고, 인슐린, 항고혈압제및항바이러스제제 (lamivudine) 복용중이었다. 신체검사소견 : 내원당시활력징후는혈압 115/61 mmhg, 체온 36.0, 맥박 130회 / 분, 호흡수 20회 / 분이었다. 심음은동성빈맥이었고, 잡음은청진되지않았으며, 양쪽하부호흡음이감소되어있었다. 왼쪽빗장위림프절에 cm로촉지되는림프절종대가있었으며, 복부촉진시에간및비장종대소견은없었다. Chest radiograph showing cardiomegaly and mild pleural effusion in both lower lung fields. 검사실소견 : 말초혈액검사에서백혈구 11,260/mm 3 ( 호중구 83.4%, 림프구 11.1%) 혈색소 11.7 g/dl, 혈소판 253,000/ mm 3 이었다. 적혈구침강속도는 48 mm/hr, C-반응단백은 6.8 mg/dl 이었다. 혈청생화학검사상총단백 6.1 g/dl, 알부민 2.5 g/dl, AST 10 IU/L, ALT 10 IU/L 였으며, 총빌리루빈 0.2 mg/dl, alkaline phosphatase 74 IU/L, LDH 283 IU/L, BUN 20 mg/dl, creatinine 0.7 mg/dl이었다. 종양표지자검사에서 CA19-9은 61.2 U/mL, CA125는 94.2 U/mL 였고, 혈액응고검 A B Figure 2. 혈청바이러스검사에서 HBs Ag 양성, Anti-HBs 음성, HBe Ag 음성, anti- HBe 양성, HBV DNA (PCR hybridization) 894,000 copies/ml, anti-hiv 음성및 ANA 음성이었다. 방사선학적소견 : 단순흉부촬영에서심비대소견과함께이전에관찰되지않던양측흉수소견이관찰되었다 ( 그림 1).

Based on immunohistochemical staining, the tumor cells were positive for CD 20 ( 200) (B). 사상 PT 68% (INR 1.33), aptt 34.6 sec이었다. 혈청바이러스검사에서 HBs Ag 양성, Anti-HBs 음성, HBe Ag 음성, anti- HBe 양성, HBV DNA (PCR hybridization) 894,000 copies/ml, anti-hiv 음성및 ANA 음성이었다. 방사선학적소견 : 단순흉부촬영에서심비대소견과함께이전에관찰되지않던양측흉수소견이관찰되었다 ( 그림 1). 심장초음파에서는소량의심낭액이존재하였고, 심실박출량이나심근벽의운동장애는발견되지않았다. 흉부및복부전산화단층촬영에서왼쪽목정맥, 빗장위, 겨드랑, 문맥대정맥및위주위림프절종대가관찰되었다 ( 그림 2A, 2B). 악성종양가능성을고려하여시행한위내시경, 대장내시경에서는특이소견발견되지않았다. 조직학적소견 : 좌측빗장위림프절이촉지되어수술장에서절제및조직생검을시행하였다. 병리소견에서크고작은림프구성세포들의침윤이보였고, 세포핵의다형성이관찰되었으며 ( 그림 3A), 면역조직화학검사에서 cytokeratin (CK) 에음성, leukocyte common antigen (LCA) 및 CD 20에양성소견 ( 그림 3B) 보여 B-미만성대세포림프종 (diffuse large B cell lymphoma, DLBCL) 을진단하였다. B symptom은없었으며, 병기는 IIIA였고, 국제예후인자는 4점, high였다. 골수검사소견 : 골수생검및흡인검사에서악성림프종의골수침범증거는없었다. 치료및경과 : 악성림프종진단당시 etanercept를사용한지 46주째였으며, 진단이후 etarnercept 및 methotrexate는모두중단한상태로항암화학요법시행예정이었으나, 환자전신상태양호하지못하여경과관찰하던중타병원으로전원 되어항암화학요법시행과정중사망하였다. 고찰류마티스관절염은관절활막의염증을특징으로하는만성염증성질환으로, T세포기능의결함이악성림프종발생의주된위험요소이며 1,7), 일반인구에비해발생위험이 2~3배높은것으로알려져있다 2). 특히, 류마티스인자가양성이면서발병후초기 5년이내에악성림프종발생위험이높은것으로보고되었다 7). 또한류마티스관절염자체뿐만아니라, 치료제로사용되는 DMARD (azathioprin, methotrexate) 도악성림프종발생을증가시킨다고알려져있다 7). TNF-α 길항제는류마티스관절염에서탁월한효과가증명된치료제로, 상대적으로안전성이입증되어있지만, 일부중요한부작용이알려졌다. 이부작용으로는감염, 주사부위부작용, 루푸스양증후군, 자가항체의발현, 심부전, 탈수초질환, 림프종, 백혈병, 고형종양등이보고되었다 8-10). TNF-α는 B세포관련종양발생을억제하는 T세포의세포독성반응을촉진시키며, 이러한이유로악성림프종발생과관련성에관심이모아졌다 7). 그러나 TNF-α가악성종양을억제하는기전과 TNF-α를억제하는것이어떻게악성종양발생을촉진시키는지여부에대해아직명백히밝혀진바는없다 12,13). 미국 Food & Drug admistration (FDA) 에서 1999년 4월부터 2000년 12월까지 TNF-α 길항제인 etanercept와 infliximab 치료후각각 18/95,000명과 8/121,000명의악성림프종발생을보고하였고, 발병까지의기간은각각평균 8주 - S

Etanercept 치료후발생한악성림프종 1 예 - 와 6주였다 5,7). 본증례의환자에서는 etanercept 치료 46주만에악성림프종이발생하였으며, 다른보고에비해치료후상당한시간이경과한후발병한예였다. 국내에서유사한보고 6) 가있었으며 64세여자가류마티스관절염치료위해 etanercept유지 8주만에 Epstein-Barr 바이러스와관련된림프증식성질환이발병하였고, etanercept 치료중지후임파선종대가호전양상을보였던예로, 치료제와림프증식성질환과의상관관계를강하게시사하였다. 본증례에서는치료 46주에악성림프종이발생하였고, 치료중지후에도임파선종대가호전되지않았으며, 바이러스와의관련성도확인되지않은점에서차이가있다고하겠다. 류마티스관절염환자에서 TNF-α 길항제치료와악성림프종발생과의밀접한관련성에대한많은연구들이있었으나위의결과들과상반되는보고도있었다 14,15). 류마티스관절염환자중주로중증도가높은 15~20% 의환자들에게 TNF-α 길항제로치료하며, 그러한환자들에서악성림프종의위험도가증가하므로질병활성도가악성림프종의발생에중요한역할을한다는주장이제기되었다 14,15). 최근의대규모환자- 대조군연구에의하면악성림프종의발생위험은치료제자체보다는류마티스관절염의질병활성도와더큰상관관계가있음을보고하였다 16). 또한 18,572명의류마티스관절염환자를대상으로 (National Data Bank for Rheumatoid Disease) 시행한연구에서는악성림프종이발생한상대위험도는대조군과비교하여 etanercept로치료한환자군에서 3.8, infliximab 로치료한환자군에서 2.6으로나타났다. 하지만이증가된위험도가류마티스관절염자체에의한것인지, 치료와관련된부작용으로증가하였는지대해서는논란의여지가있다 2,16). Wolfe 등에의하면기존에사용하던 DMARD (methotrexate) 나 TNF-α 길항제와악성림프종간의관련성을증명하지못했다고보고하였다. 이연구에의하면 methotrexate 단독치료군과 TNF-α 길항제와 methotrexate를같이치료한복합치료군사이의악성림프종발생율을조사한결과두군간의큰차이를보이지않아치료제와질병발생사이의상관관계를증명하지못했다 18). 또한기존에사용하던 DMARD, 항염증제제 (steroid) 및 Epstein-Barr 바이러스와역시도악성림프종을유발하는위험인자가아니라는보고도있었다 16). 본증례환자는 TNF-α 길항제치료후악성림프종발생까지다른증례에비해오랜시간이경과되었고, 류마티스관절염유병기간도길었던예로치료제자체의가능성과 질병활성도와관련된발생가능성을모두고려해볼수있겠다. TNF-α 길항제는류마티스관절염에획기적인치료제로현재보편적으로널리이용되는치료제로, 악성림프종발생의위험을증가시킨다는명백한증거는없지만 17) 류마티스관절염환자에게 TNF-α 길항제치료여부를결정할때신중을기해야할것으로생각된다. 요 1998년이래로 TNF-α 길항제는중증류마티스관절염의치료에탁월한치료효과와함께비교적안전하게널리치료제로이용되고있다. 그러나악성림프종발생위험을높인다는많은보고들과함께, 악성림프종발생이치료제자체보다는류마티스관절염의질병활성도와더큰상관관계가있다는주장도제기되고있는상태이다. 본저자들은아직까지국내에보고가드문 etanercept로치료한류마티스관절염환자에서발생한악성림프종을경험하여참고문헌과함께보고하는바이다. 약 중심단어 : 류마티스관절염 ; 악성림프종 ; Etanercept REFERENCES 1) Baecklund E, Askling J, Rosenquist R, Ekbom A, Klareskog L.
