The therapeutic approach should be based on the cause of pain, which should be determined by the appropriate evaluation. Recovery of range of motion, strengthening of rotator cuffs and other shoulder muscles, and scapular stabilization are the main principles and purposes of shoulder rehabilitation. In this article, we introduce diagnosis-specific management approaches for common presentations of shoulder pain such as adhesive capsulitis, impingement syndrome or rotator cuff tendinosis, and calcific tendinitis. The shoulder capsule is expanded (arrow heads) after intraarticular hydraulic distention. 1. 물리치료 유착성견관절낭염의 초기, 즉 통증이나 관절 가동 범위 의 제한이 심하지 않을 때는 핫팩(hot pack), 간섭파 전류 자극 치료(interferential current therapy)나 초음파를 이 용한 온열치료와 함께 관절 가동범위운동(range of motion exercise), 스트레칭 등의 물리치료를 시행할 수 있고 비스테 로이드성소염제의 경구투여나 패치를 이용한 국소치료를 함 께 적용해 볼 수 있다[1,2]. 온열치료를 통해 관절을 부드럽게 만든 후 관절 가동범위운동 및 스트레칭을 시행하는 것이 효 과적이며, 말단 범위(end range)에서 어느 정도의 통증을 유 발할 정도로 수동적 힘(passive range of motion)을 가하여 관절 가동범위를 늘리는 것을 목표로 한다. 특히 견관절의 외 회전과 내회전 방향으로 스트레칭을 집중적으로 시행해야 하 며 수건이나 봉 등을 이용하여 환자가 스스로 시행하도록 할 수도 있으나(Figure 1), 스스로 하는 경우 통증으로 인해 스 트레칭을 충분히 하지 않는 경향이 있어 물리치료사 등 타인 에 의해 수동적 스트레칭을 하는 것이 더 효과적이다. 2. 관절강 내 주사요법 온열치료나 스트레칭 등의 물리치료로 관절 가동범위의 회복이 지연되거나 진행이 되지 경우에는 관절강 내 스테로 이드(intra-articular steroid) 주사를 시행해 볼 수 있다. 한 차례의 주사만으로 빠른 회복이 일어나는 경우도 있지만 주 사 후에도 스트레칭을 적극적으로 시행해야 관절 가동범위 가 회복되는 경우가 많다[3,4]. 유착성견관절낭염의 경우에 는 단순한 관절강 내 스테로이드 주사보다는 국소마취제(1% lidocaine)과 생리식염수를 혼합하여 총 20 ml 이상의 주 사액(1% lidocaine 5 ml + 40 mg/1 ml triamcinolone + normal saline)을 초음파 유도 하에 관절강 내로 주입하는 수 압팽창술을 시행해 볼 수 있다[5]. 초음파 유도 하 수압팽창술 은 어깨관절의 후방에서 접근하여 초음파 유도 하에 주사침 을 관절강 내로 삽입한 뒤 주사액을 주입하는 방법으로 시행 한다. 유착이 심한 경우 시술자가 주입 과정에서 강한 저항을 느낄 수 있으며, 초음파를 통해 관절낭이 부풀어 오르는 모습 을 실시간으로 확인할 수도 있다(Figure 2). 이전에는 주사액 을 충분히 주입하여 관절낭을 파열시키는 방법이 많이 이용 되었으나, 최근에는 실시간으로 관절강 내 압력을 측정하면 서 관절낭을 최대한 부풀리되 파열시키지 않는 방법도 보고 된 바 있다[6]. 수압팽창술을 시행한 뒤에도 지속적으로 물리 치료 및 관절 스트레칭을 시행하는 것이 좋으며, 호전 속도가 느릴 경우 시술을 1-2차례 더 시행하기도 한다. 3. 기타 이외에 마취 하 도수조작법 등의 치료방법도 소개되어 있 지만 초음파 유도 하 수압팽창술에 비해 효과가 우월하지 않 고 신경이나 근육손상 등의 위험성이 있어 흔히 시행되지는 662 대한의사협회지

Informed consent was received from the subject. 않는다. 유착성견관절낭염은 2-3년 정도의 자연경과에 따 라 호전되는 것으로 알려져 있지만[7], 관절 가동범위의 제 한으로 인해 일상생활동작 수행능력이 크게 저하될 수 있기 때문에 가능한 빨리 치료를 시작하는 것이 중요하다. 충돌증후군 및 회전근개건증 회전근개건증은 퇴행성 변화나 어깨충돌증후군, 즉 어깨 관절의 생체 역학이나 구조적인 문제로 인하여 회전근개의 반복적인 마찰이 발생하는 현상 등에 의해 발생한다. 주로 팔을 어깨 높이 위로 들어올릴 때 통증이 발생하며 마찰로 인하여 윤활낭의 염증(bursitis)이나 회전근개손상이 유발되 고, 심할 경우 회전근개파열로 이어질 수 있다. 1. 염증 및 통증의 완화를 위한 주사요법 회전근개건증의 치료목적은 통증 없이 어깨를 움직일 수 있도록 하고, 염증이 가라앉고 통증이 어느 정도 해결이 되 면 운동을 통해 정상적인 생체역학을 되찾도록 하는 것이다. 초기에는 비스테로이드성 소염제를 투약하거나, 초음파 유 도 하에 스테로이드 주사를 시행하는 것이 효과적이다. 회 전근개 부분 파열이 관절낭 쪽에 나타날 경우에는 관절강 내 스테로이드 주사를 시행하고, 부분 파열이 윤활낭 쪽에 관찰되는 경우에는 견봉하 삼각근하 윤활낭(subacromial subdeltoid bursa)에 스테로이드를 주입한다. 관절강 내 주 입을 하는 경우에는 유착성견관절낭염에서와 마찬가지로 후 방에서 접근하는 것이 용이하고, 윤활낭 내 주입을 시행하는 경우에는 환자의 어깨를 내회전 시킨 뒤 극상근 위에 초음파 탐식자(probe)를 위치시켜 윤활낭 내로 주사침을 삽입한다. 주사액은 주로 스테로이드(triamcinolone 40 mg/1 ml)와 국소마취제(1% lidocaine)를 1:1로 섞어서 목표 부위에 주입 한다. 반복된 스테로이드 주사는 힘줄을 약화시키고 전신적 부작용을 유발할 수 있으므로 주의해야 하며, 환자의 스테로 이드 주사력을 정확히 파악하고 시술 여부를 결정해야 한다. 2. 운동요법 비록 약물치료나 주사치료 등으로 통증의 경감을 얻었다고 하나 이러한 치료는 염증의 감소를 통해서 얻어진 것이지 이 를 완치로 볼 수는 없다. 환자의 어깨주위의 생체역학이 이러 한 질환의 발생과 밀접한 연관을 가지고 있어 재발을 방지하 고 정상적인 생체역학을 되찾도록 하기 위한 노력이 필요하 다. 따라서 스트레칭을 통해 관절 가동범위를 정상화하고 이 후 회전근개 근력강화운동 및 견갑안정화운동을 시행하여야 한다[8,9]. 스트레칭은 물리치료사가 시행해 줄 수도 있으나 봉을 이용하여 스스로 운동하는 방법(Figure 1)이나 벽밀기 운동(Figure 3)이 도움이 된다. 벽밀기운동은 벽 앞에서 건 흔히 발생하는 어깨통증에 대한 보존적 치료 663

Informed consent was received from the subject. 측 손으로 환측 팔꿈치를 받치고 벽을 따라 팔을 서서히 위로 밀어올리는 운동방법으로, 팔이 더 이상 올라가지 않을 때 몸 을 벽으로 다가가도록 하면서 팔을 조금씩 더 위로 밀어올려 5초 정도 머물고 다시 내려오는 방법으로 시행한다. 회전근개 근력강화운동은 theraband 등과 같이 탄력성이 있는 줄 등을 이용하여 신전, 전방굴곡, 내회전, 외회전의 모 든 방향으로 시행하는 것이 효과적이다(Figure 4). 줄의 한 쪽 끝을 팔꿈치 높이 정도의 문고리에 묶은 뒤 각 방향으로 운동을 시행하는 것이 좋다. 회전근개손상이 있는 경우 견갑안정화운동을 통해 어깨관 절을 안정화시키고 주변 근육들의 근력을 강화해야 하는데, 낮은노젓기운동(low row exercise, Figure 5), 잔디깎기운동 (lawnmower exercise, Figure 6), 그리고 시계운동(Figure 7) 이나 벽닦기운동(Figure 8)과 같은 닫힌운동사슬운동(close kinetic chain exercise)이 유용하다. 낮은노젓기운동은 몸통 을 신전시키고 견갑골을 후퇴(retraction)시킨 상태로 환측 손 을 뻗어 책상 모서리에 댄 뒤, 손바닥을 뒤, 엉덩이 쪽 방향으 로 힘을 주는 운동으로 견갑 안정화의 기본이 되는 운동이다 [10,11]. 잔디깎기운동은 몸통을 구부린 상태에서 건측 방향 으로 약간 회전하여 환측 팔을 건측 무릎 부위로 오게 한 뒤, 몸통을 세우면서 다시 반대쪽으로 회전하여 견갑골을 당기는 느낌으로 환측 팔을 당겨서 손을 허리 높 이에 오도록 하는 운동방법이다[10]. 시 계 운동은 벽에서 한 발자국 정도 떨어 진 위치에서 손바닥을 어깨 높이 이하로 벽에 댄 뒤, 손바닥을 고정한 상태에서 어깨를 천천히 위, 아래, 앞, 뒤 방향으 로 순서대로 움직이면서 각 상태에서 5 초 정도 머무르는 운동이다. 벽닦기운동 은 벽 옆에 서서 건측 다리를 앞에, 환측 다리를 뒤로 놓고 환측 팔로 수건을 잡고 벽에 댄 뒤 벽을 따라 큰 원을 그리듯이 움직이는 방법으로 시행한다. 스트레칭, 근력 강화 운동 및 견갑 안정화 운동을 시행하기 직전에 온열치료, 전기치료 등 의 물리치료를 함께 시행하는 것이 효과 적이며[9], 상기 제시된 운동방법들은 보통 10-15회, 3세트 씩 하루에 3차례 정도 시행하는 것이 기본이나 운동동작이 통 증을 유발하는 경우 해당 동작은 피하는 것이 좋다[11]. 석회화건염 1. 석회쇄석술 석회화건염은 석회 침착 병기(stage)에 따라 통증의 정도 664 대한의사협회지

() The needle tip is placed at the calcium deposit and small amount of normal saline & lidocaine mixture is repeatedly injected and aspirated back and forth, mechanically breaking the calcium mass. 차례에 걸쳐 시행한다. 충격파 강도, 진 동수, 횟수 등은 프로토콜에 따라 차이 가 있으나 한 세션 당 800-2,000회의 충격파를 mj/mm2의 강도 가 다르게 나타나며, 흡수기(resorptive phase)에서 가장 심 한 통증이 나타난다. 석회가 통증을 유발한다고 판단이 되면 초음파 유도 하에 석회화가 관찰되는 부위에 주사 바늘을 삽 입하여 물리적으로 석회 덩어리를 깨거나 생리 식염수 주입 과 흡인을 소량씩 반복적으로 시행하는 석회쇄석술(calcium barbotage)을 시행할 수 있다(Figure 9) [12]. 이 과정에서 로 2-4 Hz 정도의 진동수로 가하게 된다[15-19]. ESWT는 통증감소 및 석회화 병변의 감소에 효과가 있음이 여러 문헌 에서 보고되었고[15-19], 고강도(high ESWT)로 자극할 때 만 유의한 효과가 있다는 보고도 있다[18]. 초음파 유도 하 석 회 쇄석술에 비해 시행 횟수는 많지만 비침습적이고 시술자 의 힘이 덜 들기 때문에 널리 이용되고 있다. 통증을 심하게 호소하거나 시술 후에 일시적으로 악화될 수 있기 때문에 쇄석 후 국소마취제나 소량의 스테로이드를 함 께 주입하는 것이 효과적이다. 석회쇄석술을 시행한 경우에 통증 감소 및 석회 크기 감소에 모두 효과가 있다고 보고되 어 있으며, 스테로이드만 주입한 경우와 비교했을 때 우월한 효과를 보고한 경우도 있었다[13,14]. 결론 어깨통증은 일상생활 동작이나 운동을 수행하는 데에 있 어 커다란 방해요소로 작용한다. 특히 치료가 조기에 잘 이 흔히 발생하는 어깨통증에 대한 보존적 치료 665
