Subjects and Methods:The clinical records of patients with infective, treated at Asan Medical Center between January 1989 and December 2003, were retrospectively analyzed. Results:Of the 309 case of native-valve, 17 (5.5%) cases were found to be NIE. The mean age of these 17 patients was 51±17 years, which included 9 women and 8 men. Staphylococcus aureus was the most frequent causative organism of NIE in 11 cases (65%), of which nine (82%) had methicillin-resistant strains. The prevalence of right-sided vegetation in NIE was higher than that of community acquired infective (CIE)(29 vs. 10%, p<0.05); however, left-sided vegetation was observed in more than 70% of patients with NIE (12/17). Surgeries, with or without wound infection (59%) and insertion of a central venous catheter (29%), were the two most common possible sources of NIE. In hospital mortality was significantly higher in patients with NIE than in those with CIE (47 vs. 11%, p<0.001). Conclusion: Patients with NIE, which comprises a minor portion of those with infective, show unique clinical characteristics in terms of causative organisms, risk factors, sites of vegetation and in-hospital mortality. 2 Myung-Zoon Yi, et al:nosocomial Infective Endocarditis 237 판막감염성심내막염환자들의자료를후향적으로분석하였다. 병원획득성감염성심내막염은처음입원할당시심내막염의증거가없고다른질환으로치료중적어도입원후 48 시간이지나서심내막염이발생한경우로정의하였고, 8) 심장수술후발생한인공판막감염성심내막염 (prosthetic valve infective ) 은원인균주가예측가능하고, 나쁜예후를보이는등다른대다수의병원획득성감염성심내막염과는다른양상을보이므로대상환자에서제외하였다. 3) 병원획득성과지역획득성감염성심내막염환자들의기본임상특성, 증상, 임상검사소견, 원인균주, 우종의위치, 생존곡선으로사망률을비교분석하였고병원획득성감염성심내막염환자들에서병원균유입의가능한예상경로와치료에대해조사하였다. 임상검사경흉부심초음파 (transthoracic echocardiography; TTE) 및경식도심초음파 (transesophageal echocardiography; TEE) 를단독또는함께시행하였고, TTE 단독으로시행한경우는 70명 (22.7%) 이었고, TEE 단독으로시행한경우는 17명 (5.5%) 이었으며, 함께시행한경우는 222명 (71.8%) 이었다. 심초음파소견은심장내과전문의가평가하였다. 혈액배양은한시간간격으로 3차례에걸쳐 10 ml 이상채혈하여호기성및혐기성 BACTEC culture bottle 에접종하였고, 2주이상배양하여도균이분리되지않는경우를혈액배양음성으로정의하였다. 통계데이터는평균 ± 표준편차와빈도로나타냈다. 병원획득성감염성심내막염의예측인자를평가하기위해서, 대상환자를병원획득성군과지역획득성군으로나누어임상특성을비교하였다. 연속형변수는 Student T-test 를이용하여분석하였고, 범주형변수는 Chi-square 검정또는 Fisher 의직접확률계산법을사용하였다. 생존곡선의분석은 Kaplan-Meier 법을사용하였고, 두군간의비교는 Log-rank Test 를이용하였으며, p값이 0.05 이하일때통계학적으로유의하다고보았다. 모든통계자료들은 SPSS 소프트웨어 (SPSS for windows, version 11.0; SPSS inc; Chicago, IL, USA) 를이용하여분석하였다. 결 대상환자들의기본임상특성대상환자들의기본임상특성은 Table 1과같다. 감염성심내막염으로진단된 309 명의환자중병원획득성감염성심내막염이 17명 (5.5%), 지역획득성감염성심내막염이 292 과 명 (94.5%) 이었다. 병원획득성감염성심내막염환자들의평균나이는 51±17 세였고여자가 9명 (53%) 이었다. 대부분의기본임상특성에서병원획득성군과지역획득성군사이에차이는없었고, 체중감소 (6 vs. 28%, p=0.048), 호흡곤란 (18 vs. 40%, p=0.045) 만지역획득성감염성심내막염에서더높은빈도를보였으며, 임상검사에서는혈색소수치가병원획득성감염성심내막염에서더낮았다 (9±2 vs. 11±6 g/dl, p=0.016). 병원균유입경로병원획득성감염성심내막염환자에서병원균유입이가능한예상경로는 17명모두에서확인이가능하였다 (Table 2). 수술후감염의합병증이발생하여병원체가들어오는경우 (surgical wound infection) 가 8명 (47%) 으로가장많았고, 중심정맥관삽입이연관된경우 (insertion of central venous catheter) 가 5명 (29%) 이었으며, 수술의시행과말초혈관카테터삽입이각각 2명 (12%) 이었다 (Table 3). 수술이나침습적시술후심내막염이발생하여진단되는시점까지의기간은평균 29±16 일이었다. Source of infection 원인균주 Number of patients Surgical wound infection 8 (47%) Surgery without wound infection 2 (12%) Central venous catheter 5 (29%) Peripheral venous catheter 2 (12%) 가장흔한병원획득성감염성심내막염의원인균주는 Staphylococcus aureus였고 (11명, 65%), 이빈도는지역획득성감염성심내막염에서의 12%(34 명 ) 에비해유의하게높았으며 (p<0.001), 메치실린내성균주 (methicillin-resistant Staphylococcus aureus, MRSA) 는 11명중 82%(9 명 ) 이었다. 그외에 Candida, Enterococcus faecalis, Erysipelothrix rhusiopathiae가각각 1예씩배양되었고배양검사에서균이분리되지않은경우가 3예 (18%) 있었다 (Table 4). 지역획득성감염성심내막염의경우가장흔한원인균주는 Streptococcus viridans이었으나 (90명, 31%), 병원획 득성감염성심내막염에서는한예도배양되지않았다 (p= 0.007). 그외에지역획득성감염성심내막염의원인균주는 Staphylococcus, Enterococcus, HACEK 순이었고, 배양검사에서균이분리되지않은경우가 28%(83 명 ) 였다. 우종의위치병원획득성감염성심내막염환자 17명중 12명 (71%) 에서우종은좌측심장 ( 승모판막 9, 대동맥판막 3) 에서발견되었고, 29%(5 명 ) 는우측심장 ( 삼첨판막 ) 에서발견되었다. 지역획득성감염성심내막염환자 292 명중 81%(237 명 ) 가우종이좌측심장에있었고, 10%(29 명 ) 는우측심장에있었으며, 1%(4 명 ) 의환자는양측에서우종이발견되었다. 우종의위치가좌측인경우는두군간에차이가없었고 (p=0.058), 우측심장에우종이있었던경우는병원획득성감염성심내막염환자에서 29%(5 명 ) 로지역획득성감염성심내막염의 10%(29 명 ) 에비해상대적으로높은비율을보였다 (p=0.047) (Table 5).

Site of vegetations Nosocomial Community infective acquired infective p (17 Patients) (292 Patients) Left-sided vegetation 12 (71%) 237 (81%) Right-sided vegetation 15 (29%) 129 (10%) Both-sided vegetaion 114 (11%) 치료및경과 Nosocomial infective (17 Patients) 병원획득성감염성심내막염환자 17명중 29%(5 명 ) 에서치료를위해심장수술을시행하였고, 지역획득성감염성심내막염환자는 50% 에서수술이시행되었다 (p=0.045). 병원획득성감염성심내막염환자중 14명 (82%) 에서수술이필요하였으나 9명환자중다장기기능부전 (7명 ) 과보호자의수술거부 (2명 ) 로수술을시행하지못하였다. 모든환자는원인균에따른항생제치료를받았고, 균이분리되지않은경우는경험적항생제치료를시행하였다. 연구기간동안병원획득성감염성심내막염환자의사망률은 47%(8 명 ), 지역획득성감염성심내막염환자들은 11% (33 명 ) 였고, 생존분석을시행한결과두군간에유의한차이를보였다 (p<0.001)(fig. 1). 수술의적응이되지않고내과적치료만받은경우사망률은 33%(1/3), 치료를위한수술의적응이되어심장수술을받았던경우사망률은 20% (1/5) 명으로가장낮았고, 수술의적응이되었으나다장기기능부전으로수술을시행하지못한경우사망률은 71%(5/7) 로가장높았다. 고 최근 10년간병원획득성감염성심내막염에대한연구는거의없었으며, 특히국내보고는없다. 이전의외국연구에서병원획득성감염성심내막염은전체감염성심내막염중 13~17% 를차지하였고, 4)5)9) 인공판막심내막염을제외한자연판막심내막염의경우에는그보다조금적은 9.3~ 20% 로알려져있다. 3)10) 본연구에서병원획득성감염성심 찰 Community acquired infective (292 Patients) p Staphylococcus aureus 11 (65%)* 34 (12%) <0.001 CNS 0 21 (17%) <0.252 Streptococcus viridans 0 90 (31%) <0.007 Enterococcus species 11 (16%)* 15 (15%) <0.893 Others 12 (11%)* 49 (17%) Culture negative 13 (18%)* 83 (28%) <0.335 *: candida, erysipelothrix rusiopathiae. Community acquired infective Nosocomial infective 내막염은전체감염성심내막염중 5.5% 로조사되었고외국의보고에비해다소낮은수치였다. 그러나기간에따라병원획득성감염성심내막염의비율을나누어보면, 처음 5년간 (1989~1993) 은 4.6%(2/43) 를차지하였고, 다음 5 년간 (1994~1998) 3.8%(5/133) 였으며, 마지막 5년간 (1999~ 2003) 은 7.5%(10/133) 이었다. 대부분의이전보고들과마찬가지로본연구에서도 Staphylococcus aureus(11명, 65%) 가병원획득성감염성심내막염의가장많은원인균주였다. 11)12) 메치실린내성균 (methicillin-resistant organism) 이 82%(9 명 ) 였는데이는 Chen 등 5) 의연구와는비슷한결과였고, 한예도배양되지않은 Fernandez-Guerrero 등 3) 의연구와는상이한결과로연구대상지역의항생제감수성의차이에따른결과로생각된다. Coagulase negative Staphylococci(CNS) 는정맥혈관염의주된원인균주로병원획득성감염성심내막염의 3~7% 를차지하는것으로알려져있고, 3)5)7) Terpenning 등 4) 은 41% 를 Fang 등 13) 은 33% 를차지하는것으로보고하였으나, 본연구에서는한예도없었다. Enterococci 균혈증은증가하는추세이고병원획득성감염성심내막염의경우도전체감염성심내막염중 10~30% 를차지하는것으로알려져있으나본연구에서는 6%(1 명 ) 였다. 3-5)10)14) 반코마이신저항균주 (vancomycin-resistant Enterococci) 나고농도젠타마이신저항균주 (high dose gentamycin-resistant organism) 는없었다. 중심정맥관과말초정맥관은병원획득성감염성심내막염의중요한병원균유입경로로보고된다. 3)5)9)10)15)16) Guze 등 10) 은수술부위창상감염 ( 소화기와비뇨기, 11례 ) 에의한병원획득성감염성심내막염발생을보고하였는데, 본연구에서도 8명 (47%) 에서창상감염이병원균유입경로로확인되었다. 특히흥미로웠던점은 2명에서수술부위감염이없는수술시행자체로균혈증이발생하여감염성심내막염을유발할수있었다는것으로다른논문에서보고된적은없다. 병원획득성감염성심내막염의경우중심정맥관, 수술

5 240 Korean Circulation J 2006;36: 부위창상감염등주된병원균유입의가능한예상경로는모두우측심장이되고, 따라서우측감염성심내막염이더많을것으로예상할수있다. 본연구에서도병원획득성감염성심내막염에서우측심내막염의비율이지역획득성감염성심내막염보다유의하게더높았다 (27 vs. 10%, p=0.047). 그러나절대적인수에서는병원획득성과지역획득성감염성심내막염모두우종이좌측심장에발생하는경우가더많았다. 그이유로좌측심실의펌프기능에의한압력이우측보다더높아대동맥판과승모판에와류 (turbulent flow) 를만들고이에의한심내막손상 (endothelial damage) 을더많이일으킬수있고, 좌측심장은우측심장에비해산소농도가높아서균의성장이쉬우며, 좌측심장에더많은선천성및퇴행성심질환이발생하기쉽기때문으로설명하고있다 ) 초기보고에서병원획득성감염성심내막염의사망률은 100% 였는데, 보고당시에는심장수술을제대로시행할수없었기때문이다. 10) 최근에는심장수술이가능하게는되었지만여전히사망률은높게보고되는데, 외국의경우사망률이 40~54.5% 로보고되고, 3-5)9) 본연구에서전체병원획득성감염성심내막염의사망률은 47% 로지역획득성감염성심내막염의사망률보다는현저히증가되어있다. 본연구에서치료를위한심장수술의적응이되었던경우는병원획득성감염성심내막염중 82%(14/17) 를차지하였다. 이중 5명은수술을시행하였고, 9명은다장기기능부전 (7명 ) 과수술거부 (2명 ) 로시행하지못하였다. 심장수술의적응이되어시행받았던경우환자의사망률은 20% (1/5) 로가장낮았고, 수술의적응이되었으나다장기기능부전으로수술을시행하지못한경우에사망률은 71%(5/7), 수술거부로시행하지못한경우에사망률은 50%(1/2) 이었다. 수술의적응이되지않아내과적치료만받은경우사망률은 33%(1/3) 이었다. 병원획득성감염성심내막염중수술이필요한경우는많은수를차지하였고, 다장기기능부전과수술거부등으로수술을시행하지못하는경우나쁜예후를보였다. 본연구는 3차의료기관한곳의자료를모은것으로전체감염성심내막염의양상을반영하기어려운제한점을가지고있다. 특히다른병원에서수술이나시술후상태가악화되어후송되었다가감염성심내막염으로진단받은경우가많아혈액배양의조건이나시기, 항생제투여여부등에관한정보들을체계적으로정리할수가없어이들이발병이나혈액배양양성여부에미치는효과를제대로분석할수없었던한계점이있다. 향후위험인자, 유병률, 사망률등병원획득성감염성심내막염의정확한특성을조사하기위해서는큰규모의다기관연구가필요할것으로사료된다. 요약하면병원획득성감염성심내막염은지역획득성감염성심내막염에비해빈도는비교적낮으나원인균주, 위험인자, 우종의위치등에서다른특성을보였고, 사망률이 높으며, 특히다장기기능부전을포함한다른이유등으로수술하지못한경우나쁜예후를보였다. 요 배경및목적 : 최근수술이나침습적시술에따른병원획득성감염성심내막염의증례보고가잇따르며임상적중요성이대두되고있으나, 한국에서이에관한자료는전무한실정이다. 연자들은한국인에서병원획득성감염성심내막염의발생과그임상특성을알아보고자하였다. 방법 : 1989 년부터 2003 년 12월까지서울아산병원에서치료받은심내막염환자들의자료를후향적으로조사하여, 병원획득성감염성심내막염환자들과지역획득성감염성심내막염환자들의임상특성을비교, 분석하였다. 결과 : 연구기간동안치료받은전체자연판막감염성심내막염환자는 309 명이었고이중 17명 (5.5%) 이병원획득성감염성심내막염환자이었는데, 평균나이는 51±17 세였고여자가 9명이었다. 병원획득성감염성심내막염에서 Staphylococcus aureus가가장흔한원인균주였고 (11 명, 65%), 그중메치실린내성균이 82%(9 명 ) 였다. 병원획득성감염성심내막염환자에서우종이우측심장에있었던경우가지역획득성감염성심내막염환자에비해더높은비율을보였으나 (29 vs. 10%, p<0.05), 70% 이상 (12/17) 에서우종의위치는좌측심장이었다. 병원획득성감염성심내막염에서병원균유입의가능한예상경로는수술 (10 명, 59%) 과중심정맥관삽입 (5명, 29%) 이연관된경우가가장많은수를차지하였다. 병원획득성감염성심내막염환자의사망률은 47% 로지역획득성감염성심내막염환자의 11% 에비해유의하게높았다 (p<0.001). 결론 : 병원획득성감염성심내막염은전체심내막염환자중소수를차지하였고, 원인균주, 위험인자, 우종의위치, 사망률등에서지역획득성감염성심내막염과는다른특성을보였다. 중심단어 : 심내막염 ; 원내감염. 약 REFERENCES 1) Lamas CC, Eykyn SJ.
