We tried to make 3D mucosal surface images of the sinonasal cavities, phaηnx and larynx, and evaluated the feature of normal anatomy in 28 subjects In the sinonasal cavities, 3D imaging was capable of demonstrating the inner wall of paranasal sinuses and its openings, and general status of the nasal cavity. In the nasopharynx, 3D imaging provided an easy concept of sectional images as 3D picture and displayed anatomic subsites and lesions comparable to that in fiberscope. In addition, 3D imaging had advantages in overcoming the technicallimitations in fiberscope. In the laπnx and hypophaηnx, 3D imaging gave a 3D concept of the laπngeal structures and presented additive informations not seen 10 때aliam맑 s thus enabling access to regions beyond the scope of fiberscope. In conclusion, 3D imaging allows an easy conceptu떠 ization of transaxial CT images in complex anatomic areas and provides additional informations undetectable in transaxial CT. 2 남상화외 : 부비강, 비인두및후두표면의 CT 를이용한 3 차원적영상 -based imaging 기법으로주로골격이나피부의표변영 상 (surface images) 을표현하는데각조직의신호강도 (signal intensity) 의차이를이용한다 ( 7 ). 부비강 ( paranasal sinuses), 비강 (nasal cavity), 인-후두 (pharynx - Iarynx ) 의공간은아주낮은 Hounsfield - unit ( 이 하 HU 로약함 ) 를가지는공기 (a ir) 로차여있고, 점막표 변 ( mucosal surface) 의 HU 는상대적으로높아대조적 인신호강도의차를보인다. 따라서이영역에서 thresholding-based imaging 기법에의하여점막표변의형 태를삼차원적으로재구성하는것이가능할것으로사료 Select Patient and 뻐 ages Seires 되었다. 저자들은부비강, 비강, 인-후두의점막표면의정상해부학에대한 CT 를이용한 3D 영상을처음으로시도해보았고, 또한그것의입상적유용성을알아보았다. Automatic Contour Determination Rotation, Cut and Light Source 대상및 방법 총 28 명에서 CT 를이용한 3D 영상을시도하였는데 이중 26 례는두경부의 CT 를의뢰해온환자에서병변부 위에추가적으로정상부위에서 CT images를얻었고, 2 Fig. 1. Sequence of three-dimensional reconstruc- 명의건강자는 N aso-pharyngo- Laryngofiberscope를 tlon. 시행하여재구성된 3D 영상과비교하였다. 각부위에서 의정상해부학을보이는 3D- 영상은부비동및비강이 11 례, 비인두가 8 례, 후두가 6 례였는데, 시야에서직접보 이는구인두 ( oropharynx) 는대상에서제외했다. CT 는 Somatom Plus(Siemens Medical Systems, Iselin, NJ) 로 2-3mm 두께의연속절편을얻었다. 삼차 원적재구성은 Somatom Plus 자체의 surface- rendering 방식의 Somaris" 프로그램을사용하였으며 threshold 치에따른해상도및형태변화를알기위해다양한범 위의 threshold 치를사용해보았다. 정확한해부학적구 조를보기위해재구성영역 ( area of reconstruction) 은 가능하면작게하고, 회전 (rotat i on), 절단 (cut) 및광원 기법 (Iight source) 을사용해입체감을최대한으로표현 하도록하였으며가장효과적으로시간을단축하고많은 정보를얻을수있는방법을알아보았다 ( Fig. 1). 인두및후두에서해부학적구조는 AJCC(American Joint committee for Cancer) 의 TNM c1 assification 에이용되는 regions, sites, subsites 의개염을도입하여 각부위를관찰했다 ( 12 ). 3 대한방사선의학회지 1993 ; 29 (3) : 366~372 7~ E 프 부비강및비강에서, 3D - 영상은각부비강의점막표면 의입체적형태와부비강의개구부 ( opening) 들을표현할 수있었고, 부비강의전반적인상태를한눈에보여주었으 며동시에인접한비인두와관계를관찰할수있었다 (Fig.2). 비인두 (nasopharynx) 에서, 3D 영상은횡단면 (cross - sectionai) 영상에비하여구조물의형태를공간적인개 념으로쉽게인식할수있게하였으며여러가지해부학적 구조물을내시경으로관찰하는것과유사한정도로보여 주었는데, torus t ubarius, fossa of Rosenmüller, torus levatroius 같은중요한해부학적구조물이잘보 였고, 내시경보다넓은시야를가졌다. 또한 3D 영상은 내시경이가지는각도 ( angle) 의제약이나광원의부족같 은기술상어려움을극복할수있었다 ( Fig.3) 후두와하인두 ( h y popharynx) 에서, 3D 영상은복잡한 후두의구조를삼차원적으로인지시키고, 단면영상에서 a b Fig. 3. At plus 20 (c) 3-D irnaging loses normal anatomïc pictures. 얻을수없는추가적인정보를제공했다. 그리고 3D 영상은내시경이도달할수없는부위의관찰이가능하게했는데, 후두실 (J aryngeal ventric1e), 굴콕된점막하부위, 특히성문하부 ( infra g lottis) 등의관찰이가능했다. 후두의전체적얀형태를한눈에보는데는후두의뒷부분을절단한후후방에서관찰하는것이가장효과적이었다 (Fig. 4). 부비강, 비강및인후두의점막표면의 3D 영상의재구성에있어고해상력의영상을얻기위해서는적당한조건의 CT 단면영상이요구되었는데, 2-3mm 의비교적앓은절편 (s lic e) 과떤속적이고정확한 interval 이필수적이었다. 관찰부위의자세한해부학을얻기위해가능하면작은관심영역을선택했는데, 이때관심영역의크기에따른적당한수의단층영상을선택하여이용해야했다. 3D 영상의재구성에서 threshold치의선택이영상의선명도와정확도를결정하는데, 마이너스 100에서마이너스 150의영역에서점막표면이가장정확하게묘사되었고수치가증가할수록점막표면의미세한표현이가능하나부수적으로가짜절편 ( artificial fragments ) 나가짜구멍 (artificia l hol e) 이생겨정상의형태가왜곡되었다 (Fig. 5) 기본적인 3D 영상에서회전, 절단및광원의기법을적절히사용하는것이영상판독및해부학적인이해에필수적이였다. 3D 영상의재구성에있어시간의단축은 computer 자체의속도가결정하지만, 정확한재구성영역의결정, 한번의재구성후에최대한의절단의사용이전체적인시간을좌우했다. 고 복잡한해부학적구조를가지는부위에서부분적인단 면영상만으로실지의구조를개념화 ( conceptualization) 하기어려울때 3D - 영상은많은도움이된다 (8). :'<:f E 르 CT 나 MRI 영상처럼 3D 영상도전적으로 computer 에의해서 만들어지는데, 획기적인 comput e r 의발전은실용적인 입상적응용을가져왔다. 표현기법 (rendering technique) 은 CT나 MRI의횡단 면영상정보를모형화된 ( simulated ) 3D - 영상으로만드는 데이용하는연산법 ( algorithm ) 을말하는데, 현재는대 개 thresholding 혹은 surface based(binary) 기법과 semi transparent (continum) 혹은 volume-based 기법 으로분류되며, 아직까지는 thresholding- based imag ing 기법이주로사용되고있다 ( 8-9 ). 이들기법은 3 단계 로구성되어져있는데 volume formation, classifica tion 그리고 image projection 이다. thresholding calssification 에서는각각의조직에 hi gh 와 low 두가지의 수치가부여되는데, 만약신호강도가조직이가지는 hi gh와 low threshold 사이에놓이면각 voxe l 은같은 종류의조직을포함하고있다고간주되고, 신호강도가정 해진 threshold 밖에놓이면그같은종류의조직이없는 것으로간주된다. 실지 thresholding 기법에서는 high threshold 는 CT scale 의최상치이상에있어의미가없 고, low threshold 를유일한 thre shold 로간주한다 ( 7 ). 통상적으로뼈를표현하기위해플러스 150HU 정도에 369 -

5 - :wo - 대한방사선의학회지 1993 ; 29 (3) : 366~372 서, 피부표면에는마이너스 100HU정도의 thr eshold치를취하는데 (1 0 ), 저자들의경우부비강, 비강, 인후두의표면점막을표현하기위해보통피부표면에사용되는마이너스 100 전후의값을사용하였는데점막표면을좀더상세히묘사하기위해 threshold 치를높혔었을때도리어가짜절편이나가짜구멍이관찰되어해상력이도리어감소했다. 점막표면을표현하기위해서는마이너스 100 에서마이너스 150정도의 threshold치가가장적당했다. 3D program 은체용적 (body volume) 을의미하는 CT의횡단면절편들에따라표면영상을만들게되기때문에적당한조건의 CT영상을얻어야하는데, voxel 사이의간격을줄이기위해비교적앓은절편이되며, 두경부에서는대개 2-3mm의두께를사용한다. 또한부정확한절편의간격으로생기는 interslice g aps를줄이기위해간격을절편두께와같거나약간작게사용해야하고, 빠른스캔시간이필수적이며, 이상적으로는 spiral CT 의사용이요구된다 ( 11 ). 3D -영상을임상에있어서충분히활용하려면적절한영상을짧은시간에만들어내는것이중요하다. 이를위해서는첫째는최소한작은수의절편을선택해야하며둘째는 3D 재구성시필요한기법의순서를미리결정하는것이다. 부비강및인후두의해부학적구조를보는데있어중요한구조물은대개연속해서 3cm내지 5cm 정도면모두볼수있는데, 이것은 CT 절편으로 207H 이하이며, 절편의수에따라횡단면영상에서의관심영역크기를일치시켜가능하면재구성공간을정사각형으로만들어야 3D -영상을가장크게할수있으 며여러가지기법의활용이간편함을알수있었다. 한편 중요한것은두경부의 CT 스캔시연속적으로같은 FOV (field of v i ew ) 로스캔을하는것이다. 비인두부위에서는접형동 (sphenoid sinus) 에서연구개 (soft palate) 까 지, 그리고후두및하인두를보기위해서는후두개곡 ( vallecula) 의직하부부터성문하 1. 5cm까지는언제나 같은조건으로스캔을하고, 가능하면앓은절편두께를사용한다면펼요시에언제나삼차원적재구성이가능할 것이다. 3D-surface display 는물체의음영의표현 (shaded representation) 으로, 이러한음영 (shading) 은각표면의 voxe l의평균을결정함에따라수학적으로계산되는데, light source(gradient shad ing) 의방향에의해물체의모습이달라져보일수있어적당한광원의사용이 3D -영상의분석에결정적일때가자주있다 (1 0). 3 D -영상이가지는또다른기법으로회전인데, 모든방향 과각도로의회전이가능하고, 이러한자유로운이동은 내시경이갖지못하는것으로부비강내, 성문하부위의관찰을가능하게해주었다. 부비강과비강에있어 3D -영상은횡단면영상의정보이상을얻기어려웠지만복잡한부비강의연결구조를삼차원의개념 ( 3 D - co ncept) 으로인식할수있었고, 인접한비인두의삼차원재구성시주위의비강이나부비강의해부학적지식이필요했다. 인두에서 3D 영상의적응이유용하였던부위는비인두 와하인두였다. 비인두의삼차원적재구성시측벽의구조물의관찰이중요하였는데 3D 영상에서는후두함요 (pharyngeal recess) 와이관 ( Eust ac hian - tub e ) 의개구부를동시에관찰할수없기때문에비인두의앞쪽을먼 a b Fig year-old male with left glottic tumor and cord immobility a. 6 ω남상화오 1: 부비강, 비인두및후두표연으 I CT 를이용한 3 차원적영상 저제거한후회전기법을이용하여순차적으로보는것이 다. 부비강, 비강그리고인후두의점막표면은새로운 3 가장이상적이었다. 후두는정교한연골, 미세하게조절되는근육들그리고 D 영상적응의영역이되리라기대한다. 복잡한관절과인대로구성된장기로, 그크기가성인남 자에서 4. 4x 4. 3x 3. 6cm(!ength, coronal, sagittai) 정 도이다 (13). 후두표변을조사하기위해일반적으로 CT 나 MRI 에더불어후두경검사를하는데, 실지로미세한 해부학적지식없이 CT 나 MRI 을판독하기란어렵고, 후두경검사가가지는자체의기술상한계와환자의비협 조등으로정확한검사가어려울때가많다 (1 4, 15 ). 여기에 3D - 영상은후두경검사에보조적인, 때로는대체적인검 사로이용할수있었는데, 다른부위에비해특히많은 추가적인정보를삼차원적재구성을통해얻을수있었다 (Fig.6) 후두의모양은호흡, 연화, 발성에따라다양하 게변화하기에고정된해부학을얻기가어렵고, 이에따 라단층촬영시 motion artifact'-t respiratory artifact 가생겨정확한영상을얻기어려울때가많다. 고해상력 의 3D 영상을위해필수적인정확한단층촬영술이요구 되고호흡및발성에따른후두의변화에대한사전지식 이필요하다. 일상적으로후두의 3D - 영상재구성에서먼 저정상시야 ( top v i ew ) 와저부시야 ( bottom v i ew) 를 먼저얻고, 다음에절단시야 (cutting view) 를앞쪽이나 뒤쪽에서얻으면효율적인 3D 영상을시행할수있었다. 저자들은처음으로부비강, 비강그리고인후두에서 3 D - 영상을시도하여좋은영상과정보를얻을수있었는 데그것의유용성은후두에서특히돋보였고, 또다른가 능성을보였다. 후두에서의진단은대개후두경에의한 표면영상의관찰에의해이루어지고, 직의침입정도를 CT 나 MRI 로판정하는데, 부수적으로심부조 3D - 영상은 이러한검사의일종의부산물로생기는것이기에어느정 도의짧은시간을부가하면충분한많은정보를얻을수 있을것으로생각된다. 또한 3D - 영상은내시경과달리표 면을색깔이나질감을충분히보여주지못하고, 단지용 적 ( vo lume) 의변화를나타내기에그것을판독하는데또 다른기준이있어야한다고생각된다. 추후내시경과병 리학적비교를통해서 3D - 영상에있어병소의이해를완 성시켜야하고, 후두에서의동태적변화에따른해부학을 3D - 영상을통해알아보는시도가필요하다고느꼈다. 결론적으로, 부비강, 비강그리고인후두의점막표변의 3D - 영상은복잡한해부학적구조를가지는부위에서단 면영상을쉽게개념화시켜주고, 또한단면영상에서얻을 수없는추가적인정보를줄수있다. 3D - 영상은정상해 부학을보는데있어내시경에필적하는좋은해상력을가 졌는데, 손쉬운이용과비교적짧은시간에행할수있 츠 ïl 무허 1.
