2 용법 부작용 참고문헌 1 회 1 정, 1 일 3 회복용 과민반응 제약회사자료 Minireview Interferons Interferon (INF) 은바이러스가침투했을때세포에서스스로생산되는당 ( 糖 ) 단백질의일종으로바이러스증식억제효과를나타낸다. 바이러스억제인자라고도불리는이물질은 1957 년영국의 A. Isaacs 와 J. Lindenmann 에의해알려진이후약 20 여년동안은인간의살아있는세포에서만추출이가능했기때문에생산과정제에어려움이많았다 년대후반에 DNA technology 와 monoclonal antibody 를이용하면서 INF 의인위적생산이가능해졌으며이후항종양형성, 세포증식억제, 면역조절효과가밝혀지면서항바이러스제, 항암제, 간염치료제, 면역조절제등으로널리응용되기에이르렀다 개의아미노산으로구성된 INF 은 type I 과 type II 형이있으며 INFα 및 INFβ 가 type I 형, INFγ 가 type II 형에속한다. INFα 에는다시 20 종이상의 subtype 이존재하며 β 에는 2 종, γ 에는 1 종이알려져있다. 백혈구에서생산되고바이러스에의해생산이유도되는 INFα 는분자량 20,000 정도로탄수화물을함유하지않으며 α2a 와 α2b 두가지아형간치료효과는큰차이가없는것으로알려져있다. INFβ 는섬유아세포에서생산되고바이러스나 immunoglobulin 에의해생산이유도되며분자량은약 20,000 으로탄수화물을함유한다. Type II 형에속하는 INFγ 는면역림프구에서생산되며분자량 20,00025,000, 탄수화물을함유하고있으며세포분열촉진인자 (mitogen), 종양괴사인자 (TNF), IL1, IL2 등의 cytokine 과 colony stimulating factor 등에의해생산유도된다. 현재상품화단계에까지이른 INF 은 α2a, α2b, αn1, α N3, alphacon1, β1a, β1b, γ 및 peginterferon α2b 등이며각종류별효과는큰차이가없으나 potency 면에서 γ 형이가장강하며 α 및 β 는비용과효과를고려할때유리하기때문에주로많이사용된다. INFα 는전신적으로투여될경우반감기가짧고조직침투율이낮아혈중농도를유지하기위해서는상대적으로많은량을투여해야한다. 경구투여시흡수율이매우낮고 BBB 를통과하여뇌척수액에도달하지못하기때문에중추신경계에고농도로필요할경우척수강내로투여해야한다. 주대사경로는신장이며간에서의대사는극히소량이다. INFβ 및 γ 는전신적투여후에도뇨에서발견되지않는등정확한대사경로가파악되지않고있다. 전신투여외에점안, 비강등의국소투여경로가있는데, 점안제는바이러스성각막염에적용되고있으며상기도감염의원인이되는 rhinovirus 억제목적으로비강내투여가시도되기도하나임상효과는뚜렷하지않다. INF 의결핍은선천적인경우보다는후천적혹은 2 차적인경우가더흔하며대개의원인은면역억제물질, 만성감염, 종양등이다. 선천적인결핍과관련이있는질환은다운증후군, 선택적 IgA 결핍증, Tcell 면역결핍증, 고 IgE 증후군등이며 2 차적인결핍의결과발생되기쉬운질환은 HIV, 장기레슈마니아증, 결절나병, 요독증, 림프종, 혈액종양등이다. 현재제품화단계에이른 INF 의치료영역을살펴보면 INF α2a 는만성 C 형간염, 신장암, 만성골수성백혈병에 drug of choice (DOC) 로알려져있으며다른치료에반응하지않는모상세포백혈병, 다발성골수종, 전이성흑색종, AIDS 관련카포시육종등의치료에대체약물로인정되고있다. 또한흑색종환자에게수술보강요법으로응용될수있다. INF α2b 는만성 B 및 C 형간염, 신세포암, 흑색종환자의수술보강요법에서 DOC 로인정되는데특히만성 C 형간염치료에있어 98% 의반응율을나타낸다는보고도있다. 또한 pentostatin 에반응하지않는모상세포백혈병, podophyllin 요법에반응하지않는첨형콘딜로마, 표준요법에실패한다발성골수종환자에게대체약물로투여될수있으며초기다른요법에실패한 (eg, etoposide) ADIS 관련카포시육종환자에도시도될수있다. INF αn1 은유년기후두유두종증, pentostatin 적용이어려운모상세포백혈병에다른 INFα 제에비해임상적용사례가많으나다른제제와의직접비교사례가없어우선하여추천되지는않으며 1999 년제조사에서제조및판매를중단한상태다. INF αn3 는 podophyllin, cryotherapy 의적용이어려운첨형콘딜로마병소에직접투여하는것이유일한적응증으로, 이경우 α2b 또한병소부위에직접투여될수있으나 αn3 가 potency 면에서나은것으로보고되고있다. INF alphacon1 은만성 C 형간염의초기치료제로서유효하며다른 INF 치료에반응하지않거나재발한경우에사용해볼수있다. 천연 INFβ 는유방암, B 형간염, 방광암, 다발성경화증에효과를나타내지만생산이어려워재조합형 INFβ(1b, 1a) 가임상적으로이용된다. 다발성경화증치료에있어 INF β1a 는주 1 회투여함으로써 β1b 의격일투여에비해편리성이있으며주사부위피부괴사부작용도적은것으로알려졌다. 그러나발작유발, 급성질병악화의부작용면에서는 β1b 가유리한것으로알려져있다. INF β1b 는재발이장성다발성경화증, 활동성재발진행성질환등에서신경학적퇴화속도를늦춰주는정도의역할을하므로다발성경화증의 1 차치료제로추천

3 되지는않는다. INFβ 는 INFα 만큼광범위하게연구되지못했기때문에역할과임상적용에관한연구가더필요한실정이다. INFγ 는만성육아종성질환에서항생제요법, 골수이식, 백혈구수혈등의치료에보강면역요법제로서응용되며심각한악성골화석증에도이용된다. 그러나암, 후천적면역결핍등에서의역할은정확히규명되지않았다. Peginterferon α2b 는만성 C 형간염에단독혹은 ribavirin 과병용하여투여된다. 만성 C 형간염치료에는 INF α2a, α2b, alfacon1, peginterferon α2b, 혹은 peginterferon α2b 와 ribavirin 병용요법이시도될수있는데이들 INF 의효과는대체로비슷하며 INF 단독투여가 1 차적으로추천되기도하지만최근에는경구용 ribavirin 과병용요법이권장되고있으며 INF 단독투여는 ribavirin 에적응되지않는경우에만실시하는것이치료효과가좋다. INF 은바이러스 gene 의복사를억제하고포식작용, 과산화를증진하며원충류와기생충의중요한영양분이되는 tryptophan 의대사를증진하여고갈시키는역할을함으로써감염에응용되는데콘딜로마의경우 INF 이효과적이기는하나파괴된조직이다시재생되는속도가매우빨라재발률도높다. 그러나반응율은 67% 정도로다른치료보다우세한편이므로자주응용된다. B 형및 C 형간염환자의대다수가재발되기는하지만 INF 은혈청학적개선및간조직의변화를유도하며 HIV 초기에고용량으로투여되었을경우효과가매우좋은것으로알려져있다. INF 의부작용으로가장흔한증상은발열, 오한, 발열, 근육통, 두통, 오심 ( 메스꺼움 ) 등이나대부분일시적이며며칠후엔나아진다. 수주정도지나서탈모, 체중감소, 불안, 우울, 골수억제, 갑상선기능장애등의부작용이올수있으며, 인터페론용량을줄이거나일시적으로중단했다가다시투여할수있다. B 형간염치료중 GPT 치가감소하다가일시적으로상승하는경우가있는데 HBeAg 이음성으로되는과

4 정에서나타날수있으므로치료를중단하지말고경과를관찰해야한다. 황달등의간부전소견이나타나면치료를중단해야한다. INF 은임상적용범위가광범위하기는하나치료비용에비해임상결과가탁월하지는않아 1 차선택약이아닌 2 차선택약물로만인정되고있는실정이다. 실제항악성종양및항바이러스효과면에서장기간에걸친반응률은 20% 미만이라고한다. 그러나몇몇암종에서생존기간연장이기존의항암요법과유사하다고인정되고있으며간염치료에있어특별한선택약이없는상태이므로 INF 의한계를고려한다하더라도치료학적장점은인정된다. 앞으로도 INF 의잠재적인치료효과뿐만아니라세포매개면역조절작용, 약동학적특성등은계속연구되어야한다. The regimen of irinotecan plus cisplatin consisted of four fourweek cycles of 60 mg/ m2 of body surface area(bsa) of irinotecan on days 1, 8, and 15 and 60 mg/ m2 of BSA of cisplatin on days 1. The regimen of etoposide and cisplatin consisted of four threeweek cycles of 100 mg/ m2 of BSA of etoposide on days 1, 2, and 3 and 80 mg/ m2 of BSA of cisplatin on days 1. Both regimens required hydration and administration of antiemetic drugs. The planned size of the study population was 230 patients, but enrollment was terminated early because an interim analysis found a statistically significant difference in survival between the patients assigned to receive irinotecan and cisplatin and those assigned to receive etoposide and cisplatin; as a result, only 154 patients were enrolled. The median survival was 12.8 months in the irinotecanpluscisplatin group and 9.4 months in the etoposidepluscisplatin group (P=0.002 by the unadjusted logrank test). At two years, the proportion of patients surviving was 19.5 percent in the irinotecanpluscisplatin group and 5.2 percent in the etoposidepluscisplatin group. Severe or lifethreatening myelosuppression was more frequent in the etoposidepluscisplatin group than in the irinotecanpluscisplatin group, and severe or lifethreatening diarrhea was more frequent in the irinotecanpluscisplatin group than in the etoposidepluscisplatin group. Irinotecan plus cisplatin is an effective treatment for metastatic smallcell lung cancer. The New England Journal of Medicine 2002;346:8591 Fusidic Acid Cream in the Treatment of Impetigo in General Practice: double blind randomized placebo controlled trial They performed the study in Greater Rotterdam using a randomized, placebo controlled trial. They studied 184 children aged 0~12 years with impetigo. The measurement of main outcome was clinical cure and bacterial cure after one week. After one week of treatment 55% of the patients in the fusidic acid group were clinically cured compared with 13% in the placebo group (odds ratio 12.6, 95% confidence interval 5.0 to 31.5, number needed to treat 2.3). After two weeks and four weeks the differences in cure rates between the two groups had become smaller. More children in the placebo group were noncompliant (12 vs 5) and received extra antibiotic treatment (11 vs 3), and more children in the placebo group reported adverse effects (19 vs 7). Staphylococcus aureus was found in 96% of the positive cultures; no strains were resistant to fusidic acid. Fusidic acid is much more effective than placebo (when both are given in combination with povidoneiodine shampoo) in the treatment of impetigo. Because of the low rate of cure and high rate of adverse events in the placebo group, the value of povidoneiodine in impetigo can be questioned. British Medical Journal 2002;324:15 Alteplase Use for Prevention of Central Line Occlusion in a Preterm Infant At day 50 of life, a neonate boy of 27 weeks' gestation in Neonatal Intensive Care Unit, Naval Medical Center, had a clotted peripherally inserted central catheter line. He required central access to complete a 28day course of amphotericin B. An alteplase dose of 0.3 mg/0.3 ml was instilled into the catheter, for an indwelling time of 1 hour. The initial attempt was successful, and the patient was able to finish the course of amphotericin B. When a catheter becomes occluded, the cause must be determined initially; then an appropriate treatment option can be initiated. Hydrochloric acid and ethanol are some of the pharmacologic agents available, with their use dependent on the precipitating cause of the occlusion. Thrombolytic agents, such as streptokinase and urokinase, have also been used in catheter clearance. The potential risk of anaphylactic reactions with streptokinase and the unavailability of urokinase have clinicians searching for other pharmacologic alternatives. Alteplase, a synthetic tissue plasminogen actitvator, has been shown to be a promising alternative agent. There are various reports of alteplase use in pediatric patients for catheter clearance, with different suggested doses ranging from 0.25 to 2 mg. To minimize the risk of bleeding events, initiation with low doses from the suggested range is recommended. Alteplase was shown to be safe and effective for clearance of an occluded central line in a preterm infant. The Annals of Pharmacotherapy 2002;36:2724 Plasma Homocysteine as a Risk Factor for Dementia and Alzheimer's Disease In crosssectional studies, elevated plasma homocysteine levels have been associated with poor cognition and dementia. Studies of newly diagnosed dementia are required in order to establish whether the elevated homocysteine levels precede the onset of dementia or result from dementiarelated nutritional and vitamin deficiencies. A total of 1092 subjects without dementia (667 women and 425 men; mean age, 76 years) from the Framingham Study constituted our study sample. They examined the relation of the plasma total homocysteine level measured at base line and that measured eight years earlier to the risk of newly diagnosed dementia on followup. They used multivariable proportionalhazards regression to adjust for age, sex, apolipoprotein E genotype, vascular risk factors other than homocysteine, and plasma levels of folate and vitamins B12 and B6. Over a median followup period of eight years, dementia developed in 111 subjects, including 83 given a diagnosis of Alzheimer's disease. The multivariableadjusted relative risk of dementia was 1.4 (95% confidence interval CI, 1.1 to 1.9) for each increase of 1 SD in the logtransformed homocysteine value either at base line or eight years earlier. The relative risk of Alzheimer's disease was 1.8 (95% CI, 1.3 to 2.5) per increase of 1 SD at base line and 1.6 (95% CI,

5 1.2 to 2.1) per increase of 1 SD eight years before base line. With a plasma homocysteine level greater than 14 μmol per liter, the risk of Alzheimer's disease nearly doubled. An increased plasma homocysteine level is a strong, independent risk factor for the development of dementia and Alzheimer's disease. The New England Journal of Medicine 2002;346:47683 Raloxifene and Cardiovascular Events in Osteoporotic Postmenopausal Women : Fouryear results from the MORE (Multiple Outcomes of Raloxifene Evaluation) randomized trial To determine the effect of raloxifene on cardiovascular events in osteoporotic postmenopausal women, they performed the secondary analysis of data from the Multiple Outcomes of Raloxifene Evaluation trial, a randomized, doubleblind, placebocontrolled trial conducted between November 1994 and September They studied 7,705 osteoporotic postmenopausal women (mean age, 67 years) at 180 sites in 25 countries. Patients were randomly assigned to receive raloxifene, 60 mg/d (n = 2557), or 120 mg/d (n = 2572), or placebo (n = 2576) for 4 years. Main outcome measures were cardiovascular events, including coronary events (myocardial infarction, unstable angina, or coronary ischemia) and cerebrovascular events (stroke or transient ischemic attack), collected as safety end points and subsequently adjudicated by a cardiologist blinded to therapy. Cardiovascular risk at study entry was determined by the presence of multiple cardiovascular risk factors or prior coronary events or revascularization procedure. In the overall cohort, there were no significant differences between treatment groups in the number of combined coronary and cerebrovascular events: 96 (3.7%) with placebo, 82 (3.2%) with 60 mg/d of raloxifene, and 94 (3.7%) with 120 mg/d of raloxifene. Relative risks (RRs) were 0.86 (95% confidence interval [CI], ) and 0.98 (95% CI, ) for 60 mg/d and 120 mg/d of raloxifene, respectively. Similar results were obtained when coronary and cerebrovascular events were analyzed separately. Among the subset of 1035 women with increased cardiovascular risk at baseline, those assigned to raloxifene had a significantly lower risk of cardiovascular events compared with placebo (RR, 0.60; 95% CI, for both raloxifene groups). The number of cardiovascular events during the first year was not significantly different across groups in the overall cohort (P =.94), or among women at increased cardiovascular risk (P =.86) or with evidence of established coronary heart disease (P =.60). Raloxifene therapy for 4 years did not significantly affect the risk of cardiovascular events in the overall cohort but did significantly reduce the risk of cardiovascular events in the subset of women with increased cardiovascular risk. There was no evidence that raloxifene caused an early increase in risk of cardiovascular events. Before raloxifene is used for prevention of cardiovascular events, these findings require confirmation in trials with evaluation of cardiovascular outcomes as the primary objective. The Journal of the American Medical Association 2002;287:84757 Stability of Furosemide in Human Albumin Solution To determine the chemical stability of furosemide in human albumin solution over a 28day period and to assess admixtures for microbiologic contamination, they prepared samples by mixing furosemide injectable solution and 25% human albumin solution in a 1:1 molar ratio. Six bulk containers were prepared and stored in the dark: 3 under refrigeration (~4 C) and 3 at room temperature (~25 C). Study samples were withdrawn from each bulk solution immediately after preparation and at predetermined intervals over the subsequent 28 days. Containers were observed for color change and precipitation against a light and dark background at each sampling interval. Total furosemide concentration was determined using HPLC. Additional vials were prepared and assessed for microbiologic growth at time points corresponding with chemical stability results. A mean of 94.5% ± 1.33% of the initial furosemide concentration remained after 48 hours at room temperature. Under refrigeration, 100.6% ± 1.02% of the initial concentration remained at 14 days. Beyond these respective time points, <90% of the initial furosemide concentration remained. No bacterial or fungal growth was observed. When combined with 25% human albumin solution and stored under darkness, furosemide is chemically stable and free of microbiologic contamination for 48 hours at room temperature and 14 days under refrigeration. 6 Orlistat and Antihypertensives 사례 1: 평소 Atenolol, losartan, hydrochlorothiazide 을복용하던 47 세남자환자는 orlistat 를추가복용한지일주일이내에고혈압증상이악화 (260/140 mmhg) 되었다. Orlistat 투여를중단하자혈압은정상으로회복되었다. 20 일후재투여를시도하자이완기혈압이상승 ( mmhg) 되는비슷한양상이나타났으며투여를중지하자 3 일후혈압이안정화되었다. 사례 2: Enalapril 과 losartan 을복용하던 58 세남자환자는 orlistat 투여 1 주일후고혈압증상이악화 (160/100 mmhg) 되고두개내출혈이발생하였다. Orlistat 투여를중단하고 clonidine 과 nifedipine 으로혈압은조절되었다. 사례 3: 48 세여자는 orlistat 를복용하고 2 개월후고혈압 (180/120 mmhg) 증상을경험하여복용해오던 enalapril 과 amlodipine 을 losartan 과 hydrochlorothiazide 로대체하였다. Orlistat 는계속복용하였고혈압은정상으로회복되었으나 20 일후에고혈압증세가발생하여 orlista 투여를중단하고 48 시간이내에혈압이회복되었다. 저자는위환자들의혈압조절실패의원인이 orlistat 와연관있다고생각하고 orlistat 가장운동증진, 배변촉진, 설사등을유발하여고혈압약의흡수가저하되었기때문이라고추측하였다. Annals of Pharmacotherapy 2001;35:14967 Hormone Replacement Therapy HRT 를받는 25,665 명의폐경여성을대상으로안구건조증유발에대한 cohort study 를실시하였다. HRT 를투여받지않은환자와비교했을때안구건조증유발 odds ratio 가 estrogen 단독투여시 1.69, estrogen 과 progesterone 혹은 progestin 병용투여시 1.29 였다. 안구건조발생은 estrogen 단독투여군 (9.1%) 이어떤호르몬도투여받지않은군 (5.9%) 에비해높았으며호르몬투여기간이길어짐에따라뚜렷이증가하는양상을나타냈다. 저자는호르몬특히 estrogen 단독투여가안구건조증상을증가시키는것으로결론짓고성호르몬이누액과 meibomian gland 모두에영향을미치기때문이라고추측하였다. JAMA 2001;286:21149 Warfarin and Capecitabine 2001 년 11 월 1 일 FDA 와로슈사는의약품시판후보고에서항응혈제를투여한환자가 capecitabine 투여를시작할경우 prothrombin time 과 INR 이현격히증가한다는사실을경고했다. 경구용항응혈제와 capecitabine 을함께투여하는환자는 INR 이나 prothrombin time 을모니터해야한다고주지시켰으며이사실은제품설명서에 blackbox warning 처리되었다. Cholestyramine 9 세의남자환아에게고콜레스테롤혈증으로 chloestyramine 을투여받은지약 3 개월후혈소판감소증 (PLT 94,000/uL) 증상이나타났다. 투여를중지한후혈소판수치는회복되었다. 이후두차례재투약이시도되었으며투여시작 1 개월내에증상이다시나타났다. Automatic particle counter 를통한특이사항이없었으므로혈액도말분석을실시한결과다른세포들은정상이었으나혈소판은거대화되어있었다. 환자의부모는모두정상이었으며선천적으로거대혈소판환자일가능성은배제되었다. 저자는이사례의경우 cholestyramine 에의해유도된혈소판거대증이라고주장하였으며 automatic particle counter 결과이상이나타나지않아오진할가능성이있다고경고하였다. Archives of Internal Medicine 2001;161: Nefazodone Interaction: Lack of Hypertension Control DryEye Syndrome 미국 FDA 는 2002 년 1 월 9 일 nefazodone (serzone ) 을투여받은환자에게사망내지는이식이필요할정도로심각한간부전이발생한다고경고하였다. 발생빈도는 25,000 30,000 patientyear 당 1 건꼴이며활동성간질환이있거나혈중 transaminase 농도가상승한경우에는투여를시작해서는안된다고덧붙였다. 또한간부전이의심되는임상적증상 ( 황달, 식욕부진, 복부불쾌감, 권태감등 ) 이나타나면즉각투여를중지해야한다고경고하고제조사에이경고를부착하도록지시했다. Clozapine Drug Interaction : Increased INR, FDA Safety Alert Platelete Gigantism (First Report) Lifethreatening hepatic failure Myocarditis 미국 FDA 에서는 clozapine (clozaril ) 라벨의박스경고에심근내막염발생건을추가하도록지시했다. 시판후조사에의하면미국에서 clozaril 을복용한 205,493 명중치명적증상 17 건을포함한심근내막염발생이 30 건, 캐나다에서는 15,600 명중치명적증상 1 건을포함한 7 건, 영국에서는 24,108 명중치명적증상 8 건을포함한 30 건, 호주에서는 8,000 명중치명적증상 5 건을포함한 15 건이보고된것으로밝혔다. Clozaril 을복용한환자에게피로감, 무호흡, 빈맥, 발열, 흉통, 발한등의심부전증상이의심되는증상이발생하면즉각투여를중단해야한다고경고하였다. 복약상담코너 파킨슨병 파킨슨병은 1817 년영국의의사인파킨슨이처음으로체계적으로기술하였고, 병의이름도그의이름을따서명명하였다. 파킨슨병은아직궁극적발병원인이밝혀지지않은신경퇴행성질환으로진전 (tremor), 근육강직 (rigidity), 몸동작이느려지는서동 (bradykinesia) 등의운동장애가나타나며뇌신경전달물질중의하나인 dopamine 결핍으로나타난다. Dopamine 은정상인의뇌에검게보이는흑색질 (substania nigra) 이라는부위의신경세포에서주로만들어지는데파킨슨병환자의뇌에서는흑색질이탈색되어있고현미경으로관찰하여보면이곳의신경세포들이변성되어, dopamine 의결핍이초래된다 년대후반이후 levodopa 라는약물이개발되어뇌안에부족한 dopamine 을보충하는치료를시작하게된후많은치료약과치료법이개발되어요즘에는병의초기부터적절한치료를받게되면거의정상적으로살아갈수있다. 그러므로적절한치료를적절한시기에받

7 는것이중요하다. 1. Anticholinergic agents 흑색질에서흥분성전달물질인 acetylcholine 의작용을차단함으로써진전, 강직을개선시키며, 특히진전에효과적이다. Dopamine 과 cholinergic system 의균형을맞추는약물로 dopamine 이고갈되면그반응도가떨어지므로 dopamine 부족이심각하지않는질병초기증상에효과적이다. 이약은중추성, 말초성 ( 시력장애, 구갈, 혼몽, 기억손실 ) 부작용으로인해 65 세이상된고령자의경우에는투여를피하도록한다. Table 1. Amantadine 항바이러스제제로정확한기전은알려져있지않으나 presynaptic nerve terminal 에서 dopamine 의유리를증가시키고 storage granules 로의 dopamine reuptake 를감소시켜준다. Levodopa 와병용시더큰효과를나타내며 Levodopa 의 "wears off", "onoff" 현상을줄여준다. 장에서거의완전히흡수되고투여 14 시간후에최고농도에이른다. 거의대사되지않고뇨로배설되며반감기는약 28 시간이다. 100 mg/day 로시작하여 12 주마다 100 mg 을추가한다. 유지용량은 200 mg/day 으로 1 일 2 회분복한다. 최대사용량은 400 mg/day 이다. 신부전증환자의경우에는축적될수있으므로용량조절을해야한다. 불면증을감소시키기위해하루중두번째투여량은이른오후에투여한다. 약 5% 의환자가부작용으로복용을중단한다. 구역질, 식욕감소, 변비, 설사, 구갈등이흔히나타나므로식사직후에투여한다. 입이마른경우에는적당한수분을섭취하고껌을씹거나사탕을빨아먹도록한다. 불면, 우울, 초조, 어지러움, 환각, 운동실조, 두통, 신경과민, 비정상적인꿈, 흥분, 피로등의중추신경계부작용이대부분치료를시작하거나용량을증가한지수시간 수일이내에발생하나가역적이므로감량하거나약을중단하면소실된다. 또한사지의피부에적청색을띤망상의반점형성을특징으로하는말초혈관증상인망상피반이나타날수도있다. 주로고용량을복용하는여성환자에게발생하며, 약복용을중단하면없어지나순전히미용적인측면에서만문제가되므로약을중단할필요가없다. 망상피반과관련되어무릎이붓는경우가있는데, 울혈성심부전과관련된질환이없는경우에는문제가되지않으며다리를높이올린다거나이뇨제를복용하면증상을완화시킬수있다. 필요시의사와상의해서투여량을줄인다. Amantadine 은 barbiturate 계, anticholinergic agents 등의약물의효과를증가시킨다 3. Dopamine 전구체 : Levodopa 제제모든파킨슨병의선택약물로서강직과서동에대부분효과적이다. Dopamine 이수용성이라 BBB 를통과하지못하는점에착안하여 dopamine 의전구체인 levodopa 를투여하여 BBB 를통과시킨후 CNS 내에서 dopamine 으로변화하여약효를나타나게된다. Levodopa 를 dopamine 으로전환시키는효소인 amino acid decarboxylase 는몸전체에많이존재하는효소이므로중추신경계외의다른부분에서작용시오심, 구토등의부작용을야기한다. 그러므로말초에서의 levodopa 가 dopamine 으로전환되는것을방지하기위해 amino acid decarboxylase 억제제인 carbidopa 를 levodopa 의 25% 비율로혼합투여한다. 이 carbidopa 의 BBB 통과율은 1% 로거의통과하지못한다. 소장에서 80% 흡수되며 0.52 시간후에최고혈중농도에이른다. 음식물에의해흡수가저하되고일부아미노산과경쟁하므로단백질제한식이를통해변동을최소화할수있다. 소화기, 간등에서 95% 이상이대사되며대부분뇨로배설된다. 혈중반감기는 13 시간이지만 carbidopa 나 benserazide 투여에의해증가한다. 서방형제제의경우에는 onset 이느려 1 시간정도지연되고 (onset 11.5 시간 ), 혈중농도유지력이좋으며음식물과함께복용시흡수 50%, Cpeak 25% 증가하므로식사직후에복용한다. 흡수시간을단축시킨확산정도있다. 수술이나기타다른이유로일시적으로 levodopa 제제를복용하지않은환자가증상이호전됨을관찰할수있으나이기간에운동부전이나타날수있고, 그로인해정신적, 육체적증상이악화될수있다. 운동부전으로인한 DVT, 흡입성폐렴등의위험때문에주의깊은관찰이필요하다. (Madopar dispersible 는아침또는연하곤란환자에게투여한다.) 8 정 /day (Levodopa 에서변경시 8 시간간격두고, 복용하던용량의 25% 에해당하는용량으로시작한다.) 8 정 /day 8 정 /day 8 정 /day Levodopa 제제의치료시 Paradoxical effect(high dose symptomsevere involuntary movement), Peakdose dyskinesia (Improvementdystoniaimprovement response), wears off effect (Endofdose akinesia), onoff effect(yoyo effect), morning akinesia 등의문제점이발생할수있다. 이때는 levodopa 의투여량을감소시키고투여간격을좁히거나, dopamime agonist 를투여하거나, levodopa 제제를서방형제제로바꾸거나, dopamine 의대사를억제하는 selegiline 을투여한다. 50% 이상의환자에서오심, 구토가발생하므로식사직후투여하거나용량을서서히 36 개월에걸쳐증가시킨다. 소변이나땀. 침등의분비물색이갈색이나검게변할수있지만아무문제가없다. 불면증을예방하기위해하루중제일늦게복용해야하는용량은저녁 7 시이전에복용하도록한다. 우울증이발생한경우에는우울증치료제를투여받도록한다. Levodopa 의효과감소를감소시키는약물로는 phenytoin, phenothiazines, methyldopa, guanethidine, reserpine, pyridoxine 등이있고독성을증가증가시키는약물로는제산제, MAO 저해제가있다. Anticholinergic agents 와병용시진전을감소시켜파킨슨병치료에상승효과가나타나지만 levodopa 의위장관흡수를지연시켜 levodopa 의효과를감소시킬수도있다. 4. Entacapone (COMT 저해제 : Catechol Omethyltransferase inhibitors) Levodopa 제제는 amino acid decarboxylase 뿐만아니라 catechol Omethyltransferase 에의해서도대사된다. 그러므로 COMT 억제제를복용함으로써 levodopa 의생체이용율을증가시킬수있다. COMT 억제제는 levodopa 의 Cmax 를증가시키지는않지만반감기를증가시켜 levodopa 의작용시간을연장시킨다. Tolcapone 은실제로매우효과적이라는보고도있지만부작용으로간독성이있어사용이제한을받고있다. 이에비해 entacapone 은덜강력한 COMT 저해제이며 BBB 통과율낮고반감기가짧다. 주기적인간기능검사가추천된다. 1 회에 200 mg 정도를투여하고최고 1 일 8 회투여가능하다. 최대용량은 1,600 mg/day 이다. Levodopa 제제와함께투여하며최적의효과를위해 levodopa/carbidopa 용량을 25% 정도감량시킨다. 특히 levodopa 800 mg/day 이상투여할경우에는병용투여하는것이좋다. 5. Dopamine agonists Dopamine 수용체에직접적으로흥분작용을나타낸다. Dopamine2 수용체에대한직접적인흥분작용을통하여항파킨슨효과를유도하는약물로 levodopa 의효과가감소하거나효과의변동이심해질때사용된다. Levodopa 의 "wears off " 나 "onoff " 현상을줄여주며, 투여용량감소효과가있다. Table 4. Dopamine agonists 의효과 ( 독성 ) 을감소시키는약물로는 domperidone, metoclopramide 등이있으며특히 metoclopramide 는뇌에서 dopamine 의효과를억제시키므로이를함유한약물은복용하지말아야한다. 6. Selegiline Dopamine 의대사에중요한역할을하는 monoamine oxidase B 에선택적으로결합하여내인성 dopamine 의대사를억제, dopamine 의중추활성을항진시킨다. MAOB 에선택적으로작용하기때문에 MAOA 에의해대사되는다른 catecholamine 이나 tyramine 의활성에는큰영향을미치지않는다. Levodopa 의작용시간을연장시키고필요용량을감소시킨다. 투여량의 73% 가흡수되어 0.52 시간후에최고혈중농도에이른다. 반감기는 0.52 시간이며, 대사후뇨로배설된다. 지용성이므로빠르게 BBB 통과하여, Lmethamphetamine, Lamphetamine 으로대사된다. 5 mg (1T) 으로시작하여 10 mg/day, 1 일 1 회, 또는 2 회로분복한다. 최대용량은 10 mg/day 이다. 신경자극효과로인한불면증이나타나는것을방지하기위해 1 일 2 회투여시아침과점심에투여한다. 위장관계부작용으로구역질, 식욕감소, 변비, 설사, 구갈등이흔히나타나므로식사직후에투여한다. 입이마른경우에는적당한수분

9 을섭취하고껌을씹거나사탕을빨아먹도록한다. 어지러움, 기분변화, 불수의적인운동이증가하고, 운동완서, 근육연축이나타날수있다. 불수적인움직임, 중추신경계흥분이나타나면이를알리도록한다. Meperidine 과병용하는경우초조, 섬망등이나타나고사망하는경우도있다.
