1 ORIGINAL ARTICLE Surgical Fixation of Sacroiliac Joint Complex in Unstable Pelvic Ring Injuries Kwang-Jun Oh, MD, Seok-Min Hwang, MD Department of Orthopedic Surgery, Konkuk University Medical Center, Seoul, Korea Purpose: To evaluate the effectiveness and clinical outcomes of surgical fixation of a sacroiliac joint complex in unstable pelvic ring injuries. Materials and Methods: We selected sixteen cases in our hospital from 2006 to 2010 that underwent surgical fixation of the sacroiliac joint complex and had unstable pelvis ring injuries corresponding to B and C of Tile classification. Plate fixation through an intra-pelvic anterior approach was performed for 9 cases, tension band plate fixation for 2 cases, iliosacral screw fixation for 4 cases, and spino-pelvic fixation through extra-pelvic posterior approach for 1 case. Radiological and clinical evaluations were implemented to determine the results of treatment. Results: Bone union was observed in all patients. For radiological evaluation using the Matta and Saucedo criteria, 15 cases were above the nearly-anatomic reduction. Out of 10 cases that performed rotational displacement analysis, 9 cases were above fair. All cases had above moderate clinical results as well. Conclusion: For unstable pelvic ring injuries, the satisfactory radiological and clinical results have been obtained through the anatomical reduction of the sacroiliac joint complex and firm internal fixation. And from the evaluation of fracture types, the Type C fracture and vertical shear type fractures showed relatively poor results. This is an Open Access article distributed under the terms of the Creative Commons Attribution Non-Commercial License ( org/licenses/by-nc/3.0) which permits unrestricted non-commercial use, distribution, and reproduction in any medium, provided the original work is properly cited. 따라서가게되므로, 골반환의안정성에가장중요하다고할수있는것은후방천장관절복합구조물 (posterior sacroiliac complex) 이다. 골반환의측방및후방은전방에비하여골격이크고두꺼우며천장관절을지지하는인대도매우두껍고강하다. 이로인해골반환의안정성은거의전적으로측후방, 즉천장관절복합구조물의안정성에의해좌우되는것으로평가되고있고이부위의손상시골반에불안정성을초래하게된다 1). 불안정성골반환손상은내부장기와다른근골격계손상이흔히동반되며사망률이 8.6% 에서 25% 까지보고되고있다 2). 특히전방골반환단독손상에비해후방골반환손상이동반된경우에는출혈량이세배이상이나되고합병증발생률도두배이상된다 3-5). 따라서골반환의안정성을회복시키는수술적정복과고정은출혈을감소시킬뿐아니라, 기계적안정성과관련된합병증을줄여환자의기능회복과생존율의향상등좋은임상적결과를가져온다 6,7). 후방골반환손상은천골또는장골의전위된골절, 천장 Copyright c 2012 by Korean Hip Society 139

2 관절의탈구그리고골절 - 탈구복합손상으로구분할수있다. 이러한불안정성골반환손상시후방천장관절복합구조물에대한적절한정복과고정은필수적이다. 이를위한수술적치료방법으로외고정술 (external fixation), 금속판고정술 (plating), 천골봉 (sacral bars) 뿐만아니라장천골나사못고정술 (iliosacral screwing) 도시행되고있다 8-13). 본연구에서는불안정골반환손상에서천장관절복합구조물에대해금속판고정술, 장력대금속판고정술, 장천골나사못그리고척추 - 골반고정술을이용하여고정한후정복유지의효용성과치료결과에대해알아보고자하였다. 대상및방법 2006 년부터 2010 년까지본원에서 Tile 분류상 B 와 C 에해당하는불안정성골반환손상으로내원하여천장관절복합구조물에대해수술적고정을시행한환자 16 명을대상으로후향적연구조사를시행하였다. 환자의연령, 수상기전, 골반환손상유형, 동반손상, 수상후수술까지의시간, 수술시접근방법, 고정방법, 방사선학적정복정도, 임상결과등을조사하였다. 환자의성별은남자가 11 명, 여자가 5 명이었고, 환자의평균나이는 46.1 세 (7-74 세 ) 였다. 수상의원인은교통사고가 10 예, 추락사고가 5 예, 압박손상 ( 무거운물체에눌림 ) 이 1 예로모두고에너지손상에의한것이었으며, 동반손상은 14 예에서있었다. 동반손상부위로는뇌출혈 4 예, 심폐손상 9 예, 복부장기손상 5 예, 상지골절 5 예, 하지골절 4 예이었다 (Table 1). 모든환자에서단순방사선골반골전후면사진과입구, 출구사진, 수술전전산화단층촬영사진을통해골절의양상과전위를확인하였다. Tile 14) 분류에따른골반골골절의양상은 B1 형이 1 예, B2 형이 7 예, C1 형이 7 예, C3 형이 1 예였다. Young-Burgess 15) 분류에따른골반골골절의양상은측방압박골절형이 7 예, 전후방압박골절형이 1 예, 수직전단골절형이 7 예, 복합형이 1 예였다. 후방골반환손상은단독천장관절의손상이 3 예, 천골골절이 3 예, 복합손상이 10 예이었다. 천골골절은 Denis 분류상 I 형이 4 예, II 형이 8 예, III 형이 1 예이었다. 수상후수술까지평균 10.8 일 (5-19 일 ) 이소요되었다. 동반손상과임상적인상태를고려하여조기에수술적치료를고려하였으나, 불가피한경우골견인을시행하였고, 동반된손상이안정되고환자의전신상태가회복된후수술적치료를시행하였다. 수술적방법은골반내전방접근법 (intra-pelvic anterior approach) 을통한금속판고정술을 9 예에서시행하였고, 골반외후방접근법 (extra-pelvic posterior approach) 을통한장력대금속판 (tension band plate) 고정술은 2 예, 골반외후방접근법 (extra-pelvic posterior approach) 을통한장천골나사못고정술 (iliosacral screw) 은 4 예그리 고골반외후방접근법 (extra-pelvic posterior approach) 을통한척추 - 골반고정술 (spino-pelvic fixation) 은 1 예에서시행하였다 (Fig. 1). 수술후모든환자에서침상내체위변동을허용하였고, 1-2 주에휠체어거동을시작하였으며, 2-6 주에목발사용하의부분체중부하를시행하였다. 골절부의유합소견이보이면이학적검사로동통과이상가동성을확인한후전체중부하를허용하였다. 4-8 주간격으로추시하며골반골전후면, 입구및출구방사선사진을확인하였다. 방사선적평가는우선천장관절의정복상태에대해 Matta 와 Saucedo 의기준 16) 을이용하여해부학적 (anatomic, 골반이대칭이고전위가 4 mm 이내 ), 준해부학적 (nearly anatomic, 전위가 4-10 mm 이내 ), 보통 (moderate, 전위가 1-2 cm 사이 ) 그리고불량 (poor, 2 cm 이상의전위가남은경우 ) 으로구분하였다. 또한전산화단층촬영을이용한수술전, 후골반환의회전전위 (rotational displacement) 를평가하였다 (Fig. 2). 골반환의회전전위는몸통의축에대해골반의회전정도, 즉천장관절의벌어진정도를나타낸다. 측정은전산화단층촬영을통해시행하였고수술전후전산화단층촬영을시행한 10 예 (B 형 5 예, C 형 5 예 ) 에서분석하였다. C3 형인 1 예는전산화단층촬영을시행하였으나양측손상으로인해측정방법상건측에대한기준이없어서제외하였다. 평가방법은전산화단층촬영의축상면 (axial view) 상에서천골의중심에서전후를잇는선과전상장골극 (anterior superior iliac spine) 과장골의후방끝부분을잇는선의각을수술전, 후에따라건측과환측에대해각각측정하였다. 그리고건측의각도에대한환측각도의비율을계산하여우수 ( 건측과의차이가 10% 이하 ), 양호 ( 건측과의차이가 10-20%), 보통 ( 건측과의차이가 20-30%), 그리고불량 ( 건측과의차이가 30% 이상 ) 으로구분하였다. 임상적평가는 Rommens 와 Hessmann 의기준 17) 을이용하여동통, 보행, 보행거리에따라우수 ( 동통이없고, 거리제한없는정상보행 ), 양호 ( 제한된동통, 약간의거리제한이있는다소비정상보행 ), 보통 ( 규칙적인동통, 제한된거리의비정상보행 ), 그리고불량 ( 심한동통, 거리가매우제한된비정상보행 ) 으로구분하였다. 결 과 방사선적평가에서는우선천장관절의정복상태에대해 Matta 와 Saucedo 의기준으로분석한결과해부학적정복 (anatomic) 이 7 예, 준해부학적정복 (nearly anatomic) 이 8 예, 보통 (moderate) 이 1 예로나타났으며, 수술전평균 11.0 mm 의전위가수술적고정후 4.5 mm 로감소되었다. Tile 분류상 B 형골절 8 예중해부학적정복이 5 예였고, C 형골절 8 예중해부학적정복이 2 예로나타나 B 형골절이 140

4 C 형골절보다상대적으로더만족스러운결과를보였다 (Table 2-1). Young-Burgess 분류상측방압박골절형 7 예중해부학적정복이 4 예, 준해부학적정복이 3 예였고, 전후방압박골절형인 1 예는해부학적정복소견을보이고, 수직전단골절형인 7 예는해부학적정복이 2 예, 준해 부학적정복이 4 예, 보통이 1 예, 복합형인 1 예는준해부학적정복소견을나타냈다 (Table 2-2). 유의한연관성은찾기힘들었지만수직전단골절형에서상대적으로나쁜결과를보였다. 수술적고정방법에따른결과는골반내전방접근법을통한금속판고정술을시행한 9 예중 8 예가준 A B C Fig. 1. 5 Kwang-Jun Oh et al.: Surgical Fixation of Sacroiliac Joint Complex in Unstable Pelvic Ring Injuries 해부학적정복이상의결과를나타났으며, 골반외후방접근법을통한장력대금속판고정술을시행한 2 예, 골반외후방접근법을통한장천골나사못고정술을시행한 4 예, 그리고골반외후방접근법을통한척추 - 골반고정술을시행한 1 예모두에서준해부학적정복이상의결과를보였다 (Table 2-3). 수술방법에따른결과에서는뚜렷한연관성은보이지않았다. 그리고전산화단층촬영을이용한수술전, 후회전전위 (rotational displacement) 측정상에서는분석을시행한 10 예중우수가 4 예, 양호가 1 예, 보통이 4 예, 불량이 1 예로나타났다. Tile 분류상 B 형골절 5 예중우수가 3 예, 보통이 2 예였고, C 형골절 5 예중우수가 1 예, 양호가 1 예, 보통이 2 예, 불량이 1 예로나타나 B 형골절이 C 형골절보다상대적으로더만족스러운결과를보였다 (Table 3-1). 7 Kwang-Jun Oh et al.: Surgical Fixation of Sacroiliac Joint Complex in Unstable Pelvic Ring Injuries 수술후신경손상의발생이상대적으로높았다. 신경손상증상을보인 5 명의환자중 3 명은요천추신경총손상에따른동통, 이상감각및운동장해가잔존하였으며, 2 명은동통, 저림증등의가벼운감각이상증상만이남고운동신경증상은회복되었다. 그리고수술후감염이 1 예에서보여창상세척술과항생제투여로치료하였다. 이밖에불유합, 고정물의전위, 부러짐등은관찰되지않았고, 추시기간중골절편이나탈구부위의재전위소견도관찰되지않았다. 고 찰 불안정성골반환손상의특징은고에너지손상으로발생되며, 출혈량이많아사망률또한높고, 신체내다른장기의동반손상이흔하며, 합병증및후유증의가능성이높다. 그러나해부학적구조의어려움과동반손상으로인해수술적치료는어려운문제로인식되어왔다. 이러한불안정성골반환손상의경우견인술과골반줄등에의한보존적치료는환자가오랫동안누워있어야하고정복이부정확할수밖에없기때문에사망률과장해률이높은데반해 18), 조기수술적치료는빠른회복과동시에위험도를줄일수있다 19-22). 최근들어불안정후방골반환손상시, 후방천장관절복합체를안정화시키는여러가지수술적방법이소개되고있다. 외고정장치를이용한골반환의외고정은빠르고간단하게시행할수있지만, 후방골반환에대한고정으로는견고하지못하고정복의소실이많은단점이있다 23-30). 보다적극적인수술적치료법인내고정술은이러한단점을줄이고, 기능적결과가만족스러운데 31), 본연구에서도방사선적정복율과기능적평가상대부분에서만족스러웠다. 후방천장관절복합체의내고정법중에서골반내전방접근법 (intra-pelvic anterior approach) 을통한금속판고정술은천장관절의탈구를쉽게고정할수있다는장점이있지만천추쪽이제한적이고제 5 요추신경근에가까운단점이있다 32). 골반외후방접근법 (extra-pelvic posterior approach) 을통한장력대금속판 (tension band plate) 고정술은천추골절및천장관절의탈구에모두적용될수있으나골반후면의창상의심부감염증등의합병증을가지는단점이있다 12,18). 골반외후방접근법 (extra-pelvic posterior approach) 을통한장천골나사못고정술 (iliosacral screw) 은마찬가지로천추골절및천장관절의탈구에모두적용될수있고, 다른후방고정법과비교하여생역학적으로안정적인장점이있으나 33) 마미총 (cauda equnia) 이나제 5 요추및제 1 천추신경근을손상시킬수있다는단점이있다. 이와같이각각의수술방법에따라장단점이있으므로골절형태에따라적절한수술방법을선정하는것이필요할것으로사료된다. Tile 7) 과 Pohlemann 등 11,34) 은골절의형태중 C 형에서 B 형보다임상적결과가더나쁘다고하였다. 본연구에서도마찬가지로방사선학적및임상적평가모두에서 C 형이 B 형에비해불량한결과를보였고, 수직전단골절형이상대적으로나쁜결과를보였다. 수술적접근및고정방법에따른방사선학적및임상적결과간에는증례부족으로인해연관성을찾기에한계가있었다. 본연구의제한점으로는적은증례로인해결과들간에통계학적인의의를구하기가어렵다는것이다. 따라서이에대한보다많은증례수집과장기적인추시관찰이필요할것으로고려된다. 결 론 불안정골반환손상에서천장관절복합구조물의해부학적정복및견고한내고정을통해만족할만한방사선학적및임상적결과를얻을수있었다. 그리고골절형태에따른평가상 C 형골절과수직전단 (vertical shear) 골절에서방사선적, 임상적결과및합병증에서상대적으로나쁜수술후결과를보였다. 따라서초기손상형태가환자의치료결과에많은영향을주는것으로생각되며, 수술적치료시천장관절복합구조물의손상기전에따라다양한고정방법을통해적극적인수술을시행하여천장관절의안정성을얻는다면만족스러운결과를얻을수있을것으로사료된다. 9 Kwang-Jun Oh et al.: Surgical Fixation of Sacroiliac Joint Complex in Unstable Pelvic Ring Injuries 국문초록 불안정골반환손상에서천장관절복합구조물의수술적고정 오광준 황석민건국대학교병원정형외과학교실 목적 : 불안정골반환손상에서천장관절복합구조물의수술적고정의효용성과치료결과에대해알아보고자하였다. 대상및방법 : 2006 년부터 2010 년까지본원에서 Tile 분류상 B 와 C 에해당하는불안정골반환손상에수반되는천장관절복합구조물에대해수술적고정을받고, 12 개월이상추시가가능하였던 16 예를대상으로하였다. 수술적치료의방법으로골반내전방접근법을통한금속판고정술은 9 예, 골반외후방접근법을통해서는장력대금속판고정술은 2 예, 장천골나사못고정술은 4 예, 그리고척추 - 골반고정술은 1 예에서시행하였다. 치료결과는방사선학적평가와임상적평가를시행하였다. 결과 : 수술적고정을시행한모든환자에게서유합을얻었으며, 방사선학적결과는 Matta 와 Saucedo 에의한기준을이용한평가상에서준해부학적정복이상이 15 예로나타났다. 회전전위는분석을시행한 10 예중보통이상이 9 예로나타났다. 임상적결과는모두보통이상이었다. 결론 : 불안정골반환손상에서천장관절복합구조물의해부학적정복및견고한내고정을통해만족할만한방사선학적및임상적결과를얻을수있었고골절형태상 C 형골절과수직전단골절에서상대적으로나쁜결과를보였다. 색인단어 : 천장관절복합구조물, 불안정골반환손상, 수술적치료결과 147
